Abstract

Menggigil adalah mekanisme pertahanan tubuh untuk melawan hipotermi. Menggigil berpotensi memberikan dampak buruk pada pasien termasuk peningkatan konsumsi oksigen, hipoksemia, dan memperparah nyeri operasi. Penggunaan cairan infus yang hangat merupakan salah satu strategi yang digunakan di instalasi bedah sentral (IBS) untuk membantu menjaga kenyamanan dan stabilitas suhu tubuh pasien. Penelitian ini bertujuan untuk mengetahui Gambaran pemberian cairan infus hangat terhadap pencegahan menggigil pascaoperasi pada pasien pembedahan Sectio Caesarea di RSU Mitra Delima Malang. Desain dalam penelitian ini adalah deskriptif kuantitatif, jumlah sampel sebanyak 50 responden yang diambil secara consecutive sampling. Instrumen yang digunakan adalah cairan infus hangat, jam tangan, termometer digital, dan lembar prosedur pemberian infus hangat. Alat pengumpulan data menggunakan lembar observasi kemudian data dianalisis secara deskriptif untuk menentukan frekuensi dan persentase dari kejadian menggigil pascaoperasi. Hasil penelitian didapatkan Derajat menggigil paling banyak adalah derajat 0 tidak menggigil sebanyak 42 responden (84%), derajat 2 sebanyak 2 responden (4%), derajat 3 sebanyak 5 responden (10%), derajat 4 sebanyak 1 responden (2%). Kejadian menggigil dari 50 responden yang diteliti, hanya terjadi pada 8 responden atau 16% yang menggigil, sedangkan pada 42 responden atau 84% responden tidak menggigil. Simpulan penelitian menunjukkan penggunaan cairan infus hangat efektif dalam pencegahan terjadinya menggigil pada pasien pascaoperasi sectio cesarea.

Abstract

Penggunaan general anastesi pada pasien laparatomi dapat menimbulkan kecemasan karena pasien akan kehilangan kesadaran selama operasi dan menghadapi ketidakpastian terkait proses anestesi dan efek sampingnya. Selain itu, ketakutan akan risiko dan komplikasi general anestesi juga dapat menyebabkan kecemasan yang tinggi pada pasien menjelang prosedur laparatomi. Tujuan : Menganalisis faktor kecemasan pada pasien praoperasi laparatomi dengan general anestesi. Penelitian ini adalah analitik korelasi dengan pendekatan cross sectional. Penelitian ini dilaksanakan di RST dr.Soepraoen, pada bulan Mei - Juni 2023. Populasi penelitian sebanyak 55 orang dan seluruhnya dijadikan sampel. Pemilihan sampel dengan teknik purposive sampling. Analisis data dilakukan secara univariat dalam tabel distribusi frekuensi dan analisis bivariat menggunakan uji korelasi Gamma pada taraf kepercayaan 95% (a = 0,05). Pola kecemasan pasien praoperasi laparatomi dengan general anestesi di RST dr.Soepraoen berhubungan signifikan dengan umur (p = 0,003, r = 0,714), jenis kelamin (p = 0,000, r = 0,762), pendidikan terakhir (p = 0,000, r = -0,753), dan pengalaman operasi sebelumnya (p = 0,000, r = -0,826). Umur, jenis kelamin, pendidikan terakhir, dan pengalaman operasi sebelumnya berhubungan dengan pola kecemasan pada pasien praoperasi laparatomi. Disarankan pada perawat anestesi untuk memberikan teknik relaksasi sederhana seperti pernapasan dalam, meditasi, atau visualisasi yang dapat membantu pasien mengelola kecemasan sebelum operasi.

Abstract

This study analyzes the legal authority and protection of anesthesia practitioners (penata anestesi) who provide services in hospitals without direct supervision from anesthesiologists. The issue arises from the gap between legal norms and healthcare conditions in Indonesia, particularly the unequal distribution of anesthesiologists, which creates legal challenges for practitioners in underserved regions. This research aims to assess the alignment between existing legal frameworks and actual clinical practices and to propose regulatory adjustments that strengthen legal certainty and professional protection. Using a normative juridical approach with a descriptive-analytical method, the study reviews statutory regulations, legal doctrines, and relevant literature. Qualitative analysis was conducted to examine the relationship between legal provisions and the realities of anesthesia practice. The findings show that Minister of Health Regulation No. 18 of 2016 remains restrictive and does not accommodate the limited availability of anesthesiologists. In emergencies, anesthesia practitioners may perform tasks beyond their formal authority, exposing them to potential legal liabilities. The mismatch between legal norms and practical needs results in insufficient legal protection and uncertainty in practice. This study highlights the need for adaptive, competence-based, and patient-safety-oriented regulatory reform.

Abstract

The availability of anesthesiology specialists in Indonesia remains disproportionate to the growing demand for health services. This condition compels nurse anesthetists to play a crucial role in delivering anesthesia services, particularly in hospitals that lack anesthesiology specialists. This study aims to analyze the form of legal protection for nurse anesthetists who perform anesthesia practice in hospitals without anesthesiology specialists and to assess the alignment between existing regulations and actual field conditions. This research employs a normative juridical approach with a descriptive-analytical method, examining primary legal materials such as laws and ministerial regulations, along with secondary materials including academic literature and scholarly journals on health law. The analysis focuses on identifying gaps between the legal framework and the professional practices of nurse anesthetists in hospitals. The findings reveal a significant discrepancy between normative regulations and the practical needs of medical services. Existing laws, including Law No. 17 of 2023 and Minister of Health Regulation No. 18 of 2016, do not provide sufficient legal protection for nurse anesthetists working without direct supervision. Consequently, they face legal risks in performing their duties, even when their actions are aimed at ensuring patient safety. The study concludes that reform of health law policies is urgently needed to adapt to current service conditions, particularly through the establishment of clearer delegation mechanisms and indirect supervision systems. These findings contribute to the development of health law literature in Indonesia and serve as a foundation for future research on the implementation of legal protection for nurse anesthetists.

Abstract

Patient safety is a critical component of surgical procedures, with the Surgical Safety Checklist (SSC) playing a key role in minimizing medical errors. However, its implementation in healthcare settings continues to face challenges in medical team compliance. This study aimed to examine the relationship between the type of surgical procedure and the level of compliance of medical teams in implementing SSC. An analytic survey with a cross-sectional approach was conducted at a maternal and child specialty hospital in urban Yogyakarta. Data were collected through observation of 65 surgical procedures using a checklist based on the three SSC phases (sign-in, time-out, sign-out), and analyzed using the chi-square test. Results showed elective surgeries were more dominant (55,4%), with the majority of patients undergoing cesarean section (84,6%). A significant association was found between surgery type and compliance during the time-out (p-value = 0,004) and sign-out (p-value = 0,011) phases, but not during the sign-in phase (p-value = 0,195). Overall, only 27,7% of surgeries were performed with full SSC compliance. These findings indicate that surgical procedure type significantly influences SSC compliance, particularly in the time-out and sign-out phases, and underline the need for targeted system strengthening through structured training, standardized workflow integration, and continuous compliance monitoring to improve patient safety outcomes.
