Abstract

Introduction: Esophagogastroduodenoscopy or gastroscopy is one of the types of endoscopies; the examination includes the esophagus, gastric, and duodenum. Endoscopy is a non-surgical attempt to visualize organs with abnormalities or disorders. It can also be used to obtain tissue (biopsy), remove foreign bodies in the gastrointestinal tract, and observe organs in the body. This study aims to determine the profile of gastrointestinal endoscopy in patients examined at the endoscopy unit of Primaya Hospital, East Bekasi. Methods: The type of research conducted was descriptive retrospective using secondary data in the Medical Record Installation of Primaya Hospital, East Bekasi, from October 2021 to October 2022, totaling 103 patients. Results: The results of this study showed that 51 patients (49.5%) were male, 52 patients (50.5%) were female, and the majority of the patients were aged 41-60 years as many as 44 patients, 42.7%). The most common pre-action diagnosis was dyspepsia (43 patients, 42%), and the most common post-action diagnosis (21 patients, 20.6%) was erosive gastroduodenitis. The most common location of findings in the corpus was 33 patients (32.1%), and the most common finding was mucosal erosion in 36 patients (35.0%). Biopsy was performed in 30 patients (29.1%), and anatomical pathology revealed Helicobacter pylori in 5 patients, chronic duodenitis in 4 patients, chronic gastritis in 19 patients, chronic gastroduodenitis in 1 patient, and polyp putz jeers in 1 patient. And with the most prolonged duration of action being 10-30 minutes, namely 91 patients (88.3%). Conclusion: The results of this study indicate that chronic gastritis accompanied by H.pylori infection is a diagnosis that is more commonly found through esophagogastroduodenoscopic examination and the results of anatomical pathology examination of biopsy tissue compared to other diagnoses.

Abstract

Psychological responses such as anxiety often arise from negative perceptions in situations perceived as threatening, even when they should not induce fear. These anxiety-induced responses can lead to increased hemodynamic parameters such as elevated blood pressure, heart rate, and respiratory rate—particularly prior to medical procedures. This study aimed to evaluate the effectiveness of a digital virtual reality (VR)-based therapeutic intervention in reducing patient anxiety before undergoing gastrointestinal endoscopy. A quasi-experimental design was employed, involving both an experimental group and a control group using a pretest-posttest control group approach. Anxiety levels were measured using the State-Trait Anxiety Inventory (STAI). The intervention consisted of a 6-minute immersive VR video featuring calming mountain and beach scenery. A total of 50 respondents participated, with 25 in each group. Data were analyzed using paired and independent t-tests. Results showed no statistically significant difference in mean anxiety scores between the intervention and control groups (MD = 0.23, df = 48, t = 0.59, 95% CI: -0.56 to 1.03, p > 0.001). These findings suggest that the digital VR-based therapy was not statistically effective in reducing pre-endoscopy anxiety. Future interventions may require more immersive and interactive VR experiences—such as 360-degree simulations of endoscopic procedures—to achieve more substantial effects.

Abstract

The effect of the patient's anxiety before performing an esophagogastroduodenoscopy (EGD) action affects the patients' hemodynamics, it can affect the process of such actions when they are performed, usually anxieties are affected by the feeling of being that appears as a result of poor perception of the action to be done, negative perceptions appear often caused by a lack of information about the actions to be performed. Nursing care is very influential to the patient, one of the nursing model theories that can be applied to patients undergoing esophagogastroduodenoscopy (EGD) with the theory of Human Caring by Jean Watson. The nursing process focuses more on health promotion, disease prevention, care for the sick, and physical recovery. The author differentiates the nursing case/orphanage report by applying Jean Watson's model theory using nursery process methods that include the examination, diagnosis, planning, implementation and evaluation of these nurses' orphanages. The result of the implementation that has been done is that the patient feels a change in perception in which the patient experiences a decrease in the level of anxiety after performing a personal caring approach in accordance with Jean Watson's Model Theory.

Abstract

Abstract

Background: Gastroscopy is a diagnostic procedure to view organs visually using a lat connected to a monitor, the organs viewed are the esophagus, gastric and duodenum. Anxiety in patients before gastroscopy occurs due to fear by imagining. Education is carried out as a pre-action procedure to explain the actions that will be performed. Objective: We conducted a systematic review and meta-analysis to investigate the effect of audio-visual education on patient anxiety before gastroscopy. Results: Four articles were included in this study, audiovisual education significantly reduced patients' anxiety before gastroscopy using Visual analogue scale (VAS); one study used Anxiety and Stress Scale-42 (DASS-42) and one study used State Trait Anxiety Inventory (STAI). Based on a randomized control trial, it was found that education was effective in reducing anxiety by -4.38 (95% CI: -14.3796 to 5.6198), indicating insignificant results. And obtained I² value of 99.9607%, Q value of 1017.3516, and P value <0.0001 which means that heterogeneity is high. The range of results obtained was -26.7290 to 17.9692 with three negative results and one possible positive result (Khan, et.all, 2023) with a residual value of (± 2.4977) In the regression test results showed asymmetry in the flores plot (p < 0.0001) but did not show a rank correlation test (p = 0.7500). Clinical Implications: Audio-visual education is a potentially promising method for controlling anxiety in patients undergoing gastroscopy; however, its effect in controlling anxiety is controversial.

Abstract

Background: Anxiety is an excessive and unclear feeling of worry, and it is also a response to both external and internal stimuli that causes emotional, cognitive, physical, and behavioral symptoms. Anxiety is different from fear because, in fear, the object is clearly known and threatens the well-being of the person, while in anxiety, the object is unknown. Anxiety is an experience that everyone goes through (universal) throughout their life. Anxiety has a positive function because it can motivate people to take actions that can resolve their issues. Anxiety is considered normal when it is proportionate to the situation and will disappear once the situation is resolved. Objective: To determine the effect of the deep breathing relaxation technique on anxiety levels in pre-operative patients at the hospital. Methods: This study uses a pre-experimental design, quantitative correlation, with primary data from pre-operative patients aged 17-50 years, consisting of 126 respondents. Sampling was calculated using Slovin's formula. Data collection was done using a questionnaire and interviews with the Hamilton Anxiety Rating Scale (HARS). Results: From the 126 respondents, 14 (11.1%) had no anxiety, 51 (40.5%) had mild anxiety, 58 (46.0%) had moderate anxiety, and 3 (2.4%) had severe anxiety. The Chi-square statistical test showed a p-value of 0.000 (p <0.05), indicating that there is an effect of the deep breathing relaxation technique on anxiety levels in pre-operative patients at the hospital. Conclusion: There is an effect of the deep breathing relaxation technique on anxiety levels in pre-operative patients at the hospital (p=0.000).
