Abstract

Abstract Chronic Obstructive Pulmonary Disease (COPD) is a chronic airway inflammation with resulting progressive airflow limitation that has a high incidence, morbidity, and mortality. Roflumilast is an oral phosphodiesterase-4 inhibitor as a therapy to decrease the risk of COPD exacerbations in patients with moderate-severe COPD with a history of chronic bronchitis. Roflumilast can be given orally once daily as a single or combination drug. It can be used as COPD moderate-severe but also more beneficial as COPD mild treatment. The efficacy of Roflumilast can prevent exacerbations of repetition and can fixed lung function both in terms of FEV1 and vital capacity to force. The incidence of side effects, which are diarrhea and digestive disorders. The cost-effectiveness showed that Quality of life patient with Roflumilast is better than the group without additional. This paper aimed to review systematically Roflumilast as COPD treatment for the clinical application. Keywords                   : COPD, Roflumilast, phosphodiesterase-4 inhibitorCorrespondence          : fauna@staff.ubaya.ac.id

Abstract

AbstractCOVID-19 (Coronavirus Disease 2019) has been started in Wuhan, China, and spread worldwide and resulting in many cases of death. COVID-19 attacks the respiratory tract acutely and infected both children and adults. The number of cases in children is less than in adults. By seeing from the clinical aspect, the COVID-19 case in children is milder. There are differences in immunology responses in children and adults where children have higher immunology response of COVID-19 than adults. Meanwhile, if the immunology response is slow in adults, it may cause them infected by COVID-19 with severe symptoms. There are some relations between immunization with immunology response to SARS-CoV-2 where children who already have BCG vaccination has lower infection rates of acute respiratory tract case. This study aims to know the difference between COVID-19 cases that infected adults and children seen in various aspects. Keywords                     : COVID-19,SARS-CoV-2, children, adult, severity, managementCorrespondence         : mohammad_husin@yahoo.com

Abstract

Freeze-drying technique is an alternative method on sperm preservation. However, this process can results in DNA damage. Adding antioxidants before freezing can reduce the detrimental effects of ROS on spermatozoa. Alpha lipoic acid (ALA) is one of the potent antioxidant that can be used to protect the sperm DNA. This study was aimed to determine the effect of ALA supplementation before freeze-drying on the DNA fragmentation index (DFI) of human spermatozoa. Nine semen samples were collected and evaluated for DFI (O1) then divided into 3 groups; freeze-drying without ALA (O2), freeze-drying with ALA 1,25 mg (O3) and freeze-drying with ALA 2,5 mg (O4).All three groups were stored at 4oC for 1 week. Sperm DFI were evaluated before and after freeze-drying. The result of this study showed a significant increase in sperm DFI in all three groups after freeze-drying (p<0,05) when compared to DFI before freeze-drying (14,67 ±3,295). However there was no significant difference between O2 (26,90 ±9,180) and O4 (29,29 ±5,524) group where the supplementation of 2,5 mg ALA did not have significant effect on protecting the DNA of freeze-dried human spermatozoa. The highest DFI was in the O3 group (44,64 ±11,717). Therefore, ALA supplementation before freeze-drying does not have a significant effect on protecting the DNA of freeze-dried human spermatozoa.

Abstract

Abstract  Diabetes Mellitus is a worldwide metabolic disease and becomes a challenge for the health practitioner. Unfortunately, less than half of these diabetic individuals are aware of their conditions, and less than 1% of those receiving medical treatment achieve their therapeutic targets. Acupressure is an alternative method that is believed to be an effective way of treating diabetic patients with reducing symptoms and complications. This research aims to discover the effect of period acupressure therapy on blood sugar levels in type two diabetes mellitus patients in Lumajang Acupressure Clinic. It uses an observational analytic design with pre-test and post-test approach. It involved 36 participants who were split into two groups and contained 18 participants for each group. The first group received acupressure therapy for three weeks, while the second group received eight weeks. Both groups underwent acupressure at the San Yin Jiao (SP-6) acupoint for 20 minutes three times a week. All participants in both groups had their blood glucose checked before and after acupressure therapy. The results showed that the two groups were significantly different for the paired t-test and the independent t-test. Abstract

The Leydig cells play crucial role in steroidogenesis and spermatogenesis. Those processes need complex communication in hormonal and testicular to maintain male reproductive function. Abnormal condition induced by reactive oxygen species reduce cell viability through lipid peroxidation and apoptotic pathway. Antioxidant ameliorate ROS elevation and prevent cell damage. Specifically, Leydig cells are vulnerable to ROS exposure and decline its function in mediating spermatogenesis. Therefore, it is needed to improve Leydig cells viability within antioxidant supplementation. This study aimed to determine the protective effect of antioxidant on Leydig cells induced by reactive oxygen species. This type of study is a literature review. Various studies have been reviewed through critical appraisal tool Olsen-Baisch Scoring for integrated review. Furthermore, this study highlighted the importance of the mechanism of antioxidant as protective agent of Leydig cells. Supplementation of antioxidant with the correct administration, dosage, and duration is potential to balance reactive oxygen species level and protect Leydig cells.    Keywords             : Leydig cell, antioxidant, reactive oxygen species, infertility

Abstract

Anemia is a condition that is almost always found in pregnancy. Although a program of Fe tablets has been introduced during pregnancy, the prevalence of anemia during pregnancy is still high. This situation maybe caused by another risk factor of anemia such as Thalassemia. This study was aimed to investigate the presence of carrier Thalassemia in pregnant women at the Puskesmas Kertapati, Palembang, South Sumatera using erythrocyte indices. This was a cross-sectional study with a sample size of 68 subjects, taken by purposive sampling technique. Hemoglobin level of all subjects was measured by Easy touch GCHb and confirmed by Sysmex K21. Differences of hemoglobin between Easy Touch GCHb and Sysmex K21 were analyzed by impaired t-test used SPSS program. Other data were analyzed using descriptive statistic. The mean hemoglobin levels in subjects were 10.7 g/dl. There were 68 subjects have anemia according to Easy Touch GCHb  but only 30 subjects that confirmed positive anemia by Sysmex KX 21. T-test result gavep value <0.05 that isconsidered there was a statistically significant difference between Easy Touch GCHb and Sysmex KX 21 results(p=0.001). Calculations using the England-Fraser index, Mentzer, Sirdah, and Srivastava indicate that none of the research subjects had thalassemia carrier status.

Abstract

AbstractPersistent pulmonary hypertension of the newborn (PPHN) is a condition that occurs due to increased resistance to blood vessels in the lungs that occur persistently after the baby is born. This can be attributed to congenital heart disease such as right-to-left shunts through foramen ovale (PFO) or patent ductus arteriosus (PDA) due to an error transition fetal blood circulation to the neonate. Although PPHN is always associated with births in post-term babies, PPHN cases are often found in preterm babies. Chances of babies born with PPHN are quite large, at 1.9% per 1000 live births. PPHN can be fatal, causing mortality rates ranging from 4 to 33%. The incidence of preterm births in Indonesia is estimated at 7-14%, around 459,200 - 900,000 babies per year. This study aimed to prove the relationship between premature babies and persistent pulmonary hypertension of the newborn (pphn) in Sidoarjo  Regional Hospital and to know the characteristics and analyze these variables.  This research used crossed sectional studied design; the population was all preterm babies in the NICU at Sidoarjo  regional hospital. All samples are from medical records in January-December 2018. There is a significant difference between preterm babies and PPHN (p < 0.05); besides, the results from Spearman's correlation analysis obtained a correlation coefficient (ρ) = 0.485. In the cross-tabulation analysis, the result of the proportion with the highest correlation was Late Preterm babies with severe PPHN of 46.7%. It can be concluded that there is a relationship between premature babies and PPHN in Sidoarjo  regional hospital..Keywords                      : Persistent pulmonary hypertension of the newborn, PPHN, Preterm BabiesCorrespondence            : aisyahhelmadevithalib@gmail.com
