Abstract

Preeclamsia patient experienced deterioration of glomerolus filtration and plasma protein concentration. Bupivakain is bound strongly with the plasma protein that is 95%. This research was aimed to know the pharmacokinetics profiles of isobaric bupivacaine with the noncompartemen model in preeclampsia patients who underwent Sectio Caesarea (SC) with lumbar epidural technique in RSUP Dr. Sardjito and to know correlation between protein levels and pharmacokinetics profiles of isobaric bupivacaine in preeclampsia patients. Firstly, albumin concentration of patients was checked in the laboratory and then isobaric bupivacaine 0.5% in 15ml (75mg) was given by lumbar epidural technique. The samples of blood were taken through veins vessels at 0; 15; 20; 25; 30; 45; 60 and 90 minutes. The concentration of 0.5% isobaric bupivacaine in those samples of blood was determined with High Performance Liquid Chromatography (HPLC) methods. Furthermore, pinprick test was taken to measure block sensoric effect in T10-T6 areas. Pharmacokinetics parameters of bupivacaine (AUC, AUMC, MRT) were calculated based on data of bupivakain concentration in the blood versus time. The results of this research showed that the AUC0-∞ (total AUC) was 116±37,6 (μg/ml) minutes; AUMC0-∞ (total AUMC) was 9187,5 (μg/ml) minutes2; and MRT0-∞ was 102,5 minutes. Average block sensoric effect was at T8. There was no linear correlation between the protein levels and pharmacokinetics profiles of bupivacaine (p>0,05). The conclusion was that there was no linear correlation between the protein levels and pharmacokinetics profiles of bupivacaine that was given by lumbar epidural in preeclampsia patientswho underwent section caesarea.Keywords : bupivakaine, pharmacokinetic profile, lumbar epidural, section caesarea.

Abstract

Non-steroidal anti-inflammatory drugs such as diclofenac sodium were common drug given worldwide to reduce pain in osteoarthritis. The long-term used of diclofenac sodium side effects was on liver, kidney, gastrointestinal and other organs. The earlier study result have shown the effectiveness of the combination of temulawak rhizome extract, ginger, soy and shrimp shells in osteoarthritis patients. This third phase of clinical study purpose was to monitor the side effects of temulawak rhizome, ginger, soybean and shrimp shells extract in combination compared to diclofenac sodium in patients with osteoarthritis. Forty seven patients with knee osteoarthritis diagnosed by American College of rheumatology criteria were included in this study. This subject were devided randomly into two groups to receive either the combination of temulawak rhizome 100 mg, ginger 100 mg, soy 50 mg and shrimp shells 50 mg extract (n=23) or diclofenac sodium 25 mg (n = 24) three times a day orally for 14 days. The side effect that were monitored were side effect that appear in the gastrointestinal organs, cardiovascular, central nervous system and other organs for 14 days therapy and laboratory tests of SGOT, SGPT, creatinin serum, urea, hemoglobin, leukocytes and platelets were performed before and after 14 days. Chi-Square test were used to evaluate the side effects and independent t test or Mann-Whitney tests were used to evaluate changes of levels of SGOT, SGPT, serum creatinine, serum urea, hemoglobin, leukocytes and platelets before and after intervention. Percentage of side effects on the gastrointestinal organs, cardiovascular, central nervous system and other organs in combination of ginger rhizome 100 mg, 100 mg of ginger, 50 mg of soy and 50 mg of shrimp shell extract group was not significantly different compared to diclofenac sodium 25 mg. SGPT activity in trial group was decrease significantly (p <0.05) compared to diclofenac sodium 25 mg but no significant difference in liver function (SGOT), kidney function (creatinine and urea), and bone marrow function (hemoglobin, leukocytes, and platelets).Keywords: Osteoarthritis, combination of temulawak rhizome, ginger, soy and shrimp shells, diclofenac sodium, side effects, liver function, kidney function and bone marrow.

Abstract

As an effort to boost the issue of family planning program, BKKBN has adopted a policy of IEC (Information Communication and Education) as a promotional effort and community mobilization to increase demand for contraceptives.This study aim was to analyze the role of mass media in influencing attitudes towards participation of family planning. Research was performed by using the Indonesian Demography and Health Survey data (“SKDI” 2007). Sampling location was national-wide which included all 15-49 years old ever-married women and lived in Indonesia. Informan was determined by purposive sampling technique. Total sample was 32,895 women. Data was analyzed by chi-square test. The results showed that 57.4% of ever-married women had a positive attitude towards family planning program and participated in th family planning program with p=0.005 (p value = 0.000). While ever-married women who were exposed to family planning messages through mass media and become participants KB was 58.3%, with p=0.005 (p value = 0.000). The relationship of mass media’s role in influencing attitudes towards family planning participation was observed with the control variables of education level, residence, motivation to have more children and the number of children (alive), each of which showed  a significant relationship with p=0.005 (p value = 0.000). Television is the medium which attract most viewers. Different model is required for the highly educated women. The emergence of other media such as religious leaders, community, doctors, midwives, and family planning cadres can be used as a vehicle for delivering messages and information. It needs a strategy of repetition (redundancing) messages and information, in order to remind people about the importance of the family planning programs. The right design in campaigning and informing the family planning program should be made after considering whether the type of media used is matched witu geographical characteristics, time, and forms of contraception.Keywords: mass media, attitudes, participation in Family Planning Program

Abstract

The use of drugs in patients with decreased renal function allows the occurrence of drug related problems. Pharmacist has responsibility to identify and to prevent drug related problems. This study was proposed to evaluate drug related problems in chronic kidney disease patients in Inpatient Department of Fatmawati General Hospital. It was descriptive analitic study with prospectively approach. The primary data was obtained by identifying drug related problems. The secondary data was taken from medical record of chronic kidney disease patients such as: age, sex, the stage of chronic kidney disease, comorbidity, and drug therapy. This study took place in Inpatient Department of Fatmawati General Hospital during the period of January to March 2012. Univariate analysis had done to obtain description of frequency and proportion of research variables such as the patient characteristics who received drug therapy (age, sex, the stage of chronic kidney disease, and comorbidity), the drug therapy characterictics that evaluated based on the number and type of drug therapy and drug related problems. Bivariate analysis of the Spearman correlation test was taken to evaluate whether there was correlation beetwen confounding variables and incident of drug related problems. The evaluation was taken in 40 chronic kidney disease patients with 377 number of drugs. The number of drug related problems was 98 issues (25.99% of prescribing drugs). Type of drug related problems were not optimal effect of drug treatments 62.24%, the incidence of non-allergic adverse drug events 20.41%, and the incidence of toxic adverse drug-events 17.35%. The confounding variables that significantly influence the incidence of drug related problems were comorbidities (r= 0.385; p= 0.014), and the number of drugs (r= 0.604; p= 0.000).Keywords: drug related problems, chronic kidney disease, Fatmawati General Hospital

Abstract

Every company needs a knowledge-based management including knowledge in managing intellectual property in order to make innovation continuously to anticipate dynamic change of environment. The purpose of this research was to assess the management of innovation and intellectual property management to be used as the strategy of corporate advantage on PT Cipta Sarana Kenayu Lestari (PT CSKL). This research was a qualitative study producing the descriptive data in a form of the written or spoken words from the respondents. The respondents were the top management of PT CSKL that were selected by purposive sampling. The method to collect the data were interview, observation, documentation and literature methods. Then the data were systematically analyzed by arranging the data and derive a conclusion. The result showed that PT CSKL has managed many innovations both of product and process. PT CSKL was aware of the importance of innovation and IPR protection into business process by registering to the Director General of Intelectual Property Rights (IPRs). However, from the existing regime, PT CSKL only managed brand and trade secret. This was because of limited cost and facilities classified as PT CSKL is small and medium enterprises. Some intellectual property which were not registered were logo’s company, formula of honey lipstick, website, brochure and packaging design. IPR protected by law is used as a strategy advantage of PT CSKL in doing business development strategy. This strategy starts from the management of innovation in family firms into strategic corporate level towards good corporate governance and cooperation strategic alliance, followed by a strategic business level towards to Porter’s focus strategic and competitive strategic as industrial fragmented and strategic level functional in production, marketing, finance and research and development and quality control.Keywords: innovation, intellectual property rights, strategic advantage, PT Cipta Sarana Kenayu Lestari.

Abstract

To improve the quality of service, a hospital needs a management information system that always follow the development of information technology. If the system does not provide the required information, the user will not satisfied and look for other alternatives. Satisfaction of the users can be used to evaluate the quality of information systems in organization. This study aimed to evaluate the quality of pharmacy management information system in academic hospital of Gadjah Mada University and to know theobstacles and benefits perceived by the user with customer window analysis. This study was descriptive research, with survey method. The subjects in this study were physician, pharmacist and IT Officer. The data were analyzed using SERVQUAL and Customer Window Analysis The result showed that there was negative gap value between expectations and perception of the user management information systems of the hospitals. Therefore all of the user experienced dissatisfaction with the management information systems. The biggest gap was in dimensions of assurance (-1,36) followed by dimensions of esponsiveness (-1,35), reliability (-1,15), tangibles (-1,1) and empathy (-0,94). Meanwhile, the largest gap in the information service quality was the format (-1,92), followed by completeness (-1,35), timeliness (-1,17), accuracy (-1,08), security (-1,00), ease of use (-0,98) and socio cultural (-0,96). From the analysis of customer window, the priorities of improvement were in the dimension of responsiveness, assurance and format.Keywords: Quality, Management information system RSA UGM, SERVQUAL, Customer Window Analysis

Abstract

Currently, Indonesia’s health development began to emphasize on improving the quality of health care, where service quality should be oriented to customer satisfaction. Departement of Pharmacy at X Hospital Jakarta is one of the medical support required todeliver a quality service that oriented to patient satisfaction. This research was descriptive study using primary data that obtained from the questionnaire that distributed to 400 patients at outpatient pharmacy, which was the patient who secured by insurance (A and B) and cash (F). The category of patients at outpatient pharmacy are patient that secured by corporate, corporate pensioner in Jabodetabek, patient that secured by other insurance and cash patient. The sampling technique used is nonprobability sampling, which was the quota sampling. The level of satisfaction of patients seen by SERVQUAL method and the dimension of service quality was mapped in a Cartesian Diagram.In general, patients were not satisfied with the performance of Outpatient Pharmacy at X Hospital Jakarta. The results showed that the gap between patients’ expected service and perceived service of outpatient pharmacy at X Hospitals was negative. The sequence of the biggest negative gap felt in responsiveness (-0.7100), empathy (-0.6550), reliability (-0.5800), assurance (-0.4975) and tangible (-0.4200). The sequence of the greatest dissatisfaction felt by corporate pensioner in Jabodetabek (30.59%), cash patients (29.08%), patient that secured by other insurance (22.22%) and patient that secured by corporate (18.11%). The sequence of the biggest dissatisfaction perceived by the patient for services in outpatient pharmacy F (37.49%), outpatient pharmacy A (32.47%) and outpatient pharmacy B (30.04%). Based on the mapping of Cartesian Diagram, Outpatient Pharmacy at X Hospital Jakarta should pay more attention to the dimensions of responsiveness, especially regarding speed of service and providing more crew at peak hours.Keywords: Quality of Service, Satisfaction, SERVQUAL, Cartesian Diagram

Abstract

The Pharmacy Department of Health Directorate of Army Force (Lafi Ditkesad) always continually improved in some aspects, not only focus on product quality yielded, but also on production cost management. Therefore, Lafi Ditkesad should always allocate its production factors suitably and effectively to avoid production factor waste that would harm Lafi Ditkesad. This research was aimed to know work force and machinery production costs towards production output of Floxad caplet and Lafidril syrup medicines in Lafi Ditkesad and to know the effectiveness of input use level in Lafi Ditkesad. Data collection technique was based on a documentation study from Lafi Ditkesad. This was a descriptive quantitative research and an explanatory research supported by a survey method. Analysis of the data was conducted with multiple linear regression analysis modified from a Cobb-Douglas production function model with an SPSS software. A independent variable effect towards a dependent variable was conducted by an F-test and t-test with α = 0.05. The result showed that work force (probability = 0.03) and machinery (probability = 0.00) production factors significantly affected production of Lafidril syrup medicine. Meanwhile, in Floxad caplet production, work force production factor (probability = 0.00) significantly affected the production output, but the machinery production factor (probability = 0.09) did not significantly affect production output. Floxad caplet production effectiveness was better than Lafidril syrup production process. This could be viewed from an effectiveness index from each production function where an effectiveness index of Floxad caplet medicine production was 57641.61, while Lafidril syrup was 4755.23.Keywords: Cobb-Douglas, effectiveness, work force, machine

Abstract

The problem usually found in the Jamkesmas (Society Health Insurance) realization is the difference between the real cost and INA-CBGs package tariff for patients using Jamkesmas, especially in inpatient department. This study was to find out how much the margin between the real cost and the INA-CBG’s package tariff and the factors influence the real cost, and to know the suitability of drug indications in diabetes mellitus inpatients using Jamkesmas in RSUP Dr. Sardjito Yogyakarta. This study was analytical observation. The data were taken retrospectively from the Jamkesmas claim files and patients’ medical record. Subjects were patients with diabetes mellitus type 2.The research object included the claim files and the medical record of the diabetes mellitus patients using Jamkesmas in RSUP Dr. Sardjito Yogyakarta during period of July 2010 – May 2012 with the diagnosis code INA-CBGs E-4-10-I, E-4-10-II, and E-4-10-III. The data were analyzed descriptively. Moreover, One sample test statistical analysis was conducted to find the difference between the real cost and INA-CBGs tariff and the ifference between hospital LOS and INA-CBGs package LOS. In addition,bivariate correlation test and regression linier test were used to study the relation among the factors that affect the real cost. The result of the study showed that the difference between the real costand INA-CBGs package tariff of the diabetes mellitus Jamkesmas patients with the severity level I was Rp 5,325,126 in 2 episodes of cares; severity level II was Rp -22,411 in 10 episodes of care, and severity level III was Rp -3,038,240 in 12 episodes of care. The factors that affect the real cost of treatment of patients with the severity level II were cost of clinical pathology examination, blood, and drug/medical cost, while in patient with severity level III werethe cost of visite, service in dialysis department, clinical pathology examination and drug/medical cost. From the analysis between the used drug and indication, it was found that 22 episodes of care was appropriate with the diagnosis and 2 episodes of care was not fit with the diagnosis.Keywords: Jamkesmas, INA-CBG’s, diabetes mellitus, cost components, indication of suitability

Abstract

In 2011, health insurance in patients with thalassemia major help families of patients because thalassemia treatment cost is quite high. In the implementation of INA-CBGs there are some problems, one of which is related to the payment in which difference occurs between tariff of INA-CBGs with real cost.The aim of this research was to find out the extent of the difference between the real tariff and the tariff of INA-CBGs, factors that influence real cost and the description of the appropriateness of therapy towards the Jamkesmas inpatients with Thalassaemia at Sardjito Hospital Yogyakarta.This was an analytical-observational research with of retrospective research design. The object of the research was the files of the inpatient claims and medical records of Jamkesmas patients with Thalassaemia in the period of April 2011 – March 2012 in Sardjito Hospital Yogyakarta. The data analysis was conducted with one sample test to compare between the rate of the real cost and the INACBG’s tariff. Meanwhile, the correlation test and the linear regression test were conducted to find out factors that influence the cost. The result of the research showed that a positive difference between the real cost and the tariff of INA-CBGs was IDR104.498.068 from 138 episodes of care. The average cost of healthcare for Thalassaemia based on the tariff of Sardjito Hospital at the severity level 1 was IDR1.058.823 + 591.923. The three most significant cost components included the blood bag, drugs or medical stuffs and accommodation/hospitalization cost. The result of the correlation test showed that the blood bag, drugs and medical stuffs, hospitalization/accommodation, clinical pathology examination and visite had a strong influence towards the real cost. Meanwhile, from the value of variable coefficients (B), it was known the order of variable that influence the real costs were visite, hospitalization/accommodation, blood bag, drugs and medical stuffs, and clinical pathology examination. From the analysis of the suitability of an indication, there were 5 drugs that are not consistent with the diagnosis of patients and one drug that was not appropriate for the patient.Keywords: INA-CBGs, Thalassaemia, cost, appropriateness of therapy
