Abstract

Background: Malnutrition is one common problem for hospitalized patient, estimated between 20-60%. American Dietetic Association (ADA) recommends a standardized Nutrition Care Process (SNCP) that ensures services and outcomes in the quality management of nutrition care to all patients based on an individual basis and scientific facts. In Indonesia was developed nutrition screening tool, namely the Simple Nutrition Screening Tool (SNST) which was valid and reliable.Objective: The study was to determine the effect of SNCP based on screening to improve nutrient intake and nutritional status.Method: The method using a 2x2 factorial design, with 4 groups which are: screened with Nutritional Risk Screening (NRS) and get hospital-based SNCP (A1B1); screened with NRS and algorithm SNCP (A1B2); screened with SNST and get hospital-based SNCP (A2B1); screened with the SNST and get algorithm SNCP (A2B2).Results: Nutritional screening interventions using NRS and SNST method can increase the energy and protein intake also Mid-Upper Arm Circumference (MUAC). There is no relationship between algorithm SNCP with nutrient intake and nutritional status, although the average intake of energy, protein, MUAC and albumin was better in the algorithm SNCP than in hospital-based SNCP. The interaction between the SNST and algorithm SNCP have better mean intake of energy, protein and MUAC changes than other groups. The size of the effect of energy intake, protein intake, and MUAC was 129 kcal; 4.6 g; and 0.35 cm, respectively.Conclusion: Interaction between an SNST and algorithm SNCP have a better average intake of energy, protein, and MUAC compared with other groups.

Abstract

Background: Severe malnutrition affects morbidity due to disrupted wound healing and decreased endurance against infection. There are some factors that cause malnutrition in surgical patients. Two major factors are lack of food intake and process of inflammation that cause increase of catabolism and decrease of anabolism. The condition can be identified from the decrease of albumin serum and muscle hypothrophy.Objective: The study aimed to identify impact of nutrition status of preoperative major surgical patients based on Nutritional Risk Index (NRI) indicator to wound healing and post operative length of stay.Method: The study was observational with prospective cohort study design. Nutrition screening was undertaken to the group that met inclusion criteria at the preliminary process of hospitalization using NRI indicator to identify nutrition status of surgical patients. Next, the group was split into two, one with undernourished nutrition status and the other with good nutrition status. Assessment of nutrition status of patients was based on NRI obtained from level of serum albumin and current and usual weight ratio within the last 6 months. Weight assessment was made during early hospitalization or weight within the last 6 months.Result: The result of the study showed there was signifiant association between nutrition status and wound healing and post operative length of stay (p<0.05). The result of logistic regression test showed there was impact of nutrition status to wound healing and post operation length of stay. Patients with undernourished nutrition status based on NRI contributed 4.8 times greater for the prevalence of poor wound healing than those with good nutrition status (95%CI: 1.179-19.880). Patients with undernourished nutrition status based on NRI contributed 5.5 times greater for extended length of stay > 7 days than those with good nutrition status (95% CI:4.701-50.914).Conclusion: There was impact of nutrition status of preoperative major surgical patients based on NRI indicator to wound healing and post operative length of stay.

Abstract

Background: Malnutrition and immunology change in stroke would affect the outcome of the stroke patient. The supplement of the extract of snakehead fish as an alternative is expected could reduce the effect pasca stroke.Objective: This study aims to assess the effect of snakehead fish supplementation on nutritional status, levels of albumin, TLC and TNF-α in stroke patients.Method: This type of study is a quasi-experimental, pre-test post-test group design, by providing treatment to the subject of research and treatment effects were measured and analyzed. Subjects were divided into two groups; a first group which receives 3 x 2 capsule of snakehead fish supplementation as well as diet and nutrition education (intervention group) and the second group that received only diet and education (control group).Results: From the results, it was found that nutritional status, which was obtained by measuring UAC in cases and controls are good (SG ≥ 85%) but no significant differences between them (p=0.914). There are significant differences between albumin levels before and after intervention (p=0.023) with an average increase of 0.2 g/dl, but not in the control group (p=0.931). For TLC values increased in the intervention group (990,5 sel/mm3) and a decrease in the control group (645,03 sel/mm3) with a significant difference in both groups (p=0.034). There was a decrease of TNFα in both group; for cases (mean=-2.91) and controls (mean=-2.58) with significant differences between the two groups (p=0.001).Conclusion: The supplementation of an extract of snakehead fish could increase the albumin and TLC level and decrease the TNFα in stroke patients.

Abstract

Background: The high prevalence of anemia pregnant women anemia in Indonesia reaches 50%–70%, and it is caused by many factors. First, it may be caused by low compliance of iron supplementation and second, it may be caused by bad distribution of iron supplementation. This prevalence results some effect: high maternal mortality rate, neonatal mortality rate, anemia in infancy, and low productivity. One solution to overcome this prevalence is by giving iron supplementation to pregnant women with husband’s supervision.Objective: This study aims to examine the relationship between iron supplementation with compliance, Hb value, and average of age gestation under husband’s supervision.Methods: This was quasi experimental non-equivalent control group design. Subjects were pregnant women aged 20–28 weeks whose hemoglobin level 8 gr/dl to 15 gr/dl. Subject were taken from three sub-districts of Bantul District, Special Teritory of Yogyakarta. Subject were divided into two groups, the first group (n=55) received 60 mg iron supplementation with husband’s supervision and the second group (n=65) received 60 mg iron supplementation without husband’s supervision.Results: The compliance of first the group was higher than the second group (x2=19,48 p<0,01). In the first group, iron supplementation was effective to increase Hb level it was statistically insignificant (p>0,05). In the second group, iron supplementation was also effective to increase Hb level and it is statistically significant (p<0,05). The difference of Hb level between two groups was insignifi cant. After the predictor variable was controlled using multivariate regression test, it showed that iron supplementation with husband’s supervision was effective to increased Hb level 0,8 gr/dl and statistically signifi cant (p<0,05). The gestation average of two groups was similar, but after predictor variable was controlled using multivariate test, it showed that iron supplementation with husband’s supervision had one week gestation longer than those group without husband’s supervision.Conclusion: Iron supplementation with husband’s supervision are effective to lengthen one week age gestation.

Abstract

Background: Effective control of blood glucose and activities of antioxidant are key factors that prevent diabetes mellitus (DM) and its complications. There are lots of herbal plants that have those both effects. Ceplikan leaves (Ruellia tuberosa L.) is a traditional medicine which is empirically used to lower blood glucose level. Instead of antioxidant compound, there is assumed other compound in ceplikan leaves that has side effect to pancreatic beta cells.Objective: To identify the effect of ceplikan leaves extract to blood glucose level and pancreas histology description in white diabetic rats (Rattus norvegicus).Method: Thirty subjects of Wistar strain male white rats of 2-3 months old and of 150-200 grams weight were made diabetic with aloxan and randomly divided into 5 groups. Group I consisted of diabetic rats with aquadest, group II with glibenclamide, and Group III-V were given extract of ceplikan leaves in different concentrations that were 1.6 mg, 3.2 mg, and 6.4mg, respectively. Treatment was given orally per day within 30 days. Level of blood glucose was measured in the day of 0, 3, 4, and 30. Statistical analysis used repeated measures and t-test.Result: The supply of ceplikan leaves extract could reduce level of blood glucose of diabetic rats, although the decrease was insignificant. Average diameter of wider Langerhans island occurred to the group of diabetic rats that were given extract of ceplikan leaves dosage 6.4 mg. There was no significant difference (p > 0.05) in changes of blood glucose level before and after experiment in diabetic rats. Pancreas histological description of rats showed that there was improvement as indicated by greater quantity of Langerhans Island and wider diameter of Langerhans Island.Conclusion: Ceplikan leaves was safe and efficacious, so that self-medication of DM using ceplikan leaves could be sustained through formal approach.

Abstract

Background: Obesity can lead to various health problems including psychosocial problems. Obesity also relates to body image which may affect psychosocial health.Objective: To examine the relationship between obesity, body image, and perceived stress among college students in Yogyakarta Province.Method: This study is a cross-sectional design in 150 college students (75 boys and 75 girls) at Universitas Gadjah Mada and Universitas Technology Yogyakarta. Perceived stress was measured using 10 items of the Perceived Stress Scale (PSS-10), body image was evaluated using 16 items of the Body Shape Questionnaire (BSQ-16) and obesity was estimated using the percentage of body fat calculated from anthropometric measurements. Chi-square test, Spearman correlation analysis, and odds ratio were used to test the hypotheses.Results: There was significant correlation between obesity and body image in male (r= 0.32, p<0,01) and female students (r= 0.23, p<0,05). Obese male students were 7 times likely to have negative body image than non-obese male students. While no significant relationship was found between obesity and perceived stress,  non-obese students had almost two times likely to perceive stress. Similarly, body image was not significantly related to perceived stress in male and female students, however, it was likely that students with negative body image had about 1,5 times to feel stress than those with positive body image.Conclusion: Obesity was significantly associated with body image among male and female college students in Yogyakarta Province. However, no significant relationship was found between obesity and perceived stress as well as between body image and perceived stress among the students.

Abstract

Background: The prevalence of obesity is markedly increasing, both in developed and developing countries. Whether physical activity contributes to the obesity in Indonesian adolescents is still unknown.Objective: To assess whether physical activity is associated with obesity in junior high school adolescents in Yogyakarta. Methods: A case control study was conducted in Yogyakarta Province in 2003. Subjects were 140 obese and 140 non- obese junior high school adolescents in Yogyakarta and Bantul, randomly chosen from an obesity survey performed previously. Pattern and duration of activity were assessed using IPAQ modified questionnaire. The activities were than sorted into light (sedentary), moderate and vigorous activities.Results: There was significant difference in the distribution sedentary activity between adolescents of Yogyakarta and Bantul (p<0.0001). The mean duration of sedentary activity in Yogyakarta was 12.4 hours/day while in Bantul was 11.0 hours/day. Obese adolescents spent longer time in sedentary activity than non-obese adolescents (p=0.002). The odds of being obese in adolescent whose sedentary activity was longer than 13 hours/day were almost doubled.Conclusions: Urban adolescents spent more time for sedentary activities than rural adolescents, so were obese adolescents. The association of sedentary activities to obesity is independent from other factors such as calorie intake and parental obesity status.

Abstract

Background: Low birth weigth babies (LBW) are most likely a cause of the infant mortality and morbidity. Many factors causes LBW babies as maternal health, behavior during pregnancy, environmental  as well as fetal and placental factors. Negative behavior during pregnancy such as exposure of cigarette smoke can affect the body’s supply of oxygen from mother to fetus and placenta. Exposure to cigarette smoke may also reduce maternal folic acid level which causes disruption of fetal growth inside the womb.Objective: This study was aimed to fnd out the risk of low birth weight baby in the passive smoker pregnant woman.Method: The study used a case control design. The subject consisted of mothers who delivered at RSU dr. Zainoel Abidin in Banda Aceh municipality. The case group were mothers who delivered babies of less than 2,500 grams and the control group were those who delivered babies of 2,500 grams or more. Each group consisted of 105 mothers (1:1). Sample were taken using non probability with consecutive sampling method. Data analysis used univariable method with frequency distribution, bivariable with chi square and multivariable with logistic regression.Results: Passive smoker pregnant woman exposed the 1-10 cigarettes a day had risk 2,4 times greater of getting low birth weight baby (OR=2,42; 95%CI 1,24-4,71). Pregnant mother exposed to ≥11 cigarettes a day had the risk 3,1 times greates in having LBW infants than those not exposed (OR=3,1; 95%CI 1,50-6,70).Conclusion: Passive smoker of pregnant mother increased the risk of low birth weight babies.

Abstract

Background: Province of Yogyakarta Special Territory is a province of “elderly” structure population, as viewed from the number of elderly which is 15% of total population. Anemia is one of nutrition problems of elderly which reaches as high as 50%. This is caused by low intake of iron and other substances related to iron metabolism process in forming hemoglobin. Low hemoglobin concentration can be one of causes of elderly declining physical fitness. Efforts to overcome anemic problems can be done through the supply of iron and zinc for the elderly twice a week.Objective: To know effect of iron and zinc supply to hemoglobin concentration and physical fitness for anemic elderly at Bantul District, Province of Yogyakarta Special Territory.Methods: The study was a randomized double blind controlled trial design. Subject of the study were anemic elderly that fulfilled inclusion criteria. The supply of iron, combination of iron and zinc to trial group was given twice a week on different days, supply of iron was given on Monday and Thursday, while supply of zinc was given on Tuesday and Saturday. Laboratory and physical fitness examination to both group were done prior and after treatment. Nutrition intake data were achieved through food record. Next, to know effect of iron, iron and zinc supply to elderly hemoglobin concentration and physical fitness, covariant analysis was done.Results: The result of the study showed that after 3 months’ treatment, there was significant difference of hemoglobin concentration caused by different treatment of group with iron supply, iron and zinc combination supply and control group (p=0.012) although iron and combined iron and zinc supply had equal effectiveness in increasing hemoglobin concentration (p=0.404). There was no significant relationship between elderly hemoglobin concentration and physical fitness (p=0.448). This showed that treatment given affected increase of hemoglobin concentration but the supply and  increase of  hemoglobin concentration did not affect increase of elderly physical fitness.Conclusion: The supply of iron and combined iron and zinc increase hemoglobin concentration, but the supply and in- crease of hemoglobin concentration did not increase the elderly physical fitness.

Abstract

Background: Malnutrition is one of major public health problem in Indonesia. Prevalence of malnutrition in Indonesia in 2005 was 23.8% and in the Province of Yogyakarta Special Territory in 2002 was 15%. District of Sleman had prevalence of malnutrition as much as 11.39% and Cangkringan as one of heavy iodine deficiency disorder endemic areas at district level had 12.5% of malnutrition prevalence. Children at iodine deficiency disorder endemic areas might likely have growth and development disorder.Objective: To identify relationship between nutritional status and mental psychomotoric development of children of 12 to 24 months old at iodine deficiency disorder endemic areas.Method: The study was observational with cross sectional design carried out at Subdistrict of Cangkringan, District of Sleman. Subjects of the study were 75 children aged 12–24 months chosen by using consecutive sampling technique. Dependent variables of the study were mental and psychomotoric development, independent variable was nutritional status of children based on anthropometry with length/age indicator. Confounding variables were birth weight, mothers’ iodine status, level of education, job and family economic status. Other independent variable affecting nutritional status of children was mothers’ height. Data were analysed using chi square, Fisher’s exact test, and independent t-test.Results: Prevalence of low nutritional status was 13.33%. Children with normal nutritional status had average mental development index (MDI) 2.49 points and psychomotoric development index (PDI) 5.37 points higher than those with low nutritional status. However, relationship between nutritional status and mental and psychomotoric development was statistically insignificant.Conclusion: Relationship between nutritional status and mental and psychomotoric development of children of 12–24 months old was not statistically significant. Children with normal nutritional status tended to have higher MDI and PDI score than those with low nutritional status.
