Abstract

Background: The metabolic syndrome, which is characterized by hyperglycemia, obesity, dyslipidemia, hypertension, prothrombic and proinflamatory state, is a risk factor for cardiovascular disease and diabetes mellitus. The synbiotic yogurt of tanduk banana (Musa paradisiaca fa. corniculata) flour contains lactic acid bacteria, fructooligosaccarides, flavonoids, and vitamin C which all of them play role in improving the lipid profile.Objective: The aim of this study was to prove the effect of synbiotic yogurt of tanduk banana flour on lipid profile of metabolic syndrome rats.Method: This study was an true experimental with pre-post test  control group design. The subject of this study were 18 metabolic syndrome male Wistar rats divided into 3 groups,i.e control group (standart diet), intervention group I (standart diet and synbiotic yogurt of banana flour 0,009 ml/g weight/day), and intervention group II (standart diet and synbiotic yogurt of banana flour 0,018 ml/g weight/day). The intervention period was 2 weeks. Different test before and after intervention used paired t-test. The difference test between groups used One-Way ANOVA and Kruskal wallis.Results: The result showed that both of intervention group improved lipid profile significantly (p<0,05). The intervention group II showed more effective improvement of lipid profile significantly (p<0,05) than intervention group I. The intervention group II decreased triglycerides level 41,56%; total cholesterol level 41,39%; LDL-cholesterol level 57,5%; and increased HDL-cholesterol level 139,62%.Conclucion: The intervention of synbiotic yogurt of  tanduk banana flour can decrease triglyceride level, total cholesterol level, LDL-cholesterol level, and increase HDL-cholesterol level.

Abstract

Background: The production of tempeh milk is an effort to diversify tempeh-based food processing potential as functional food. However, as vegetable milk tempeh milk has the disadvantages of tasting bitter and being easily precipitate. The addition of gelatine is potential as stabilizer that is expected to counter the disadvantages and improve the taste of tempeh milk.Objective: To identify the effect of gelatine addition to stability and acceptability of tempeh milk among students of Nutrition Department of Health Polytechnic Yogyakarta.Method: The study was experimental with a completely randomized design, comprising four experiments, i.e. tempeh milk with gelatine addition as much as 2%, 3%, 4% and no gelatine addition as control. Organoleptic test was made by 25 skilled panelist, students of Nutrition Department of Health Polytechnic Yogyakarta, to identify acceptability of tempeh milk. Whereas viscocity test and visual observation through the assessment of tempeh milk precipitation level within 5 hours storage were made to identify emulsion stability. Data analysis used Anova and Duncan advanced test.Result: Stable emulsion could be achieved through gelatine addition of 2% at emulsion consistency 89.10%, viscocity 0.225 poise. Better emulsion stability was achieved through gelatine  addition of 3% at emulsion consistency 91.10%, viscocity 0.249 poise. Best emulsion stability was achieved through gelatine addition of 4% at emulsion consistency 95.58%, viscocity 0.254 poise and lowest emulsion stability was found in the control without gelatine addition at emulsion consistency 80.84%, viscocity 0.216 poise. The result of proximate nutrition value of tempeh milk with gelatine addition 4% (most stable) were energy 55.54 cal, protein 2.14gr, fat 1.82g, carbohydrate 7.65%, water 86.23g, ash 0.15% and raw fbre 2.01%.Conclusion: There was no effect of gelatine addition at various concentration to acceptability of tempeh milk. There was effect of gelatine addition at various concentration to stability of tempeh milk. The highest emulsion stability was achieved in tempeh milk with gelatine addition of 4%.

Abstract

Background: Breast cancer is the most common malignancy in women in the world. Vitamin D helps control the majority of gene expression in female reproductive tissues. Body mass index (BMI) influences the bioavailability of vitamin D. Diet is one of the main sources of vitamin D and it is directly converted into 25-hydroxyvitamin D [25 (OH) D] in the liver.Objective: This study aimed to analyze the relationship between BMI and vitamin D intake and 25 (OH)D serum in patients with breast cancer.Method: Analytic observational with cross sectional design was used in this study. A total of 37 breast cancer patient visited Dr. Moewardi Hospital in Surakarta was selected as research subjects using a purposive sampling technique. BMI data was obtained by measuring body weight and height. Vitamin D intake was determined using 1 x 24-hour food recall and semi-quantitative food frequency questionnaire. While, 25(OH)D serum was measured using enzyme-linked immunoabsorbent assay. Data analysis used Spearman correlation test.Results: The averages of BMI, vitamin D intake and 25(OH)D serum of breast cancer patients were 21.96±3.63 kg/m², 3.50±3.30 µg/day and 16.01±14.67 ng/mL respectively. Most breast cancer patients had less vitamin D intake and 25(OH)D serum deficiency. There was a weak relationship between BMI (r=0.188, p=0.266) and vitamin D (r=0.113, p=0.507) and 25 (OH)D serum in breast cancer patients.Conclusion: There were no significant correlations between BMI and vitamin D intake with 25(OH)D serum in breast cancer patients. Further study is required on the effect of chemotherapy regimens on vitamin D metabolism.

Abstract

Background: Diabetes Mellitus (DM) is a chronic disease with its increasing prevalence worldwide, including in Indonesia and in Bali province especially among adult group. The increasing prevalence of DM is followed by the increasing prevalence of obesity affected by changes in lifestyle and unhealthy dietary patterns.Objective: The study was conducted to investigate relations between dietary patterns and obesity with type 2 DM in Sanglah Hospital, Denpasar.Method: The study was observational analytic with matched case control study. Subject of the study was divided into two groups, i.e. case and control with comparison case and control 1:2 which matched to age and sex. The cases were type 2 DM patients who had recently diagnosed on first visited in Sanglah Hospital. Control 1 was outpatients of non-type 2 DM in Sanglah Hospital and control 2 were taken from the case-neighboring household. Total subject were 147, taken by consecutive method. Collected data were subject identity, dietary patterns, physical activity, weight, height, waist circumference and fasting blood sugar. Dietary patterns were collected using Food Frequency Questionnaire (FFQ), physical activity was collected using modified International Physical Activity Questionnaire (IPAQ), and fasting blood sugar were estimated using biosensor of glucose. Multiple logistic regression models were used to identify risk factors associated with type 2 DM.Result: This study showed that there were 5 variables significantly related with type 2 DM, i.e. interaction of overall obesity with the dietary fiber intake (OR:10.7; 95% CI: 3.5-33.7), family history (OR:5.5, 95% CI: 2.2-13.3), source of high fat animal protein (OR:4.9, 95% CI:1.5-16.1), source of low fat animal protein (OR:0.1, 95% CI: 0-0.5), hypertension (OR:3.7, 95% CI:1.4-9.9)Conclusion: The interaction between overall obesity with dietary fiber intake, family history, source of high fat animal protein, source of low fat animal protein and hypertension were the risk factors of type 2 DM occurrence in Sanglah Hospital, Denpasar.

Abstract

Background: Chronic kidney disease causes progressive deterioration of kidney function and is commonly irreversible, leading to nausea and vomiting as a consequence of high ureum and creatinine in the blood. This condition results in declining appetite which greatly affects food intake and degrading nutritional status that causes high mortality among chronic kidney patients. Therefore efforts are made on how to increase intake of chronic kidney patients.Objective: To identify the effect of nutrition therapy through commercial diet modification to nutrition intake and status of pre-hemodialysis chronic kidney patients at DR. M. Djamil Hospital, Padang.Method: The study was a quasi experiment which used pre-test and post-test with control group design. Population of the study were all patients of pre-hemodialysis chronic kidney disease hospitalized at DR. M. Djamil Hospital of Padang. Samples were patients of pre-hemodialysis chronic kidney disease at internal medicine ward. The experiment group consisted of 11 patients who got modified diet within 6 days and control group consisted of 11 patients who got hospital standard diet with α = 0.05 and ß = 10%. The study were collected from January-March 2008. Data of intake were analyzed using Nutri Survey program. Statistical t-test and chi-square were used to identify the effect of commercial modified diet to food intake, nutritional status, and increase of ureum and creatinine. Absorption level was measured from the quantity of commercial nutrition used. Perception of the patients about commercial diet was obtained through questionnaires and analyzed to get the score.Results: The group with modified diet had better energy and protein intake than the group with hospital standard diet (p < 0.05). However, there was no significant difference in nutritional status, ureum and creatinine changes between patients who got modified diet and those who got hospital standard diet (p < 0.05). The result of analysis on perception of patients about commercial diet showed that patients could accept commercial diet.Conclusion: Commercial modified diet could increase intake of energy and protein of pre-hemodialysis chronic kidney patients.

Abstract

Background: The differences in geographic, sosioeconomic and lifestyle between children in rural and urban areas infl uence their nutritional status. The urban children tended to be less active. The evidence suggested that physical activity improves cardiorespiratory fi tness and mental health in young people but study that compare those indicators among rural and urban children was scarce.Objective: To compare the nutritional status, physical fi tness, and quality of life between elementary school children in rural and urban areas.Method: A cross-sectional study was conducted at Semarang (urban) and Mlonggo (rural) Central Java in 2009. Inclusion criteria was school children aged 9-11 years and had no physical disability. Body composition was measured by Bioelectrical Impedance Analysis, physical activity by Global Physical Activity Questionnaire (GPAQ), physical fi tness by 20-m shuttle run test and quality of life by Pediatrics Quality of Life Questionnaire (PedQoL 4.0). The differences between nutritional status and physical activity were assessed by Chi Square test, while the differences between physical fi tness and quality of life were tested by independent t test (p<0.05).Result: One hundred sixty-two subjects were enrolled in the study (Semarang 82 students; 36 boys, 46 girls and Mlonggo 80 students; 40 boys, 40 girls). Body mass index (18.97 vs 15.16 kg/m2) and body fat (26.03% vs 18.03%) of urban children were higher than that of the rural’s (p<0.001). Children in rural area were signifi cantly more active, have higher physical fi tness level (VO2 max 28.54±1.79 ml/kg/min vs 21.57+1.79 ml/kg/min) (p<0.01), and have higher score in quality of life (2243 + 295,8) compared to the children from urban area (2133 + 369.4) (p<0.05). Conclusion: Rural school children have lower nutritional status than urban children, but they are more active and have higher physical fi tness level. They also have better quality of life.

Abstract

Background: The prevalence of diabetes mellitus (DM) is increasing globally, including in Indonesia. Studies by Diabetes Control and Complication Trial (DCCT) and U.K. Prospective Diabetes Study Group (UKPDS) prove the benefit of medical nutrition therapy (MNT) to glycemic control. Similar studies have not been done much in Indonesia.Objective: To identify the effects of MNT to nutrient intake, control of nutrition status, blood glucose and blood pressure of type 2 DM outpatients.Method: This was a randomized controlled trial. Subject of the study consisted of 60 people. The experiment group got MNT; whereas the control group got conventional nutrition counseling. Data were processed using computer, nutrients were analyzed using nutrisurvey. Statistical t-test was done to identify the effect of intervention.Result: Intake of energy, fat and carbohydrate of the experiment group decreased signifcantly close to the necessity at subsequent decrease as much as -14.05 (p= 0. 009), -35.64 (p=0.019) and -19.14 (p=0.035). Body mass index In the experiment group decreased -0.41kg/m  and waist circumference -0.24cm, where as the control group -0.23 kg/m  and -014cm. Blood glucose level during fasting decreased -23.6 mg/dl in the experiment group and -14.03 mg/dl in the control group (p=0.036). Systolic and dyastolic blood pressure of the experiment group decreased -3.33 mmHg (p=0.035) and 2 2 -1.67 mmgHg (p=0.895), whereas the control group + 0.5 mmHg  and -1.33 mmHg; the change was insignificant.Conclusion: Intake of nutrient, nutrition status, blood glucose level and blood pressure of type 2 DM patients that got MNT was more controllable than of those that got conventional nutrition counseling.

Abstract

Background: The decline in nutritional status of hospitalized patients was reported to be assossiated with longer length of stay and higher hospital charges. However, the effect of changes in nutritional status on hospital outcomes in Indonesia is still unknown.Objective: To determine the effect of changes in nutritional status on length of stay and hospital charge among adult hospitalized patients.Method: A total subjects of 262 adult patients who were admitted to internal and neurology departments of Dr. Sardjito, Dr.M.Jamil, and Sanglah hospitals were included in this study. Nutritional status of each patient was assessed using Subjective Global Assessment (SGA) method. Information on length of stay and hospital charge was collected based on medical records.Results: Subjects with nutritional status declined from normally to moderately, normally to severely, and moderately to severely malnourished were 6,3 (OR=6.32, 95% CI=1,3-29,8); 11,9 (OR=11.94, 95% CI=1,02-139,1); and 6,90 (OR=6.9, 95%CI=1,5-32,0 )times more likely to stay longer than those with nutritional status stayed normal during hospitalitation. They also had 3,3; unlimited; and 1,76 times risk on higher hospital charges than reference group (95% CI=1,123-9,529; unlimited; and 0,590-5,245).Conclusions: The declines of nutritional status from normally to moderately, normally to severely, and moderately to severely malnourished in hospitalized patients influenced to longer length of stay. Normally to moderately and normally to severely malnourished in hospitalized patients influenced to higher hospital charges.

Abstract

Background: Wasting is an acute malnutrition form that interrupts immune function, prolong duration of infection and in the end increase mortality risk of children. It is not clear yet which risk factor leading to wasting, simply because wasting-suspected risk factors were also involved in another kind of malnutrition such as stunting and underweight.Objective: To determine whether infant and young child feeding practices is risk factor for wasting in under two year children.Method: This research was mix method research which used matching case control design for quantitative research and focus group discussion to gather information in qualitative research. Case were wasting children age 3-23 months old (z-score WHZ <-2 SD) while control were non-wasting children age 3-23 months old (z-score WHZ ≥-2 SD). This research involved 106 under-two years old children and 14 nutritionist from 14 community health center which randomly chosen. Control group was matched by age and socio economic to case group.Results: Case group had more proportion of inappropriate feeding practices (26.41%% vs 20.75%). More than half respondent failed to meet Indonesian recommended dietary allowance for energy (57.55%). Inappropriate infant and young child feeding practice significantly did not increased risk of wasting in under two children at Yogyakarta (OR=1.4; 95% CI:0.62-3.36; p=0.523). Nutritionist from community health center focused in consultation and counseling in effort to increase infant and young child feeding practice.Conclusion: Feeding practices in under-two children in Yogyakarta was already good. Infant and young child feeding practice was not risk factor for wasting in under two years old children at Yogyakarta.

Abstract

Background: Coronary heart disease (CHD) is responsible for a substantial amount of early deaths, reduced quality of life and significant costs to the health and social care system. More than 3/4 CHD cases can prevented by lifestyle changes and focus on earlier risk factors management. Physical activity become a reference for the most important of primary and secondary prevention.Objective: To determine the relationship between physical activity and coronary heart disease in Indonesia.Method: This study used a cross-sectional design. Coronary heart disease’s history, physical activity, age, sex, economic status and consumption of fatty food were obtained from Basic Health Research (Riskesdas) 2013. Riskesdas 2013 used multistage cluster sampling. Subject in this study was 374.506 women and 347.823 men above 15 years old in Indonesia who answered the coronary heart disease questions which was selected purposively. Data was analyzed by using descriptive analysis, Chi-Square and multiple logistic regression.Results: Those who did not perform vigorous-intensity physical activity or who only did it less than 80 minutes per week had a higher prevalence of CHD than those who were more active  [2.63 (2.44-2.86); p=0.00].Conclusion: Physical activity had a significant association with CHD events in people above 15 years old in Indonesia.
