Abstract

The ideal route of administration for any medication is one that achieves serum concentration sufficient to produce the desired effect withoutproducing undesired effect. Patients may be concidered candidates for switching from intravenous to oral therapy once the patient has shown clinicalimprovement and is medically stable. Purpose of this study is the evaluation of selection antibiotics and timeliness to switch therapyfrom iv to oralantibiotics therapy.A prospective observation study, data were collected from all hospitalized patients with community-acquired pneumonia at internal medicinedepartment and respiratory care department at Dr. M. Djamil Public Hospital in Padang Indonesia from September until November 2013receivedantibioticswitchtherapy. Objective criteria were used to define time to switch therapy. Result showed, 49 patients fulfill inclusive criteria in thisresearch, 41 patients from internal medicine department and 8 patients from respiratory care department. All use ceftriaxone intravena, but 32 (65%)patients wrong in switched to oral therapy antibiotics, where 23 to cefixime, 7 to azithromycin, 1 to cefadroxyl and 1 to amoxcillin + clavulanic acid.Only 17 patients (35%) with prompt conversion of intravena antibiotic therapy to oral, 12 to ciprofloxacin and 5 to levofloxacin, only 12 patients rightin time to conversion intravena to oral antibiotics, with time to switch therapy was the fifth days. 49 patients have been discharged in goodconditionand doctor's approval.

Abstract

Acne is a common skin condition in the community. Inflammation of acne can be caused by the bacteria Propionibacterium acne, Staphylococcus epidermidis, Staphylococcus aureus. Cream is a form of acne preparations that circulate on the market, usually cream contains various chemicals, one of the side effects can irritate the skin, thus it is necessary to solution the use of materials from plants that are as antibacterial. One of the herbs that have been scientifically proven to contain antibacterial agents and that can be used to treat acne is Avocado leaves (Persea americana Mill.). This study aims to form the water extract of avocado leaf (Persea americana Mill.) To be formulated in a stable anti-acne cream preparation and can inhibit the bacteria Staphylococcus aureus. Formulation of avocado leaves water extract cream was made at concentrations of 12.5%, 15%, 17.5%, 20%. The results of this study indicate that the cream of avocado leaves water extract is good and physically stable for 4 weeks of storage, and is effective as an anti-acne. It is expected that this study will provide information that the natural ingredients of avocado leaves can inhibit the bacteria Staphylococcus aureus as the cause of acne bacteria.

Abstract

Sembung rambat (Mikania micrantha Kunth) is one of the plants that contain flavonoid compound. The flavonoid have the potential as a sunscreen, because it have chromophores group which can abssorption (UV A) dan (UV B) rays). The purpose of this research was to know the concentration of extract potentially as sunscreen and determine the value of SPF (Sun Protection Factor) , so it can be continued into natural sunscreen preparation. The sunscreen activity test method was performed in vitro using a UV-VIS spectrophotometer. Sunscreen activity test done by calculating the percent value of erytema transmission (% Te) and percent value of pigmentation transmission (% Tp) and SPF value. The results of the calculation of percentage value of erytema transmission (% Te) and sunscreen category at concentration 100, 150, 200 dan 250 ppm were 11,3024 % (standard suntan); 3,5238 % (extra protection); 1,3865 % (extra protection) dan 0,3838 % (sunblock). The calculation of percentage value of pigmentation transmission (% Tp) and sunscreen category at concentration 100, 150, 200 dan 250 ppm were 11,0254; 3,7011; 1,7447 and 0,5462 % all with the sunblock category. The SPF value at 250 ppm was 42,8810 with high protection category. The concentration was potentially developed into a natural sunscreen preparation that is at a concentration of 250 ppm.

Abstract

Periodontal prevalence in all age groups in Indonesia is 96.58%. The main cause ofperiodontal is microorganisms in dental plaque. Control of plaque reduction can be done using natural ingredients which contain antibacterial, one of which is areca nut seed (Areca catechu L.). This research aims to determine the type of bacteria on the gums, and to determine the antibacterial activity of ethanol extract of areca nut (Areca catechu L.) on gums' bacteria isolate. Antibacterial activity test used Agar diffusion method with concentration series 10%, 20%, 30%. The results of phytochemical screening of areca nut seed ethanol extract showed the presence of alkaloids, terpenoids and flavonoids. The results from the identification test based on macroscopic, microscopic, and biochemical observations obtained two Gram-positive bacteria (Staphylococcus epidermidis and Streptococcus sp) and one Gram-negative bacteria (Enterobacter aerogenes). Antibacterial activity test of ethanol extract of areca nut seed (Areca catechu L.) on gums' bacterial isolates was analyzed statistically with One Way Anovaand alternative test withKruskal-Wallis . Antibacterial activity of ethanol extract of areca nut seed (Areca catechu L.) on Staphylococcus epidermidis and Streptococcus spbacteria at concentrations of 10%, 20%, and 30% showed significant difference diameter of inhibitory power compared to positive and negative controls on Enterobacter aerogenes bacteria at concentration of 10 %, 20%, and 30% showed that the diameter of inhibitory power was significantly different compared to positive controls and not significantly different compared to negative controls.

Abstract

Abstrak Inhibitor tirosinase digunakan dalam kosmetik sebagai pencegah terjadinya hiperpigmentasi. Penelitian ini bertujuan untuk uji aktivitas inhibisi terhadap enzim tirosinase dari ekstrak etanol daun pepaya (Carica papaya L.) secara in vitro. Pemilihan inhibitor dari bahan alam memiliki tingkan keamanan yang lebih aman daripada sintesis. Ekstrak etanol daun pepaya diperkirakan memiliki aktivitas inhibitor tirosinase karena mengandung senyawa flavonoid. Daun pepaya diekstraksi secara maserasi dengan etanol 70%, hingga diperoleh ekstrak kental. Pengujian dilakukan dengan L-DOPA sebagai substrat, ekstrak etanol daun pepaya dengan konsentrasi 20000, 10000, 5000, 2500, 1250, 625 dan 312.5 µg/ml dengan kontrol positif asam kojat lalu diukur serapannya dengan menggunakan microplate reader pada panjang gelombang 490 nm. Hasil penelitian menunjukkan bahwa ekstrak etanol daun pepaya (Carica papaya L.) memiliki aktivitas inhibitor tirosinase dengan nilai IC50 sebesar 12158,8751 µg/ml dan potensi relatif sebesar 6,879 x 10-3 kali asam koja Abstract Tyrosinase inhibitors used in cosmetics as a deterrent occurrence of hyperpigmentation. This study aimed to test the activity of the enzyme tyrosinase inhibition of the ethanol extract of Carica papaya (Carica papaya L.) in vitro. Election inhibitors from natural materials have security safer than synthetic. The ethanol extract of papaya leaves is estimated to have activity of tyrosinase inhibitor compounds that contain flavonoids. Papaya leaves extracted by maceration in 70% ethanol, to obtain a thick extract. Testing is done with L-DOPA as a substrate, the ethanol extract of papaya leaf with a concentration of 20000, 10000, 5000, 2500, 1250, 625 and 312.5 ug / ml with the positive control kojic acid and absorbance was measured using a microplate reader at a wavelength of 490 nm. The results showed that the ethanol extract of papaya leaves leaves of papaya (Carica papaya L.) have activity of tyrosinase inhibitor with IC50 value of 12158.8751 ug / ml and the relative potency of 6.879 x 10-3 times kojic acid.

Abstract

Potentially Inappropriate Medications (PIMs) is the use of drugs associated with the risk of side effects where there are safer alternative to treat the same condition. The number of drug is one of the factors that can cause errors in the prescription of drugs and may result in PIMs events. This study aims to look at the number of drugs, the incidence of PIMs, the number of PIMs events and PIMs categories as well as see the relation of the amount of drugs used against PIMs. This is an observational analytic descriptive research with cross sectional approach for 287 prescriptions of geriatric hypertension patients using the purposive sampling technique in Sidomulyo health center in Pekanbaru. Based on the number of drug, there were 60 patients who got the number of drug ≥5 and 226 patients who got the numbert of drug <5 . The incidence of identified PIMs was 77 patients, the highest number of PIMs received was 1 PIMs in 75 patients and found the one category which is the most accepted category by the patients. The correlation analysis of Contigency Coefficient showed that the number of drugs gives a moderate correlation and significant (r = 0.414; p = 0,000) to the incidence of PIMs and the number of PIMs events. It can be concluded that the number of drugs provides a significant correlation in the incidence of PIMs in geriatric patients.

Abstract

Guava fruit (Psidium guajava. L.) is consist at phenolics content that have antioxidant activirt and the possibility of using as sunscreen. The purpose of this research was to determine the concentration of extracts that have a potential sunscreen profile by determining the value of erythema transmission, pigmentation transmission and sun protection factor (SPF). Sunscreen activity was carried out in vitro using UV-Vis Spectrophotometer. Ethanol extract, red guava and white guava fruit are made to concentration of 100; 150; 200; 250 ppm, each absorption was measured in the UV A and UV B areas. As result the extract of red guava at a concentration of 250 ppm had %Te value was 15.32% which was included in the category fast tanning, %Tp was 25.4% which was included in the sunblock category and the SPF value was 8,228 which was categorized as maximum protection. The extract of white guava at a concentration of 250 ppm had %Te value of 12.51% which was included in Suntan standard category, the % Tp value was 21.97% which belongs to the sunblock category and the SPF value was 8, 945 which was categorized as maximum protection. Abstract

Bauhinia semibifida Roxb. is a plant has potensial as antioxidant. The purpose of this research is to isolate alkaloids fraction from methanol extract and to examine the activity of antioxidant with DPPH method. The extraction was done by using maceration method with metanol solvent and an acid-base was performed on the extract with acid citric 2% and amonia, and fractionation with ethyl acetat and butanol solvents. The ethyl acetat fraction was separated by column chromtography. The isolates from column chromatography was labeled SA fraction, which a yellow colour oil. Based on phytochemicals data analysis, the spectroscopy of UV-Vis, FT-IR, and LC-MS, it can be concluded that the SA fraction is a compound has a NH functional group (alkaoids), and has very weak antioxidant activity with IC50 429,64 mg/mL.

Abstract

The antibacterial activity of the Manila Fruit Juice (Manilkara zapota (L.) van Royen) Raw and Ripe againt Escherichia coli bacteria has been done with a solid dilution method. The purpose of this research is to know and compare the antibacterial activity of fruit juice sawo manila raw and ripe to inhibit the growth of Escherichia coli bacteria. The results of the Antibacterial Activity of Manila (Manilkara Zapota (L.) van Royen) Raw with a concentration of 100%, 80%, 60%, 40% and 20%, respectively, of 120 colonies/ml colonies, 154 colonies/ml, 179.33 colonies/ml, 195.33 colonies/ml and 196.67 colonies/ml with an average number of early colonies of bacteria 197.67 colonies/ml. The results shown that the concentration of 100% will be decreasing of 39% the colony. While, the antibacterial activity test’s results of manila fruit juice (Manilkara zapota (L.) van Royen) ripe with 100%, 80%, 60%, 40% and 20%, respectively, with the average number of colony 221.00 colonies/ml, 221.67 colonies/ml, 221.67 colonies/ml, 221.67 colonies/ml and 221.67 colonies/ml with an average number of early colonies of bacterial 222.00 colonies/ml. This indicated that the raw manila has antibacterial activity, while the manila ripe fruit juice shown no antibacterial activity against bacteria of Escherichia coli.

Abstract

Telah dilakukan penelitian isolasi metabolit sekunder, uji aktivitas antibakteri dan antioksidan dari ekstrak metanol kulit batang meranti buaya (Shorea uliginosa Foxw). Ekstraksi dilakukan dengan metode maserasi menggunakan pelarut methanol. Isolasi metabolit sekunder dari ekstrak metanol dilakukan dengan menggunakan kromatografi kolom dengan metoda bergradien (Step Gradient Polarity). Hasil isolasi yang diperoleh berupa senyawa murni yang diberi label RS5 sebanyak 6 mg dengan titik leleh 240-242 ℃. Berdasarkan analisis spektroskopi UV, spektroskopi IR, dan pemeriksaan fitokimia dengan menggunakan pereaksi Liebermann-Bourchard menghasilkan warna merah bata dan disimpulkan senyawa RS5 adalah golongan Terpenoid. Hasil penelitian uji aktivitas antibakteri menunjukan bahwa ekstrak metanol kulit batang meranti buaya (Shorea uliginosa Foxw) memiliki daerah diameter hambat terhadap bakteri Staphylococcus aureus dan Escherichia coli pada kategori sedang hingga lemah, dan terhadap bakteri Bacillus cereus dan Salmonella typhi pada kategori kuat hingga lemah. Sedangkan hasil uji aktivitas antioksidan ekstrak metanol memiliki aktivitas antioksidan yang kuat dengan nilai IC50 40.1513 µg/ml dan senyawa RS5 memiliki aktivitas antioksidan sedang dengan nilai IC50 107.1968 µg/ml.
