Abstract

Lactate dehydrogenase (LDH) is an enzyme that catalyzes the conversion of pyruvate to lactate and on the contrary lactate to pyruvate. LDH is a tetramer consisting of two types of subunits M (LDHA) and H (LDHB). LDHA catalyzes the conversion of pyruvate to lactate, while LDHB converts lactate to pyruvate. LDHA increases its expression in cancer cells and is associated with cancer aggressiveness. Lactate that continues to increase gives the effect of an acidic microenvironment so that there is sufficient availability of oxygen for metabolism, which increases anaerobic glycolysis and decreases oxidative phosphorylation. Inhibition of LDHA can provide hope in cancer therapy because many studies have shown that inhibition of LDHA reduces cancer growth. This review will describe several cancer treatment approaches using LDH as a cancer therapy target until now.

Abstract

Background: Low back pain (LBP) is the most common musculoskeletal complaint and the largest contributor to the burden of disability in the world. One of the risk factors for LBP is having a higher body mass index (BMI). Although the relationship between BMI and LBP in the age group susceptible to LBP has been widely studied, the relationship in the young adult age group is still under-researched. Meanwhile, several studies report that lifestyle changes that occur during the COVID-19 pandemic might lead to an increasing number of overweight obese sufferers, including in the young adult age group. Given the context, this research aims to determine the relationship between BMI and LBP in UPH Medical Faculty (FK UPH) students during the COVID-19 pandemic.Method: A cross-sectional method of unpaired categorical comparative analytical study design was conducted with a population of 414 medical students. Data were collected by an online survey of BMI measurement and the Nordic Musculoskeletal Questionnaire. Statistical analysis was done using SPSS Statistics with Chi-squared or Fischer's exact method.Result: The study involved a sample of 372 people with 188 people (50.5%) being LBP sufferers. Most of the sample belongs to the category of BMI < 23 kg/m2, which is 215 people (57.8%).Conclusion: The result from this research concludes that there is no significant relationship between BMI and LBP in FK UPH students during the COVID-19 pandemic (P = 0.420); significant relationships were found between LBP with gender (P = <0.001) and smoking habit (P = 0.022).

Abstract

Background: Spinal cord intramedullary cavernous malformation (SICM) is kind of rare vascular disease, and the therapeutic strategy is still under debate. Cavernous malformation (CM) can be found throughout the central nervous system (CNS) but only rarely occur within the spinal cord. The purpose of this article is to describe natural history, clinical presentation and outcome of SICM case which treated surgicallyCase Presentation: A 70-year-old healthy woman presented with lower extremity weakness. A posterior laminectomy was performed, and a diagnosis of intramedullary spinal cord cavernous malformation was established. Neurological improvement was seen in one month after surgeryConclusion: Intramedullary CM is a rare disease but one with significant consequences if not managed appropriately. whenever safely feasible, gross total resection is suggested, to prevent rebleeding and further worsened of neurological deficit.

Abstract

Background: Non-Hodgkin large cell B-lymphoma (DLBCL: Diffuse Large B-cell Lymphoma) were classified into two subtypes of Germinal Center B-Cell Like (GCB) and non-Germinal Center B-Cell Like (non-GCB), which GCB has a better prognosis. The Ki-67 antigen is one of the most reliable markers of cell proliferation. This research aims to establish the relationship between proliferation index Ki-67 with DLBCL subtypes and to evaluate the utility of Ki-67 proliferative index as a predictive marker for predicting stages of lymphoma.Methods: We obtained 60 cases of patient samples DLBCL in Siloam Hospital Lippo Karawaci in 2014-2018. Clinical and pathological data were obtained from medical records. Chi-square methods were used to analyze data.Result: There were more ≤60 years compared to >60 years. In this study, there were more male patients than female patients. lymphoma provided in extranodal were higher than lymphoma in nodal form.Conclusions: There is a relationship between Ki-67 proliferation index with lymphoma GCB and non-GCB subtypes and stage of lymphoma, which can be used as a predictive factor in predicting the stage of lymphoma.

Abstract

Introduction: Dysmenorrhea is menstrual pain that often occurs in young women. The pain felt in every women can be different from one another. Anxiety experienced by each women can also worsen the menstrual pain. Efforts to treat dysmenorrhea can be in the form of non-pharmacological therapy, one of which is music therapy.Aim: This study uses an experimental method with random allocation to divide 122 respondents who are medical students of Pelita Harapan University into a control group and an intervention group.Methodology: The intervention group has been given anexperiment of random allocationin the form of classical music therapy for 20 minutes on the first day of the respondent getting menstruation. As for the control group given 40Hz sound for 20 minutes. The data analyzed using chi-square and wilcoxon test.Results: The results of the study found that the average menstrual pain in the control group before the intervention was 6.11 and after the intervention was 6.07. Meanwhile, in the intervention group, the average menstrual pain before being given classical music therapy was 6.46 and after the intervention was 3.39.Conclusion: So that classical music therapy is effective for reducing anxiety by 0.77 and the menstrual pain scale is 3.07 in the group given classical music therapy with P value <0.001.

Abstract

Background: Severe pneumonia dan anemia happened to many children under five years old. Anemia leads to hypercapnia and slowing down red blood cell maturation and facilitate ischaemic syndrome. In the other side, pneumonia may increase hepcidin that suppressed erythropoiesis, hence anemia could worsen pneumonia. The aim of this research is to find association of hemoglobin level and severe pneumonia under five years old.Methods: Cross sectional study was done in medical record RSUD Wangaya Denpasar starting from May-August 2022. Data was taken from subject with severe and mild and moderate pneumonia age 6-59 months old that admitted to PICU and Kaswari ward from January 2020-June 2022. Variables processed in this study are gender, pneumonia severity, hemoglobin level, and length of stayResult: This study admits 56 subjects that fulfilled inclusion and exclusion criteria. Boy and girls subjects are found equally (28 subjects equally in both groups). Severe pneumonia found in 18 (32.1%) subjects and 25 subjects (44.6%) has anemia. Length of stay for severe pneumonia is 4.83 ± 1.54 days. Hemoglobin level in severe pneumonia is 10.93±1,96 mg/dL and subjects with mild and moderate pneumonia is 11.69 ±1.41 mg/dL. The result of chi-square test between haemoglobin level and severe pneumonia is p= 0.26Conclusions: This study shows that there is no correlation between hemoglobin level and severe pneumonia. Further study is needed since the correlation between them is still controversial.

Abstract

Background: Peripheral nerve block for lower extremity surgeries requires a minimum of two block injections. Although the combination of the lumbar plexus block and the sciatica block is a procedure that requires large doses of local anesthetic, which may induce cardiotoxicity, a recent study suggests that change in the hemodynamic system is not clinically significant.Case Description: We report a case of a 53-year-old male presenting with a worsened shortness of breath two days before hospital admission. He also experienced a sudden high fever and pain on his right thigh. The patient had a history of Heart Failure (EF 28%) and Congestive liver disease and was diagnosed with right femur cellulitis with right phlegmon. Surgical debridement was planned to treat sepsis. Right lumbar plexus block and right sciatica block were performed as surgical anesthesia. The surgery duration was one hour, and the patient was transferred to the PACU. The patient was treated in ICU before being admitted to the general ward and was discharged on day three post-surgery.Conclusion: The combination of Sciatic nerve and lumbar plexus nerve block is an effective anesthesia and analgesia technique for lower limb surgery in individuals with a severe heart condition.

Abstract

Background: Medulloblastoma is the most common malignant brain tumor of childhood. Prognosis is mostly favorable, but may be affected by histological and molecular subtypes. Long-term therapy-related morbidity also remains a significant concern.Case Presentation: A 6-year-old boy with brainstem/midline cerebellum tumor. Histopathology found area of necrosis, sheets of malignant undifferentiated round cells with hyperchromatic nuclei and scanty cytoplasm, no nodule and no anaplasia. Immunohistochemistry found p53 wild-type staining pattern along with b-catenin diffuse cytoplasmic and focal nuclear staining. This indicated a diagnosis of WNT-activated medulloblastoma, World Health Organization (WHO) grade IV, with classic histological features.Discussion: WNT-activated medulloblastomas with classic histological features and no anaplasia were reported to have the most favorable prognosis. The current patient showed negative staining for GFAP, Olig2, EMA, H3K27M, EZHIP, and LIN28A, with retained staining for INI1 and BRG1, thus excluding several differential diagnosis such as atypical teratoid/rhabdoid tumor, embryonal tumor with multilayered rosettes, small cell glioblastoma, Ewing sarcoma, high-grade neuroepithelial tumor with BCOR alteration or diffuse midline glioma. Histopathology in combination with immunohistochemical and molecular subtyping of medulloblastoma can help to refine diagnosis, exclude differential diagnosis, and improve counseling in regards to overall prognosis.

Abstract

Introduction: Diabetic Ketoacidosis (DKA) is an acute complication of diabetes mellitus (DM) which present as the first manifestation in fulminant type 1 diabetes mellitus (FT1DM) without significant elevation of HbA1C. We present a unique case of a 52-years-old male with DKA without previous history of DM and HbA1c of 6.6.Case Illustration: A 52-years-old male was brought to hospital due to persistent vomiting. He experienced polyuria and unexplained weight loss but denied previous history of DM. He was diagnosed with DKA due to his blood glucose of 1107 mg/dL along with elevated blood ketone and metabolic acidosis. However, his HbA1c was a mere 6.6. His serum lipase was increased, consistent with pancreatic damage. He was then discharged with basal-bolus insulin.  Discussion: Although the symptoms of this patient was consistent with hyperglycemic crisis, the presentation of DKA is usually associated with elevated HbA1c, approximately 10.4 - 16.9%. This unusual HbA1c points to the diagnosis of FT1DM. It is caused by sudden beta-cell destruction triggered by viral infections, alongside genetic disposition, leading to sudden depletion of insulin occurred less than a week and presents with ketoacidosis. It is characterized by low HbA1c (< 8.7%) yet extremely high blood glucose (≥ 288 mg/dl) and elevated pancreatic enzymes (depicting pancreatic damage). A diagnosis of FT1DM can be ascertained in this patient as his presentations matches its characteristics.Conclusion: FT1DM is a sub-type of T1DM which can suddenly occur in patients and associated with discordant HbA1c. It is important to recognize and treat it accordingly to avoid fatal outcomes.

Abstract

Background: Abducens nerve is the second longest intracranial path out of all of the cranial nerves. Abducens nerve pasly had a prevalence 11.3/100.000 and mostly seen in adults. Isolated abducens nerve palsy due to neoplastic lesion are rare and mostly related to skull base tumor.Case Presentation: We present a case of 57 years old woman with complaints of red right eye and worsening headache associates with diplopia on the right lateral gaze for three days prior admission. She was later diagnosed with isolated abducens nerve palsy associated to neoplasm cause in Dorello’s canal. Patient was then discharged with symptomatic therapy and educated for head posturing and avoid triggers.Conclusions: Abducens nerve palsy is the most common isolated ocular nerve palsy. The incidence of mass formation such as neoplastic lesion or cyst are rare.
