Abstract

Background: Health service quality reflects how well services meet patient satisfaction as it affects patient trust and adherence while also demonstrating the accountability of primary health care facilities. This study aims to examine the impact of service quality on outpatient satisfaction at UPT Puskesmas Sangatta Utara. Methods: This study utilized an analytical survey with a cross-sectional design. The population included community members seeking care at the North Sangatta Health Centre. A total of 271 respondents were selected based on the Lemeshow formula, using purposive sampling as the sampling method. Data were collected through a structured questionnaire that included demographic characteristics and research variables. Data analysis was conducted using a combination of univariate, Chi-square, and multiple logistic regression methods to explore the relationships and effects among the variables.Result: The results showed that variables significantly associated with outpatient satisfaction were technical competence, facility accessibility, service effectiveness, continuity, safety, comfort, information, timeliness, and interpersonal relations (p < 0.05). Service efficiency was not significantly associated with patient satisfaction (p > 0.05). The most influential factor was interpersonal relations, with the highest odds ratio of 2.294 (95% CI: 1.381–3.809).Conclusion : This study demonstrated that outpatient satisfaction at the North Sangatta Health Centre was influenced by multiple service aspects, with interpersonal relations emerging as the most dominant factor. These findings emphasized the importance of improving healthcare workers’ communication and interaction with patients to enhance overall satisfaction.

Abstract

Background: The Gunungkidul District in Indonesia achieved malaria elimination status in 2014; however, the risk of reintroduction remains due to environmental suitability and population mobility. This study aimed to identify malaria-receptive areas in Gunungkidul District through spatial and environmental analyses to support the strategic maintenance of malaria elimination. Methods: A descriptive observational study was conducted using secondary data from a vector survey conducted in October 2023 across 18 sub-districts. Geographic Information System (GIS) mapping and larval habitat index (LHI) calculations were used to identify potential Anopheles breeding sites. Climate data from 2020 to 2023 were analyzed to assess environmental receptivity.Result: Rivers were the most common breeding sites for Anopheles mosquitoes (59.41%), followed by lakes and springs. Gedangsari and Karangmojo sub-districts showed the highest larval habitat indices, indicating localized malaria receptivity in these areas. Climatic factors, such as high rainfall and humidity during the wet season, support year-round mosquito breeding. In 2023, four imported malaria cases were reported, underscoring the district's continued vulnerability. Conclusion : Spatial identification of malaria-receptive areas provides operational evidence to support the maintenance of the elimination status in the Gunungkidul District. Targeted surveillance and vector control in high-risk sub-districts, along with a rapid response to imported cases, are essential to prevent malaria re-establishment.

Abstract

Background: Hypertension, commonly referred to as "the silent killer," has become one of the degenerative diseases that still show high case numbers in society. Coastal communities are one of the groups that can experience hypertension due to diet, limited access to healthcare services, and other factors. This study was conducted to analyze non-modifiable risk factors (age, gender, family history) and modifiable risk factors (diet, physical activity, occupation, education, smoking behavior, access to healthcare services) associated with the incidence of hypertension. Methods: The research uses an analytical and cross-sectional with a sample size of 127 people through simple random sampling using the Chi-square test with a 95% confidence level (α = 0.05). The variables used in this study are age, gender, family history, education, occupation, economic status, diet assessed using the Food Frequency Questionnaire (FFQ), physical activity evaluated using the Global Physical Activity Questionnaire (IPAQ), smoking behavior, and access to health services.Result: Based on the risk factors studied, there was a significant association between diet (p = 0.001 & PR 1.534), education (p = 0.000 & PR 1.766), and age (p = 0.038 & PR 1.301) with the incidence of hypertension. However, other variables such as gender, family history, economic status, access to health facilities, physical activity, and occupation did not have a significant relationship with hypertension. Conclusion : The research shows a significant relationship between dietary patterns, education, and age with the incidence of hypertension in coastal communities.

Abstract

ABSTRAKBackground: Stroke remains a leading cause of disability among older adults, often resulting in impaired motor abilities and a diminished quality of life. This study aimed to evaluate the impact of combining Creative Movement Therapy (CMT) and Occupational Therapy (OT) on motor function and quality of life among post-stroke elderly individuals in Gresik District, Indonesia. Methods: A quasi-experimental study with a pretest–posttest control group design was conducted from August to September 2025 at Puskesmas Alun-Alun Gresik. The control group (n = 35) received conventional rehabilitation, while the intervention group (n = 35) participated in combined CMT and OT sessions for four weeks, lasting 45–60 minutes each. Motor function was assessed using the Fugl-Meyer Assessment (FMA), and quality of life was measured using the Stroke-Specific Quality of Life (SS-QOL) scale. Results: Participants in the intervention group achieved significantly higher mean ranks for both FMA (46.00) and SS-QOL (43.61) compared to the control group (25.00 and 27.39, respectively; p < 0.05).Conclusion: The integration of Creative Movement Therapy and Occupational Therapy provides an effective, community-based rehabilitation strategy that holistically enhances motor function and overall quality of life among post-stroke elderly individuals

Abstract

Background: Stroke survivors in coastal areas face major challenges, such as limited access to health services, minimal post-stroke education, and inadequate monitoring. Without a targeted tertiary prevention strategy, the risk of recurrence and disability remains high. This study aimed to develop a stroke management model to enhance self-control and support community-based health policy planning. Methods: A cross-sectional study was conducted among 45 stroke survivors in the Puger Subdistrict, Jember Regency, using a total sampling approach. Data were collected through questionnaires and measurements of blood pressure, cholesterol, and random blood glucose using touch-based devices and digital sphygmomanometers. Path analysis was performed using Partial Least Squares structural equation modeling. Results: Clinical needs, such as elevated blood pressure and cholesterol levels, encouraged survivors to be more active in managing their conditions and increased their interactions with health services. However, paradoxically, poor self-management, such as infrequent blood pressure monitoring and unhealthy diets, places a greater burden on the health system. This indicates that an increased system response alone is insufficient without strengthening the individual roles. Family history and blood glucose levels also contribute to indirect pathways. Conclusion: Preventing stroke recurrence requires a comprehensive epidemiological management approach, including clinical control, dietary education, and sustainable, community-based interventions. This model can serve as a basis for designing health programs and policies that are more responsive to the needs of coastal communities in the future.

Abstract

Background : Exposure to household chemicals, especially those containing endocrine-disrupting compounds, can cause serious damage to reproductive health. These findings highlight the importance of developing and implementing preventive environmental health strategies. Policies should be designed to integrate ecological-reproductive risk management, particularly targeting rural communities that experience high exposure to household and agricultural chemicals. Methods : This cross-sectional epidemiological study involved 48 couples of reproductive age, selected using cluster-stratified sampling in rural agricultural areas. Data collection included structured interviews and household observations to assess exposure to common household chemicals such as floor cleaners, aerosol air fresheners, and pesticide residues.  Statistical analysis involved univariate and bivariate tests (Chi-square), as well as multivariate logistic regression, to determine independent relationships.Result: The prevalence of primary infertility was 35.4%. Bivariate analysis revealed significant associations between infertility and exposure to floor cleaners (p = 0.021), air fresheners (p = 0.034), and indoor pesticide residues (p = 0.015). Multivariate analysis confirmed indoor pesticide exposure (aOR = 3.92; 95% CI : 1.12–13.68) and air freshener use (aOR = 2.74; 95% CI : 1.01–9.15) as independent predictors of infertility. Conclusion: Exposure to household chemicals can cause serious damage to reproductive health. This risk increases when such exposure is combined with agricultural chemicals. These findings highlight the importance of developing and implementing preventive environmental health strategies. Policies should be designed to integrate ecological-reproductive risk management, particularly targeting rural communities that experience high exposure to household and agricultural chemicals.

Abstract

Background: The city of Bogor has recorded a steady increase in the number of TB cases over the past three years, reaching 6,142 cases in 2023 with the highest treatment-related mortality rate in West Java (270 deaths). This study aims to analyze the relationship between demographic and environmental factors and TB incidence, as well as analyze the spatial distribution of use in the city of Bogor in 2023.Methods: The study employed an ecological design based on administrative areas, with six districts as units of analysis: Bogor Barat, Bogor Timur, Bogor Tengah, Bogor Selatan, Bogor Utara, and Tanah Sareal. Secondary data were obtained from the Bogor City Health Office (TB case data by age and sex, 2023), Statistics Indonesia (BPS) of Bogor City (population density and housing density, 2023), and the Bogor Class II Meteorological Station (BMKG) (average air temperature and humidity, 2023). Spatial analysis was conducted using QGIS and bivariate analysis using Pearson correlation.Result: A total of 6,142 TB cases were recorded in 2023. The highest number was in Bogor Barat (1,419 cases), followed by Bogor Selatan (1,320) and Tanah Sareal (1,147), while the lowest was in Bogor Timur (539 cases). Spatial mapping showed higher case concentrations in the western and southern parts of the city. The sex variable (male r = 0.880, female r = 0.901) showed a strong correlation with TB cases. Conclusion : This study recommends targeted TB interventions in high-burden areas and consideration of demographic aspects as risk indicators.

Abstract

Background: Mental disorders affect approximately 20% of Indonesia’s population, with East Kalimantan reporting 2,679 cases, including 130 in the Sangatta Utara area. Despite the growing awareness of the critical role families play in supporting individuals with mental disorders, there is a still lack of studies that specifically explore family responses, particularly in Indonesia. This study aims to fill this gap by examining the factors that are associated with family responses to individuals living with severe mental disorders in Sangatta Utara, East Kalimantan. Methods: This observational and cross-sectional study targeted 130 families of individuals diagnosed with severe mental disorders. Data were collected using structured questionnaires. Statistical analysis was performed using chi-square tests and binary logistic regression. This statistical model aimed to identify the factors most influential on family responses. Results: Significant associations were found between family burden, social support, and family responses to individuals with severe mental disorders. The analysis revealed that family burden was strongly associated with positive family responses (Exp(B) = 24.22, p = 0.002), indicating that higher caregiving burden significantly increased the likelihood of positive family responses. Similarly, social support showed a significant positive association with family responses (Exp(B) = 5.19, p = 0.040).Conclusion: Family burden and social support are important factors that influence family responses to severe mental disorders. This study emphasizes and suggests the importance of targeted family counseling and community-based support programs in North Sangatta.

Abstract

Background : Katingan Regency has experienced an increase in dengue fever cases in Central Kalimantan Province over the past three years, from 2020 to 2023, with an incidence rate in 2023 of 168.4 per 100,000 population and a Case Fatality Rate (CFR) of 0.66%. This is due to the Maya Index, which is a potential breeding ground for Aedes sp mosquitoes, remaining a problem. Therefore, research will be conducted to detect the Maya Index of Aedes sp mosquitoes in dengue fever endemic areas in Katingan Regency. Methods : This study was conducted using an observational research method with a cross-sectional approach in April 2024. The population in this study consisted of all cases of dengue fever in January–February 2024, with a sample size of 113 samples. The sampling technique used was random sampling. Data were obtained through direct observation and observation of the home environment using the visual larval method.Result : The Breeding Risk Index (BRI) was in the moderate category, with 52 (46%) homes, while the Hygiene Risk Index (HRI) was also in the moderate category, with 55 (48.7%) homes. in dengue endemic areas, it is in the moderate category (BRI 2/HRI 2), which is 27 (23.9%) houses. Most houses in dengue endemic areas in Katingan Regency are in the high Maya Index status, which is 51 (45.1%) houses. Conclusion : Houses with low to moderate BRI will have less potential for having positive larval containers than houses with high BRI. Houses with low to moderate HRI have higher positive larval containers than houses with high HRI.
