Abstract

Objective: to determine the patient’s characteristics, correlation between staging and chemotherapy response, furthermore two years overall survival rate. Methods: a retrospective analytic study was conducted. Data were obtained from medical records in January 2008 to December 2014 and analyzed using SPSS 17.0. All of correlation between staging and chemotherapy response, primary tumor staging (pT) and metastasis (M), regional lymph nodes staging (N) and metastasis (M), serum tumor marker and chemotherapy response were tested by Spearman correlation test. Two years overall survival rate was analyzed by Kaplan-Meier. Data ratio with normal distribution was tested by Paired T-test. Results: the mean age of patients were 31.03 ± 13.751 years, with seven patients (15.6%) had previous history of undescended testis. Based on TNM staging, we found that most patients had already develop into stage pT3 (46.7%), N3 (57.7%), and M0 (57.7%). A significant correlation between staging and chemotherapy response was shown with stage I of testicular seminoma had completely chemotherapy response (100%) and two years overall survival rate in stage I was 100%, whereas in metastatic seminoma (stage II and III) was 60%, with Hazard Ratio 0.63 (p=0.294; 95%CI 0.276-1.476). Conclusion: patients in early stage of testicular seminoma will give a better response to chemotherapy and will have better survival rate compared to those with metastatic seminoma.

Abstract

Tujuan: untuk mengetahui faktor-faktor yang berhubungan dengan kejadian hipertensi vena perifer paska pembuatan akses fistula arteriovena untuk hemodialysis. Metode: penelitian ini menggunakan rancangan cross sectional. Subyek penelitian adalah pasien dengan Chronic Kidney Disease (CKD) Stadium V dengan akses fistula arteriovena yang menjalani hemodialisis di RSUP Sanglah selama periode Desember 2016-Desember 2017 sebanyak 141 sampel. Hasil: dilakukan uji multivariat dengan analisa regresi logistik untuk masing-masing faktor, dengan mengendalikan faktor perancu, yaitu usia, jenis kelamin, etiologi CKD, lama hemodialisis, dan status gizi pasien. Nilai rasio prevalens untuk lokasi fistula arteriovena brakiosefalika sebesar 15,2 dengan nilai p<0,001. Nilai rasio prevalens untuk riwayat pemakaian kateter double lumen ipsilateral sebesar 20,9 dengan nilai p<0,001. Simpulan: dari hasil analisis data secara statistika dapat disimpulkan lokasi fistula arteriovena brakiosefalika dan riwayat penggunaan kateter double lumen secara statistika bermakna terhadap kejadian hipertensi vena perifer pada pasien CKD stage V yang menjalani hemodialisis regular di RSUP Sanglah Periode Desember 2016-Desember 2017.

Abstract

Tujuan: untuk mengetahui perbedaan karakteristik kanker payudara stadium dini dengan dan tanpa metastasis kelenjar getah bening aksila di RSUP Sanglah. Metode: penelitian ini adalah penelitian deksriptif pada pasien kanker payudara stadium dini dengan dan tanpa mengalami metastasis kelenjar getah bening aksila di RSUP Sanglah. Data dicatat dari rekam medis mengenai, usia, status menopause, letak kanker payudara, ukuran tumor, varian histopatologi, subtipe, dan status hormonal. Data disajikan dalam bentuk tabel dan dianalisis bivariat dengan uji Chi-Square atau t-independent dengan interval kepercayaan (CI) 95% dan p<0,05. Hasil: penelitian ini dilakukan pada masing-masing 22 pasien kanker payudara stadium dini dengan dan tanpa metastasis ke kelenjar getah bening aksila di RSUP Sanglah. Ada perbedaan yang bermakna usia (45 tahun dan 53 tahun; p=0,012) dan status menopause (68,2% premenopause dan 31,8% postmenopause; p=0,016) pada subjek kanker payudara stadium dini dengan dan tanpa metastasis KGB aksila. Tidak ditemukan perbedaan bermakna mengenai letak kanker payudara, ukuran tumor, varian histopatologi, subtipe, dan status hormonal. Simpulan: usia dan status menopause berbeda secara bermakna pada subjek kanker payudara stadium dini dengan metastasis kelenjar getah bening aksila di RSUP Sanglah.

Abstract

Background: pituitary tumor accounted for 15% of benign intracranial tumors. Clinical symptoms found in the form of visual disturbances, and hormonal manifestations, surgery is one of the main managements. Endoscopic endonasal transsphenoidal surgery (EETS) is a minimally invasive approach for this case. The advantages of EETS is better efficacy than traditional technique, and shorter hospital stays, less mean blood loss, and earlier return to daily activities. Case: we reported a rare example of a male patient aged 37 years old with the main clinical features were visual disturbances, sexual malfunction and bitemporal hemianopsia on physical examination. Magnetic resonance imaging (MRI) showed macroadenoma pituitary, the size 2.41 x 2.98 x 3.20 cm. We explained about minimally invasive surgery such as EETS and the patient was agreed to performed EETS. Conclusion: endoscopic endonasal transsphenoidal surgery is a minimally invasive technique for tumor pituitary cases and safe when compared to traditional medicine.

Abstract

Background: Bochdalek hernia is a congenital defect on posterolateral diaphragm with an abnormal connection between the thoracic cavity and the abdominal cavity. This disease causes protrusion of abdominal organs into the thoracic cavity. Case: an 8-day-old baby girl admitted to hospital with shortness of breath 24 hours after delivered. The baby was born spontaneously assisted by midwife. Upon born, the baby was crying strongly and meconium came out 2 hours after birth. On physical examination, the abdomen was inspected flat. Darm contour and darm steifung was observed, and peristaltic sound was heard on left lung. Radiological examination demonstrated a diaphragmatic hernia with ileus obstruction. The patient underwent laparatomy and stomach, ileum, transverse colon, and spleen, was found on foramen Bochdalek. Post-surgery chest X Ray showed favourable result. Ten days after treatment, the patient was discharged in a good condition with no respiratory or digestive problems. After 1 months the patient’s condition remained good and there were no respiratory or digestive complaints. Conclusion: In a rare case like Bochadalek hernia, laparotomy performed as a promising attempt to return the anatomic position of organ.

Abstract

Background: Extranodal lymphoma appears in about 40% of all lymphoma patients and has been reported in almost all tissue and organs. The most common site is the intestinal tract, followed by head and neck. There are many reports about extranodal lymphomas that arise in orbits, paranasal sinus and oral cavities, nasal, thyroid and salivary glands. These lymphomas usually do not destroy adjacent bone, unlike squamous cell carcinoma. Case: We would like to present an uncommon case of diffuse large B-cell extranodal lymphoma arising maxillofacial bone on buccal area that infiltrating to maxillary sinus and orbital, with bone destruction as well. The patient responded well with CEOP regimen chemotherapy and showed significant tumor regression with symptoms reduction. She just underwent the seventh series of chemotherapy now and planned to be restaging after eighth chemotherapy. Conclusion: Chemotherapy remains the treatment of choice for non-Hodgkin lymphomas. Surgery also takes place in modality choice of treatment of maxillary cancer, either a partial or total maxillectomy alone, or maxillectomy with craniofacial resection.

Abstract

Background: Benign breast disease have been broadly classified into non-proliferative lesions, proliferative lesions without atypia and hyperplasia with atypia. Proliferative disease, such as usual ductal hyperplasia, is associated with a 1.5 to 2 fold increased risk of developing invasive carcinoma. We reported a case of usual ductal hyperplasia in a young male. Case: A 22-year-old male complained of discomfort and enlargement of unilateral breast. Physical examination at that time revealed a palpable mass in the lateral upper quadrant of the patient’s left breast, three centimeters from nipple areola complex. The examination of axilla didn’t reveal any lymph node enlargement on both sides. His vital signs were normal without any abnormalities found on examination. Ultrasonography examination demonstrated fibroglandular tissue in the left breast with the volume of 11.13 cm3, consist of 4.8 cm length, 2.9 cm width and 0.8 cm depth. Excisional tumor biopsy was done on his left breast. Usual duct cell hyperplasia was present in microscopic examination with chronic inflammatory cells spreading around the fibrotic stromal cell. Physical examination, radiologic examination, and biopsy were all performed in this patient. Although the accuracy of the triple test is high, benign concordant results do not obviate further surveillance of a palpable mass. We advised our patient to routinely follow-up his condition every 6 months for 1 to 2 years, especially if there any changes found on his breasts. Conclusion: Any guidelines and further studies regarding patient’s follow-up examination after biopsy for male breasts tumor are needed in order of better understanding about this disease.

Abstract

Aim: This study aims to study the profile of patients with gastric perforation caused by peptic ulcer. Method: This is a retrospective study by obtaining patients data from medical records in the Medical Record Centre of Dr. Soetomo General Hospital during the period of January - December 2016 and analyzed descriptively. Result: 66 patients were identified (45 male and 21 female), most of them were between age 60 - 69 years old, and live in Surabaya (45.45%). 30.3% of patients No malignancy was found during the anatomic-pathological examination. 30.3% of patients have a long term NSAID taking, while 53.03% of patients came with a habit of traditional medicine consumption. Perforations were mostly located 1 - 5 cm pre-pyloric and between the range 0.5 - 1 cm in diameter (56.6% and 69.7%, respectively). Conclusion: With the lack of information, it still needs further studies with more completed data for better accuracy.

Abstract

Aim: To know the patterns of fracture site and management of maxillofacial cases in the Department of Trauma and Acute Care Surgery in Sanglah General Hospital Denpasar Bali. Methods: this is a retrospective study, based on medical record were concluded, samples taken in Sanglah General Hospital from January 2012 to December 2018. All of maxillofacial trauma medical records were taken. The data of age, gender, patterns of fractures site and management were taken and described. Results: There were total of 257 cases of maxillofacial trauma managed in the Department of Trauma and Acute Care Surgery in Sanglah General Hospital. Two-hundred and forty-one medical records of maxillofacial trauma were included in this study. About 16 medical records were excluded due to incomplete medical records and could not be contacted. Mostly cases found in male, aged 18-40 years old. The site of fractures majorly located in the mandible (60.12%). About 48% fractures were identified at symphysis or parasymphysis of mandible, followed by the body and angular of mandible. Open reduction and internal fixation (ORIF) were the gold standard of the treatment (83,73%) followed by Archbarr (16,27%). Conclusion: The most common site of maxillofacial fracture was mandible, specifically at symphysis or parasymphysis part. ORIF miniplate, together with Archbarr and interdental wiring fixation were the most common modality of management.

Abstract

Aim: Objective of the study is to find out the demographic and clinical profile of tuberculous mastitis (TM) patients. Methods: The study was carried out for a period of one year from August 2017 to August 2018 at Tebet General Hospital. Histopathologically diagnosed cases of TM were retrospectively studied. Results: All eight patients diagnosed as suffering from TM were females with the mean age of 37.25 years. Of these, two patients were lactating, six others were not, and 75% were multiparous. Unilateral involvement was observed in all cases. The right breast was affected in 62.5% and left breast in 37.5% cases. Patients presented with a solitary lump on one breast (87.5%), swelling of the breast (37.5%), and breast pain (100%). In the present study, two of the patients had previous positive tuberculosis (TB) contact and more than half found to be below an average economic level. All the patients were negative on the HIV test; also, there was no focus of TB elsewhere in the body. Conclusion: Tuberculosis should be considered as one possible cause of either mastitis or breast abscess. Histopathology examination played an important role in the diagnosis of TM. Standardized sequence for the diagnosis of TM is needed to increase awareness of the disease.
