Abstract

Introduction: During the COVID-19 pandemic, oral surgery services were prioritized for emergency cases such as malignancy, head and neck infection, and open or comminuted fracture. COVID-19 that spread by droplets need specific handling considering the work area of an oral surgeon is around the oral cavity and often has close contact with patients. Review: It is important to understand more about COVID-19. In emergency cases that need to be treated immediately, the management to prevent transmission by doing pre-operative, operative, and postoperative procedures will help oral surgery services can be done without the risk of transmission between dentist, patient, and paramedics. Conclusion: Management of emergency cases in oral surgery during pandemic COVID-19 can still be done, if everyone performs proper preventive procedures, not only the dentist but also the patient and paramedics. Conducting telescreens before the visit, early assessment of the patient, preoperative and operative procedures, management of medical waste, and also disinfection of the working area will help to avoid the transmission of COVID-19.

Abstract

Introduction: Regenerative therapies for periodontium aims to regenerate damaged periodontal tissue. Stem cell-based regenerative therapy may represent an effective therapeutic tool for periodontal regeneration. Periodontal ligament stem cells (PDLSCs) is a type of somatic stem cell that shows the potential to differentiate into several cell types and perform strong self-renewal. Therefore, PDLSCs are considered as a highly promising stem cell population for regenerative therapy in periodontium and need further knowledge regarding to the potential of using PDLSCs as a regenerative periodontium therapy. Literature Review: The periodontal ligament is a marker for stem cell, which is STRO-1. The STRO-1 marker is a colony-forming osteogenic precursor that can be isolated from bone marrow. The STRO-1 marker has the potential to be a phenotype of adipocytes, chondrocytes, and osteocytes. Markers of expression of STRO-1 from the surface of the periodontal ligament show that periodontal ligament cells have osteogenic precursors that can differentiate into osteoblasts. The potential multilineage differentiation of PDLSC including the adipogenic, chondrogenic, and osteogenic potential of PDLSC makes a promising cell source for cell-based regenerative periodontal therapy. Conclusion: The ability of PDLSCs in self renewal, multipotency, and stem cell immunomodulators, provides satisfactory results in the treatment of periodontal defects.

Abstract

Introduction: Systemic diseases such as leukemia can manifest on the periodontium. The symptoms of this disease can be varied such as gingival enlargement, bleeding, oral ulceration, petechiae, mucosal pallor, noma, and trismus, but most clinicians are unaware of these conditions. This literature study purposed to review the literature concerning the oral manifestations and periodontal managements consideration of patients with leukemia. Review: The most frequent oral manifestations associated with leukemia are represented by the paleness of oral mucosa or abnormal color of the gum, petechiae, ecchymosis, bleeding associated with painless gingival hyperplasia, hemorrhage, ulcerative necrotic lesions, and buccal infections. Clinicians should take precautions when encountering those symptoms, such as referring the patient to specialized professionals. Besides that, clinicians should have a certain protocol to maintain the oral health of patients and to prevent complications that may occur from dental procedures. Patients with leukemia have a bleeding tendency and increased risk of infection which required special attention on periodontal and dental management to prevent the development of opportunistic infections and other complications. However, many patients are unaware of their disease and clinicians hold a pivotal role to identify leukemia through oral manifestations and giving feasible dental management. Conclusion: This review describes the oral manifestation of leukemia, which might be useful in early diagnosis. By knowing the diagnosis, clinicians should concern about special considerations in providing dental treatment to patients with leukemia.

Abstract

Introduction: Patients with fixed orthodontic devices are at risk of oral hygiene problems. Dental students are considered to have adequate knowledge and should be able to use the right and proper way to maintain their oral hygiene when having fixed orthodontic appliances. The purpose of this study was to describe the methods used by students using fixed orthodontics at the Faculty of Dentistry, Baiturrahmah University Class of 2017-2019 in maintaining their oral hygiene. Methods: This is a descriptive study using a cross-sectional approach, the research sample was 53 students from the 2017-2019 class who used fixed orthodontic appliances by using a total sampling technique. Result: Almost all respondents declared that the use of fixed orthodontic appliances made food waste easier to adhere to (43.4% always experience it and 56.6% occasionally experience it). Almost all respondents had trouble cleaning their orthodontic appliance, (20.8% always experience it and 60.4% occasionally experience it). Most toothbrush they use for daily is conventional toothbrush (52.8 %). Other oral hygiene measures used daily by patients were dental floss (11.3%), interdental brush (20.8%), and mouthwash (13.2%). Most of the respondents clean their teeth after eat (60.4%). Few respondents use toothpicks where 9.4% always use them and 47.2% sometimes use them. Conclusion: This study concludes that the number of routine use of special orthodontic toothbrushes, dental floss, interdental brushes, and mouthwash is still low, and fixed orthodontic users who use toothpicks are still found.

Abstract

Introduction: Chlorhexidine in mouthwash or gel is used as an adjunct therapy for periodontal treatment but the effect of using chlorhexidine chip in 3 months was unclear. Objective: The present study aimed to evaluate the effect of chlorhexidine chip on reduction of pocket depth and clinical attachment level gaining in patients with periodontitis. Methods: The literature search was done in PUBMED for the use of chlorhexidine chip as an adjunct to scaling and root planing in improving periodontal status, especially pocket depth and clinical attachment level in periodontitis patients within 3 months. The risk of bias was estimated for each study based on the Cochrane Handbook for Systematic Review of Interventions. Meta-analysis was conducted for reduction of pocket depth and gain of clinical attachment. Results: Electronic search provided 51 records and 5 studies were included. The meta-analysis indicated that there was a significant difference in reduction pocket depth (p<0.05; I2 = 97%) and gain of clinical attachment level between chlorhexidine chip plus SRP and SRP alone (p<0.05; I2 = 98%). The mean difference of reduction of pocket depth was 0.26 (0.13 – 0.40) mm and the mean difference of gain of clinical attachment level was 0.31 (0.14 – 0.47) mm. All studies had fair quality based on AHRQ standards. Conclusions: Chlorhexidine chip can be an adjunct therapy for periodontal treatment to improve the clinical condition in periodontitis patients.

Abstract

Introduction: Implant placement in posterior maxilla alveolar ridge might be challenging due to progressive ridge resorpsion and poor bone quality caused by intrasinus positive pressure, vascular interruption to bone plate, and absence of occlusal loads. Many attempts were made to develop convenient and predictable bone augmentation techniques in the area. Maxillary sinus floor elevation is the most predictable and frequently used method nowadays. The use of platelet-rich fibrin (PRF), which is an autologenous biomaterial, has various advantages in sinus floor augmentation. This article has purpose in review the literature on the current possibilities of PRF usage in sinus floor augmentation. Literature review: Maxillary sinus floor augmentation is done by lifting the Schneiderian membrane and inducing bone formation in the created space. There are two types of approaches; lateral and trans-alveolar approach. Schneiderian membrane perforation may cause complications in the augmentation procedure. PRF is made of fibrin matrix, containing high concentrations of platelets, growth factors, fibrin, and leukocytes, which promotes wound healing and natural bone regeneration. PRF can be used as a sole filling material, as an addition to graft materials, as a membrane to seal bone graft in lateral sinus augmentation approach, to protect sinus membrane, and to aid healing in membrane perforation. PRF acts as an optimized blood clot, providing stability and releasing growth factors in promoting bone formation. Conclusion: Within the limitations of this review, it is shown that PRF provides a safe, reliable, and easy method as a filling material, protective membrane, and in managing ruptured membrane.

Abstract

Introduction: Periodontal regenerative therapy goals are to prevent and repair the soft and hard tissue that has been damaged by the infection. The membrane has been used as a material of soft tissue periodontal regenerative therapy. Resorbable and non-resorbable membranes were used as physical barriers to avoiding the growth of connective and epithelial tissue into the defect area. This review will be discussed about the new development of membrane materials for Guided Tissue Regeneration (GTR) which is used in regenerative periodontal therapy along with the advantages of each biomaterial. Review: The barrier membranes used for GTR can be divided into three generations. The first-generation membrane has many structural, mechanical, and biofunctional limitations. The second and third generation of membrane materials is still being developed. The third generation is claimed to function not only as a barrier but also as an antimicrobial agent. It also enhances tissue adhesion and releases the growth factors which are needed for periodontal regeneration. The development of materials used in GTR therapy has been upgraded from only as a barrier to the perspective from being only a barrier to a delivery agent such as antibiotics, growth factors, adhesion factors in the third generation. Conclusion: The GTR procedure has long been used in periodontal regenerative therapy. The latest development for GTR materials not only works as a barrier but also has both bioactive and multilayered nanofibrous membranes that are potential for use in periodontal tissue engineering.

Abstract

Introduction: Dental caries is a disease that is produced by the dissolution of local chemical surfaces of the teeth caused by product of microbial fermentation from dietary carbohydrates in the biofilm that attached to the teeth. Sugar especially sucrose is the most important dietary factors in the etiology of dental caries. Dietary modification have been recommended in order to reduce the fermentable carbohydrate intake needed by the cariogenic microorganism in order to produce acid. The used of sugar substitutes is a common procedure to reduce caries risk. Stevia rebaudiana Bertoni leaves extract is a natural sweetener which is used as a sugar substitute. The aim of this literature review is to study the noncariogenic potencially of Stevia rebaudiana Bertoni leaves extracts as natural sweetener. Review: Stevia rebaudiana Bertoni leaves extract is a non caloric natural sweetener alternative source to traditional sugar (sucrose) with diterpene glycosides are responsible for its high sweetening taste ranging 250-400 times sweeter as compare to sucrose. Studies have revealed the noncariogenic potential of Stevia leaves extract because its non fermentable, improved the pH biofilm, pH saliva and buffering capacity of the saliva in a high caries risk patient and reduce cariogenic organisms in biofilm and saliva. Stevia rebaudiana Bertoni leaves extracts also have been scientifically proven to be safe and effective to protection against dental caries because of its less demineralization email as compared to sucrose. Conclusions: Stevia rebaudiana Bertoni leaves extracts as natural sweetener has a excellent noncariogenic potency

Abstract

Introduction: Gingival recession represents a clinical condition in adults frequently encountered. Gingival recession is the main complaint of patients who come to the dentist due to an unsatisfactory aesthetic problem. Handling this condition requires action depending on the severity of the case gingival recession. the dentist must know the best treatment for the case. This article purpose to review some literature regarding the treatment means for the gingival recession to achieve an esthetic gingival appearance. Review:  Gingival recession is the most common clinical manifestation of all oral diseases. The condition can be corrected by two approaches to gain an aesthetic appearance. The first option is mucogingival surgery or gingival plastic surgery. The second option is gingival replacement with artificial substitutes. Treatment of gingival recession is essential to rectify the esthetic and functional deficiencies of the patient and prevent further periodontal destruction. Gingival recession can be treated with different surgical procedures and the root coverage can be obtained from the surgical approach taken. Conclusion:  Increased patient awareness of dental esthetic procedures has increased the demand as well as the responsibility of cosmetic dentistry to have outstanding skill and create pleasing results.

Abstract

Introduction: A periodontal treatment includes surgical and nonsurgical therapy which the successful wound healing after periodontal treatment is influenced by many local and systemic factors. One systemic factor that influences healing is stress. Recent studies have shown that stress plays a role in predicting the severity of pain, periodontal disease, and can even inhibit wound healing. This article aims to review how stress inhibits periodontal wound healing. Review: Stress can occur as a result of stressors or external pressures in the form of household conflicts, financial problems, the death of the closest person referring to emotional and physiological reactions, exceeding one's ability to deal effectively with the situation. The process of healing periodontal tissue is divided into two processes, namely regeneration, and repair. Wound healing in periodontal tissue is the same process that occurs in other parts of the body. Research on the effects of stress on healing periodontal wounds has been carried out. Stress induces glucocorticoid production. Increased cortisol hormone can inhibit the formation of collagen, thinning capillary blood vessels and preventing the formation of new bone. Psychological stress is associated with increased interleukin IL-1β and IL-6 in the gingival crevicular fluid. Other studies have shown that increased stress causes the development of periodontal inflammation due to an increase in IL-1, IL-4, and IL-8 which results in greater periodontal damage. Conclusion: From this literature review it can be concluded that stress inhibits periodontal wound healing.
