Abstract

Introduction: Pain conditions can cause physiological responses in the form of increased pulse rate. A local anesthetic procedure with a syringe is given to minimize pain during an invasive procedure in children's dental treatment, but the procedure can also cause pain. A jet injector is a new innovative anesthetic tool that can anesthetize certain areas without using a needle with minimal pain, but the shape and size of the jet injector, as well as the shocking sound of the device, can affect the pain perception of children, therefore a distraction method is needed to divert attention children from pain stimulus, one of them by using virtual reality. This research purposed to determine the effect of virtual reality distraction on changes in pulse during local anesthetic procedures with jet injectors in pediatric dental care. Methods: The research method used in this study is a quasiexperimental with Pretest-Posttest non-Equivalent Control Group Design. This design is used to find out the comparison between the treatment group and the non-treatment group. Changes in pulse rate before and during the anesthesia procedure in both groups were analyzed using the Independent T-Test analysis. Results: Based on the results of the Independent T-Test, it was found that the T-test Sig. (2- tailed) is 0.001 (p <0.05) so the data obtained is significant. Conclusion: Virtual reality distraction influences changes in pulse rate smaller than without distraction during local anesthetic procedures with jet injectors in pediatric dental care.

Abstract

Introduction: Periodontal disease is a chronic inflammatory condition that affects supporting tissue around the teeth which results in periodontal tissue damage. If left untreated, periodontal disease can cause tooth loss. When this condition occurs around dental implants, the disease is called peri-implantitis. Along with the development of science and technology, various treatments for periodontitis and peri-implantitis have also been found. One of them is gene therapy. Gene therapy is a treatment that is offered and is proven to provide maximum results. Therefore, in this review, we present gene therapy in the application of periodontal tissue and peri-implant regeneration. The purpose of this article is to review the literature on the current of gene therapy in treatment of periodontitis dan peri-implantitis. Review: Periodontitis and peri-implantitis treatment with gene therapy is based on treatment by transferring genetic material to introduce, suppress, or manipulate certain genes that direct a person to have his own cells to produce therapeutic agents. Gen therapy can achieve greater bioavability of growth factors in periodontal wound so that they have greater regenerative potential, both in-vivo and ex-vivo with viral and non-viral vectors. Conclusion: Within the limitation of this review, it was shown that gene therapy could be an alternative in overcoming the weakness of various growth factors and showed promising results.

Abstract

Introduction: The main etiology of periodontal disease is the presence of dental plaque and poor oral hygiene. This condition can be exacerbated through systemic drug therapy by modifying the inflammatory and immunological responses of periodontal tissue to bacterial plaque. In addition to the use of systemic drug therapy, the use of illicit drugs also affects changes in periodontal tissue. The prevalence of drugs has increased globally. Understanding the effects of drugs on periodontal tissue helps clinicians determine diagnoses and treatment plans. This article aims to understand the medication affecting the changes of periodontal tissue. Review: The literatures were obtained using a variety of different database sources (Medline via PubMed, Wiley Online Library, and Google Scholar), with the keyword systemic drugs AND periodontal tissue; Illicit drugs AND periodontal disease; recreational drugs AND periodontal disease; sex hormone AND periodontal tissue. This literature outlines the various periodontal tissue changes as an effect of systemic drug therapies including anti-hypertensive, anti-convulsion, immunosuppressant, anti-inflammatory, hormones and sedatives such as cannabinoids, opioids, stimulants, and hallucinogens. Periodontal tissue changes can be exacerbated by drug interactions, both systemic therapy drugs and illicit drugs. Some studies have found an association between systemic drug consumption and the patient's periodontal condition. Some medications have different effects on periodontal tissue which affect the time and type of periodontal treatment. Conclusion: Understanding the effect of drugs on periodontal tissue helps the clinician determine diagnoses and treatment plans that are appropriate for the patient's periodontal condition.

Abstract

Introduction: The leaves of Altingia excelsa were found to strongly inhibit SP-C1 human tongue cancer cell lines. This study was focused on identifying antiproliferative compounds found in A. excelsa leaves and assesment their action mechanism. Methods: Extracts of Methanol A. excelsa were fractionated based on their solvent polarity using n-hexane, ethyl acetate, and water. The anti-proliferative testing were tested in vitro against SP-C1 human tongue cancer cell lines using the MTT assay. Isolated the active compound used column chromatography and identified by the spectroscopic method. The isolated compound was also tested for its anti-proliferative testing and ELIZA method of apoptotic induction. Results: This work resulted in the isolation of a flavonoid, which was identified to be kaempferol and quercetin. The compound inhibited SP-C1 proliferation of cell in a time- and dosedependent manner with IC50 values of 0.72 µg/mL and 0. 70 µg/mL for the 24 hours treatments, respectively. Furthermore, promoting apoptosis via the intrinsic pathway, thereby increasing the activity of caspase-8 and caspase-9. Conclusions: These results suggest that kaempferol and quercetin the anticancer compound found in A, excelsa provided a basic for its potential use in cancer disease treatment management

Abstract

Introduction: Alveolar bone is the most affected of the periodontal diseases. Bone grafts such as autograft, allograft, xenograft and, alloplastic often are used, but they have limitations. So, the bone graft is still a major problem that continues to inspire the design of new bone materials. The purpose of this article is to review the clinical and biologic function, advantages, and disadvantages of dentin as an autograft and allograft bone in periodontal therapy. Review: Autograft is still the gold standard in bone augmentation because of its excellent osteoinductivity and osteoconductivity. Bone and dentin are mineralized tissues that have almost similar chemical compositions. Dentin matrix obtained from a tooth has to be demineralized before using it as a bone graft material. Dentin consists of 18% collagen, 2% noncollagenous proteins, 70% hydroxyapatite, and 10% body fluid. Matrix is a repository for growth factors, such as Bone Morphogenetic Proteins (BMP), Transforming Growth Factor-β (TGF-β), Insulin-like Growth Factor (IGF), and Basic Fibroblast Growth Factor (bFGF). Demineralized Dentin Matrix (DDM) is highly biocompatible and can support cell growth and proliferation, enhances the bone remodeling capabilities and autograft leads to the absence of antigenicity. Autograft DDM was further suggested to be an ideal scaffold for stem cells and bone growth factors, therefore autograft tooth could be recycled as the innovative biomaterial. Conclusion: The present results indicate that the autograft dentin materials can be used as a bone graft.

Abstract

Introduction: Acrylic denture in its use often results in fractures and one of the causes is due to poor mechanical resistance, especially to impact strength. Several methods to prevent broken acrylic, include the addition of fibers that function as reinforcement. There are 2 types of fibers, synthetic and natural fibers. Synthetic fibers are relatively expensive, so an alternative is needed, namely the use of natural fibers (Ramie and banana stem). This study purposed to determine the effect of adding ramie fiber and banana stem to impact and flexural strength of acrylic denture base. Methods: Experimental Laboratory, number of 60 samples consisted of 30 samples for flexural and impact tests. The sample was an acrylic plate; size 65x10x2.5 mm for Flexural Test and 80x10x4 mm (Flatwise impact method without notched) for Impact Test. The fiber concentration added was 1.6% of the sample weight. The sample consisted of 3 groups, namely control, the addition of ramie and banana stem fibers. Flexural test with UTM and Impact test with Charpy method. Results: Flexural and impact strength mean value of the ramie fiber group had the highest value (132.7459MPa; 7.9859 x 10-3Joule/mm2), followed by the control group (99.8499MPa; 4.2757 x 10-3Joule/mm2) and smallest value was in banana midrib fiber group (91.6865MPa; 3.0303 x 10-3Joule/mm2). The LSD test of significance(p) between each group in flexural and Impact Strength tests showed a significant difference (p<0.05). Pearson correlation test showed a significant positive correlation between the two tests. Conclusion: The addition of ramie fiber can increase the impact and flexural strength of denture acrylic, but it is different from the addition of banana midrib fiber because the presence of flavonoids (phenol compounds) it contains causes a decrease in the strength of denture acrylic.

Abstract

Introduction: An acquired diastema may be a warning sign signaling advanced periodontitis. Gingival inflammation, reduced bony support, and tooth mobility may contribute to the drifting of teeth, a condition called pathologic tooth migration, and formed tooth gap or space between two teeth or more. Identification of the causative factors involved in pathologic tooth migration is crucial for successful treatment. Case and Management: Case 1 - A 38-year-old woman reported to the Department of Periodontics with the chief complaint of bleeding gums. An intraoral examination revealed highly inflamed periodontal tissue with heavy plaque and calculus deposition. And in case 2 - A 34-year-old woman had a chief complaint of an increasing gap between her maxillary anterior teeth. After clinical and radiographic examinations, the patient was diagnosed with chronic periodontitis and gingival enlargement, pathological tooth migration with diastema. In Case 1, the patient was performed scaling, root planning, and curettage. Re-evaluation after 1 month revealed the reduction of visible inflammation and persistence of pockets and complete closure of spacing between the lower incisive. In case 2, the patient was performed scaling, and gingivectomy, and gingivoplasty. Re- evaluation after 3 months there is a complete closure of spacing between the upper incisors. Conclusion: These cases indicate that migrated teeth sometimes return to their original positions after the etiologic factor has been eliminated such as gingival inflammation, attachment loss, gingival overgrowth

Abstract

Introduction:  Chronic periodontitis is a multifactorial disease that causes rapid destruction of the tissues supporting the teeth and that may lead to tooth loss. Matrix metalloproteinase-13 (MMP‐13) levels have been higher in the gingival crevicular fluid of chronic periodontitis patients and tissue inhibitor of metalloproteinase-1 (TIMP‐1) is an endogenous inhibitor of MMPs implicated in inflammation.  This review purpose to discuss the relationship between matrix metalloproteinase-13 and tissue inhibitor of metalloproteinase-1 in chronic periodontitis. Review: The level of MMP-13 plays a role in the pathogenesis of chronic periodontitis. MMP-13 (collagenase-3) is the third member of the collagenase subfamily of MMPs and it is a potent gelatinolytic and collagenolytic enzyme expressed by different cell types. The activity of these MMPs is primarily controlled by tissue inhibitors, known as TIMPs, which have an N-terminal domain that is able to inhibit MMPs. Some studies showed that MMP-13 is associated with soft and hard tissue damage during the progression of chronic periodontitis. The balance between MMPs and TIMPs plays an important role in the degradation of the extracellular matrix. In healthy periodontal conditions, TIMP levels are generally higher than in unhealthy conditions, in which MMP levels exceed the level of TIMP. Conclusion: Chronic periodontitis is characterized by the increase of MMP-13 expression during the progression of the disease, the active sites show a decrease of TIMP-1 levels in association with the elevation of MMP-13.

Abstract

Introduction: One of the most developed tissue engineering techniques to improve and spur the wound healing process in damage of soft and hard tissues of the oral cavity is the application of Platelet-Rich Fibrin (PRF). PRF contains many growth factors. Many studies have shown differences in the PRF preparation process especially in the variable speed and centrifugation time resulting in varied clinical trial results. Purpose: This study aims to find the effects of speed and time of centrifugation on the number of thrombocytes. Methods: In this experimental laboratory study, the samples were rat blood that was put into a vacuum tube centrifuged at different speeds and times. The sampling comprises 25 rat blood samples for the variable of the centrifugation speed and 25 rat blood samples for the centrifugation time, after which the number of histologically-formed thrombo-cytes was calculated. One-way ANOVA test was used for data analysis. Results: One-way ANOVA parametric test-assisted statistical analysis in the centrifugation speed group obtained a sig value of 0.000 <0.05, suggesting that the centrifugation speed was significantly affected the number of thrombocytes. One-way ANOVA test in the centrifugation time group obtained a sig value of 0.000 <0.05, indicating that all tested groups significantly affected the number of thrombocytes. Conclusion: Speed and time of blood centrifugation affect the number of thrombocytes in the process of Platelet-Rich Fibrin preparation

Abstract

Introduction: The black triangle is a condition where the interdental papila does not cover the space between teeth which can interfere with aesthetics, pronunciation and cause food impaction, so preservation of papila in the aesthetic area is something that must be considered in periodontal treatment. This article aims to review the literature on definitions, etiology, classification, and various considerations in handling black triangle cases in aesthetic dentistry. Review: Interdental papilae not only function as biological barriers for the periodontal structure beneath but also have an important role in aesthetics. Handlers in black triangle cases include non-surgical involves crossdepartment (Periodontia, Orthodontia, Conservation) and surgical. The etiology of interdental papila loss is multifactorial, so it is important to establish an adequate diagnosis to obtain an appropriate treatment plan. Non-surgical techniques can be performed by making prosthesis, restoration, and orthodontic treatment. While surgical techniques include reconstructive, preservation, and recontouring of interdental papilae. Conclusion: Black triangle treatment can provide maximum results by considering various factors, both etiology, appropriate treatment plan, and alternative treatments so that it can provide satisfaction for patients.
