Abstract

Hypertension is a non-communicable disease which is the main cause of morbidity and mortality in the world. This disease is known as a silent killer because it often shows no symptoms in the early stages, but can cause serious complications such as heart disease, stroke and kidney failure. Hypertension control does not only depend on pharmacological therapy, but is also influenced by various interrelated multidimensional factors. This research aims to analyze in depth the factors that influence blood pressure control based on literature studies from national journals in the last five years. The method used is a literature study with a qualitative descriptive approach to scientific articles that discuss psychosocial, cognitive and behavioral aspects in hypertension sufferers. The results of the study show that patient motivation, family support, health literacy, as well as diet and physical activity are the main factors that determine the success of controlling blood pressure. The implementation of these factors in the field is currently supported by health technology such as the CERDIKA application with a medication reminder feature and Mobile JKN. These three factors interact with each other and form a complex system in managing hypertension. Therefore, a comprehensive, integrative and sustainable approach is needed. Abstract

Pneumonia is still the main reason children under five years old die worldwide, and nearly all of these deaths happen in poorer countries. In Pekanbaru City, the Payung Sekaki Health Center had the most cases of toddler pneumonia in 2021, with a total of 97 cases recorded. Objective: To look at how the home environment and the way parents act can influence the chances of a toddler between 12 and 59 months getting pneumonia. Methods: A quantitative study with cross-sectional design. A group of 49 young children was chosen using a method where every child had an equal chance of being selected. Data was gathered using observation forms and examined with univariate and bivariate analysis, including the Chi-Square test. Results show that 51% of the people surveyed had homes where conditions were not good, 75.5% had parents who acted in ways that weren't positive, and 55.1% of the young children had pneumonia. The analysis looked at two factors at a time and found that environmental factors had a strong impact on pneumonia, with a p-value of 0.007 and a POR of 6.333. Parental behavior also had an effect, with a p-value of 0.038 and a POR of 5.538. In summary, having a bad home environment makes a child six times and three tenths more likely to get pneumonia, and if parents don't behave well, it makes the child five times and five tenths more likely to get pneumonia. Education about PHBS and efforts to improve healthy home standards should be given top priority. Keywords: environmental factors, parental behavior, pneumonia, toddlers, Pekanbaru

Abstract

The growing demand for electrical power and the heavy reliance on fossil energy resources encourage the use of sustainable renewable energy, especially solar energy. Padang Pariaman Regency has considerable potential for solar power development, indicated by an average solar irradiation of 4.62 kWh/m²/day, so the implementation of an off-grid Solar Power Plant (PLTS) system is the right solution to support energy independence and reduce carbon emissions. The objective of this study is to analyze the planning of an off-grid photovoltaic (PV) system at the Operational Building of Balai Teknik Perkeretaapian (BTP) Kelas II Padang from technical and economic aspects and evaluate its investment feasibility. The research method includes collecting primary data in the form of building electrical load profiles and site observations, as well as secondary data in the form of solar radiation data, component specifications, electricity consumption data, interest rates, and inflation rates. Technical and economic analyses are conducted through manual calculations to determine the capacity of solar panels, batteries, and inverters, as well as economic analyses in the form of Net Present Cost (NPC) and Cost of Energy (COE) calculations, which are then validated using HOMER software simulations. The analysis results show that both methods produce relatively comparable system configurations. Technically, the off-grid solar power system is designed with a solar panel capacity of 39,4 kW based on manual calculations and 45,2 kW based on HOMER software simulations, both of which are capable of meeting daily energy needs of 169,5 kWh with the support of a battery capacity of ±340 kWh that provides autonomy time of up to 48 hours and an inverter capacity of 80 kW which is above the peak load of 52,245.50 W. From an economic aspect, manual calculations produce a Net Present Cost (NPC) value of IDR 1,606,156,628 with a Cost of Energy (COE) of IDR 1,327/kWh, while the HOMER simulation produces an NPC of IDR 1,624,727,000 with a COE of IDR 1,405/kWh. Both methods show a positive NPV value, a Profitability Index greater than one, and a Discounted Payback Period below the project life, so that the off-grid solar power system can operate optimally is declared technically and economically feasible.Keyword: technical and economic analysis, solar energy, HOMER, Investment feasibility, off-grid solar photovoltaic system
