Abstract

COVID-19 has spread rapidly throughout the world that affecting the community life. The government has strictly taken health and social measures that have been imposed by all countries to decrease of COVID-19 spreading. Health care units are the forefront of dealing with health problems in the community due to COVID-19. Therefore, the role of Puskesmas in the detection, prevention and response very crucial as the health care unit in the first level. In supporting that matter, the implementation of epidemiological surveillance system can be used to monitor the trend of COVID-19 transmission from local, national and global levels in conducting early detection in the areas without virus transmission, monitoring cases in the areas with virus transmission, including vulnerable populations identification, providing epidemiological information to carry out risk assessments and to evaluate the impact of pandemic on health and social service. This study aimed to provide an overview the implementation of COVID-19 epidemiological surveillance system at the puskesmas level in Kendari City, 2021 to improve the epidemiological surveillance system quality. A descriptive qualitative with phenomenological design was carried out. The informants consisted of 2 key informants (surveillance programmer and head of puskesmas) and 1 ordinary informant, KTU of Nambo Health Center. The results showed that the implementation of COVID-19 epidemiological surveillance system at the Nambo Health Center in the aspects of data collection and dissemination have been carried out optimally, but the data analysis has not been carried out properly. The data processing and data interpretation aspects are not implemented.

Abstract

Patients in the emergency room (ER) often experience anxiety due to acute or severe pain, which may indicate a life-threatening condition, leading them to expect immediate treatment from nurses. Nurse response time plays a critical role in saving patients and has a direct impact on their anxiety levels. Patients who experience delays in receiving services from healthcare providers may feel afraid and worried about their condition. The aim of this research is to determine the relationship between nurse response time and patient anxiety in the ER at the Dinoyo Health Center, Malang City. The study employed a correlation research design using a cross-sectional approach. The population included all 50 patients in the ER, with a sample of 47 patients selected using systematic random sampling techniques. The independent variable was response time, while the dependent variable was patient anxiety. The instruments included response time observation sheets and the Hamilton Anxiety Rating Scale (HARS) questionnaire. Data were analyzed using the chi-square test. The results indicated that the majority of nurses exhibited good response times (70.2%), and the majority of patients reported no anxiety or were within the normal range (63.8%). A significant relationship was found between nurse response time and patient anxiety within p-value = 0.000 (0.05). Suggestions for future research include identifying patient anxiety levels in the ER based on factors such as the level of emergency, payment method, and availability of family assistance.

Abstract

The aging process in the elderly population can lead to changes in body posture, often resulting in kyphosis, which exacerbates postural balance disorders. This study aims to determine the relationship between kyphosis posture incidence and postural balance disorders among the elderly in Jebres, Surakarta. The research design employed was an observational analytic correlation with a cross-sectional design, utilizing purposive sampling techniques. The independent variable was kyphosis, while the dependent variable was balance disorders. The instruments used included the Unteberger test for assessing balance and the Occiput-to-Wall distance test for measuring kyphosis. Results from the study revealed that out of 67 respondents from a total population of 75 elderly individuals, 30 (44.77%) experienced kyphosis, with 23 (34.32%) presenting hyper kyphosis. Additionally, 53 (79.10%) respondents exhibited balance disorders, while 14 (20.89%) did not have kyphosis or balance disorders. Bivariate analysis using the Spearman rho test demonstrated a significant correlation between Balance Disorders and Kyphosis Posture, with a Sig (2-tailed) value of 0.001 and a correlation coefficient of 0.759. The relationship between these two variables was found to be unidirectional, indicating a significant association between the incidence of kyphosis posture and postural balance disorders in the elderly in Jebres, Surakarta. For future research, it is recommended to further explore the specific magnitude of kyphosis angles and their impact on postural balance disturbances

Abstract

Violence behaviour poses a potential risk of physical harm to oneself or others. Violence behaviour must be promptly addressed by providing intervention to avoid detrimental impacts on the patient, others, or the surrounding environment. This research aims to identify the types of interventions for patients with violent behaviour in psychiatric hospitals. This review adopts a scoping review approach, following the analysis procedure by Arksey and O'Malley. Articles were identified using the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses) method. Inclusion criteria for this review article include English or Indonesian language articles, original articles, articles using quantitative, qualitative, and mixed research methods published within the last 5 years. From the review results of 1.128 articles, 8 articles met the inclusion criteria. The analysis results indicate various types of interventions proven to be effective in reducing violent behaviour in patients in psychiatric hospitals. Treatments involving physical, psychological, and belief-based activities are suitable for reducing violent behaviour in patients in psychiatric hospitals. Furthermore, to determine the real impact of reducing violent behaviour, supplemental research is needed to develop interventions in patients in psychiatric hospitals.

Abstract

The presence of comorbid diseases has become a crucial concern in confronting the COVID-19 pandemic, underscoring the need for the implementation of GERMAS (Healthy Living Movement) policy as a preventive measure initiated by the government. GERMAS aims to control the increase in non-communicable disease cases, which have the potential to disrupt human resources and productivity, as well as negatively impact social and economic development. In its implementation, GERMAS must adhere to the principles of Good Governance to ensure transparency, community participation, accountability, effectiveness, efficiency, responsiveness, and consensus. This research aims to analyze Transparency, Participation, Accountability, Effectiveness and Efficiency, Responsiveness, and Consensus. This research employs a qualitative approach with a narrative method and in-depth interviews with 12 informants. While GERMAS has been implemented since the President's directive in 2017, there are still several challenges in enhancing its effectiveness. Analysis of Good Governance principles highlights issues such as lack of community participation, low transparency and inter-agency coordination, and insufficient attention to inactive GERMAS forums. Further research is needed to deepen understanding of GERMAS implementation in Kutai Barat, focusing on aspects such as inter-sectoral communication, community participation, and enhancement of accountability of relevant agencies. Concrete steps such as comprehensive performance audits and increased social media outreach can help improve policy implementation.

Abstract

Intrapersonal conflict is a condition where an individual has two desires that cannot be fulfilled simultaneously. This research aims to identify the description of intrapersonal conflict among nurses. The study design uses a scoping review method. Articles were identified using the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses) method. Inclusion criteria for this review article include articles in English or Indonesian, original articles, and articles that use quantitative, qualitative, and mixed research methods published within the last 5 years. A search of articles in this review yielded 819 articles, and the review identified 4 articles that met the inclusion criteria and were subsequently analyzed. The results of the analysis indicate the description of intrapersonal conflict among nurses, namely: (1) Career-related conflicts, hindered by further study; (2) Family-related conflicts. Nurses want to contribute to the family's economy but their partner asks them to take care of the children and the house; (3) Work-family life conflicts or sleep duration; (4) Religion-related conflicts, with an example of a Muslim nurse newly employed at a hospital but facing a mandatory rule to remove the hijab. The conclusion of this review article shows various descriptions of intrapersonal conflict among nurses and suggests that research be conducted to reduce or manage these conflicts.

Abstract

Bleeding disorders outside the menstrual cycle are reproductive system disorders commonly experienced by women during their reproductive years. These disorders are characterized by changes in menstrual periods, including alterations in cycle length, duration of menstruation, or volume of bleeding, with a worldwide prevalence ranging from 3% to 30%. Hormonal factors are often implicated as causes of bleeding disorders outside the menstrual cycle. Preliminary findings from a study conducted at RSIA Puri revealed that three female patients seeking treatment experienced bleeding disorders outside the menstrual cycle, with two of them exhibiting low estrogen levels (below 20 pg/ml) upon examination. The purpose of this study was to analyze the relationship between hormone levels, particularly estrogen, and bleeding outside the menstrual cycle in women of childbearing age at RSIA Puri Malang. The research employed a quantitative analysis through a cross-sectional approach. Primary and secondary data were obtained from laboratory examinations of estrogen levels in blood serum on the third day of menstruation. The sample consisted of 34 mothers who met the criteria of experiencing menstrual disorders and visited RSIA Puri Malang, and who willingly participated as respondents, selected through quota sampling. Data analysis was conducted using the Chi-square test, and the results revealed a significant relationship between estrogen levels and bleeding outside the menstrual cycle, or metrorrhagia (p-value = 0.018; OR = 0.107). This suggests that estrogen hormone regulation plays a crucial role in preventing menstrual irregularities. In conclusion, maintaining optimal estrogen hormone levels is essential for preventing and managing menstrual irregularities and bleeding disorders outside the menstrual cycle.

Abstract

Communication failures in information transfer during intraoperative handover can pose a risk to patient safety incidents. One of the factors influencing the success of intraoperative handover is the existence of a structured handover checklist. This study aimed to explain the implementation of a structured intraoperative handover checklist to improve information transfer during intraoperative handovers. The design employed was a literature review. Articles were selected from databases including ScienceDirect, Scopus, PubMed, and Springer Link. The criteria for article selection included publication between 2017 and 2022, full text in English, and the use of keywords such as 'intraoperative handover,' 'handover checklist,' and 'information transfer.' The PRISMA flowchart stages were utilized to filter appropriate articles. The critical assessment of article quality employed The Joanna Briggs Critical Appraisal Tools 2020 and Mix Methods Appraisal Tool 2018. The results of the review of 8 articles indicated that a lack of good coordination during intraoperative handovers often occurs due to the absence of structured guidelines regarding handover content. The application of an intraoperative handover checklist has been shown to increase the completeness of data transfers and reduce the percentage of information omission during the handover process. The implementation of structured intraoperative handovers has a positive influence on improving the quality of communication between surgical teams and reducing incomplete information conveyed. A structured intraoperative handover checklist is needed as a guide to reduce the number of errors in information transfer, thus enhancing patient safety.

Abstract

Pulmonary tuberculosis (TB) is the number one deadly infectious disease in the world caused by the bacterium Mycobacterium tuberculosis. Indonesia ranks third globally, and East Java ranks second in Indonesia. Secondary data from Dr. Soeroto Ngawi Regional Hospital in 2022 indicate that there will be 15 patients resistant to drugs. The research aims to determine the influence of knowledge and family support on medication adherence among tuberculosis patients at Dr. Soeroto Ngawi Regional Hospital. The research design is quantitative observational with a cross-sectional approach. The study population consists of all pulmonary TB patients aged ≥ 12 years recorded from January to July 2022 at Dr. Soeroto Ngawi Regional Hospital, with a selected sample of 48 respondents using simple random sampling probability techniques. Research instruments include direct observation, interviews, and questionnaires. The research period was from December 4, 2022, to April 9, 2023. The results showed that the majority of pulmonary tuberculosis patients are female, aged between 26 and 45 years, with educational levels ranging from junior high school to high school. Most respondents have good knowledge (56.2%) and good family support (60.4%). The analysis indicates that there is an influence of knowledge factors and family support factors on medication adherence in pulmonary tuberculosis patients. The most dominant factor affecting medication adherence in pulmonary tuberculosis patients is family support.

Abstract

Transmission of infectious diseases is one of the risks in the blood transfusion process. The Blood Donor Unit (UDD), as a blood provider institution, must ensure that blood products distributed in the community are safe, especially from infectious diseases that can be transmitted through blood transfusion (TTI). UDD PMI Surabaya is a first-class UDD and CPOB-certified facility that serves blood needs not only in East Java but also as a reference for the Eastern Indonesia region. The location of UDD is in a big city, the number of donors is quite large, the coverage of the area of origin of donors and the target users of blood products is quite broad, and a complete screening test method are interesting factors to study regarding the prevalence of TTI disease. This study aims to determine the prevalence of infectious diseases transmitted through blood transfusion among blood donors in UDD PMI Surabaya in 2018–2022, as well as the sociodemographic characteristics of reactive donors. This research is a retrospective, descriptive study. Data for 2018–2022 comes from the in UDD PMI Surabaya database with total number of 639580 blood donors. The research results show that hepatitis B is a TTI disease with the highest prevalence compared to other TTI diseases. Most donors come from the 17–30 year age group who have reactive TTI test results, and this decreases with increasing age except for hepatitis B. Reactive donors are more dominated by women than men, except for HIV disease.
