Abstract

The massive use of synthetic insecticides in vector and pest control has caused various problems, including insect resistance, environmental pollution, and the risk of toxicity to non-target organisms. Therefore, the development of natural insecticides based on tropical plants is becoming a more environmentally friendly and sustainable strategic alternative. One plant with potential as a bioinsecticide is Annona muricata (soursop). The method used is a systematic review study on scientific publications for the period 2020–2025 that report data on toxicity (LC₅₀), mortality, and safety against non-target organisms from various plant parts (seeds, leaves, and bark) against disease-vector insects, agricultural pests, and domestic insects. Data were analyzed descriptively to assess the effectiveness of insecticides based on plant parts, extract types, and target species. The review results show that Annona muricata has acetogenin with strong and broad-spectrum insecticidal activity. Insecticide activity has been reported against Spodoptera frugiperda, Plutella xylostella, Bactrocera sp., Periplaneta americana, Culex sp., Aedes albopictus, and Rhipicephalus spp., with mortality rates increasing as extract concentrations increase. Some studies have shown safe limits on non-target organisms at certain concentrations, indicating potential ecological selectivity. Formulation development, extract standardization, and advanced toxicity tests are still needed to optimize the application of Annona muricata-based bioinsecticides on a field scale.

Abstract

Patient safety is a system designed to make patient care safer, including risk assessment, risk identification and management, and incident reporting and analysis. The World Health Organization emphasized in 2020 the need for increased attention to patient safety in hospitals. This study aimed to determine the relationships among patient safety culture, the work environment, and patient safety in the surgical ward of RSUD Jenderal Ahmad Yani, Metro City, Lampung Province, in 2025. This research used a quantitative, analytical, cross-sectional design. The study was conducted from 16 May to 28 May, 2025. The population consisted of 70 nurses working in the surgical ward, with a sample of 52 respondents selected. Data were collected using an observation sheet to assess work culture and questionnaires to measure the work environment and patient safety. Patient safety was treated as the dependent variable. In contrast, work culture and work environment were the independent variables. Data analysis was performed using the chi-square test. The results showed that 29 respondents (55.8%) had good patient safety, 37 respondents (71.2%) had a positive work culture, and 35 respondents (67.3%) experienced a conducive work environment. Statistical analysis indicated a significant relationship between work culture and patient safety (p = 0.001) and between the work environment and patient safety (p = 0.018). In conclusion, work culture and the work environment are significantly associated with patient safety. Hospitals are recommended to strengthen nursing management by fostering a supportive work environment in the surgical ward.

Abstract

Previous studies have shown that an ASiR-V rate of 60% is optimal for contrast-enhanced abdominal CT scans in patients with kidney stones. In this study, however, the ASiR-V variations tested were 60%, 70%, and 80%, with a focus on clinical intra-abdominal tumors. The objective was to measure the optimal signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) values for CT images of these tumors. The research employed a quantitative retrospective experimental design with a sample of 10 patients who met the inclusion criteria. The findings reveal notable differences in SNR, CNR, and Hounsfield Unit (HU) values across the ASiR-V variations, with the 70% ASiR-V showing the highest values for both SNR and CNR.

Abstract

Maternal mortality remains a critical public health challenge in Lampung Province, which consistently records one of the highest maternal mortality rates in Sumatra and has not yet reached the national Sustainable Development Goals (SDGs) target. Understanding the temporal patterns and dominant risk factors of maternal deaths is essential for designing more targeted preventive strategies. This case-control study included 193 respondents: 64 cases (maternal deaths) and 129 controls (surviving mothers), selected through purposive sampling from health facilities across Lampung Province. Data were obtained from medical records, maternal death audit reports, and structured interviews, then analyzed using univariate, bivariate (chi-square), and multivariate (binary logistic regression) approaches. The majority of maternal deaths occurred in the postpartum period (78.3%), with the highest concentration within the first 24 hours (63.1%), indicating this as the most critical vulnerability window in Lampung. Hemorrhage was the leading cause of death (53.9%), with most fatalities occurring within hospital settings (71.2%), suggesting that referral delays and suboptimal emergency obstetric care remain persistent challenges in the province. Binary logistic regression revealed that chronic diseases were the most dominant risk factor (OR = 3.80; 95% CI: 1.56–9.24), followed by irregular Antenatal Care visits (OR = 3.77; 95% CI: 1.58–8.99) and history of pregnancy complications (OR = 3.48; 95% CI: 1.45–8.34). Gestational age showed no significant association (p = 0.635). Urgent need for Lampung Province to strengthen early detection of chronic diseases through integrated ANC programs at the primary care level (Community Health Centers), improve emergency obstetric care quality, and reduce referral delays across districts to prevent avoidable maternal deaths.
