Abstract

Background: Common nutritional problems in pregnant women are chronic energy deficiency (CED). CED in pregnant women is a state of the mother lack of energy and protein intake during the period of pregnancy which can lead to the occurrence of health problems in the mother. This study aims to determine factors assoIKated with CED among pregnant women at UPT Puskesmas I Pekutatan, Jembrana, Bali.Methods: A case control study was conducted among 36 respondents, 12 cases (KEK) and 24 control (Non-KEK). The sampling technique used is purposive sampling technique. Affordable populations in this study were pregnant women recorded in medical records data at UPT Puskesmas I Pekutatan period 01 January-31 May 2017. The data were collected by interview using questionnaire and assessed using SPSS software version 20 for Windows. Existing data were analyzed by univariate and Fisher's exact bivariate with odds ratio calculation (OR).Results: About 13 respondents (36,1%) are <20 years old /> 35 years old with the most recent education is graduated from SMA that is 20 respondents (55,6%). Most respondents did not work (91.7%) and had low economic status (30.6%). Most of the respondents had children <2 (69.4%). Economic status (OR = 115; 95% IK: 9,3-1418), education level (OR = 2,3; 95% IK: 0,5-9,5), age (OR = 7,6; 95% IK: 1.6-35.9), gestational distance (OR = 11; 95% IK: 1.7-69), number of parities (OR = 7.6; 95% IK: 0.7-83.7) , And visit frequency of ANC (OR = 5; 95% IK: 0,9-26,4).Conclusion: The characteristic and frequency of ANC visit among pregnant women are risk factors of KEK occurrence in pregnant mother, so it is expected to provide information related to risk factors of KEK by health personnel in Puskesmas I Pekutatan, state of Jembrana.

Abstract

Introduction: A couple being unable to get pregnant result out of unprotected sex and have been trying for one year is defined as the inability to conceive or infertile. Other reasons can be due to the male lifestyle itself, for example obesity, alcohol consumption and also smoking. This study aims to look for various things that might be risk factors for infertility in male students of semester III and V, in the Faculty of Medicine of Udayana University in 2017.Method: This study used a descriptive cross-sectional design carried out during the 2017 period using the questionnaire given to respondents, a total of 116 male respondents in this study. Identification of lifestyle that can be a risk factor for infertility was assessed in all respondent.Result: Through this research, it can be known that lifestyle risk factors that cause infertility are using a laptop on the lap for hours (29.3%), injury in groin area (14,%), alcoholism (11.2%), continuous fast food consumption (12.1%), and obesity (6.9%), and active smoker (1.7%). In the other side of the person still doing a sedentary lifestyle and low physical activity.Conclusion: Sedentary lifestyles with low physical activity are common lifestyle finding among medical students so that this can be a lifestyle that becomes a risk factor for the onset of infertility.

Abstract

Introduction: Diarrhea is still a public health problem in developing countries, including Indonesia, because of its high morbidity and mortality. Breast milk is one of the important factors to reduce mortality and morbidity due to diarrhea. The aim of this study was to determine the relationship of exclusive breastfeeding to the occurrence of acute diarrhea. This research is analytic research with cross sectional method.Method: The study was carried out in Community Health Centre III Subdistrict of North Denpasar from October to November 2018. The research subjects were taken by consecutive sampling. Data analysis used the prevalence ratio calculation and chi square test to determine the relationship between exclusive breastfeeding and the incidence of diarrhea.Result: In this study the results of acute diarrhea patients in the exclusive breastfeeding group were 17 children and non-exclusive breastfeeding as many as 31 children. Subjects who did not experience acute diarrhea in the excluded breastfeeding group were 28 children and in the non-exclusive breastfeeding group as many as 14 children. The results of statistical analysis showed the value of p=0.003 (p<0.05)  PR=0.514 CI 95% (0.115-0.656).Conclusion: Exclusive breastfeeding has a significant relationship with the incidence of acute diarrhea and is a protective factor for the incidence of diarrhea.

Abstract

Background: Traumatic Brain Injury is among the leading causes of mortality and morbidity in the world, with the increase of incidence rate every year especially in the developing country concomitant with the rise in motor vehicle usage. In Indonesia, although the number is high, the study concerning the incidence and characteristic of patients with traumatic brain injury are still limited. This study has objective to show the characteristic pattern of traumatic brain injury cases at Umbu Rara Meha general hospital that, as the biggest hospital in Sumba Island, can be the representative for the area.Method: This study is a descriptive study with a retrospective design using the medical record in Umbu Rara Meha general hospital from January 1st until December 31th as secondary data. A non-probability sampling technique using consecutive was conducted. Data were analyzed using SPSS version 25 and depicted in mean, median, frequency, and percentage. Result: A total of 186 cases was included and from there it is shown that the highest prevalence rate of traumatic brain injury was in males (67,7%), and was dominated by the adolescence group (25,3%), although by occupation the highest rate was farmer and fisherman (32,3%). The leading cause of traumatic brain injury is an accident (82,2%) with mild traumatic brain injury (67,7%) as the most frequent type of severity. The average length of stay for traumatic brain injury was two days.Conclusion: The high number of the incidence rate of traumatic brain injury mainly from the accident and on adolescence group can be used as a reference for the local government or related institution to look more carefully about the awareness of safe driving or riding among the society and can be implemented through a program or other governing instrument.

Abstract

Background: The use of interactive media as a health promotion media has continued to develop in the past few decades. This technology is used to manage various acute and chronic diseases, one of which is diabetes mellitus. This systematic review aims to describe an intervention that utilises technology as a tool in the form of interactive media in conducting preventative health promotion and glucose control for type 2 diabetes.Methods: The databases used include ProQuest, PubMed, Science Direct and Google Scholar. The keywords used in article search are research, interactive media, type 2 diabetes, health promotion, telemedicine, web-based, obesity, diabetes exercise, walking, glucose control. Inclusion criteria are 1) articles published in 2014-2018 in English, 2) articles which the main content is about using interactive media for diabetes type 2 problems. Exclusion criteria are 1) literature that is not relevant to the purpose of writing, 2) researching the interactive media but not applied to the problem of type 2 diabetes, 3) duplication, and 4) published articles incomplete. Thus, there were 1442 articles and only 12 articles that were used in accordance with the inclusion and exclusion criteria.Results: This systematic review found that the effective results in the implementation of preventive health promotion and glucose control of type 2 diabetes by utilising technology particularly interactive media.Conclusion: The 12 articles used in this study revealed that the implementation of preventive health promotion by utilising technology particularly interactive media was sufficient to control the glucose levels of among patients with type 2 Diabetes Mellitus.

Abstract

Background: Alcoholic cardiomyopathy is a form of toxic cardiomyopathy which develops as a result of long-standing chronic ingestion of alcohol. Its clinical findings share similar features to other forms of dilated cardiomyopathy. Diagnosis can be made based on the history of long term alcohol abuse, clinical findings, and supporting diagnostic studies. This case report aims to overview alcoholic cardiomyopathy; from establishing the initial diagnosis to the treatment given for the condition.Case Description: A 41 years old male came to the emergency department unit with shortness of breath since last few days. Shortness of breath was said to be getting worse over time and progressed to breathlessness at rest. Other complaints were palpitations, fatigue, and long-standing cough. The patient was found to have a history of drinking alcohol in large quantities since 27 years ago. The blood pressure was 110/70 mm Hg, pulse rate 105 beats/min, and respiratory rate was 20 breaths/min. Slight edemas on both legs were also found.  From auscultation examination, bibasilar crepitation on both lungs was found. Cardiac examination showed systolic murmur of mitral regurgitation (MR) and tricuspid regurgitation (TR).  On supporting investigation, a chest x-ray showed cardiomegaly (cardiothoracic ratio; 57%). ECG examination showed sinus tachycardia and left atrial (LA) and left ventricular (LV) enlargement. Trans-thoracal echocardiographic (TTE) examination revealed; RWMA: global hypokinetic-akinetic, multi-chamber dilated and severe LV systolic dysfunction (EF 29%) and diastolic function (Grade IV (E/A Ratio>4)). Valve examination showed moderate-severe MR and moderate TR (TVG 40 mmHg).Conclusion: Alcohol cardiomyopathy is one of the most common causes of heart failure in young age and carries a poor prognosis if not treated properly. Treatment of alcoholic cardiomyopathy mainly includes total alcohol abstinence along with drugs used to treat systolic heart failure to reverse the condition and correct any nutritional deficiencies.

Abstract

Latar belakang: Prevalesi kanker kulit semakin meningkat di seluruh dunia. Walaupun insiden tertinggi pada kelompok berkulit putih, namun mengetahui epidemiologi dan tren penyakit dari kanker kulit diperlukan agar dapat menentukan pencegahan dan penanganan yang tepat.Tujuan: Untuk mengetahui karakteristik kanker kulit pada pasien yang telah dilakukan skin flap di Departemen Dermatologi dan Venereologi RSUP Sanglah.Metode: Penelitian ini merupakan studi deskriptif retrospektif dari catatan medis semua penderita kanker kulit yang memperoleh tindakan skin flap sejak Januari 2015 sampai dengan Desember 2019. Sampel diambil melalui total sampling. Karakteristik yang tercatat meliputi jenis kanker, jenis kelamin, umur, tipe kulit, pekerjaan, dan jenis flap yang dilakukan. Data yang terkumpul dianalisis secara deskriptif dengan program SPSS Ver21.Hasil: Terdapat 41 pasien kanker kulit yang memperoleh tindakan skin flap, terdiri dari 43,9% (n=16) BCC, 54,6% (n=10) melanoma, 21,9% (n=9) SCC, 9,8% (n=4) jenis kanker lain. Jenis flap yang umum digunakan yaitu simple advancement. Lokasi kanker paling umum ditemukan pada wajah dan kejadian kanker paling tinggi terjadi pada usia ≥50 tahun dan bekerja di luar ruangan.Simpulan: Kasus kanker tertinggi yaitu BCC dengan usia lebih dari 50 tahun dan bekerja di luar ruangan sebagai karakteristik paling umum pada semua kasus kanker.

Abstract

Background: Basal cell carcinoma (BCC) has a very slow-growing and rarely metastatic nature but tends to be locally invasive and relapse after therapy. Elliptical excision is one of the surgical procedures in the field of dermatology which is most often used with the application of tumescent techniques.Case Description: A woman, 64 years old, no. CM. 18.03.11.71, Balinese tribe, on July 30, 2018, came to the Dermatology and Venerology Polyclinic at Sanglah General Hospital with a frequent symptom of arising from wounds on the forehead. The lession began one year ago. Initially, it was a small reddish lump which was then felt more and more dilated as well as became blackish and accompanied by itching. Dermatology examination revealed hyperpigmentation, solitary, oval, 3x2cm, multiple erosion, firm boundary, round shape, 0.3-0.4 cm in diameter, covered in black crusting on top.Conclusion: The diagnosis of BCC determined on anamnesis, physical examination, dermoscopy, and histopathology regard with standard ellipse excision surgery.

Abstract

Introduction: Physiological jaundice found in infants and most symptoms are often mild. In conjugated jaundice defects in intra-hepatic production, transmembran transport from bile, i.e. cholestasis intrahepatic, or extra-hepatic obstruction/cholestasis occur, resulting in bile barriers. This study was conducted to look at the differences in the clinical and laboratory manifestations of IH and EH cholestasis in infants.Method: A descriptive retrospective study was performed on 102 infants with cholestasis who came to Sanglah Hospital Denpasar, during the period of January 2015 – December 2018. Data analysis was performed with pearson Chi-square test and independent t-test.Result: Subjects consisted of 60 infants boys and 42 infant girls, intrahepatic cholestasis were 79 and extra-trahepatic cholestasis were 42. Significant differences in characteristic of nutritional status with intrahepatic and extra-hepatic cholestasis were found (p=0,015), but there isn’t significant differences in age, birth weight, gestational age were observed. On examination of stool color, there was a significant difference (p<0,001), as well as laboratory results of SGOT, SGPT (p<0,05) between intra and extra-hepatic cholestasis. Meanwhile there isn’t significant differences in GGT, total bilirubin, direct bilirubin, indirect bilirubin, and ALP.Conclusions: There were differences in the characteristics of nutritional status, differences in laboratory results of SGOT, SGPT, and fecal clinics between intrahepatic and extrahepatic cholestasis.

Abstract

Introduction: The mechanism of nerve damage in ENL patients is important to be revealed because the problem of nerve damage is not getting enough attention in handling ENL patients. Some pro-inflammatory mediators that have been studied are risk factors for nerve damage in ENL.Aim: This study aim in knowing the mechanism of nerve damage in the relation with its risk factors, which are CD64 neutrophil, IL-17, MMP-9, and iNOS with PGE-2 and IL-10 protective factor and IL-10 in ENL patients.Method: This is an analytic cross-sectional design study with tissue samples from 30 leprosy patients with ENL in Dr. Soetomo General Hospital Surabaya. Samples from biopsy examined using IHC method to evaluate S-100, IL-10, IL-17, PGE-2, iNOS, MMP-9, and CD64 neutrophil expressions. The result calculated within 20 view field with 1000 times magnification.Result: CD64 neutrophil (p=0.022, r=-0.417), IL-17 (p=0.001, r=-0.560), iNOS (p=0.005, r=-0.502) and MMP-9 (p=0.025, r=-0.409) has significant correlation with S-100. There is no significant correlation between IL-10 (p=0.061) and PGE-2 (p=0.407) with S-100.Conclusion: Expressions of IL-17, CD64 neutrophil, MMP-9, and iNOS in tissue correlate with nerve damage in ENL patients.
