Abstract

Indonesia is one of the countries with the largest number of tuberculosis (TB) cases in the world. Environmental factors play significant roles in infection and disease development in children living with adult active TB cases. The aim of this study was to explore the environmental factors affecting TB risk in children, including humidity and number of people living in the same house with the children.  Using a cross-sectional study design, children living with active TB adults for at least 8 weeks were recruited. The subjects underwent clinical examination, tuberculin skin test (TST) and radiological investigations. Home visits were conducted three times daily, namely in the morning, at midday, and in the evening, to measure humidity by digital hygrometer and to observe home conditions. Of 56 index cases living with active TB adults in an urban low socio-economic setting, 64 contact TB children were recruited. These children were classified as class I with negative TST and no clinical signs, class II with positive TST and no clinical signs, and class III with both positive TST and clinical signs. There were 32 (50%) class I, 10 (15.6%) class II and 22 (34.3%) class III children. However, a sub-sample of 43 showed positive results for Mtb 16S rRNA, indicating that all children were infected. The humidity cut-off point was set at 75%, p=0.04 and EP 2.09 (CI= 1.32-3.29), signifying that children living in houses with a humidity higher than 75%, were twice more likely to be tuberculin positive. Environmental conditions playing a role in disease development were humidity and number of people living in the house.

Abstract

Objective: To evaluate the efficacy of superoxide dismutase (SOD) in lung function (FEV1 reversibility) and respiratory symptoms (drug scores, symptoms scores) in asthmatic and house dust mite allergic children receiving house dust mites immunotherapy.Methods: Forty subjects aged 6–17 years old with asthma and tested positive for house dust mite allergy on skin prick test, and received immunotherapy were enrolled in this study. All subjects completed clinical based assessments and diary-based assessments for drug and symptom scores. Following a four-week baseline assessment, all subjects were randomized to receive SOD or placebo. Respiratory symptoms (drug and symptoms score) and FEV1 were evaluated at the end of the 1st, 2nd, 3rd, and 4th weeks after randomization. Drug score, symptoms score and FEV1 reversibility test results were analyzed using a Paired t test and repeated measure of ANOVA. Results: There was a significant difference in drug scores, symptoms score and FEV1 reversibility test outcomes between SOD and placebo. SOD group showed a significant decrease in all outcome measures compared to those in placebo group. Conclusions: The use of SOD as antioxidants is effective in accelerating symptom relief for children with asthma and house dust mite allergy receiving house dust mite immunotherapy. Abstract

Background: Midazolam hydrochloride is an injection form of benzodiazepines and included in the highalert category in the Dr. Soetomo Teaching Hospital Intensive Care Unit (ICU), Surabaya, Indonesia.Nowadays, the technical preparation of midazolam drugs in the ICU room by doctors and nurses wasperformed by reconstituting drugs in the injection syringes to be stored for a while in the room. Objectives:To evaluate the preparation process for midazolam hydrochloride injection which has been analyzed inDr. Soetomo Teaching Hospital. Methods: Physical, chemical, and microbiological stability of 1 mg/mLmidazolam hydrochloride in aqua pro injection solvents have been evaluated at points 0, 8, 12, and 24 hoursafter preparation, as well as a comparison of stability in storage conditions at room temperature comparedto refrigerator temperatures. Results: The results showed all the preparations were no foreign particlesin the sample container. The midazolam did not experience precipitation in both room and refrigeratortemperatures. The pH of midazolam solution during storage proved that the preparation was relatively stable(pH 4.3–4.5). There were no significant differences of midazolam levels in both storages which was in rangeof 95%. The Miccrobiology Stability Test showed negative germ growth after 24 hours incubation in bothstorage. Conclusion: This study showed no changes in physical, chemical, or microbiological stability in themidazolam injection samples up to 24 hours after the manufacturing process.

Abstract

The prevalence of allergic diseases has increased in the last decade. Therefore, precise and reliable in predicting allergy in children were needed. However, in daily practice, most misdiagnosis occurs because it is only based on a family history of allergy. This study aims to evaluate the chronicity, recurrence of symptoms with similar exposure, and family history of allergy to be used as a predictor of allergy in children with respiratory symptoms. Cross sectional study was conducted in children who referred to the Pediatric Allergy and Immunology Outpatient Clinic, Dr. Soetomo General Hospital, Surabaya from January 1st - July 31st 2019. Children with a suspected allergy who developed respiratory symptoms was included. The data was taken from standardized medical record. The sensitivity and specificity of the 3 characteristics of atopic with positive SPT were calculated. A total of 115 children were admitted and 109 children fulfill the inclusion criteria. Gender 60% male and 40% female. The most common group of age founded was age 5-<10 years 39.4%, followed by 1-<3 years (25.8%), 3-<5 years (22.9%), 10 years 8.3%, and <1 year (3.6%). Fifty-four patients (49.5%) fulfilled 3 characteristics of atopy and there were 64 patients (58.7%) with positive SPT results. The sensitivity, specificity, positive predictive value and negative predictive value were 81.3%, 95.6%, 96.3%, and 78.2%, respectively. The 3 characteristics of atopy have good sensitivity and specificity to predict allergy in children.

Abstract

Background: Children with Down syndrome tend to have accompanying comorbidities, such as hypothyroidism, and late detection of this ailment leads to reduced growth of the child. This study aimed to assess the growth patterns in children with Down Syndrome and hypothyroidism at diagnosis.Methods: This cross-sectional study was conducted from September to December 2020 with 56 subjects at the Pediatric Endocrine Outpatient Clinic of Dr. Soetomo Hospital. Diagnosis of Down Syndrome was confirmed by karyotyping, while the weights and heights were assessed using a standardized curve for children with Down Syndrome and then calculated using Peditools. Fifty-six children with Down Syndrome and hypothyroidism were included, comprising 32 boys and 24 girls (mean age, 37.75 ± 34.26 months). Majority of the subjects had normal weight, height, and Body Mass Index (36/56 [mean z-score, −1.62 ± 2.36], 33/56 [mean height-z-score, −0.43 ± 2.74], and 30/56 [mean z-score, −2.00 ± 2.06], respectively). Furthermore, the mean onset of diagnosis was 17.07 ± 32.23 months, where 23 out of the 56 children had short stature and had gotten diagnosed at over 12 months of age.Conclusion: From the results obtained, hypothyroidism can be said to be associated with reduced growth in children with DS.

Abstract

Background: Allergic rhinitis (AR) is the most common chronic disease in children. Allergic symptomsaffect daily activities and increase risk of psychosocial emotion and attention deficit hyperactivity disorder(ADHD). Immunotherapy has been proven in improving AR symptomsObjective: To identify prevalence of the risk of psychosocial emotion disorder and ADHD and its relationwith immunotherapy duration in AR children.Methods: A cross-sectional study was held in AR children aged 4-18 years at Allergy Immunology OutpatientClinic, Dr. Soetomo Hospital, during March 2017. Immunotherapy duration categorized into 0-6 months, 6months-1 year, 1-2 years, 2-3 years. Psychosocial emotion disorder risk assessed using Pediatric SymptomsChecklist 17 (PSC-17), scored into four different subscales: Internalizing, Externalizing, Attention, andTotal Score. ADHD risk was assessed using Abbreviated Conner’s Rating Scale (ACRS). Statistical analysisusing One-Way ANOVA and Eta test, with a value of p< 0.05 considered as significant.Results: Total of 37 children included. Based on immunotherapy duration 0-6 months, 6 months-1 year, 1-2years, and 2-3 years, prevalence risk of ADHD are 20.6%, 15.4%, 12.5%, and 12.5%, and prevalence ofpsychosocial emotion disorder risk are only in immunotherapy duration 6 months-1 year 12.5%. There wereno correlation between immunotherapy duration with risk of psychosocial emotion disorder (p = 0.945) andsignificantly correlated to ADHD (p = 0.049, r = 0.326).Conclusion: Prevalence risk of ADHD decrease as the immunotherapy duration increase and immunotherapyduration weakly correlated with risk of ADHD.

Abstract

Leprosy is a chronic infection which is caused by Mycobacterium leprae. Leprosy has long been considereda complex disease, in which host and environmental characteristics are as important as the pathogen todetermine disease outcome. Leprosy is an excellent model for examining the immunoregulatory functions.Children are believed to be the most vulnerable group to leprosy infection due to their immature immunity.This review is aimed to summarize the findings of previous studies about immune dysregulation in childhoodleprosy. This study highlighted the plausible maternal-fetal connection which are related in immunedysregulation in childhood leprosy.

Abstract

Background: Leprosy is a chronic infection caused by Mycobacterium leprae. In endemic locations,children become vulnerable as a result of being continuously exposed to foci of active transmission from avery young age, especially from their mothers. This study aims to find association between environmentalrisk factors and leprosy in mothers and children in endemic areas.Methods: This study is a case-control study in endemic areas in Tuban Regency, East Java Province,Indonesia. Retrieval of data was done using structured questionnaire and direct measurement. Chi-squarewas used to assess the association between environmental factors and leprosy in mothers and children groups.Results: 22 pairs of cases and 57 pairs of controls were examined. It is found that access to clean wateris associated with leprosy in mothers in endemic areas (p value = 0.047, OR 3.080, CI 95% 2.232-4.251).Environmental factors are not associated with pediatric leprosy in endemic areas.Conclusion: The results of our study show that environmental factors are not associated with pediatricleprosy in endemic areas. Access to clean water is associated with leprosy in mothers in endemic areas.

Abstract

Leprosy remains endemic in several country, where the disease is still considered as a health burden. The development of the disease is determined by several factors, amongst which that play significant role are close household contact and impaired immunity. Maternal index case significantly associated with leprosy case in children and nutritional status plays a pivotal role in shaping the immune response against the bacteria Mycobacterium leprae. Thus, this paper aims to evaluatethe association between nutritional status and leprosy, especially in maternal and child leprosy. The study was conducted in Tuban, Indonesia. We foundsignificant difference in haemoglobin, red blood cells, and haematocrit levels in subject with maternal leprosy in the group of child without leprosy and leprous mother compared to the control group. The difference in haemoglobin and haematocrit level are also associated with child leprosy in the group of child with leprosy and mother with leprosy.In addition, although no significant association on BMI were observed, we found that the child whose mother contracted leprosy has a lower BMI compared to the other groups
