Abstract

Health is a basic need of every individual which is an important part in improving the quality of human life. The quality of health facility services is one of the main indicators in assessing the success of a country's health care system. Good services not only contribute to improving the health status of the community, but also reflect the effectiveness of the management system and health policy as a whole. Good service quality will have a direct impact on increasing public trust and the effectiveness of patient handling. This study used a quantitative approach with descriptive analytic research. The number of samples determined based on the Slovin formula with a margin of error of 5% amounted to 100 respondents. Based on the results obtained, the variables of officer attitude, availability of medical personnel, and infrastructure have a significant influence on improving service quality (p < 0.05). The variable waiting time has an influence, but is not statistically significant (p> 0.05). Thus, improving service quality at the Karang Anyer Health Center can focus on developing the attitude and competence of officers, equitable distribution of medical personnel, and providing more patient-friendly service facilities.

Abstract

Failure to adhere to the prescribed diet can contribute to a rise in hypertension cases. A key preventive measure for hypertension is maintaining a balanced diet, which necessitates sufficient knowledge about nutrition, particularly in relation to hypertension. The degree of compliance with the recommended diet significantly influences the occurrence of hypertension. Therefore, the objective of this study was to assess the impact of nutritional counseling on the knowledge and adherence to the hypertension diet among hospitalized patients at Sibuhuan General Hospital in Padang Lawas Regency. This research utilized a Quasi-Experimental design with a one-group pre-test and post-test approach. The study's population consisted of 80 hypertensive patients admitted to Sibuhuan General Hospital. A sample of 36 individuals was selected based on specific inclusion and exclusion criteria, and data were collected using questionnaires. The findings revealed a notable effect of nutritional counseling on improving both knowledge and dietary adherence, with a significance value of 0.000 (p-value = 0.000 < 0.05). In conclusion, nutritional counseling effectively enhances both knowledge and compliance with the hypertension diet among patients. It is strongly recommended that healthcare providers take more initiative in spreading information about hypertension.

Abstract

Background: Stunting remains a major public health issue in Indonesia, with long-term effects on children’s development. Nutrition education for pregnant women is a crucial early intervention strategy for stunting prevention. Objective: This study aimed to analyze the effectiveness of nutrition education by health workers in improving pregnant women’s knowledge and attitudes toward stunting prevention. Research Methods: A quantitative study with a cross-sectional approach was conducted among 96 pregnant women in the Lubuk Pakam Health Center area. Data were collected through questionnaires and analyzed using chi-square tests and multivariate logistic regression. Results: Most participants received good nutritional education (60.42%), had high knowledge (54.17%), and showed positive attitudes (64.58%). Good nutrition education significantly increased the odds of having high knowledge (OR = 2.90; p = 0.004) and positive attitudes (OR = 3.29; p = 0.002). Conclusion: Nutrition education is effective in improving knowledge and attitudes of pregnant women regarding stunting prevention. Structured and accessible nutrition education should be strengthened in antenatal care services.

Abstract

Background: Medical records play a crucial role in supporting safe, high-quality, and continuous healthcare services. However, the quality of medical record services at many healthcare facilities, including Granmed Hospital Lubuk Pakam, still faces various challenges.Objective:This study aims to analyze the factors affecting the quality of medical record services, including individual factors (knowledge, perception, workload), organizational factors (policies, training, supervision), and technological factors (facilities and infrastructure). Research Methods: A quantitative analytic observational study with a cross-sectional design was conducted. A total of 75 healthcare and administrative workers involved in medical record management were selected using total sampling. Data were collected through structured questionnaires and medical record audits, then analyzed using chi-square tests and multiple logistic regression. Results: There were significant relationships between knowledge (p=0.002), workload (p=0.015), training and supervision (p=0.049), and availability of facilities and infrastructure (p=0.022) with the quality of medical record services. Most respondents reported a heavy workload and inadequate facilities.Conclusion:Knowledge, workload, training-supervision, and facility availability significantly influence the quality of medical record services. Efforts to strengthen human resources, manage workloads better, enhance supervision, and improve infrastructure are essential to enhance the quality of medical record documentation.

Abstract

Background: Stunting remains a serious public health issue in Indonesia, with a prevalence rate of 24.4% according to SSGI 2021. Specific nutrition interventions require strong managerial capacity among nutrition personnel. Objective: This study aimed to analyze the relationship between the managerial capacity of nutrition personnel and the achievement of specific nutrition interventions at community health centers (puskesmas) in Deli Serdang Regency. Research Methods: This was a quantitative study with a cross-sectional design. A total of 41 nutrition personnel were selected using proportional random sampling. Data were collected through structured questionnaires and reports on specific nutrition interventions, and analyzed using chi-square tests with a significance level of p<0.05. Results: Most nutrition personnel (63.4%) demonstrated good managerial capacity, and 68.3% achieved high outcomes in specific nutrition interventions. Statistical analysis showed a significant relationship between managerial capacity and intervention achievement (p=0.001), where those with better managerial skills achieved higher intervention targets.Conclusion: Managerial capacity significantly influences the achievement of specific nutrition interventions for stunting prevention. Strengthening managerial skills through continuous training is essential to enhance the effectiveness of nutrition programs at the primary healthcare level.

Abstract

Background: Healthcare quality is a key indicator in assessing the performance of health systems worldwide, and medical record management plays a crucial role in achieving high-quality healthcare. Objective: This study aims to analyze the impact of the implementation of Ministry of Health Regulation No. 24 of 2022 on medical records on healthcare quality. Research Methods: This study employs a cross-sectional design with a sample of 120 healthcare workers. Data were collected through questionnaires and medical record audits, then analyzed using univariate analysis for data description and bivariate analysis with Chi-Square. Results: Most respondents at RS Grandmed Lubuk Pakam were aware of Ministry of Health Regulation No. 24 of 2022 (85%), but only 72% felt its implementation was effective. Healthcare quality was generally satisfactory (83.3%), though there were some errors in medical records (11.7%). The analysis revealed a significant relationship between the implementation of the regulation and healthcare quality (? = 0.478, p = 0.002). Training for healthcare workers and available infrastructure had a significant impact on healthcare quality, while work experience did not. Conclusion: The implementation of Ministry of Health Regulation No. 24 of 2022 at RS Grandmed Lubuk Pakam has a positive impact on healthcare quality, despite challenges related to infrastructure and training. The policy has improved patient satisfaction but requires further improvements in facilities, training, and time management for greater effectiveness. Routine evaluation is necessary to ensure the continued enhancement of healthcare quality.

Abstract

Background: Tuberculosis (TB) remains a major health issue in Indonesia, closely linked to environmental risk factors such as poor ventilation and inadequate sanitation. Objective: This study aimed to analyze the implementation of environmental risk factor monitoring in TB control efforts at Hutabaginda Health Center, North Tapanuli District. Research Methods: a qualitative approach with a case study design was used. Data were collected through in-depth interviews, participatory observations, and document reviews, and analyzed thematically with data triangulation. Results: The study found that although policies for controlling environmental risks exist, implementation faces challenges including limited trained personnel, inadequate sanitation facilities, weak supervision, and suboptimal coordination with local governments. Conclusion: the health service management system at Hutabaginda Health Center has not yet been optimal in controlling environmental risk factors for TB. It is recommended that the health center strengthen healthcare worker training, improve sanitation facilities, enhance internal supervision, and intensify cross-sectoral coordination to improve TB control effectiveness.

Abstract

Patient safety is a crucial indicator of healthcare service quality. In Indonesia, inpatient units are among the most vulnerable areas for patient safety incidents due to the complexity of medical procedures and prolonged patient contact. Risk assessment plays a key role in minimizing adverse events by identifying potential hazards, evaluating risk levels, and implementing preventive measures. This study aims to analyze the relationship between the implementation of risk assessment and the reduction of patient safety incidents in the inpatient department of RSU Sembiring Delitua. A cross-sectional quantitative study was conducted in April–June 2025, involving 50 healthcare workers selected through total sampling. Data were collected using validated questionnaires and secondary data from the hospital's Patient Safety Committee. Univariate and bivariate analyses (Chi-Square test) were used. The results showed that 76% of respondents routinely identified risks, 70% actively reported incidents, and 66% followed up on assessments. Incident data from 2023 to 2024 showed a 43.75% reduction in total incidents, particularly medication errors and patient falls. The Chi-Square test revealed a significant relationship between risk assessment and the incidence of patient safety events (p = 0.018). The study concludes that effective risk assessment significantly contributes to reducing patient safety incidents. This finding emphasizes the need for continuous risk management practices, strong institutional support, and the cultivation of a safety culture across all levels of hospital services.

Abstract

Background: The management of hazardous chemical waste in healthcare facilities largely depends on human resource preparedness and the effectiveness of internal policies. Objective: This study aimed to evaluate the preparedness of human resources and internal policies in supporting hazardous chemical waste management at RS Grandmed Lubuk Pakam. Research Methods: This quantitative study with a cross-sectional design involved 44 respondents selected through stratified random sampling. Data were collected via structured questionnaires and document review, and analyzed using Spearman correlation and multiple logistic regression. Results: a total of 63.6% of respondents were categorized as prepared in hazardous waste management, and 56.8% perceived the hospital's internal policies as good. Bivariate analysis showed significant relationships between human resource preparedness (r=0.621; p=0.000) and internal policies (r=0.587; p=0.000) with waste management compliance. Multivariate analysis indicated that human resource preparedness (OR=6.65) and internal policies (OR=4.68) significantly affected compliance. Conclusion: Human resource preparedness and internal policies significantly influence compliance with hazardous chemical waste management; thus, continuous training, strengthened policy dissemination, and regular monitoring are recommended to enhance hospital waste management practices.

Abstract

Background: Adverse drug reactions (ADRs) are a serious concern in drug use, potentially endangering patient safety if not properly monitored. Objective: This study aimed to determine the incidence of ADRs and identify the drugs causing ADRs among hospitalized patients at RSUD Drs. H. Amri Tambunan, Deli Serdang Regency. Research Methods: a descriptive non-experimental study with a retrospective approach was conducted. Data were collected from medical records between September 2022 and February 2023. Analysis was performed descriptively using the Naranjo scale. Results: a total of 139 suspected ADR cases were identified. Based on the Naranjo scale, 1 case (0.72%) was definite, 86 cases (61.87%) were probable, and 52 cases (37.41%) were possible ADRs. Antibiotics (61.87%) were the most common drug class causing ADRs, with ciprofloxacin and levofloxacin being the main drugs. The most common ADR manifestation was redness (23.74%). Dexamethasone (41.73%) was the most frequently administered medication to treat ADRs. Conclusion: The incidence of ADRs at RSUD Drs. H. Amri Tambunan remains relatively high, predominantly linked to antibiotic use. Strengthening the adverse drug reaction monitoring system through enhanced reporting, healthcare worker training, and regular antibiotic use evaluations is crucial to improving patient safety.
