Abstract

Pulmonary tuberculosis (TB) remains a significant health problem in Indonesia due to its contagious nature. Having an adequate understanding of TB is very important, especially for students in health education institutions, considering their potential role as pioneers in efforts to prevent and control this disease. This study was conducted to examine the level of knowledge of students regarding pulmonary tuberculosis at the Medistra Health Institute, Lubuk Pakam. This study used a quantitative descriptive method with a cross-sectional approach. A total of 100 students were used as research samples selected through purposive sampling techniques. Data collection was carried out using a closed questionnaire that had undergone validity and reliability tests. The data were then analyzed descriptively with a frequency distribution approach. The results showed that the majority of students, namely 65%, had a good level of knowledge about pulmonary TB. Meanwhile, 25% of respondents had knowledge in the sufficient category, and 10% were classified as lacking. The knowledge that was most widely mastered included the symptoms and transmission of TB, while aspects of prevention and treatment still need to be improved. The conclusion of this study shows that students at the Medistra Health Institute, Lubuk Pakam, generally have good knowledge regarding pulmonary TB. However, increased education on certain aspects is still needed. The results of this study are expected to be a reference for institutions in designing more comprehensive health promotion and education programs related to tuberculosis.

Abstract

Background: The prevalence of Diabetes Mellitus in Aceh has increased, reaching 1.68% in 2018 compared to 1.3% in 2013. In Bireuen District, the number of diabetes mellitus patients receiving healthcare services also rose in 2022, with 11,300 patients compared to 11,218 in 2021 (Riskesdas, 2018).Objective: This study aims to evaluate food service in relation to the satisfaction of diabetes mellitus patients. Research method: This research is a descriptive qualitative study. Informants include the head of the nutrition installation, ward nutritionists, food service staff, DM patients, and their families. Data were collected through a combination of in-depth interviews, focus group discussions, and observations. Data analysis was conducted manually using content analysis without software, and data validity was ensured through triangulation. Results: The average food waste of patients remains above 20%. Although the prescribed diet is generally appropriate, changes often occur based on doctors' recommendations. Delays in meal delivery still occur, especially during menu cycles involving chicken and meat. Menu variations follow a 10-day cycle; however, some ingredients are occasionally unavailable due to stock shortages.Conclusion: The satisfaction of DM patients with food service at RSUD dr. Fauziah, Bireuen District, still needs improvement, particularly regarding high food waste, delays in meal delivery, limited availability of ingredients in the menu cycle, and suboptimal communication by food service staff with patients.

Abstract

Background: The implementation of hand hygiene (HH) is an essential aspect of healthcare management, influencing the health of both healthcare workers and the general public. Objective: This study aims to identify the factors associated with compliance in the implementation of hand hygiene among healthcare workers. Research Method: This research is a quantitative study using a cross-sectional design, conducted at Puskesmas Sialang, Bangun Purba District, Deli Serdang Regency. Results: The analysis showed that several factors had a significant relationship with hand hygiene compliance among healthcare workers, including knowledge (p-value = 0.010), attitude (p-value = 0.022), availability of facilities (p-value = 0.009), and supervision by the Infection Prevention and Control (IPC) team (p-value = 0.000). Conclusion: There is a significant influence of knowledge, attitude, facility availability, and IPC team supervision on hand hygiene compliance among healthcare workers at Puskesmas Sialang. Therefore, it is recommended to conduct awareness campaigns on hand hygiene and the associated infection risks, establish a reward and recognition system for healthcare workers who consistently practice proper hand hygiene, ensure the availability of hand hygiene facilities in easily accessible locations for healthcare workers, and enhance communication between the IPC team and healthcare workers regarding hand hygiene practices.

Abstract

Background: RSUD dr. H. Kumpulan Pane (RSKP) has not yet successfully implemented real-time incident reporting as stipulated by the Minister of Health Regulation No. 11 of 2017. Objectives: Based on this situation, this study aims to analyze the condition of the patient safety incident reporting system in the inpatient unit at RSKP, identify the inhibiting factors, and propose priority policy alternatives to improve the system. Research method: To achieve these objectives, the Grounded Theory method, emphasizing qualitative data analysis from informants including structural officials, healthcare workers, and the quality and patient safety committee at RSKP, was used to reveal the conditions and inhibiting factors of patient safety incident reporting in the inpatient unit at RSKP. Meanwhile, to obtain priority alternatives for improving the incident reporting conditions, the researcher used the Analytical Hierarchy Process method. Results: The results of the study indicate several weaknesses in the reporting conditions, including very minimal incident reporting actions, incomplete implementation of system-oriented approaches, lack of coordination regarding assurance of immunity from punishment for incident reporting actions, unguaranteed independence from external parties, and partial patterns of patient safety incident resolution.  Conclusion: Patient safety incident reporting in the inpatient unit of RSKP remains suboptimal due to a weak reporting culture, insufficient training, unsupportive leadership, and an underdeveloped system. Strategic actions such as timely disbursement of service incentives, the establishment of a dedicated SPO improvement team, and the implementation of proportional sanctions are essential to enhance the overall quality of incident reporting.

Abstract

Background: The Gerakan Sayang Ibu (GSI) is a movement implemented by the community in collaboration with the government to improve the quality of life for women, particularly by accelerating the reduction of maternal mortality during pregnancy, childbirth, and postpartum (Manuaba, 2019).Objectives: The study aims to explore the implementation of the Community-Based Sanitation Program’s first pillar in Sipahutar Subdistrict, North Tapanuli Regency. Research method: This research is a qualitative study with a descriptive approach. Results The results of this study indicate that the communication and dissemination of policy information from policymakers to policy implementers are carried out in a structured manner.  Conclusion: Based on the research conducted in Sipahutar Subdistrict, it can be concluded that the implementation of the Gerakan Sayang Ibu program is structured, supported by adequate facilities, village funds, and clear Standard Operating Procedures (SOPs). However, the main challenges include a lack of human resources, low participation of pregnant women due to difficult access, limited family education, and insufficient health budgets. Despite these challenges, cross-sectoral coordination demonstrates a strong commitment to reducing maternal and newborn mortality risks.

Abstract

Undernutrition among toddlers is a major public health issue in Indonesia, as it directly affects children's physical growth, cognitive development, and quality of life in the future. This study aims to evaluate the effectiveness of a Supplementary Feeding Program (PMT) using locally sourced food ingredients as a form of nutritional intervention for undernourished toddlers in the working area of Puskesmas Nelayan, Gresik Regency. A quantitative research design with an analytic experimental approach was applied. The study involved 56 toddlers aged 15–56 months who were identified as undernourished based on the weight-for-age (W/A) indicator. The intervention lasted for 56 days and included a five-day cycle menu composed of staple foods and snacks made from local ingredients rich in carbohydrates, animal and plant-based proteins, vitamins, and minerals. The intervention was closely monitored by nutritionists, midwives, and community health volunteers. Statistical analysis using a paired sample t-test showed a significant change in Z-scores before and after the intervention (p = 0.000), indicating that the locally based PMT was effective in improving the nutritional status of the toddlers. In addition to weight gain, the program also enhanced parental knowledge about balanced nutrition and the use of local food ingredients in daily menu planning. Thus, the PMT program not only had a physical impact on toddlers but also served as an educational tool that empowered families to adopt healthy eating habits sustainably at home.

Abstract

Congenital heart disease can be experienced early in life. This disorder can cause growth disorders in children, one of which is malnutrition. Malnutrition can be caused by food intake that does not meet the needs and can be influenced by chronic diseases. This study aims to determine the results of the implementation of a standardized nutritional care process in pediatric patients with OBS dyspnea, VSD, Pneumonia, FTT, Malnutrition. This case study was conducted for 3 days on October 01, 2024 - October 03, 2024 located at z hospital in Surabaya. this research method uses an analytic observational design on outpatients.  The nutritional care process begins with an assessment based on medical record data and interviews consisting of food intake, anthropometer, biochemical, and physical/clinical data. Followed by diagnosis, intervention, monitoring and evaluation. The results of this study showed an increase in the patient's food intake but still did not meet the patient's needs, the patient's physical/clinical examination improved but the patient's airway was still ineffective. The conclusion of the study is that the management of the patient's nutritional care process can optimize the patient's condition but still does not reach the target needs.

Abstract

Cognitive function is related to the brain's ability to obtain, retain, and understand the information obtained. Decreased cognitive function can have an impact on a person's activities. Impaired cognitive function generally occurs with increasing age. Several studies show the effect of giving lutein as an antioxidant that can maintain brain health. Several studies have stated that there is a link between the body's plasma lutein levels and better cognitive function. This study was carried out with the aim of analyzing research results related to intervention of lutein and its isomers on cognitive function in adulthood. The literature analyzed is publications in the last 10 years with research designs in the form of RCTs and double blinds. Literature search using keywords (lutein or carotenoids) and (brain or cognition or cognitive function). Supplementation is considered to be able to help concentration in answering questions. However, supplementation is also considered to have no significant effect on cognitive function. Through analysis of all literature, it was found that lutein is able to maintain cognitive function in adults, especially memory. Further research is needed regarding lutein supplementation with other substance activators.

Abstract

The medical committee plays an important role in the hospital quality management system, especially in ensuring the professionalism of medical personnel, implementing clinical audits, and formulating service policies. However, the implementation of the role of the medical committee in regional hospitals often faces various structural and operational challenges. This study aims to analyze in depth how the medical committee plays a role in improving the quality of service at Batu Bara Regional Hospital and to identify the obstacles and strategies implemented. This research method uses a qualitative descriptive approach. Data were collected through in-depth interviews with the chairman and members of the medical committee, the hospital director, and medical personnel, documentation studies of policies and committee meeting reports, and direct observation of medical committee activities. The analysis was carried out using a thematic approach and source triangulation techniques to ensure the validity of the data. The results of the study indicate that the medical committee has carried out the functions of credentialing, medical audits, and formulating clinical policies with active involvement. However, the development of medical personnel and monitoring of clinical practices are still not optimal due to limited human resources, minimal ongoing training, and lack of information system integration. The medical committee is also a communication bridge between management and medical staff in clinical decision making. Abstract

Background: Health Human Resources (HRH) include professional health workers, strategic personnel, non-professional staff, and supporting personnel who contribute to healthcare management and delivery to achieve the highest possible standard of public health. The quality of HRH is a key determinant of healthcare service quality, particularly  at  the clinic level. Objectives: To assess the quality of HRH at Santa Maria Palipi Clinic, identify the barriers to improving HRH quality, and describe the efforts made to enhance HRH standards. Research Methods: This study employed a descriptive qualitative approach involving 15 informants: 2 key informants, 6 staff members of Santa Maria Palipi Clinic, and 7 patients or their families. Data were collected through observation and in-depth interviews. Results: The study found that the quality of HRH at Santa Maria Palipi Clinic has not fully met the established standards. Major challenges include difficult-to-change individual characteristics, lack of compliance with existing regulations, limited personnel, and poor collaboration among clinic staff. Efforts to address these issues include regular evaluations and training provided by both the clinic leadership and supporting foundations. Conclusion: Santa Maria Palipi Clinic must continue to improve HRH quality to meet service standards. Strong commitment, continuous training, and better teamwork among health workers are essential strategies for enhancing healthcare service quality.
