Abstract

Background: Occupational Safety and Health (OSH) is a crucial aspect of workforce management to create a safe, healthy, and productivity-supporting work environment. In the healthcare sector, the implementation of OSH is particularly vital, especially for health workers such as nurses who often face various occupational risks. Objective: This study aims to identify the factors influencing the implementation of OSH among nurses in the Emergency Department of Dr. Soedarso Regional Public Hospital. Research Method: This study used an analytical survey method with a cross-sectional approach. The sample consisted of 34 nurses working in the Emergency Department, selected using total sampling technique. Data analysis was performed using the chi-square test and multivariate analysis through logistic regression. Results: The study showed that the predisposing factor of work experience was not significantly associated with OSH implementation (0.300 > 0.05), while predisposing factors such as knowledge (0.007 < 0.05) and attitude (0.037 < 0.05), as well as the enabling factor of facilities (0.037 < 0.05), were significantly associated with OSH implementation. The hospital is expected to select nurses with longer work experience in accordance with their expertise, organize seminars and training, conduct supervision, provide supporting Occupational Safety and Health (OSH) facilities, and carry out regular evaluations. These steps aim to improve the quality of nursing staff and encourage better and more positive OSH implementation in the hospital's emergency department.

Abstract

Background: Immunity is one of the important things for the body's defense against viral and bacterial infections. In controlling the immune system, immunomodulators are needed, which usually come from compounds found in plants.  Immunomodulatory compounds can be found in the leaves of the sambiloto plant. The content of sambiloto leaf compounds in the form of diterpenoids, flavonoids, and polyphenols is proven to be immunostimulants. Objective: This study aimed to determine the immunomodulatory effects of sambiloto leaves. Methods: This study used literature review method on in vivo and in vitro experimental studies that have been conducted.  The article search used online databases Sciencedirect and Pubmed. Results: A total of eight articles were used in this study. Discussion: Based on the literature review, it is proven that sambiloto leaves have compounds that give rise to immunomodulatory effects. Conclusion: According to this study, the leaves of sambiloto (Andrographis paniculata) have the potential to be used as a herbal immunomodulatory medication that has anti-inflammatory and immune-enhancing effects. However, further research is needed to explore the molecular mechanism, more complex clinical trials, determine the optimal dose and dosage form of sambiloto leaves.

Abstract

According to WHO, hospitals provide comprehensive curative and preventive services, including quality medical record (MR) management, assessed by completeness, accuracy, timeliness, and legal compliance. This study aimed to analyze factors related to incomplete MR documentation at Sibuhuan Regional Hospital in 2022 using an analytical observational cross-sectional design with 74 health workers as total respondents. Data were analyzed using binary logistic regression. Results showed that supervision, training, and sanctions were significantly related to incomplete MR filling (p<0.05), while knowledge, attitude, and length of work were not (p>0.05). Sanctions were the most dominant factor. It is recommended that the hospital conducts monthly audits to improve MR documentation completeness

Abstract

The maternal diet during breastfeeding was related to breast milk production and indirectly affected the nutritional status of infants. During the breastfeeding period, maternal nutritional needs increased, making it important to maintain a balanced food intake. The amount and type of food consumed influenced the volume and quality of breast milk. This study aimed to analyze the relationship between maternal diet and the frequency of exclusive breastfeeding in infants in the Pegajahan Health Center work area. The method used was quantitative research with a cross-sectional study approach. The population in this study consisted of 165 mothers who had babies aged 0-6 months, with a sample of 61 mothers selected through purposive sampling techniques. The independent variable in this study was the diet of breastfeeding mothers, while the dependent variable was the frequency of exclusive breastfeeding. Data collection was carried out through interviews and questionnaires, with hypothesis analysis using the chi-square statistical test. The results showed that the majority of mothers were in the age range of 20-30 years (65.57%), had a final education in high school (42.62%), and most of them worked (81.96%). Most mothers had a diet classified as frequent (77.04%), and the frequency of breastfeeding was more than eight times a day (78.68%). Bivariate analysis showed a significant relationship between maternal diet and the frequency of exclusive breastfeeding, with a p-value = 0.000. This study concluded that there was a significant relationship between maternal diet and the frequency of exclusive breastfeeding in mothers at the Pegajahan Health Center, Serdang Bedagai Regency. Consistent efforts by healthcare professionals are essential in delivering nutritional counseling to breastfeeding mothers.

Abstract

Malnutrition in cancer patients undergoing radiotherapy can worsen outcomes. This study analyzed the effect of nutrition counseling on macronutrient intake, BMI, and muscle mass using a randomized controlled trial with intervention and control groups. Measurements were taken before and after intervention using 24-hour dietary recall, BMI, and bioelectrical impedance analysis. Data were analyzed with Wilcoxon Signed-Rank and Mann–Whitney U tests.. The results indicated that nutrition counseling improved nutritional status and muscle mass in intervention group. However, the increases in mean values across all variables were not statistically significant, including energy intake (from 1041.0 ± 359.7 to 1102.6 ± 418.7 kcal; p = 0.406), protein intake (from 40.2 ± 15.4 to 40.1 ± 18.1 g; p = 0.977), BMI (from 24.2 ± 4.6 to 24.7 ± 4.6; p = 0.087), as well as muscle mass across various body segments such as whole body (from 23.9 ± 4.7 to 24.2 ± 3.3 kg; p = 0.832), leg (from 36.6 ± 7.2 to 37.5 ± 5.2 kg; p = 0.221), arm (from 26.1 ± 6.6 to 27.0 ± 5.7 kg; p = 0.194), and trunk (from 18.2 ± 4.4 to 18.4 ± 3.4 kg; p = 0.769).Further research with a more robust study design and a multimodal approach is recommended to optimize nutritional interventions in cancer patient care.

Abstract

Background: Cadre performance reflects the effectiveness of cadres in fulfilling their assigned duties and responsibilities. Poor compliance of mothers of toddlers in attending Posyandu and participating in growth and development monitoring has led to two cases of malnutrition, 20 cases of severe malnutrition, and 10 cases of stunting. Improving maternal compliance can help address these issues. Objective: This study aimed to analyze the relationship between health cadre performance and maternal compliance with Posyandu visits and toddler growth and development monitoring. Methods: The study involved 86 mothers of toddlers selected through purposive sampling. A quantitative analytical design was used, with data collected via a validated and reliable questionnaire. Data were analyzed using factor analysis techniques. Results: There was a significant relationship between health cadre performance and maternal compliance with Posyandu visits (p < 0.05), as well as a significant relationship between cadre performance and growth and development detection among toddlers in the working area of Pon Village Health Center, Sei Bamban District, Serdang Bedagai Regency, in 2025 (p < 0.05). Conclusion: Health cadre performance is significantly associated with maternal compliance to Posyandu visits and toddler growth and development detection. It is recommended that cadres enhance service quality, particularly in monitoring growth and development through routine child monitoring and providing education on child health and the risks of malnutrition.

Abstract

Background: Protein intake is essential for maintaining muscle mass, accelerating postoperative recovery, and improving chemotherapy tolerance in ovarian cancer patients. However, few reviews have specifically addressed its impact on prognosis and quality of life. Objective: To review scientific evidence on the effect of protein intake in ovarian cancer patients. Methodology: Literature was searched through PubMed, Scopus, and Google Scholar using keywords such as protein intake, ovarian cancer, muscle mass, fatigue, and quality of life. Studies were selected based on clear methodology and relevance of findings. Results: Protein consumption ?1.5 g/kg body weight/day was associated with increased muscle mass and reduced muscle loss during chemotherapy. Protein supplementation before and after surgery supported gastrointestinal recovery and shortened hospital stay. Adequate protein intake also enhanced immune function and reduced the risk of fatigue and malnutrition. Patients with higher protein intake showed better progression-free survival outcomes. Discussion: Sufficient protein intake supports physical recovery and therapy tolerance in ovarian cancer patients. Nutritional interventions, including protein supplementation, may serve as an important part of comprehensive cancer management. Conclusion: Adequate protein intake plays a significant role in improving clinical outcomes and quality of life for ovarian cancer patients. Nutrition should be personalized as part of cancer care. Recommendation: Nutritional assessment and planning for protein intake should be integrated into the care protocols for ovarian cancer patients, particularly during the perioperative and chemotherapy phases. Collaboration among physicians, dietitians, and oncology teams is essential.

Abstract

This research explores the determinants of adherence to antihypertensive medication among patients at the Primary Care Clinic Polkes 01.10.02 Lubuk Pakam. Hypertension refers to a consistent elevation in blood pressure, typically above 140/90 mmHg. Clinical records indicated a growing number of hypertension cases: 1,001 in 2021, 1,115 in 2022, and 1,074 in 2023. The highest recovery rate was recorded in 2022, with 1,056 patients showing improvement after undergoing treatment. A quantitative approach was adopted, utilizing an analytical survey with a cross-sectional design. A total of 95 hypertension patients were selected purposively from the Pegasing Health Center. Data analysis included univariate, bivariate, and multivariate techniques. The study identified significant associations between medication adherence and several factors: level of education (p = 0.000), patient knowledge (p = 0.000), distance to healthcare facilities (p = 0.000), family involvement (p = 0.000), length of time on medication (p = 0.000), and health education received (p = 0.005). Among these, the duration of medication use emerged as the most influential factor (p = 0.017; 95% CI = 2.088–1614.65). The findings suggest that these six factors play a critical role in patients' consistency with hypertension therapy. Strengthening healthcare personnel's involvement and implementing strategic interventions is advised to enhance adherence and lower hypertension rates

Abstract

The JKN mobile application is a digital transformation of the BPJS health business model which originally consisted of administrative activities carried out at branch offices or health facilities. Now, participants can use this application wherever and whenever they want (self service). By using the JKN mobile application, the public and participants can obtain many conveniences, such as ease of paying and changing membership data, ease of accessing information about family participant data. The aim of this research is to implement health services based on the JKN mobile application to reduce queues at the Karang Anyer Health Center. The type of research taken is qualitative research. The subjects in this study were 5 people. Meanwhile, data collection techniques use observation, interview and documentation techniques. The research results show that the National Health Insurance (JKN) mobile application at the Karang Anyer Community Health Center is very effective and efficient in helping patients with registration, referrals and doctor consultations at the Community Health Center. Conclusion By using this application, patients can speed up the registration and referral process and get information more quickly. Patients can also make direct referrals to specialist facilities, which saves time and speeds up the service process. The JKN mobile application for doctor consultations can also be accessed directly by patients via the application. This helps doctors plan their schedules better.

Abstract

Surveillance of Health Care Associated Infections (HAIs) is a dynamic and continuous process of collecting, identifying, analyzing, and interpreting health data. This study aims to analyze the HAIs surveillance system based on input, process, and output components using a mixed-method approach. Phase I (quantitative) assessed input (human resources and infrastructure) and output (epidemiological data and HAIs incidence rates) using secondary data analyzed univariately. Phase II (qualitative) explored input and process components through interviews and FGDs with the IPC team (IPCN and IPCLN), analyzed using thematic analysis. Results from Phase I showed the highest HAIs cases in patients over 41 years, with VAP incidence in February (32.5 ‰) exceeding the standard. Other peaks included non-SC IDO in February (1.58 ‰), SC IDO in April (5 ‰), and phlebitis in March (1.11 ‰). Phase II revealed issues in HR quality, incomplete SOPs, limited infrastructure, and lack of indicators to assess data completeness and accuracy
