Abstract

Background: Community Health Centers (Puskesmas) play a crucial role in providing basic health services. However, the work motivation of health workers is often affected by high workloads, limited infrastructure, and the complexity of cases handled. Objective: This study aimed to analyze the influence of transformational leadership style and interpersonal competence on the work motivation of health workers at Bandar Khalifah Health Center, Percut Sei Tuan District. Research Methods: A quantitative approach with a correlational design was used. A total of 40 health workers were selected using a total sampling technique. Data were collected through a Likert-scale questionnaire and analyzed using Pearson correlation and multiple linear regression with SPSS software. Results:The results showed a significant positive influence between transformational leadership style and work motivation (r = 0.650; p < 0.05) as well as between interpersonal competence and work motivation (r = 0.680; p < 0.05). Regression analysis indicated that transformational leadership (? = 0.480; p = 0.000) and interpersonal competence (? = 0.420; p = 0.004) significantly contributed to improving health workers' work motivation.Conclusion:Transformational leadership style and interpersonal competence significantly influence the improvement of health workers' work motivation. Strengthening these factors is recommended to enhance performance and the quality of health services.

Abstract

Background: Improving the quality of services in the health sector, especially in hospitals, has become one of the main concerns in various countries, including Indonesia. Quality health services are essential in creating patient satisfaction and improving the effectiveness of the health care system as a whole. Objective: This study aims to determine the extent of the relationship and influence between the variables of quality management system implementation on service quality at RSUD Dr. Pirngadi Medan City. Methods: This research is a quantitative study with an associative approach, associative research is used to test the cause-and-effect relationship between two or more variables through statistical analysis. The sample amounted to 86 respondents. Data were collected using a structured questionnaire and the outcome report was analyzed with the chi-square test at a significance level of p<0.05. Research Results: the implementation of the quality management system has a positive and significant effect on service quality at RSUD Dr. Pirngadi Medan City. The hospital needs to continue to improve the implementation of the quality management system consistently, especially in the aspects of HR training and the implementation of continuous quality audits, in order to improve the quality of services received by patients. Conclusion: hospitals need to pay attention to improving physical facilities, such as the cleanliness of treatment rooms and the provision of more modern medical equipment. With these improvements, hospitals can improve the tangible dimension of service quality, which in turn will increase overall patient satisfaction.

Abstract

Background: The accuracy of prescription services is a key indicator of hospital pharmacy service quality. One of the factors influencing service timeliness is effective drug inventory management. Objective: This study aimed to analyze the effect of drug inventory management on the accuracy of prescription services at the Pharmacy Installation of Grandmed Lubuk Pakam Hospital. Research Methods: This research employed a quantitative approach with a descriptive-analytic design. Samples were randomly selected prescriptions served. Data were collected through direct observation, interviews, and document analysis, and analyzed using simple linear regression. Results: The average prescription service time for non-compounded drugs was 40 minutes and 75 minutes for compounded drugs, exceeding the standards set by the Ministry of Health. Regression analysis showed that drug inventory management significantly influenced prescription service timeliness, with a regression coefficient of 0.45 (p=0.002). Conclusion: Optimal drug inventory management contributes to the acceleration of prescription service time. It is recommended that the hospital improve the stock monitoring system, optimize procurement processes, and strengthen interdepartmental coordination to support pharmacy service efficiency.

Abstract

The performance of posyandu cadres reflects the results of the implementation of tasks and responsibilities carried out in health service activities for the elderly. Optimal performance plays an important role in creating satisfaction for the elderly as recipients of posyandu services. This satisfaction reflects the expectations and perceptions of the elderly regarding the quality of services provided by cadres during the activities. This study aims to evaluate the quality of posyandu cadre performance and its relationship to the level of satisfaction of the elderly in the work area of ​​the PON Village Health Center. This study used an associative design with a quantitative approach, involving 134 elderly respondents as samples. The instrument used in data collection was a structured questionnaire. The results of the analysis showed that the cadre performance variable consisted of two main factors, namely attitude and activity. Meanwhile, the elderly satisfaction variable was formed from three factors, namely reliability, empathy, and the suitability of expectations with reality. Statistical analysis showed a significant relationship between cadre performance and the level of satisfaction of the elderly, with a P value of 0.000 (p <0.05). Based on these findings, it is recommended that posyandu cadres continue to improve their productivity and quality of service. Cadres are also expected to actively participate in coaching programs from the Health Center and be more optimal in conveying information to the elderly during the implementation of integrated health post activities.

Abstract

This study aims to identify and analyze the factors influencing the low absorption rate of the General Allocation Fund (DAU) for the health sector by the Bengkalis Regency Health Office in 2024. The research employed a qualitative approach using data collection methods such as direct observation, interviews, and document analysis. Data were analyzed using a Fishbone diagram to identify root causes, the USG (Urgency, Seriousness, Growth) method to determine problem priorities, and the Delphi method to formulate solutions. The findings revealed that by the third quarter of 2024, DAU absorption was only 58%, far below the target. The main issues included poor inter-sectoral coordination, administrative delays, and limited human resource capacity. Proposed strategies include developing SOPs, establishing cross-sectoral teams, and implementing technology-based monitoring systems to enhance efficiency, transparency, and accountability. This study provides practical recommendations for optimizing DAU absorption to support better public health services.

Abstract

Contact dermatitis is often associated with work and is also referred to as occupational contact dermatitis. About 90% of occupational dermatitis is contact dermatitis. This disease attacks workers who are often exposed to toxic or allergic substances, including workers in the agricultural and plantation sectors. The process of planting oil palm seeds until the harvest stage causes workers to be exposed to many chemicals in the form of pesticides, which is one of the causes of occupational skin diseases. This study aims to analyze how high the incidence of contact dermatitis is and what the risk factors are among employees of PT Perkebunan Nusantara 1 Langsa. This study is a quantitative research with a cross sectional design. The population in this study was all 47 employees of Afdelling 1 PTP Nusantara 1 Langsa, and the research sample was used as total sampling. Data collection was carried out directly by distributing observations and distributing questionnaires to employees. Then, continue with the data analysis process using the Chi Square test and logistic regression. The results showed that there were length of work (p=0.000, PR=3.4; 95% CI 2.201-5.294), use of PPE (p=0.006, PR=2.6; 95% CI 1.197-5.676), personal hygiene ( p=0.000, PR=5.2; 95% CI 1.816-14.906) and family history of allergies (p=0.000, PR=3.2; 95% CI 1.615-6.430) against contact dermatitis. Furthermore, the dominant factors related to the incidence of contact dermatitis through the binary logistic regression test were the variables length of work and personal hygiene, where length of work increased the risk of contact dermatitis 7 times and personal hygiene increased the risk of contact dermatitis 6 times.

Abstract

Bronchopneumonia is an inflammation of the lung parenchyma that extends to the bronchioles. This condition occurs due to infection that spreads directly through the respiratory tract or bloodstream, causing the accumulation of inflammatory exudate in and around the alveoli that interferes with normal gas exchange and has the potential to trigger serious respiratory disorders. In Indonesia, in 2022, bronchopneumonia was recorded as the leading cause of death for toddlers aged 12-59 months. Risk factors such as nutritional status are known to play an important role in the increasing incidence and mortality due to bronchopneumonia. This study aims to analyze the relationship between nutritional status and the incidence of bronchopneumonia in toddlers at the Cut Meutia Hospital, North Aceh in 2023-2024. This study is an observational analytical study with a cross-sectional design. Data were obtained from medical records of 65 toddlers with bronchopneumonia collected using the total sampling technique. The results of this study showed that the majority of sufferers were female, with a median age of toddlers with bronchopneumonia being 16 months. The median weight of toddlers was 8,6 kg and the average height reached 73,37 cm. Most toddlers affected by bronchopneumonia had good nutritional status and mild disease severity. Statistical analysis using the Chi-Square test showed a p value <0,05, indicating a significant relationship between nutritional status and the incidence of bronchopneumonia in toddlers at the Cut Meutia Hospital, North Aceh in 2023-2024. The results of this study indicate that monitoring nutritional status in toddlers is very important to reduce the severity of bronchopneumonia. Therefore, health intervention programs, such as nutritional education for parents and the provision of balanced nutritious food, need to be intensified to reduce the severity of bronchopneumonia.

Abstract

Radiotherapy installations are among the high-risk healthcare service units due to the use of ionizing radiation in cancer treatment. Evaluating the implementation of risk management is essential to ensure the safety of patients, healthcare workers, and the surrounding environment. This study aims to evaluate the extent to which risk management has been implemented in the Radiotherapy Installation at Grandmed Hospital Lubuk Pakam in 2023. A descriptive qualitative method was used through observations, interviews with healthcare personnel, and document analysis of standard operating procedures (SOPs) and incident reports. The results of the study indicate that risk management practices have been implemented, but not yet optimally, particularly in aspects such as incident reporting and regular training. Strengthening the reporting system, continuous training, and internal audits are necessary to improve the effectiveness of risk management in the facility.This study employed a descriptive approach using qualitative methods. Data were collected through in-depth interviews with radiotherapy unit staff, direct observations, and document reviews related to the risk management system. The evaluation was conducted based on the ISO 31000 risk management standard and guidelines from the Nuclear Energy Regulatory Agency (BAPETEN).The evaluation results indicated that most hospitals have implemented the basic components of risk management, such as hazard identification, risk analysis, and risk control. However, it was found that the processes of risk monitoring and evaluation have not been carried out systematically. In addition, not all installations had complete risk documentation or conducted regular training updates.The implementation of risk management in radiotherapy installations is at a promising early stage, but improvements are still needed, particularly in the areas of documentation and the strengthening of safety culture. Strong management commitment and ongoing training are essential to enhance the effectiveness of the risk management system to ensure the safety of both patients and medical personnel

Abstract

Malaria remains a global health problem and affects many tropical countries. Indonesia ranks second in Asia with the highest number of deaths from malaria. The Karo tribe has a kinship system of rakut si telu orat si waluh which is projected to be a community asset in implementing malaria prevention. This study is a qualitative study using in-depth interviews with malaria patients, three key informants, and eight relatives of malaria patients based on the rakut si telu orat si waluh system. The results found that malaria sufferers are often exposed to rain and activities outside the home at night, in the house there is a water reservoir, a lot of land in Rampah Village which was previously not taken care of so that the conversion of forests into gardens and residential areas, the community gives attention to malaria sufferers both in terms of informing the place of treatment and also facilitating sufferers to seek treatment, Karo tribal kinship based on rakut si telu orat si waluh also has a role in helping malaria treatment where each relative based on their kinship position has different tasks in helping sufferers treat malaria. Malaria prevention in the Karo community can be done by applying community capital based on the local perspectives of rakut si telu and orat si waluh. People involved in the rakut si telu and orat si waluh kinship systems have their respective roles in dealing with families suffering from malaria in terms of planning, prevention, handling and treatment.

Abstract

Work stress is a state of tension that arises due to an imbalance between physical and psychological demands. Factors contributing to work stress include both job-related and individual factors. This study aims to analyze the factors associated with the risk of work stress among crew members of the Kelud Ship at Belawan Port in 2025. The study used a descriptive analytic design with a cross-sectional approach. The research was conducted from July 2024 to March 2025. The sampling technique used was accidental sampling, involving 50 crew members of the Kelud Ship at Belawan Port. The results showed significant relationships between age (p=0.035), physical work environment (p=0.001), workload (p=0.005), salary or wages (p=0.001), and interpersonal relationships (p=0.000) with the risk of work stress. The most dominant factor influencing work stress risk was workload. Therefore, it is recommended that the management of the Kelud Ship organize crew activities more efficiently, manage working and rest times appropriately to prevent fatigue, implement fair task rotations to avoid monotony, and maintain a balanced workload. Additionally, fostering open communication between the crew, captain, and management is essential to reduce misunderstandings and create a more harmonious working environment that promotes mutual support among crew members.
