Abstract

WHO recommends that every infant should be exclusively breastfed, but until now the coverage of exclusive breastfeeding is still low due to factors such as knowledge, breastfeeding efficacy, family support, lactation problems, and barriers in the health care system.  This study aims to determine the effectiveness of Small group education of cadres with pregnant women on breastfeeding efficacy among third trimester pregnant women on Enggano Island. The research design was a quasi-experimental pre-test and post-test with a control group. The treathird trimesterent group was pregnant women who received Small group education of cadres with pregnant women while the control group was pregnant women who received conventional education. The study was conducted at Enggano Health Centre from January to November 2023 with a sample of 50 pregnant women in the third trimester of pregnancy from 25-36 weeks of age, selected by purposive sampling.  The research instruments were modules and leaflets on exclusive breastfeeding, developed through a needs assessment, while breastfeeding efficacy was measured using the Breastfeeding Self-efficacy Scale-Short Form (BSES-SF). Data were analysed univariate, bivariate with  wilcoxon and multivariate with spearman rank test. The results showed that there was an effect of Small group education of cadres with pregnant women class education on breastfeeding effectiveness with p=0.00 mean difference of 19.56. There was an effect of conventional class education on breastfeeding effectiveness in the control group with p=0.46 mean difference of 1.12. The class was more effective in improving breastfeeding practices among pregnant women with p=0.00 mean difference of 47.26. Class and education level were the most dominant factors in improving breastfeeding efficiency among pregnant women. The conclusion of this study is that the small group education of cadre with pregnant women is effective in improving breastfeeding efficacy in pregnant women in the third trimester.  Keywords: Breastfeeding, Education, Efficacy, Small Group

Abstract

Effective communication between nurses and patients is very important in providing quality services. Inaccurate information can have a serious impact on patients, incidents are caused by poor communication, less than optimal communication. The aim of this research was to determine the effect of consideration training on nurses' communication skills in the inpatient room at the Surakarta Muhammadiyah Ummah Welfare Development Hospital. This research uses a preexperimental research method with one group pretest-posttest. The research sample was nurses who worked in inpatient rooms. Sampling was taken through purposive sampling, with the inclusion criteria of nurses with 1 year of work experience. The instruments used were a consideration and acceptance questionnaire adopted from Payne as well as a consideration and acceptance implementation checklist quoted from Parke & Mishkin. Data analysis used the Paired Sample-T test with a significance level of 5%, CI-95% confidence interval. The analysis results indicate that there is no significant difference in communication skills before and after the training (p=0.083). Handover training activities have no effect on nurses' communication skills, and they do not significantly improve the quality of nurses' communication abilities in the inpatient ward. Several things that can be recommended include a commitment to carry out considerations in the form of policies, direction and evaluation of the implementation of considerations, for the sake of continuity of nursing care which has an impact on increasing nurses' communication skills in implementing patient safety as well as adding samples to ensure sample representation in further research and optimize patient admission training by providing strict sanctions for nurses who do not carry out patient admission proceduresKeywords: Communication, Handover, Nursing, Skills, Training

Abstract

One of the information systems needed in a hospital to support services is the inpatient information system. Several field constraints that often occur and can impede patient services include: limited bed space, hindered coordination between units or rooms, sudden changes in bed space requirements, the lack of integration of information systems, incomplete and inaccurate documentation of patient mobilization, the length of the patient discharge process, unavailable facilities in rooms, and the existence of hospital service indicators that must be achieved. Based on the above, a real-time and integrated information system is required, starting from the registration of inpatients to their discharge, in order to improve the quality of service and patient satisfaction. The design of the Inpatient Monitoring and Evaluation System for the Executive Report is an enhancement of the hospital information system that integrates all inpatient patient service activities, including supporting facilities, to generate useful information for monitoring and developing inpatient care services. This information can be used as a basis for decision-making by the leaders. The design of the Inpatient Monitoring and Evaluation System for the Executive Report utilizes the Systems Development Life Cycle (SDLC) method in the form of a prototype, with  the drafting phase starting from design to development. The final result of this information system design consists of comprehensive, integrated, and real-time information related to patient activities from admission to discharge, presented in a dashboard that is useful for monitoring and evaluating inpatient care services. Abstract

The prevalence of exclusive breastfeeding in various cities of Bangka Belitung Province varies, where in 2018 from January to June, the prevalence of exclusive breastfeeding in Bangka Belitung Islands Province was relatively low (57.6%). The problem of milk production is influenced by the reduced stimulation of oxytocin hormone. One of the non-pharmacological therapies that can be done is relaxation by doing Mindfulness therapy. The aim was to determine the effect of mindfulness-based stress reduction (MBSR) on oxytocin levels and stress in breastfeeding mothers for 2 weeks consisting of 8 sessions in 3 meetings. The research method used was quantitative with a quasi experimental research design (pretest-posttest control group design). Penelitian ini dilakukan at Regional General Hospital Dr. (HC) Ir. Soekarno, Bangka Belitung Province in 2023. Dilaksanakan pada bulan Januari sampai dengan Maret 2023. The population in this study were breastfeeding mothers at Regional General Hospital Dr. (HC) Ir. Soekarno, Bangka Belitung Province with a sample of 36 using purposive sampling technique. Data were collected using a questionnaire. Data analysis was univariate and bivariate dengan uji t-test and Mann Whitney test . The results of mindfulness-based stress reduction increased oxytocin hormone levels in breastfeeding mothers for 2 weeks consisting of 8 sessions in 3 meetings lasting 150 minutes per meeting and mindfulness-based stress reduction decreased stress scores in breastfeeding mothers for 2 weeks consisting of 8 sessions in 3 meetings lasting 150 minutes per meeting. It is recommended that midwives provide information about Mindfulness therapy to increase the hormone oxytocin in breastfeeding mothers and can play an active role in overcoming the problems of breastfeeding mothers who experience anxiety or worry about producing breast milk and breastfeeding babies. Keywords : Brestfeeding mothers, MBSR, Oxytosin levels

Abstract

With the rapid advancement of information technology in the healthcare sector, the implementation of electronic medical records has become a dominating trend. The transition from manual medical records to electronic medical records is not just a technological transformation but also a revolution in healthcare services. This research aims to explore the hospital's experience in transitioning from manual medical records to electronic medical records at dr. R. Soeharsono hospital, a level iii hospital. This research utilizes a qualitative approach with a phenomenological design. Data collection is conducted through in-depth interviews, and data analysis follows the colaizzi method. Triangulation methods were carried out by means of interviews with electronic medical records users, document analysis, and field observations The study involves four participants. The results reveal four main themes: Benchmarking, Finding suitable service platforms, EMR facilities, EMR training and Implementation of EMR. Electronic medical records training emerges as a crucial factor in preparing for electronic medical records usage, supported by regulations mandating electronic medical records service platforms. The use of external service platforms is acknowledged as an effort to enhance data security. The success of the implementation of electronic medical records also relies on the availability of supporting facilities and the selection of the right service platforms. This research provides important insights for hospitals and related parties to understand the transformation process from manual medical records to electronic medical records optimally. Keywords: Electronic Medical Record, technology

Abstract

Elderly refers to a more advanced phase of a physiological process marked by a decline in the body's capacity to adjust to external pressures. One contributing factor to the rise in Diabetes Mellitus is a deficiency of understanding, which can subsequently influence the adoption of harmful behaviors. Health providers propose dietary behavior as a means to normalize blood sugar levels in elderly individuals with Diabetes Mellitus. The objective of this The aim of this research is to examine the relationship between levels of knowledge and adherence to dietary recommendations among elderly adults diagnosed with diabetes mellitus who live in Karang Rejo Village, Langkat Regency. In this study, a quantitative approach is employed, utilizing a cross-sectional research design. The present investigation was carried out at Karang Rejo Village, located in the Langkat Regency, spanning from February to October 2023. The research sample consisted of 780 older individuals diagnosed with Diabetes Mellitus, residing in Karang Rejo Village, Langkat Regency. The study utilizes a basic random sampling technique to ensure that each participant has an equal probability of being chosen for the sample, without considering their individual features. Consequently, the sample size for this study consisted of 42 old individuals. The primary instrument employed for data collection in this study was a questionnaire. The present study employed bivariate analysis chi-square test for data analysis, and utilized software help in the form of SPSS version 20 for data processing. The findings indicated a statistically significant correlation (p-value = 0.001) between the extent of knowledge and adherence to dietary recommendations among older individuals diagnosed with diabetes mellitus residing in Karang Rejo Village, Langkat Regency. Abstract

Water is a necessity of life, such as washing clothes, washing vehicles, bathing, defecating, cooking, drinking, purifying, ablutions, watering yard plants, providing water for livestock and pets, and other needs. Poor water quality has an impact on health and can even be life-threatening. This study aims to determine the relationship between education about the suitability of drinking water sources and the level of knowledge in Gempol Village, Karanganom District, Klaten Regency. This study is a quantitative study with a research design and a cross-sectional approach. The research was conducted in Gempol Village, Karanganom District, Klaten Regency, from October to November 2023. The study population included residents of Gempol Village, consisting of 18 neighbourhood associations with 800 family cards. A total of 116 respondents, who were selected through proportional sampling calculations, were involved in this study. Data were collected through questionnaires and observation sheets. Bivariate data analysis with Fisher exact test processed using SPSS version 26. The results showed a relationship between education level and knowledge about the feasibility of drinking water sources (p-value = <0.0001) <0.05. It is recommended that the Gempol village government implement an ongoing educational program to increase community awareness about the importance of clean water and how to ensure the suitability of drinking water sources. This program can include counselling, workshops, seminars, and public campaign. Abstract

Non-communicable diseases are still the leading cause of death worldwide, affecting not only the elderly but also the productive age group. Type 2 diabetes mellitus (DM) is one of the non-communicable diseases whose prevalence continues to rise, including in Indonesia. North Sumatra Province also experienced an increase in DM prevalence, with the highest number of cases in Medan City. The development of type 2 DM is a complex process and is associated with several risk factors, one of which is psychological stress. This study aims to analyze the effect of stress on the incidence of type 2 DM in the 30-60 years age group in Medan City directly and indirectly through sleep quality and hypertension. The research method used a case control design with a sample size of 68 cases and controls each. Data analysis was carried out univariately and multivariately using a path analysis model. The results showed that there is a direct effect between stress (b=0.892; 95% CI=0.149-1.636; p=0.019) on the incidence of type 2 DM in the 30 -60 years age group in Medan City. Stress increases the risk of type 2 DM indirectly through poor sleep quality (b=0.886; 95% CI=0.188-1.583; p=0.013) and hypertension (b=0.859; 95% CI=0.117-1.602; p= 0.023). In addition, sleep quality (b=0.820; 95% CI=0.076-1.565; p=0.031) and hypertension (b=1.046; 95% CI=0.249-1.841; p=0.010) also have a direct effect on type 2 DM. Avoiding and managing stress, getting enough and regular sleep, and controlling blood pressure can help prevent type 2 DM.Keyword: Hypertension, Sleep Quality, Stress, Type 2 Diabetes Mellitus

Abstract

Hospitals as health service providers need to study the equipment used in health services, which is called a Health Technology Assessment. This research aims to determine the assessment of electronic medical record information technology using the Health Technology Assessment concept at regional General Hospital X Jepara. This research uses a qualitative method with a descriptive design. The research was conducted at the regional general hospital X Jepara which was carried out from March to June 2023. There were 13 informants in this research with 9 main informants and 3 triangulation informants. Data collection techniques use in-depth interviews, observation, and document study. Data analysis for this research is data reduction, data presentation, and conclusion. The results of this research on the concept of effectiveness can increase productivity, be more efficient, make it very easy to publish patient medical records in summary and detail, and doctors and nurses can easily access patient data by logging in using their password. Patient privacy is also maintained with limited access to open it. Service is fast and can increase satisfaction and improve hospital services from an economic perspective using health technology assessment methods. The characteristic aspect of using electronic medical records is that it makes hospital services easier. The security aspect of patient data is guaranteed because system access can only be accessed by users with an account, and the rights of each user are guaranteed. Abstract

Every year, millions of patients suffer injuries or die because of unsafe and Not Good-quality health care. Many medical practices and healthcare-related risks have emerged as major challenges to patient safety and contribute significantly to the burden of harm from unsafe care. The aim of this research is to determine the relationship between the nurse handover process and the implementation of patient safety targets in the surgical ward and ICU of the Royal Prime Hospital Medan 2022. This research is a type of quantitative research with a pre-experimental design method type one group pretest-posttest (single group initial test-final test), with a population of all nurses working in the surgical ward (26 nurses) and ICU (26 nurses). 25 nurses) Royal Prime Hospital Medan, totaling 51 people. Research sample collection techniques are primary data, secondary data and tertiary data. The instrument used is a questionnaire. Research data analysis is univariate analysis and bivariate analysis. The results of the research showed that there was a significant increase in the implementation of the nurse handover process before the treatment was 24.75 and it increased after the treatment to 39.22. Shows that there was a significant increase in the implementation of patient safety targets before treatment, amounting to 27.65 and an increase after treatment to 43.39. And showing a p value (0.005) < 0.05, it shows that there is a significant relationship between the handover process and the implementation of patient safety targets at the RSU.
