The Review of Primary Care Practice and Education is a bilingual open access journal which provides scientific information on the field of ‘Primary Care’ and ‘Family Medicine’ in the form of research-based scientific articles, case reports, policy briefs and new findings from experts, civitas akademika and medical practitioners. This journal is an official journal of the College of Indonesian Primary Care Physicians in collaboration with the Department of Family and Community Medicine of the Faculty of Medicine, Universitas Gadjah Mada, with the inaugural publication in January, 2018. The limited number of scientific journals of primary care and family medicine in Indonesia encouraged the College of Indonesian Primary Care Physicians to create a journal which focuses on the primary care, family and community medical sciences, and to disseminate the information to academics, health practitioners, and the public. This journal contains scientific information on strengthening efforts of primary care, various health problems in primary care, principles of education and family medicine services, and the efforts to create a healthier and prosperous community in Indonesia and the world.

Abstract

Background: Tuberculosis (TB) is still a problem in developing countries and even the world. TB is one of the top 10 causes of death and the first cause of death from an infectious agent worldwide. The government plans to eliminate TB by 2030, which is carried out with the National Strategy for TB Control. Family doctors play an important role in which the role of family doctors is not only to cure but also to promote health and prevention of TB disease. Purpose: The application of evidence-based medical doctor family services to patients by identifying risk factors, clinical problems, and management with a patient-centered approach and family approach. Method: The study conducted was a Case Report. Primary data were obtained through history taking and physical examination by making home visits, filling family folders, and filling patient files. The assessment was carried out based on the initial holistic diagnosis, process, and end of the visit quantitatively and qualitatively. Results: Based on a holistic diagnosis, it was found that an 87-year-old patient complained of coughing, difficulty in swallowing, and weight loss since three months ago, three weeks ago the patient was diagnosed with TB. Physical examination showed a BMI of 14.2. The patient had difficulty swallowing, with the examination of nerves 10 within normal limits. Internal risk factors include a lack of knowledge about TB, and aging. External factors are often exposed to dust. Functional degrees 4. Four visits were made. The interventions provided in the form of education about TB include the definition, symptoms, treatment and prevention, counseling with the family, and providing a high-calorie, high-protein diet for patients. The evaluation results show an increase in patient and family knowledge about TB and an improvement in the patient's diet that the family has done. Conclusion: The diagnosis of TB with malnutrition and dysphagia is good. Management provided is in accordance with the guidelines, visible changes in behavior, and knowledge of patients and patients' families after an intervention based on evidence-based medicine that is patient-centered, family approach.

Abstract

Background: Medical records are an important element in the process of health care in health facilities. Several primary health facilities have used electronic-based medical records to improve service quality. Electronic medical records are patient medical records in electronic form that include health information of an individual that can be accessed using a computer from a network to provide effective, efficient, and integrated health services. The purpose of the study was to determine the development of the use of electronic medical records in primary healthcare facilities in Indonesia with a systematic review before the mandatory use of electronic medical records in Indonesia. Methods: This research is a systematic literature review of the use of electronic medical records in primary health facilities. Researchers searched the literature on google scholar with the keyword electronic medical records and obtained a total of 60 articles, 4 of which were inaccessible, 40 articles were not conducted in primary health facilities, and 3 articles do not contain the aspect we look for so only 13 articles were used in literature studies. Results: The implementation of medical records is carried out by all health workers who provide services from registration, triage, clinical examination, treatment, and drug prescribing. Medical record data is distributed automatically by computer. Health workers have different electronic medical record display menus according to their duties. Conclusions: The implementation of electronic medical records in primary health facilities can be carried out by medical recorders and non-medical health workers who are trained and has a different electronic medical record display menu in each service flow.

Abstract

During prenatal and post-partum period, 10-20% of women experience depression. The quality of life and functional capacity of these women were also significantly impacted by depression. This condition also has an adverse effect on fetal development and newborn. Furthermore, home visit have been used to reduce maternal depression. However, the evidence of this still low. Hence, this review aims to evaluate the effectiveness of home visiting in preventing maternal depression. Articles for this systematic review were collected from several databases (PubMed, Scopus, Google Scholar, and ScienceDirect) using terms related to maternal depression, prevention, and home visiting. The quality of included studies were assessed using the Newcastle-Ottawa Scale (NOS). This systematic review is comprised of 13 high quality randomized clinical trials with 4.804 participants. Eleven studies indicate that home visiting effectively reduce depressive symptoms and a study shows that mothers receiving home visiting are twice less likely to develop depressive symptoms. Home visiting are also beneficial for low-income women amidst the low rate of mental health services. Only two studies state that there is no evidence that home visiting effectively reduces depressive symptoms and increases caregiving quality. However, some studies state that the mother's cognitive, child growth, and maternal depression may improve with home visiting integrated with cognitive behavioral therapy, counseling, education such as lectures, or video. Home visiting are effective in preventing maternal depression. Further studies are needed to compare the effectiveness of the intervention plan in home visiting.

Abstract

Hypertension is a condition when a person has systolic blood pressure ≥140 mmHg and/or diastolic blood pressure ≥ 90 mmHg when measured at a clinic or health facility. Hypertension can be a risk factor of important organs damage such as the brain, heart, kidneys, retina, large blood vessels (aorta) and peripheral blood vessels. To prevent the complications, hypertensive patients must control their blood pressure. One approach to achieving controlled blood pressure is involving the family in hypertension management, which is called Family Centered Care. This approach has been implemented at Mitra Sehati Clinic, a private clinic located in Bandung Regency, West Java. This study aims to determine the implementation of hypertension management using the Family Centered Care approach at Mitra Sehati Clinic. This research is a qualitative research using a single case study method. The data were obtained through in-depth interviews, observation and documentation and then analyzed using interpretive analysis methods in which emerging themes were interpreted based on the theory of Family Centered Care and hypertension so they became the basis for describing certain conclusions. The results obtained show that all elements of Family Centered Care in the management of hypertension have been carried out by health workers at the Mitra Sehati Clinic which are also included in several programs. In addition, a positive impact was found on patients, their families and health workers. However, in its implementation, obstacles were found, it was limited time. The solution was to use online consultations via the doctor's or nurse's personal cellphone number. Implementation of Family Centered Care in the management of hypertension has a good impact on patients, their families and health workers.

Abstract

Background. Family medicine is a new specialty, so it is necessary to know the interests of students. Objectives. To find out the perception and interest of clerk students toward family physician careers. Methods. This research was a cross-sectional study at the Faculty of Medicine YARSI. Consecutive sampling in the period January to April 2023. Data was collected through online questionnaires. The analysis carried out includes univariate, bivariate, and multivariate analysis. Results. The sample involved 103 people. Students interested in becoming family physicians amounted to 46 people (44.7%). Individual characteristics are not significantly related. The determinant of student interest was the perception that family physician’s practice was attractive (p = 0.036; OR= 2.518 (95% CI= 1.060-5.981)). Conclusions. Family physicians play an important role in solving health problems in primary care. Intensification of learning about family medicine and primary care is needed to increase student interest.

Abstract

Objective. This study investigated the effect of shared decision-making (SDM) approaches on medication adherence among individuals with chronic diseases receiving primary healthcare from studies conducted between 2012 and 2023. Method. A literature review was conducted in PubMed, Scopus, ProQuest, and EBSCO databases, resulting in 220 articles. Only randomized controlled trials (RCTs) were included; the two studies that matched the inclusion criteria were examined. Results. Two RCTs in primary healthcare settings examined SDM interventions. The study on hypertension patients found no significant effect of SDM training on medication adherence, while the study on depression patients using a decision aid also found no significant impact on medication adherence. Discussion & Conclusion. SDM interventions did not significantly improve medication adherence in chronic diseases such as hypertension and depression. Factors identified include short training duration, the complexity of Adherence, and a need for consistent evidence. The conclusion suggests that SDM may not significantly affect chronic disease patients receiving primary healthcare medication adherence. However, given the limited number of eligible studies, more research is needed in diverse settings and populations.

Abstract

It is a general consensus that investing more in primary care will lead to better health outcomes overall, and therefore, primary care should be the focus of development for health services. A body of evidence that intervention in primary care is cost-effective and reduces health system burden and disparities (1–7). Making primary care the focus of health development is crucial for the sustainability of the health system, particularly amidst the changing demographic landscape and disease burden. Designing health service or needs to start at primary care at its heart. This is concurrent with WHO 2008 report on Primary Care (8): “… focus of reform that reorganizes health services as primary care, i.e., around people’s needs and expectations so as to make them more socially relevant and more responsive to chancing world while produced better outcomes –  service delivery reforms.”
