Abstract

Thalassemia major is one of the types of thalassemia that need a routine blood transfusion. If not treated immediately, the patient will only last for 1-8 months. Blood transfusions performed at least or more than 10 times causes iron overload. Excess levels of Fe ions in the body will be stored in the form of ferritin. If the ferritin level is high, cell damage will occur in the presence of a fat peroxidation reaction or Malondialdehyde (MDA). Cell damage can trigger proinflammation, which increases neutrophil counts and decreases lymphocyte counts. The Neutrophil-Lymphocyte Ratio (NLR), which measures the ratio between Absolute Neutrophil Count (ANC) divided by Absolute Lymphocyte Count (ALC) with a manual peripheral blood picture. This study aimed to determine the correlation of ferritin levels with MDA and NLR in iron overload. This study used an analytical observational design with a cross-sectional approach, with samples were thalassemia patients who received repeated blood transfusions at the General Hospital Dr. R Soetrasno, Rembang City and Regional General Hospital Dr. R Soedjati, Grobogan Purwodadi. Inclusion criteria were age 10-18 years, transfusion 10-20 times, normal body temperature. Exclusion criteria were Fe therapy orally, leukocytosis, chronic kidney disease. In the MDA levels, there was no significant difference where p=0.25 by Spearman test. In the NLR there was no significant difference where p=0.91 by Spearman test. There is no correlation between ferritin levels with MDA and NLR in iron overload.

Abstract

Antiphospholipid antibody syndrome (APS) is a systemic autoimmune disease associated with venous and/or arterial thrombosis with the presence and persistence of antiphospholipid antibodies (aPL). One of the currently discussed markers related to a high risk of thromboembolism is increased Mean Platelet Volume (MPV). This study aimed to know whether an association exists between MPV and thrombosis event in APS patients. We systematically searched and reviewed studies from MEDLINE, Science Direct and the Cochrane Controlled Trials registry (CENTRAL) from October until November 2018. We use appraisal tools from Critical Appraisal Skills Programme (CASP-UK) for cohort studies. We found two relevant studies to be included in our review.In total, 389 patients consisting 92 APS patient and 297 APS-negative and healthy controls were included. In two studies, the mean of MPV in APS group with thrombosis ranged from 7.85 to 9.22 fl. MPV in APS group with thrombosis was higher than in the APS group without thrombosis and in healthy controls. The platelet size, measured as MPV, reflects platelet reactivity, including aggregation, glycoprotein IIb-IIIa expression and production of more thrombogenic factors. In summary, MPV has a positive correlation with thrombosis event in APS patient. MPV may also be a potential clinical predictor for recurrence of thrombosis in APS patient. We urge that more future prospective studies with larger sample size to be done in order to validate this potential marker.Keywords : Mean platelet volume, thrombosis, antiphospholipid syndrome

Abstract

Infection caused by Methicillin-Resistant Staphylococcus aureus (MRSA) is a healthcare-associated infection thatreceives the most significant attention worldwide due to its resistance. Administration of precise and rational antibiotics canprevent high MRSA rates in hospitals. This study aimed to determine the antibiotic susceptibility pattern of MRSA at theintensive care room of Ulin General Hospital, Banjarmasin, between 2016 and 2018. This study was an observational analyticstudy by taking the results of culture and antibiotic susceptibility pattern data of the MRSA isolated from patients treated atthe intensive care room retrospectively. The results showed 37 data of patients suffering from MRSA at the intensive careroom in 2016-2018, with a percentage of 23.81%, 25.81%, and 35.19%, respectively. The most common sources of MRSAisolate in this study were sputum (32.39%), blood (29.27%), and pus (16.67%). From 2016 to 2017, there was a decreasedsusceptibility to macrolide antibiotics, aminoglycosides such as Gentamicin, and quinolones such as Moxifloxacin. In 2018,there was an increased susceptibility pattern of some antibiotics compared to the previous period. Antibiotics with thehighest susceptibility in period of 2016-2018 were Linezolid, Quinupristin/Dalfopristin, Tigecycline, Nitrofurantoin, andTrimethoprim/Sulfamethoxazole. Also, the antibiotic with the lowest susceptibility was Tetracycline. It was concluded thatthere had been changes in some antibiotics' susceptibility pattern to MRSA within 2016-2018.

Abstract

Managing anemia in Chronic Kidney Disease (CKD) patients with hemodialysis (HD) is a challenge to physicians. The present consensus does not address the proper time of blood sampling in HD patients, but higher ultrafiltration (UF) volume (a process of removing fluid excess during HD) may alter hematologic parameters. The objective of this study was to compare some parameters of the Complete Blood Count (CBC); hemoglobin (Hb), hematocrit (Hct), leukocyte (WBC), and platelet counts (Plt) before and after HD. This method was a cross-sectional study performed in the HD Unit, Dr.Soetomo Hospital, including 51 CKD patients selected consecutively, divided into two groups based on the UF volume (2 L and >2 L). Complete blood count pre- and post-HD were measured using Sysmex XN 1000. The results were 25 males and 26 females in this study, age ranged from 20 to 74-year-old, and 36 patients with UF volume >2 L. Only HD with UF >2 L showed significant increases for Hb (9.35g/dL to 10.00 g/dL), Hct (29.80% to 31.15 %), and Plt (209.00x103/µL to 213.00x103/µL) but WBC did not change significantly. These changes were believed to be caused by ultrafiltration. The conclusion was Hb, Hct, and Plt increased significantly with UF ≥2 L in HD CKD patients.

Abstract

The obesity index is a parameter that describes general and central obesity. Obesity prevalence tends to increase in the rural areas of Indonesia. The increased prevalence of obesity is correlated with increased cardiovascular prevalence and diabetes. Dyslipidemia is also a risk factor for cardiovascular disease and is closely related to obesity. Obesity is an accumulation of excessive triglyceride levels and stored in adipose tissue that can contribute to the onset of coronary heart disease. This was an analytical observational study with a cross-sectional design. Research subjects were the rural community in Bonjoroyo village, Kulonprogo Regency, DIY which fulfilled the inclusion criteria. The data obtained were obesity index data (Body Mass Index/BMI, Waist Circle/LP, Waist Round/RLPP Ratio, Body Waist Ratio/RLPTB) and triglyceride levels which were then analyzed statistically with normality test and continued with correlation Spearman test with a 95% confidence level. The results showed that BMI, LP, and RLPTB measurements were significantly positively correlated with moderate-to-weak correlation strength in fasting blood triglyceride levels (r:0.435; p:0.000; r:0.362; p:0.002; r:0.347; p:0.004) in males. Measurements of BMI, LP, and RLPTB were positively correlated with a weak correlation strength against fasting blood triglyceride levels (r: 0.193; p: 0.028; r: 0.226; p: 0.010; r: 0.214; p: 0.014) in females. The RLPP obesity index did not show significant correlations to fasting triglyceride levels in rural communities.

Abstract

Diabetes Mellitus (DM) is a metabolic disease characterized by hyperglycemia due to abnormal secretions and/or insulin activity. Osteopontin (OPN) is an important component of inflammation and insulin resistance, and vitamin D decreases insulin resistance. This study aimed to analyze the correlation between OPN and glycemic control and total 25-OH vitamin D in type 2 DM. An observational analytic study with a cross-sectional approach was performed in Dr. Moewardi Hospital, Surakarta, from May to September 2018. Plasma OPN levels were measured by a sandwich enzyme immunoassay kit from Elabscience 96T Human OPN (USA), and a total of 25-OH vitamin D was evaluated using the ELFA method from Biomerieux SA (France). Data were tested by Pearson correlation (r). Type 2 DM subjects consisted of 45 (54.2%) males and 38 (45.8%) females, 36 (43.45%) well- and 47 (56.65%) poorly-controlled. The average age was 56.81±9.76 years old. The mean of OPN level in poorly-controlled cases was significantly higher (20.27±3.20 ng/mL) than well-controlled ones (15.04±3.34 ng/mL) with p=0.001. There was no significant difference in total 25-OH vitamin D between well- and poorly-controlled groups (19.84±6.65 vs. 17.24±6.78 ng/mL, respectively, p=0.085). The correlation of OPN with glycemic control (fasting glucose, 2-hour post-prandial glucose, HbA1c) and total 25-OH vitamin D in all subjects with type 2 DM were r=0.241 (p=0.028), r=0.378 (p=0.0001) r=0.529 (p=0.0001) and r=-0.151 (p=0.173), respectively. This study suggested that plasma OPN level was correlated with glycemic control but not with serum total 25-OH vitamin D in type 2 DM. Further research was needed in populations of other types of DM and other research variables related to inflammation or insulin resistance.

Abstract

Sepsis is a cause of non-cardiac death in the hospital. Early and rapid diagnosis of septic patients is a challenge toincrease the expectancy of life. IL-6 and TNF-α are groups of pro inflammatory cytokines that initiate an initial inflammatoryresponse. Procalcitonin is a specific marker of bacterial infection. This study aimed to analyze the correlation of serumcytokine IL-6, TNF-α, procalcitonin and leukocyte count in suspected sepsis patients. This was a cross-sectionalobservational study consisting of 45 patients with suspected sepsis with procalcitonin level > 0.5 ng/mL. Procalcitonin levelwas measured with Enzyme-Linked Fluorescent Assay (ELFA) (VIDAS), IL-6 and TNF-α levels were measured with theU-CyTech Human Elisa kit (Bioscience, INC) and leukocyte counts were measured with SYSMEX-XN 1000. It was found in thisstudy that IL-6 levels ranged in 0 pg/mL – 73.29 ng/mL (mean 29.43 ng/mL), TNF-α levels were 0 pg/mL – 390.5 pg/mL (mean27.62 pg/mL), and the mean value of leukocytes was 20,139/μL. There was no correlation between leukocyte counts with IL-6(p=0.798 and r=0.040), TNF-α (p=0.304 and r = -0.160), and procalcitonin (p=0.323 and r = 0.154). There was no correlationbetween IL-6 levels with TNF-α levels (p=0.871 and r = -0.025), and procalcitonin levels (p = 0.466 and r = 0.112). There wasa weak negative correlation between TNF-α level and procalcitonin levels (p=0.006 and r = -0.403) and there was a weaknegative correlation between procalcitonin and TNF-α levels in suspected sepsis patients.

Abstract

A compatibility test as part of a pre-transfusion test is mandatory to ensure blood compatibility between patients anddonors. Diamed-ID as the first gel-based product is commonly used as a reference for the compatibility test. The presence ofnew products such as DG Gel Coombs encourages research to compare them with reference methods. This study aimed toanalyze the suitability of DG Gel Coombs to Diamed-ID in the compatibility test with the same sample. This cross-sectionalanalytic observational study was conducted during November 2017-February 2018 at the Blood Transfusion Unit Dr.Soetomo Hospital, Surabaya using blood samples (n=40), which met the inclusion and exclusion criteria. Simultaneoustesting of the two products was according to the manufacturing requirements of each product (using different LISS reagentsfor red blood cell suspensions and centrifugation arrangements). The suitability of results was tested with Cohen's kappaand significant differences with McNemar. There was a minimum suitability of DG Gel Coombs to Diamed-ID for majorcompatibility tests, κ 0.307 (95% CI: -0.029-0.643), significance 0.007 (p < 0.05) and moderate for minor, κ 0,678 (95% CI:0.454-0.903), significance <0.0001 (p < 0.05). McNemar's significance was 0.016 (p < 0.05) for major compatibility test and0.031 (p < 0.05) for minor. Referring to Diamed-ID's results, false negatives were found on DG Gel Coombs for majorcompatibility tests (n=7) and minor (n=6). The suitability of results from DG Gel Coombs and Diamed-ID is not strong forcompatibility testing.
