Abstract

Patients with Severe Mental Disorders (PSMD) "Pasung" require long-term care, which causes a financial burden on the family. The Indonesian government has guaranteed funding for the treatment. However, there are still PSMDs who do not make regular visits to health services. This study aimed to describe the Relationship between National Health Insurance Ownership and Compliance with Access to Health Services for PSMDin Blitar Regency, Indonesia. This study used an analytic observational design with a retrospective cohort study approach. Total sampling techniques selected 50 PSMD "pasung" recorded health insurance and several visits to primary health services. The data was obtained from the primary health services reporting records from January to March 2020. This study used the biserial point correlation (CI: 99%) test for analyzing data. This study has ethical clearance approval. The results showed that there was a positive relationship between ownership of NHI and compliance with access to health services (t-obs (3,491) ≥ critical (2,678); p-value = 0,001; r-bis = 0,450; α = 0,01). Indonesia has guaranteed the cost of PSMD treatment with shackles, but patients and families who use these facilities are not optimal. In conclusion, national health insurance ownership is related to compliance with access to health services for PSMD "Pasung". Health agencies can pay attention to policy-making for enhancing access to health services of PSMD "Pasung".

Abstract

Tuberculosis (TB) alert health cadre training is an important thing that can be done to help control TB in the community. TB control includes increasing early detection and adherence to treatment for TB patients. To solve TB problems in the community, a synergy of roles is needed between health workers and the community, one of which is through empowering health cadres by increasing their capacity. We conducted interactive TB control health education training program that aim to improve health cadre knowledge, attitudes, and practice in early detection of TB and assistance to Drug Swallowing Supervisors/”Pengawas Menelan Obat (PMO)”. The training was provided using lecture method, two-way discussion, and demonstration for one day by internal medicine specialists, assisted by multi-professional students from medical and nursing program. A total of 36 health cadres from Sumberharjo village participated in this training. Before and after the training, cadres took pre-test and post-test. The survey results were analyzed statistically using descriptive quantitative methods for the respondent’s characteristics, whereas the score of knowledge, attitude, and practice were analyzed using nonparametric test. The median and the minimum-maximum of the pre-test score before training for knowledge was 10.5(7-13), attitude score was 15(11-15), and practice score was 15(8-15). Meanwhile, the median and the minimum-maximum post-test score after training for knowledge was 11(8-14), attitude score was 15(10-15), and practice score was 15(10-15). The results of statistic test obtained were p=0.003 for knowledge, p=0.272 for attitude, and p=0.111 for practice. It showed that there was a significant difference in the cadre's knowledge scores before and after training, but there were no significant differences in their attitudes and practice scores before and after training. Training for health cadres is recommended to increase capacity of health cadre, especially the knowledge of health cadre controlling TB in the community.

Abstract

Background: Mental disorders can affect people of any age, including teenagers of school age, which can disrupt their productivity and quality of life if not managed properly. There is growing evidence that nutrition is vital in mental health, which helps manage mental disorders. Simple interventions for mental disorders among teenagers through health promotion, education, and nutritional interventions are needed to prevent mental disorders or provide early treatment for those in need. Objective: Our project aimed to train counseling teachers and students of senior high schools in Yogyakarta to have the capability of performing simple management of mental disorders and promoting a healthy nutritional status. Methods: Our project consisted of three phases; phase I (webinar and workshop), phase II (practice and implementation at schools), and phase III (monitoring and evaluation). We used pre- and post-tests to assess respondents’ knowledge and questionnaires to assess their perceptions of the project. Results: Twenty counseling teachers and forty students from 20 senior high schools in Yogyakarta participated in this project. Our project slightly improved students' knowledge of the importance of nutrition and mental health but not the teachers’ knowledge. All participants welcomed our project positively. Most participants agreed they learned new and important information and stated that the project helped combat nutritional problems and mental disorders. Conclusion: Our project was perceived positively and improved participants’ skills and knowledge of nutrition and mental health issues. This suggests that the project has empowered them to perform basic management of nutritional problems and mental disorders among teenagers at their schools. Additionally, more projects are still needed to widen and maximize the coverage of these important topics.

Abstract

Pneumonia has long been and remains one of the leading causes of death, especially among children and the elderly. Until this study was released, only the Jakarta province fulfilled the national target of pneumonia case findings. Thus, it is important to determine the characteristics and prevalence of pneumonia-related deaths in Sleman District based on verbal autopsy. This cross-sectional study used secondary data from Sleman HDSS cycles 1 to 7. The subject of this study was a person aged over 28 days who passed away due to pneumonia between 2015 and 2022 and met the criteria for sample inclusion. Fisher’s exact and Pearson’s chi-square tests were used to analyze the data. The number of subjects who died due to pneumonia was 65 subjects (33,3%), and the number of subjects who died due to other causes was 130 (66,7%). The prevalence of those who died due to pneumonia was lower than non-pneumonia death for all of the predetermined risk factors, including elderly age, male sex, low education level, high occupational risk, smoking history, alcohol consumption, asthma history, COPD history, stroke history, tuberculosis history, hospitalization history, and very thin or malnourished body with sex, occupational status & type, and nutritional status being statistically significant. The prevalence of pneumonia-related deaths in the Sleman HDSS VA population is 33,3%, with some risk factors that may affect it.

Abstract

Communities residing in proximity to volcanoes face inherent risks from lahars. The eruption of Indonesia's Merapi volcano in 2010 had far-reaching consequences, impacting multiple districts and leading to 386 fatalities, as well as substantial financial losses amounting to approximately US$403 million. In order to bolster disaster response and mitigation efforts, resilience plays a crucial role by considering hazards, risks, vulnerability, and capacity. This research aimed to examine the relationship between disaster health literacy, risk perception, and preparedness, with the ultimate goal of enhancing resilience within a community susceptible to volcanic activities. The study utilized a cross-sectional design, involving 258 participants residing in hazard zone III areas near Yogyakarta's Merapi mountain, selected through proportional clustered sampling. Between January and February 2023, the researchers employed several assessment tools, including the Indonesian versions of the communities advancing resilience toolkit (CART-AS), disaster health literacy mitigation and preparedness questionnaire (DHLQ), disaster risk perception questionnaire (DRPQ), and disaster preparedness questionnaire (DPQ). The collected data underwent analysis utilizing descriptive statistics and the Pearson correlation test. The results unveiled disaster health literacy (β=0.22, p<0.001), risk perception (β=0.26, p<0.001), and preparedness (β=0.38, p<0.001) were all significant predictors of disaster resilience in a volcano-prone community. In conclusion, this study underscores the significant interplay between disaster health literacy, risk perception, and preparedness in relation to disaster resilience. It emphasizes the significance of fortifying disaster health literacy, fostering accurate risk perception, and promoting preparedness measures to enhance overall resilience in the face of disasters. These findings highlight the necessity for targeted interventions and educational programs aimed at augmenting disaster health literacy, improving risk perception, and enhancing preparedness levels within communities, thereby bolstering their resilience and response capabilities effectively.

Abstract

The subject matter of science, especially related to human anatomy, is one of the most challenging to understand. To help understand science materials, especially human body anatomy, we conducted community service by developing the Anato-Smart book as a learning media for students and teachers of 5th grade of Muhammadiyah 1 Elementary School Surakarta in 2021. The method of implementation in this service includes several stages, including determining the topic of the body system for the learning media that would be developed through the Focus Group Discussion for teachers at Muhammadiyah 1 Elementary School Surakarta, followed by the development of interactive and educational story media Anato-Smart with the title “The Adventures of Andi and Si Balung”, then media trials to 5th graders at Muhammadiyah 1 Elementary School Surakarta, as well as interviews and outspread questionnaires for the evaluation of Anato-Smart media. This service activity resulted in an Anato-Smart teaching media developed to hone the students' analytical skills on limb and skeletal topics. Teachers can use this book to augment interactive lessons and as one of the educational resources for students to learn about potential risks and disorders to the limbs and skeleton in daily life. From the evaluation, the story and education concepts in this Anato-Smart media book are intriguing and facilitate comprehension, however, it is better to lessen the words and increase the number of images to make it more interesting for children. We will use this feedback to improve the final results of this media so that it can be used by other schools, not only by Muhammadiyah 1 Elementary School Surakarta. Collaboration with other parties to develop teaching media in the future is needed to produce educational and interactive teaching media.
