Abstract

Background: Tuberculosis is a type of infectious disease which is still a health problem in developing countries, including Indonesia. WHO states that in 2019 tuberculosis cases are expected to reach 10 million cases with 44℅ cases in the Southeast Asia region. In Indonesia, in 2020 the number of tuberculosis cases found was 543,874 cases, this figure slightly decreased when compared to 2019 where there were 566,623 cases found. Meanwhile, the number of Tuberculosis cases in Lampung Province in 2020 reached 16,006 cases with a case discovery rate of 54.6%. Objective: Application of holistic management with family doctor service approach by identifying risk factors, clinical problems, management and prevention of disease in patients based on a problem-solving framework with patient-centered, community-oriented and family-oriented approach. Method: This study is a case report analysis. Primary data is taken from autoanamnesis and alloanamnesis, physical examination, and visits to the patient's home. Secondary data was taken from patient medical records. Results: Based on holistic diagnosis. The patient is diagnosed with pulmonary tuberculosis and has internal risk factors in the form of a lack of knowledge about tuberculosis transmission and behavior to prevent transmission. Visits were made 3 times. Evaluation results show clinical improvement, and changes in patient and family behavior. Conclusion: Holistic management with the principles of family medicine is effective in providing clinical improvement and changes in patient and family behavior.

Abstract

Hypersensitive Pneumonitis (HP) is an inflammatory lung disease caused by type 3 and 4 hypersensitivity reactions to allergens present in the environment and arises after chronic exposure, usually associated with inhaled residential or occupational allergens. The disease is relatively difficult and rare, but can cause significant and irreversible disability. The presence of pulmonary fibrosis on HRCT has important implications for prognosis and management. Patients with HP should be monitored regularly to assess progression. As far as possible, trigger antigens should be avoided. Immunosuppression is commonly used in the treatment of HP but has not been shown to slow the progression of fibrosing disease. Nintedanib, a tyrosine kinase inhibitor, is an approved treatment option for fibrotic HP with a progressive phenotype. Keywords: Hypersensitivity pneumonitis, HRCT, management

Abstract

Hospital is a health institution that provides complete individual health services. A high level of brand equity, especially in healthcare organizations increases customer satisfaction, their rebates, and also the level of loyalty, which is a key role that brands play in business success. This research is to identify the role of service, trust, and loyalty in the progress of hospital brand equity. A literature search in this systematic review was conducted on 3 (three) databases: PubMed, Cochrane, Embase Google Scholar, published in 2013 until 2023. Selection of literature based on predefined inclusion and exclusion criteria. This systematic review identified 13 studies that examined service quality, trust, and loyalty in building patient-based brand equity. Service quality has a positive impact on customer loyalty, and customer loyalty has a significant positive relationship with brand equity. The quality of service, both services and facilities, has a significant effect on patient trust and satisfaction. Improving service quality by paying attention to various aspects of both services and facilities has an impact on patient trust and loyalty to the hospital. Patient loyalty has an impact on brand equity.

Abstract

Kulit neonatus dan bayi berbeda dengan kulit orang dewasa. Perbedaan ini sering tidak disadari sehingga menyebabkan pemaparan kulit neonatus dan bayi dengan berbagai bahan yang dapat membahayakan. Kelainan kulit bawaan pada bayi baru lahir memiliki banyak jenis. Pemeriksaan dermatologi bayi baru lahir sangat penting untuk membedakan dermatosis jinak sementara dan penyakit berat sehingga dapat dilakukan diagnosis dini dan mengobati kelainan kulit bawaan secara tepat dan cepat Kelainan yang paling banyak terjadi ada tiga berdasarkan epidemiologinya, yaitu Erythema Toxicum Neonatorum 75%, Benign Neonatal Cephalic 50%, dan Miliaria Pustulosa 40-50%. Penulisan artikel ini juga bertujuan untuk mengetahui definisi, etiologi, epidemiologi, patofisiologi, penegakan Diagnosis, tatalaksana, prognosis, dan komplikasi dari Erythema Toxicum Neonatorum, Benign Neonatal Cephalic, dan Miliaria Pustulosa.

Abstract

Diabetes Mellitus (DM) is a chronic disease characterized by blood glucose (blood sugar) levels exceeding normal, namely blood sugar levels at or above 200 mg/dl, and fasting blood sugar levels above or equal to 126 mg/dl. In the last few decades, the content of sea cucumber secondary metabolites has been widely studied, such as inhibiting the growth of new blood vessels, preventing excessive blood clotting, having antibacterial, anti-inflammatory, anticancer properties, including antidiabetes One of the sea cucumber compounds is Fucoidan, which is an acidic polysaccharide consisting of fucose monosaccharides and its chemical structure differs depending on the type of sea cucumber. Fucoidan has been widely proven to have potential in the treatment of diabetes.

Abstract

Complex Regional Pain Syndrome (CRPS) is a chronic pain condition (lasting more than six months) that most often affects one limb (arm, hand or leg) after an injury. CRPS is believed to be caused by damage to, or malfunction of, the peripheral and central nervous systems. The most common injury causing CRPS is a fracture. A large multicenter prospective study found that 48.5% of patients developed CRPS (IASP criteria) after sustaining ankle, wrist, and metatarsal fractures. Management in CRPS patients is a combination of physical/occupational therapy, psychological therapy, neuropathic pain medication, and anti-inflammatory drugs. The management aims to reduce pain, increase mobility, and improve the quality of life of CPRS patients.

Abstract

Latar belakang: Hipertensi merupakan penyakit kardiovaskuler dan termasuk penyakit Silent Killer. Penyakit hipertensi jika tidak ditangani secara tepat bisa mengancam nyawa. Kejadian hipertensi sering terjadi pada lansia. Perubahan tekanan darah pada lansia yang tidak disadari bisa menimbulkan kecemasan. Tujuan: Mengetahui korelasi hipertensi grade II dengan tingkat kecemasan lansia di Puskesmas Mojolangu Kota Malang. Metode: Metode yang digunakan adalah kuantitatif dengan pendekatan cross sectional dan menggunakan uji Spearman’s Rank. Populasi pada penelitian ini adalah lansia dengan hipertensi dan teknik pengambilan sampel menggunakan purposive sampling sebanyak 65 responden. Pada penelitian ini menggunakan instrumen kuesioner Hamilton Anxiety Ranting Scale (HARS). Hasil: Hasil penelitian ini didapat bahwa dari 34 responden dengan hipertensi grade II mengalami kecemasan ringan 14 responden (31,2%), sedang 5 responden (14,7%) dan 15 responden (44,1%) tidak mengalami kecemasan. Rata-rata tekanan darah hipertensi grade II 169,24/108,47 mmHg dan non hipertensi grade II 138,61/89,48 mmHg. Pada uji analisis didapat nilai p-value 0,000 (<0,05) dan kekuatan korelasi -0,607 yang artinya sedang. Kesimpulan: Ada hubungan hipertensi grade II dengan tingkat kecemasan pada lansia di Puskesmas Mojolangu Kota Malang.

Abstract

Many studies for chronic kidney patients are being conducted around the world, but they all focus on a variety of factors that have an impact on the patients' day-to-day lives. When a patient starts dialysis, they may face a range of changes and challenges. Patients' usual routines were significantly modified because of the treatment. Most of the studies being conducted among dialysis Muslim patients focus on certain issues such as fasting, and emotion distracted due to the situation. In this case study, the aim was to explore the living experiences of a Muslim widow who was a dialysis and chronic kidney disease patient. This is one of the 16 participants who were interviewed semi-structured based on the researchers' criteria for Muslim dialysis patients. Only one of them was chosen for this case study. All of the interviews, including this one, were taped, transcribed, analysed, and categorised into themes. She had talked about how she had been diagnosed as a patient with chronic kidney disease and the difficulties she had faced along the way. She was a chronic kidney patient, having dialysis three times a week, which dramatically altered her everyday routine. The circumstances had a significant impact on her life. She took the initiative to learn more about dietary requirements because she had to watch what she ate and drank. Considering the circumstances, she becomes more devoted to Allah Almighty as she looks up information about ibadah that is pertinent to her situation as a Muslim. She apparently accepted the situations that drastically altered her life and treated them as a challenge in life. Researchers concluded that in this case study, how a chronic kidney patients handle problems from dialysis regular changes throughout her life, would be made public.

Abstract

This study highlights the importance of effective communication in healthcare, particularly during patient transitions among healthcare professionals. To address communication inefficiencies during these transitions, the structured communication tool SBARe (Situation, Background, Assessment, Recommendation, and Evaluation) is introduced among nurses in public hospitals. The methodology involves a narrative approach using in-depth interviews (IDI) and thematic analysis to analyze data. The study's findings identify three main themes: positive perceptions of the SBARe format in patient transition reports, with sub-themes on clarity and simplicity of the format; the applicability of SBARe in daily routines with improved document reporting quality and the evaluation element; and the positive impact of SBARe on communication, knowledge, and willingness to apply it in patient care practices, with sub-themes of enhanced effective communication, increased knowledge, and readiness for SBARe implementation. In conclusion, the use of SBARe yields positive participant views on its effectiveness and utility in daily healthcare practices, emphasizing the crucial role of effective communication in healthcare and suggesting potential for broader implementation in the healthcare domain as a whole.

One of the best ways to evaluate a person's health quality is by measuring their Quality of Life (QoL). In assessing health, the QoL concept has exploded in popularity during the previous four decades. It has been used as the sole outcome in studies aimed at helping unhealthy patients improve their physical or emotional well-being. As a result, a specific evaluation method for health-related QoL is needed for this rapidly rising global population. This review aims to discuss components of QoL.
