Abstract

The high global cesarean section rate, ranging from 10% to 40%, imposes an economic burden at national and individual levels. Due to the increased risk of complications associated with repeated cesarean sections and the safety of vaginal birth after cesarean (VBAC), VBAC has become a preferred strategy. It is crucial to offer mothers appropriate counseling on VBAC, considering potential benefits and risks, as well as understanding other factors influencing the choice of childbirth mode. An analytical observational study with a cross-sectional design was conducted, utilizing data from medical records of patients who gave birth at Dr. Tjitrowardojo Hospital Purworejo from January 2022 to December 2022. Data from 108 subjects were analyzed using Chi-square to identify variables correlated with VBAC spontaneous delivery. Mothers who successfully delivered VBAC at a non-risk age numbered 32 (55.6%). Mothers with a parity history of ≥2 who successfully delivered VBAC numbered 47 (45.2%). Mothers who delivered babies with normal birth weight and successfully delivered via VBAC numbered 39 (41.5%). Abnormal birth weight was significantly associated with VBAC failure (PR=3.526 (1.031-12.062), p=0.036). There were 8 (72.2%) babies with an Apgar score <6 at 5 minutes after VBAC. Abnormal birth weight is associated with the failure of spontaneous VBAC delivery at Dr. Tjitrowardojo Hospital Purworejo.

Abstract

In Southeast Asia, including Indonesia, antibiotic use is very high, reaching more than 80% in several provinces in Indonesia. This will lead to antibiotic resistance, one of the top three public health threats of the 21st century. Research needs to be carried out regarding the level of knowledge and behavior of mothers in using antibiotics for children's diarrhea. This was quantitative research with an analytical observational method with a cross sectional approach. Respondents filled out a questionnaire regarding their knowledge and behavior of using antibiotics which has been tested for validity and reliability. The data were analyzed using univariate and bivariate analysis using chi square. There were 100 people. The research results showed that most research subjects were mothers aged 26-30 years (29%). Based on the level of knowledge, most subjects were in the sufficient category (56%), and based on maternal behavior the majority were in the poor category (69%). There is no relationship between mother's knowledge and behavior in using antibiotics in children with diarrhea with a p value of 0.831. It can be concluded that here is no relationship between mother's knowledge and behavior in using antibiotics in children with diarrhea with a p value of 0.831.

Abstract

The quality of service in the hospital is very important to support the quality of the hospital in order to strive to create the best service and treatment for every patient. The purpose of this study was to determine the overall level of outpatient satisfaction with the quality of service and complaint handling at the pharmaceutical installation of Kendari City Hospital. In this study, it is descriptive with qualitative and quantitative approaches. Then the data obtained is processed using gap analysis, custumer satisfaction Index (CSI), and Importance Performance Analysis (IPA) to identify the strengths and weaknesses of the service and determine opportunities to develop strategic planning. The results of the gap analysis show that each dimension of the servqual is at a negative index, which means that the patient is not satisfied with the services provided, namely tangibles -0.24, reliability -0.17, responsiveness -0.22, assurance -0.16, empathy -0.54. The results of the CSI analysis of the servqual dimensions and overall complaint handling have not reached 100% (excellent) but have received a value of 91.75% (excellent). The results of the IPA analysis show that quadrant A is a priority in improvement, namely: the tangibles dimension, namely the availability of drugs and the responsiveness dimension, namely the waiting time for drugs.

Abstract

BPH or Benign Prostatic Hyperplasia is a histological diagnosis due to hyperplasia of stromal cells and epithelial cells of the prostate gland. Thought to occur due to the influence of hormones, dietary patterns, microtrauma, inflammation, obesity, and physical activity Occurs in elderly males and has testicles that still produce testosterone. This paper was created by searching the literature from clinical practice guides, scientific literature, websites, and textbooks on the topic of BPH. Results and Discussion: BPH often occurs in old age, has to do with diet, comorbidities, physical activity/exercise, smoking, work and education. Some treatments, namely conservative therapy, medicamentosa, phytopharmaceuticals and surgery. The selection of therapy starts from the least invasive aimed at improving quality of life. BPH is difficult to prevent, preventive measures are very important for prevention. Conclusion: BPH can decrease quality of life although not deadly. Management depends on the level of the patient's complaint, complications, available facilities, and the patient's choice. The ability to recognize diseases, diagnose and provide therapy is not the same because of differences in facilities and human resources. It is hoped that doctors in remote areas can recognize and diagnose BPH early in order to handle patients as well as possible.

Abstract

In the era of Indonesia's National Health Insurance or Jaminan Kesehatan Nasional (JKN), antenatal care services (ANC), have become more affordable and accessible for pregnant women. However, there are still some referrals that are based on the patient's own wishes or are not in accordance with BPJS Health Policy. This study was aimed to describe the incompatibility of ANC referrals for BPJS Health participants. This research is a descriptive study with a cross-sectional method. This study included all BPJS Health participant data referred from FKTP to the Obstetrics and Gynecology Clinic of Reda Bolo Hospital, Southwest Sumba, in the period March–June 2023. The appropriateness of ANC referrals in this study is based on Berita Acara Kesepakatan Bersama Panduan Penatalaksanaan Solusi Permasalahan Klaim INA-CBG BPJS Kesehatan 2019 No. JP.02.03/3/1693/2020. The majority of patients had a history of visiting 0-1 times in each trimester. In the first trimester, only a small proportion (1.1%) made more than one visit. In addition, the frequency of more than one visit was most common in the third trimester (15.6%). A total of 27 patients (15.1%) were categorized as ANC referrals that were not in accordance with BPJS Health Policy. There were still a number of patients who experienced referral discrepancies. Further research can be conducted to explore the factors that influence referral discrepancies and their impact on maternal and infant health.

Abstract

Background: The national health insurance system which implements a tiered referral system requires BPJS participants to visit the community health center as a First Level Health Facility (FKTP) before seeking treatment at an Advanced Level Health Facility (FKTl). Research Objective: To determine the availability and distribution of Jambi City Advanced Level (FKTL) facilities. Research method: Quantitative with an exploratory descriptive design. Research results: Show that all sub-districts in Jambi City have been reached by FKTL, Putri Ayu Community Health Center made the most referrals to FKTL at 5,927 due to being close to several FKTL, while the FKTL with the highest referrals was DKT Jambi Hospital, namely around 65,375. The most common illnesses referred to FKTL are hypertension, diabetes mellitus, coronary heart disease, stroke and kidney failure. Raden Mattaher Regional Hospital is a hospital that receives many referrals for this disease, one of the reasons is because it has the largest number of specialist doctors in Jambi City. Conclusion: The distribution of Jambi City Hospitals is quite even, the number of FKTL referrals can be influenced by the distance or distribution of health centers close to the FKTL and the availability or ability of medical personnel to treat referral diseases.

Abstract

Academic procrastination is an activity of procrastinating work that students do when they get their main assignment, namely studying. This research was conducted with the aim of finding out whether there is an influence and relationship related to stress and local control on students who take part in student organization activities at the Faculty of Health, Dian Nuswantoro University through academic achievement index scores and questionnaires, especially students who are in the core organizational structure. This research will be carried out at 6 student organizations at the Faculty of Health, Dian Nuswantoro University in October 2023 – January 2024 (Suhandianto and Pratitis Nindia, 2020). Using survey methods with a quantitative correlational approach.(Siska Martiana et al., 2022)The results of the multiple regression analysis together show that there is an influence between the independent variables stress (X1) and Locus of Control (X2) on the dependent variable academic procrastination (Y) of 0.093, which means that there is an influence between the two independent variables on the dependent variable of 09.3 % and the remaining 90.7% is influenced by other variables not studied at this time. The results of the second hypothesis research test, namely the independent variables Stress (X1) and Locus of control (X2), which were tested simultaneously, have an influence on the Academic Procrastination variable (Y) which can be seen from the T and F tests. Where are the results of the T and F tests? The comparison value is obtained as Fcount equal to 2.971 < Ftable 3.156, then together with the stress variable (X1) we get a value of tcount 2,345 > Ttable 1.672 and the variable locus of control (X2) obtained a value of tcount 0,171 < ttable 1,672. In conclusion, there is a very significant influence between stress and student academic procrastination and there is an insignificant influence between locus of control and academic procrastination. This means that if the locus of control results are low, students are classified as having an internal locus of control which has little influence on the occurrence of academic procrastination.
