Abstract

Objectives: This research aims to investigate how Diabetes Mellitus relates to the shapes seen in mandibular X-rays. Fractal analysis can be used to examine variations in bone structure and to determine whether panoramic radiography is a beneficial screening technique for osteoporosis in people with diabetes. Review: This investigation is a thorough review of existing literature conducted from August to September 2024, adhering to PRISMA guidelines. Research articles were gathered from PubMed, Scopus, Elsevier, and DOAJ databases, focusing on terms related to diabetes, radiomorphometry, and fractal analysis. The articles included were published between 2015 and 2024, in English, open-access, and relevant to using panoramic radiography or CBCT for identifying changes in bone due to DM. Initially, 860 articles were found, but after screening, only three cross-sectional studies met all the criteria. The analysis of these studies indicated that individuals with diabetes had lower scores on radiomorphometric measures such as the Mental Index (MI), Antegonial Index (AI), and Gonial Index (GI) should be consulted as a group when compared to measurements from healthy individuals. The Mandibular Cortical Index (MCI) was observed to have a negative correlation with bone mineral density in individuals diagnosed with type 2 diabetes.   Furthermore, fractal evaluation revealed a decline in the trabecular bone architecture of individuals with diabetes type 1 diabetes, showing that DM negatively impacts the shape and structural quality of bones. Radiographic panoramas indicate that they might be an effective screening method for assessing osteoporosis risk in diabetics, and they can also demonstrate changes in the structure of the mandible. Conclusion: These outcomes point to the necessity for more research utilizing larger groups of participants and prospective study frameworks to more clearly define the connection between DM and changes in bone structure with Panoramic.

Abstract

Objectives: This research aims to describe the frequency and distribution of characteristics of maxillary central incisor impaction based on panoramic radiographs at RSKGM FKG UI and RS UI using the Mitchell and Bennet, Smailiene, and modified Winter classifications. Materials and Methods: A total of 48 cases of maxillary central incisor impaction that met the inclusion criteria were analyzed using secondary data. The characteristics of impaction were evaluated based on the Mitchell and Bennet, Smailiene, and modified Winter classifications. Intraobserver and interobserver reliability were assessed using Cohen’s Kappa test. Results: Of the 23,750 panoramic radiographs evaluated, the prevalence of maxillary central incisor impaction ranged from 0.12% to 0.35%. The most frequently observed impaction patterns were the far category in the Mitchell and Bennet classification, apical third depth (v3) in the Smailiene classification, and labiopalatal orientation in the modified Winter classification. Cohen’s Kappa values indicated very good reliability. Conclusion: Evaluation of impaction classifications demonstrated diverse impaction patterns.

Abstract

Objectives: This case report aims to report the incidental finding of tooth impaction and a dentigerous cyst in an asymptomatic patient, identified through a panoramic radiograph as the initial examination before prosthodontic treatment, and to discuss the role of further examination using CBCT in confirming the diagnosis and treatment planning. Case Report: A 47-year-old female patient came to RSGM Udayana University complaining about missing teeth in the right and left posterior regions, and wanted a denture. Clinical examination showed a mesiodens and unerupted tooth 23, without other complaints. Based on the medical history, the patient claimed to have a history of hypertension. The patient was referred to the radiology department of RSGM Udayana University to undergo a panoramic radiograph examination. Based on the result, it revealed some incidental findings of a dentigerous cyst marked by an oval-shaped, well-defined radiolucency in the region of teeth 37 and 38 and multiple impactions. Furthermore, a CBCT examination was performed on the patient to further diagnose the condition. The CBCT examination showed a radiolucent lesion surrounding the crown and causing destruction of the lingual aspect of the cortical bone, and inverted impaction of tooth 38 and horizontal impaction of tooth 23. Conclusion: Panoramic examination has successfully revealed multiple impactions and an incidental finding of a dentigerous cyst. Furthermore, CBCT examination is needed to assess the lesion in three dimensions, size, extent, position, relationship to surrounding anatomical structures, and assist in comprehensive treatment planning.

Abstract

Objectives: This research aims to describe mandibular condyle linear measurements via panoramic radiographs and extraoral conditions in both partial and full edentulous patients at Gusti Hasan Aman Dental and Oral Hospital (RSGM GHA) in Banjarmasin, South Kalimantan. Materials and Methods: This research uses an observational descriptive design with a cross-sectional approach. Samples were selected using a purposive sampling technique, consisting of 50 edentulous patients aged 20-70 years. Clinical extraoral examinations were performed to evaluate mandibular movement deviation or deflection, facial asymmetry, and temporomandibular joint clicking. Linear measurements of the anterior joint space (AJS), superior joint space (SJS), and posterior joint space (PJS) were analyzed using ImageJ software by measuring the distance between the condyle and the glenoid fossa. Results: The results showed that mean condylar linear measurements in both partial and full edentulous patients were in the abnormal range. Full edentulous patients exhibited the lowest mean values for SJS (1.68±1.05 mm) and PJS (1.58±1.36 mm). Extraoral manifestations, including facial asymmetry, TMJ clicking, and mandibular deviation or deflection, were predominantly identified. Conclusion: The condylar position in both partial and full edentulous patients is within the abnormal range, accompanied by various extraoral clinical manifestations. Kennedy Class III maintained a relatively centered condylar position, whereas the full edentulous group exhibited greater postero-superior condylar displacement.

Abstract

Objectives: This research aims to describe the length and mandibular gonial angle using panoramic radiographs and extraoral manifestations in edentulous patients at Gusti Hasan Aman Dental Hospital, Banjarmasin. Materials and Methods: This research uses an observational descriptive study with a cross-sectional design. The sample consisted of 46 patients aged 20- 70 from June 2025 to March 2026 at Gusti Hasan Aman Dental Hospital, Banjarmasin. The subjects included 25 partial edentulous and 21 full edentulous patients selected using purposive sampling. Data were collected through intraoral and extraoral examinations and panoramic radiograph analysis using ImageJ software to measure mandibular length and gonial angle. Data were analyzed descriptively using mean and standard deviation. Results: The results of this study on 46 edentulous patients at Gusti Hasan Aman Dental  Hospital, Banjarmasin, showed that the mean mandibular length in the partial edentulous group was 136.81 ± 11.47 mm, higher than in the full edentulous group (135.16 ± 10.97 mm). The mean gonial angle in the full edentulous group was 133.14 ± 8.16°, higher than in the partial edentulous group (126.46 ± 9.39°). Extraoral manifestations observed included mandibular deviation and deflection, temporomandibular joint pain, and facial asymmetry. These changes may be influenced by subjective factors such as unilateral chewing habits, denture usage, and bruxism. Conclusion: Mandibular length was greater in partial edentulous patients, while gonial angle was greater in full edentulous patients. Panoramic radiography can be used to evaluate mandibular morphological changes in edentulous patients.

Abstract

Objectives: This review aimed to evaluate the performance of convolutional neural networks (CNN) in the automated identification of periodontal bone loss (PBL) on panoramic radiographs. Review: This task-based narrative review was developed from a structured literature search of PubMed, Scopus, ScienceDirect, and reference lists for full-text studies published in 2013-2023 that applied CNN-based methods to PBL on panoramic radiographs. Of 336 records, 34 duplicates were removed, 302 records were screened, 14 full texts were assessed, and 11 core studies were included. Ten studies incorporated classification, nine detection, and five segmentation. Reported performance was generally promising, but direct comparison was limited by heterogeneous datasets, disease definitions, architectures, data splits, and outcome metrics. Classification accuracy in studies reporting this metric was approximately 76-95%; segmentation studies reported high Dice and intersection-over-union values in selected tasks. Newer studies published after the original search show continued movement toward multicenter datasets, periodontitis staging, defect-pattern analysis, and longitudinal assessment. Conclusion: CNN shows strong potential for the accurate and rapid automated identification of PBL on panoramic radiographs through classification, detection, and segmentation approaches, and may assist dentists in diagnosing periodontal disease; however, small dataset size, limited image quality, and the absence of accompanying clinical data remain limitations that should be addressed in future research.

Abstract

Objectives: Cleidocranial dysplasia (CCD) is an autosomal dominant malformation syndrome in which a mutation occurs in the RUNX2 gene on chromosome 6 and affects bone and tooth development. This article aims to describe the CBCT radiographic findings and diagnostic evaluation of a case with multiple impactions. Case Report: A 21-year-old patient came to RSGM with complaints that his teeth felt messy. The orthodontist who examined him found that there was a lot of delay in eruption, and then CBCT radiography was performed.   After an examination using CBCT, it was seen that there were many impacted teeth, starting from the central incisor, left and right premolars, both. Conclusion: Cleidocranial dysplasia (CCD) is a developmental disorder characterized by delayed tooth eruption, supernumerary teeth, and clavicular abnormalities

Abstract

Objectives: This study aimed to analyze the differences in maxillary sinus volume measurements between the linear dimension method and the semi-automatic segmentation method using CBCT images. Maxillary sinus volume is an important anatomical parameter that reflects variations in sinus morphology and pneumatization and may support clinical assessment and treatment planning. Materials and Methods: This comparative analytical study used secondary CBCT radiographic data of patients aged 20–40 years from the Radiology Unit of RSGM FKG USAKTI. A total of 50 samples, or 100 maxillary sinuses, were analyzed using two measurement methods, namely the linear dimension method and the semi-automatic segmentation method. Results: The mean maxillary sinus volume measured using the linear dimension method was 24,501.85 mm³ on the right side and 25,004.67 mm³ on the left side. In contrast, measurements obtained using the semi-automated segmentation method yielded mean volumes of 17,720.65 mm³ for the right maxillary sinus and 18,455.25 mm³ for the left maxillary sinus. The paired t-test demonstrated mean differences of 8,325.4 mm³ on the right side and 10,701 mm³ on the left side between the two measurement methods. These results indicate a difference between maxillary sinus volume measurements obtained using the linear dimension method and the semi-automatic segmentation method. Conclusion: Volume measurements obtained using the semi-automatic segmentation method are closer to the average normal maxillary sinus volume
