Abstract

Unmet need is a condition when women of childbearing age have a need to use contraception but it is not met. This condition can occur in women who do not want to have any more children or want to space out pregnancies, but do not use contraception. This study aims to analyze the relationship between socio-economic and patriarchal culture and the incidence of unmet need for family planning. Method: Uses a cross-sectional design with quantitative methods. The population consists of fertile couples (PUS) at the Siti Khadijah Clinic, with a sample of 60 respondents taken using a purposive sampling technique. Data were collected using a Standard questionnaire and analyzed using the chi-square test. Results: The analysis shows that Respondents with a low economic level are more vulnerable to experiencing unmet need, as shown by the significant value between socioeconomic (p=0.001) and patriarchal culture (p=0.000) with the incidence of unmet need. Conclusions: Socio-economic and patriarchal culture play a significant role in the incidence of unmet need for family planning. Cultural-based interventions and economic empowerment programs are needed to increase access and participation of fertile couples in family planning service programs as well as assistance and guidance to prospective family planning participants.

Abstract

The high total fertility rate (TFR) and unwanted births are caused by the high level of unmet demand for family planning among women of childbearing age. Age, expertise, and spousal support are three aspects that play a role in this. Objective to analyze unmet need in relation to age, education, and husband's support. Method using a cross-sectional approach, 89 respondents were selected from a group of fertile-age couples (PUS) in BPM Umayyah, Muaragembong District, Bekasi Regency in 2024, data collection by observation and interviews. Data analysis used the chi-square test. The majority of fertile couples did not have unmet needs, based on the univariate analysis findings (65.2%), 76.4% were of reproductive age, 60.7% had sufficient knowledge, and 51.7% had supportive partners. Unmet need was found to be associated with three variables in the bivariate analysis: age (p = 0.000), knowledge (p = 0.001), and husband support (p = 0.004). The incidence of Unmet Need is correlated with husband support, age, and expertise. To avoid misunderstandings about the use of contraceptives and to motivate mothers to use contraceptives to prevent pregnancies that impact their health.

Abstract

The implantation of a permanent pacemaker has become a primary strategy for managing cardiac arrhythmias in elderly patients to enhance quality of life, reduce cardiovascular symptoms, and improve physiological function. While pacemakers have been proven effective in enhancing mobility, emotional well-being, and physical endurance, their impact varies among patients depending on health conditions, social support, and psychological readiness. Therefore, further exploration of the clinical and psychosocial implications of pacemaker implantation in elderly individuals is required. This scoping review aims to identify and analyze the impact of permanent pacemaker implantation on the quality of life of elderly patients, with a focus on patient safety, psychosocial adaptation, and implementation challenges. Methods: A systematic search was conducted in ScienceDirect, ProQuest, and PubMed to identify relevant articles published between January 2015 and December 2024. Out of 1,321 articles initially identified, 33 articles were selected for full-text review after applying inclusion and exclusion criteria. A critical appraisal using the Joanna Briggs Institute (JBI) framework determined that 9 articles met the final criteria for analysis. The studies included in this review involved elderly patients with pacemakers across various healthcare settings, including hospitals, outpatient clinics, and community healthcare services, to assess the impact of pacemakers on physical and mental well-being as well as patient satisfaction. Results: Pacemaker implantation generally improves the quality of life of elderly patients, particularly in terms of cardiovascular function, mobility, and independence. Additionally, pacemakers help reduce the risk of arrhythmia-related complications, enhance patient confidence, and improve adherence to medical therapy. However, several challenges persist, including post-implantation infection risks, psychosocial disturbances, and limited access to pacemaker technology in certain regions. The studies also identified research gaps, particularly in longitudinal evaluations of the impact of pacemakers on elderly patients with comorbidities and cognitive impairment. Conclusion: Pacemaker implantation is an effective intervention for improving the quality of life of elderly patients with cardiac arrhythmias. However, a more comprehensive patient-centered approach, including psychosocial support, pre-implantation education, and optimized post-procedural monitoring, is crucial. Further research is recommended to explore the long-term effects of pacemaker implantation and develop more adaptive care strategies for vulnerable elderly populations.

Abstract

Infection in Catheter Double Lumen (CDL) tunneling is a complication that often occurs in patients with hemodialysis. The prevention of infection in tunneling CDL can be achieved through nurses' possession of adequate knowledge, attitudes, and behaviors regarding the care of tunneling CDL access, as well as the provision of proper information to patients and families. The objective of this study is to ascertain the relationship between nurses' knowledge, attitudes, and behaviors regarding the prevention of infection in tunneling CDL at Hospital X Jakarta. The research method employed is quantitative, utilizing a descriptive correlation research design with a cross-sectional approach. The sample collection technique employed total sampling of 46 hemodialysis nurses. The results of univariate analysis indicated that the majority of respondents were female (93.5%), aged 31-40 years (47.8%), had a Diploma 3 education level (69.6%), exhibited good category knowledge (65.2%), good category attitude (65.2%), and good category behavior (69.6%). The results of Kendal's Tau B analysis demonstrated a relationship between nurses' knowledge and attitude regarding the prevention of CDL infection (p-value 0.014) and a relationship between nurses' behavior and the prevention of CDL infection (p-value 0.004). The knowledge, attitudes, and behaviors of nurses have been identified as a crucial component in the prevention of infection in the context of CDL tunneling. Hospitals are expected to implement training programs, disseminate Standard Operating Procedures on infection control in CDL, and conduct ongoing monitoring and evaluation to mitigate the risk of CDL infection.

Abstract

Telemedicine services can be a diversion of most offline or classic consultations into online consultations through telemedicine services. The use of telemedicine continues, especially through telemedicine services provided by the Ministry of Health for patients who need it, such as about prevention of certain diseases, or patients who have experienced symptoms of certain diseases can consult through telemedicine services. This observational analytical study with a cross-sectional design uses a quantitative approach aimed at analyzing factors related to the level of satisfaction of telemedicine service users in Aceh Province. Data collection used a googleform questionnaire that was distributed using social media for 2 weeks. The sample in the study was the population of Aceh who met the inclusion criteria totaling 468 people. Sampling used the accidental sampling technique. Data analysis used a logistic regression test. factors related to the use of telemedicine application services in Aceh Province are the use of the JKN mobile application (OR=1.61; 95%CI=1.06-2.54; p-value=0.025), income < Minimum Wage (OR=1.64; 95%CI=1.13-2.37; p-value=0.008), and 4G/LTE internet network (OR=1.69; 95%CI=1.01-2.84; p-value=0.046). The most dominant factor related to the use of telemedicine is non-BPJS Kesehatan insurance users (AOR=2.42; 95%CI=0.53 – 11.05). It is expected that respondents should use the telemedicine application counseling facility provided for control and encouragement in supporting and assisting efforts to recover quickly and prevent disease. Respondents also pay attention to the type of application used because each telemedicine application has its own advantages and disadvantages.

Abstract

COVID-19 is a global pandemic that threatens everyone's health. The increase in confirmed COVID-19 cases continues to occur, including in Aceh Besar Regency. Public health intervention efforts have proven effective in reducing the incidence of COVID-19 and community leaders can be involved in these public health intervention efforts. Public health interventions are expected to cause changes in health behavior, but these behavioral changes require health beliefs. This study is a qualitative study with an exploratory approach to determine the beliefs of community leaders towards COVID-19. The number of informants in this study was 8 community leaders who were selected by purposive sampling. Data collection was carried out by conducting in-depth interviews. Data analysis in this study was carried out before entering the field, while in the field and after finishing in the field. The results of the interviews showed that the health beliefs of community leaders towards COVID-19 were influenced by the sources of information obtained, beliefs about COVID-19, the attitudes of community leaders towards people around them who were infected with COVID-19 and knowledge about the dangerous impacts of COVID-19. Good knowledge will increase the trust of community leaders and improve the attitudes of community leaders to participate in the COVID-19 vaccination which is one of the preventions of COVID-19. Community leaders are also involved in helping health workers provide information about efforts to prevent and handle COVID-19. It is hoped that community leaders will play an active role in efforts to prevent and handle COVID-19 by increasing knowledge and attitudes about COVID-19 which can be obtained through online and print information media. In addition, community leaders are expected to work together with health workers in preventing COVID-19.
