Abstract

Spinal tuberculosis (TB) is a rare but severe infection that can lead to deformities, back pain, and neurological deficits. Preoperative administration of anti-tuberculosis drugs (OAT) is commonly used to reduce bacterial activity and inflammation before surgery. However, the optimal duration of OAT preoperatively is still debated. This study aims to systematically review and analyze the relationship between preoperative OAT duration and clinical outcomes post-surgery in patients with spinal TB. A systematic review and meta-analysis were performed on studies comparing the effects of different OAT durations preoperatively in spinal TB patients. Data were collected from PubMed, Scopus, and EMBASE. Key outcomes evaluated included Visual Analog Scale (VAS) for pain, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), wound healing, and bone fusion. The meta-analysis showed that preoperative OAT duration of 1 to 2 weeks significantly reduced inflammation, with a notable decrease in ESR and CRP post-surgery (p = 0.006 for ESR, p = 0.02 for CRP). However, no significant differences in functional recovery or wound healing were observed between shorter (1-2 weeks) and longer (2-4 weeks) OAT durations. Bone fusion results were similar across both groups. A shorter duration of OAT preoperatively (1-2 weeks) is effective in reducing inflammation and improving postoperative recovery, though it does not significantly impact bone healing or fusion. Thus, a shorter preoperative OAT regimen can be considered a viable option in managing spinal TB patients.

Abstract

Medical malpractice represents a significant violation within healthcare services, posing serious risks to patient safety and eroding public trust. In Indonesia, incidents of malpractice are relatively frequent, yet many remain unresolved due to limited legal awareness and inadequate access to justice for patients. This article examines the legal dimensions of medical malpractice by focusing on two critical aspects: the legal protection afforded to patients and the legal liability borne by medical practitioners. Employing a normative juridical approach, alongside an analysis of relevant legislation and case studies, this paper aims to provide a comprehensive understanding of the existing legal frameworks and the practical challenges faced in their enforcement. The findings are expected to serve as a valuable reference for stakeholders, including patients, healthcare professionals, and legal institutions.

Abstract

Quality hospital services are the main demand in the modern era that prioritizes patient satisfaction as the main indicator of the success of health services. This study aims to evaluate the quality of hospital services using a combined approach between the SERVQUAL and Quality Function Deployment (QFD) methods. The research was conducted qualitatively using in-depth interview techniques and participatory observation at one of the referral hospitals in Banten Province. The SERVQUAL method was used to identify gaps between patient expectations and perceptions of five service dimensions: tangibles, reliability, responsiveness, assurance, and empathy. The results showed that the responsiveness and empathy dimensions had the most significant gaps, indicating the need for improvement in the aspects of service speed and attention to patients. Furthermore, the QFD method was applied to translate patient needs into technical specifications of hospital services through the preparation of the House of Quality (HOQ). Some of the key patient needs identified included improving the quality of staff communication, waiting room comfort, and utilization of digital service systems. The literature review supports that the combined approach between SERVQUAL and QFD is able to produce solutions that are applicable and oriented towards systemic improvement of health services. This research makes an important contribution to hospital management in designing strategies based on patient needs as well as a reference for sustainable service development. The results also highlight the important role of service innovation in improving hospital competitiveness at the regional level.

Abstract

Cloud computing has become a strategic solution in improving the efficiency of data management in the education sector, including in Banten Province. This research aims to explore the implementation of cloud computing and its impact on academic data management in educational institutions. Using a qualitative approach with case studies, data were collected through in-depth interviews and observations of institutions that have adopted cloud systems. The results showed that cloud computing accelerates administrative processes, increases flexibility of data access, and reduces administrative workload. In addition, the cloud supports the development of personalized learning and innovation of digital-based education services. However, the study also found significant challenges, such as limited digital literacy of educators and dependence on internet network infrastructure. Organizational support, through visionary leadership and regular training, proved to be a key factor for successful cloud implementation. On the other hand, data security is a major concern, requiring strategies such as multi-factor authentication, data encryption and role-based access control to reduce the risk of data breaches. By properly managing these challenges, cloud computing not only improves efficiency, but also encourages continuous educational innovation. This research recommends the need for synergy between educational institutions, cloud service providers and local governments to strengthen the education digital ecosystem. A secure, adaptive, and innovation-oriented cloud implementation is expected to accelerate the transformation of education in Banten Province towards competitive and inclusive global standards.

Abstract

Sagittal balance refers to the equilibrium between external forces acting on the spine, particularly gravity, and the body's response to maintain an upright position with minimal muscular energy expenditure. Disruption of this balance can lead to sagittal imbalance, which is linked to poor clinical outcomes in conditions such as adult scoliosis and degenerative spondylolisthesis. The sedentary lifestyle, especially in office workers who spend significant time sitting, makes understanding spinal alignment changes in various positions crucial. This study aims to assess the differences in lumbopelvic sagittal balance in standing, upright sitting, and relaxed sitting positions in a healthy population. This analytical study used a case series approach with 25 healthy subjects aged 18-40. X-ray images were taken in three positions: standing, upright sitting, and relaxed sitting. Parameters including lumbar lordosis (LL), sacral slope (SS), pelvic tilt (PT), pelvic incidence (PI), and PI-LL mismatch were measured using RadiAnt DICOM Viewer. Data were analyzed using ANOVA to compare the sagittal balance parameters across different positions. differences were found in LL, SS, PT, and PI-LL mismatch between positions (p < 0.05). LL was highest in standing and lowest in relaxed sitting. PT increased significantly in relaxed sitting, while SS was highest in standing. PI did not show significant changes across positions. Lumbopelvic sagittal balance varies significantly between standing and sitting positions, highlighting the need to consider sitting postures in spinal assessments. These findings provide valuable insights for spine surgery planning and postural assessments.

Abstract

The rising popularity of contemporary beverages among adolescents, despite their high sugar content and low nutritional value, has raised significant public health concerns. This study investigates the key factors influencing the consumption of contemporary beverages—such as boba milk tea, cheese tea, milk coffee, and Thai tea—among secondary school students in Bandung, Indonesia. Employing a quantitative, cross-sectional descriptive method with a sample of 110 respondents aged 15–19, data were collected via structured questionnaires and analyzed using descriptive statistics. Findings reveal that price is the most influential factor driving consumption interest (75%), followed by packaging (71%), quality (68%), and brand (68%). Adolescents prioritize affordability and functional packaging features such as portability and spill resistance, while sensory preferences (e.g., taste, temperature, and texture) and customizable options (e.g., sugar and ice levels) enhance perceived quality. Although brand recognition contributes to purchasing decisions, it is secondary to practical and economic considerations. The study underscores the need for targeted health communication strategies and product reformulation to align adolescents' beverage preferences with healthier consumption behaviors. These insights are valuable for stakeholders in the beverage industry and public health sectors aiming to balance market demands with adolescent nutrition education.

Abstract

Central Sulawesi is one of the former conflict areas and terrorism bases in Indonesia. The government continues to prevent the community especially the youth community from becoming radicalized. This research aims to find the right policy to prevent terrorism in the future. The data analysis technique used is Soft System Methodology (SSM). The method used is descriptive qualitative, by conducting interviews with the government, community and non-profit organizations. The research findings showed that the Integrated Action Plan program for Managing Social Conflict in the Poso Regency implemented dominated by a military sience 15 years ago but unable to prevent radicalization. A deliberative approach must be used to handle long-term radicalization, involving non-governmental actors as policy agents. The deliberative approach used the power of social capital, discussions with the citizen and involving the community in decision-making.

Abstract

Digital transformation in the banking sector has brought convenience to financial transactions through mobile banking applications. However, the quality of User Experience (UX) and User Interface (UI) is still a challenge, especially for people with low digital literacy. This service activity aims to improve the understanding and skills of the community in Banten Province in using digital banking applications effectively and safely. The implementation method includes an initial survey, simulation-based training, focus group discussions (FGDs), pre-post test evaluations, and the formation of village digital communities. The participants consisted of MSME players, housewives, and students, with a total of 120 people. The results of the activity showed a significant improvement in the digital literacy aspect, where the participants' understanding score rose from 52.3% to 83.7%. As many as 88% of participants were able to complete digital transactions independently after the training. The focus on digital security also increased user trust in the app. FGD findings indicated the need for improvement in application design, particularly interface navigation, icon size, and language simplification. Community-based UX/UI recommendations have been compiled and submitted to partner banks. The outputs of the activity include scientific articles, service reports, and the formation of a digital community forum for banking application users. This activity proves that an educative and participatory approach is able to answer the challenges of banking digitalization in real terms. This program is expected to become a replication model in an effort to increase digital financial inclusion based on local needs.

Abstract

Lateral condylar humerus fractures are common pediatric elbow injuries, often resulting from trauma or falls. These fractures are typically seen in children aged 6 to 10 years and require appropriate fixation to prevent complications like malunion and nonunion. This systematic review and meta-analysis compare the outcomes of K-wire pin and screw fixation methods for pediatric lateral condylar humerus fractures. Data from studies published in the last decade were analyzed, focusing on clinical outcomes such as healing time, complications (delayed union, non-union, infection, stiffness), and functional recovery. Results show no significant difference in delayed union or non-union between the two methods. However, screw fixation was associated with a higher risk of non-union and avascular necrosis, although these differences were not statistically significant. In contrast, K-wire pin fixation was linked to a significantly higher risk of infection (RR 6.53) and stiffness (RR 1.77) compared to screw fixation. No significant difference was found in lateral overgrowth, fishtail deformity, or cubitus varus. These findings suggest that while screw fixation offers greater stability, K-wire pin fixation may be more prone to complications. Clinicians should carefully consider fracture type and patient-specific factors when choosing the fixation method.
