Abstract

Diabetes Mellitus (DM) is a chronic metabolic disease with an increasing prevalence and requires optimal self-management to prevent complications and improve the quality of life of patients. One approach that can be done is Diabetes Self Management Education (DSME) through bedside teaching to improve patients' ability to take care of themselves and meet their self-actualization needs. This study aims to analyze the application of DSME nursing care through bedside teaching in meeting the needs of self-actualization in Diabetes Mellitus patients at Royal Prima Hospital Medan in 2026. The method used was a case study with a nursing process approach including study, diagnosis, intervention, implementation, and evaluation of nursing for 3×24 hours in a patient Mrs. N aged 56 years with a medical diagnosis of Diabetes Mellitus. The results of the study showed that patients experienced instability in blood glucose levels with a KGD of 460 mg/dL, a deficit in knowledge related to the disease, and a risk of situationally low self-esteem due to insulin use. Interventions provided were in the form of hyperglycemic management, health education through DSME bedside teaching, and self-awareness promotion. After three days of nursing treatment, there was an improvement in the patient's condition which was characterized by a decrease in blood glucose levels to 150 mg/dL, an increase in the patient's knowledge about DM management, adherence to treatment, and an increase in the patient's confidence in accepting the use of insulin. The implementation of DSME through bedside teaching has been proven to help improve patients' self-management skills and support the fulfillment of self-actualization needs in Diabetes Mellitus patients.

Abstract

Hypertension is one of the non-communicable diseases that occurs in the community and can cause various serious complications such as stroke, heart disease, and kidney disorders. One of the complaints that hypertension patients often experience is headache in the nape of the neck due to increased blood pressure. Pain management is not only done pharmacologically, but can also use nonpharmacological therapies such as autogenic relaxation techniques. This study aims to analyze the application of the provision of autogenic relaxation to pain management in hypertension patients at Royal Prima Hospital Medan in 2026. The results showed a decrease in pain levels and blood pressure after autogenic relaxation therapy. On the first day, the patient's blood pressure was 170/90 mmHg with a pain scale of 5, then decreased on the third day to 130/80 mmHg with a pain scale 2. Patients also appear to be more relaxed, less restless, and able to rest better. Final evaluation showed nursing problems of acute pain, activity intolerance, and anxiety could be resolved. The conclusions of this study show that autogenic relaxation techniques are effective in helping to lower headaches and blood pressure in hypertensive patients. This technique can be used as a non-pharmacological supportive therapy in providing nursing care to hypertensive patients.

Abstract

Chronic Kidney Disease (CKD) is a progressive kidney function disorder that causes the kidneys to be unable to maintain the balance of fluids, electrolytes, and remove the body's metabolic waste optimally. Patients with end-stage CKD generally require lifelong hemodialysis (HD) therapy to maintain quality of life. This study aims to describe nursing care for CKD on HD patients at the Inpatient Installation of Royal Prima Hospital Medan in 2026. The method used is a case study with a nursing process approach including nursing assessment, diagnosis, intervention, implementation, and evaluation. The subject of the study was Mr. N aged 84 years with a medical diagnosis of regular CKD on HD accompanied by anemia. Data were obtained through interviews, observations, physical examinations, and documentation studies. The results of the study showed that the patient experienced shortness of breath, edema in both legs, nausea and vomiting, decreased appetite, weakness, and decreased urine output. Laboratory tests showed Hb 7.0 g/dL, urea 187 mg/dL, and creatinine 10.79 mg/dL. Established nursing diagnoses are that hypervolemia is associated with excess fluid intake, ineffective breathing patterns are associated with obstruction of breathing efforts, and nausea is associated with biochemical disorders due to increased urea. The nursing interventions provided include hypervolemia management, respiration monitoring, and vomiting management according to SDKI, SLKI, and SIKI for 3×24 hours. Evaluation showed that the problem of ineffective breathing pattern and nausea was resolved, while partially resolved hypervolemia was characterized by a decrease in edema to degree I and continued with a regular HD program. Comprehensive and continuous nursing care is indispensable to improve the clinical conditions and quality of life of CKD on HD patients.

Abstract

Access to adequate sanitation and drinking water is a fundamental human right that remains challenging in many regions of Indonesia, including Central Sulawesi Province. This study aims to evaluate the achievement of adequate sanitation and drinking water services and formulate appropriate policy recommendations to address access disparities, particularly related to poverty. The research employs a quantitative descriptive approach, using secondary data from the Central Statistics Agency (2019–2024), and a qualitative approach using the USG (Urgency, Seriousness, Growth) method and policy alternative evaluation based on the Bardach framework (2012). The results indicate that adequate sanitation access increased from 72.5% (2019) to 80.1% (2023), and adequate drinking water access from 82.0% to 87.8%. However, the nexus between poverty and access to basic services emerged as the primary problem with the highest USG score (14). An evaluation of five policy alternatives shows that the whole-of-government approach achieved the highest score (18/20), followed by the infrastructure-based (13), community-based (15), social protection (17), and place-based (16) approaches. The conclusion is that an integrated cross-sectoral policy is most effective in breaking the poverty trap chain and improving sustainable access to sanitation and drinking water. The main recommendation is the issuance of a Governor's Regulation on the Cross-Sectoral Integration Framework in Central Sulawesi Province.

Abstract

A woman who gives birth to a baby weighing ?4000 grams has a higher risk of experiencing perineal rupture. This condition occurs due to pressure from the fetal head or shoulders during the delivery process. The purpose of this study was to determine the relationship between birth weight and perineal rupture during normal delivery in the VK Room of Sitti Khadidjah Hospital, Gorontalo City. This research used a quantitative method with a descriptive analytical design through a cross-sectional approach. The population consisted of all mothers experiencing perineal rupture, with a sample of 33 respondents selected using the total sampling technique. Data were collected using observation sheets and analyzed using the Chi-Square test. The results showed a significant relationship between birth weight and perineal rupture during normal delivery, with a p-value of 0.000 (<0.05). In conclusion, there is a relationship between birth weight and perineal rupture during normal delivery in the VK Room of RSIA Sitti Khadidjah, Gorontalo City. It is recommended that health workers improve their knowledge and competence in managing normal childbirth to prevent complications, including perineal rupture.

Abstract

L-asparaginase is an enzyme that catalyzes the conversion of L-asparagine into L-aspartate and inhibits the growth of leukemia cells that depend on asparagine. This study aims to isolate and characterize endophytic fungi from plants of the Amaryllidaceae family, and to evaluate the ability of these isolates to produce the L-asparaginase enzyme. This study employed an experimental method conducted in a pharmaceutical microbiology laboratory. Endophytic fungi were isolated using surface sterilization techniques, followed by cultivation on Potato Dextrose Agar (PDA) media. Enzyme production was performed in liquid fermentation, whereas enzyme purification was conducted using ammonium sulfate fractionation with concentrations of 0–20%, 20–40%, 40–60%, 60–80%, and 80–100%. The activity of L-asparaginase enzyme was tested using the Nesslerization method, with Asparagus used as an inducer. The findings showed that several endophytic fungal isolates produced L-asparaginase, as indicated by a yellowish-brown color in the Nessler test. The purified fractions exhibited varying enzyme activities at different levels of ammonium sulfate saturation. Therefore, endophytic fungi from plants of the Amaryllidaceae family have potential as alternative sources of the L-asparaginase enzyme, which can be further developed in pharmacy and biotechnology.

Abstract

Family anxiety in the Emergency Department (ED) commonly occurs due to uncertainty regarding the patient’s condition and limited understanding of initial treatment procedures. This condition may influence the family’s ability to receive information and support patient care. This study aimed to analyze the relationship between family anxiety levels and knowledge of initial patient management in the Emergency Department of RSUD M.M. Dunda Limboto. A quantitative correlational study with a cross-sectional design was conducted involving 91 respondents selected through accidental sampling. Anxiety levels were measured using the Hamilton Anxiety Rating Scale (HARS), while knowledge was assessed using a structured questionnaire on initial patient management. Data were analyzed using the Chi-Square test with a significance level of 0.05. The results showed that most respondents experienced mild anxiety (42.9%) and had low knowledge levels (36.3%). Statistical analysis revealed a significant relationship between family anxiety levels and knowledge of initial patient management (p = 0.000). These findings indicate that family anxiety is associated with their understanding of emergency care procedures. Strengthening health education and effective communication by healthcare professionals is necessary to reduce anxiety and improve family comprehension during emergency services.

Abstract

Hypertension is one of the major health problems among the elderly and can lead to serious complications if not properly controlled. Medication adherence is an important factor in the management of hypertension. This study aimed to determine the relationship between medication adherence and blood pressure control among elderly patients with hypertension. This study employed an analytical design with a cross-sectional approach involving 60 respondents selected using a purposive sampling technique. Data were collected using a medication adherence questionnaire and blood pressure measurements and were analyzed using the Chi-Square test and Fisher’s Exact Test with a significance level of 0.05. The results showed that most respondents were non-adherent to medication (85.0%), and the majority had uncontrolled blood pressure (91.7%). Statistical analysis revealed no significant relationship between medication adherence and blood pressure control (p = 1.000). It can be concluded that medication adherence was not significantly associated with blood pressure control among elderly hypertensive patients. Therefore, increased education and a comprehensive approach are needed in managing hypertension among the elderly.

Abstract

The workload of health workers at the West City Health Center is characterized by high patient visits, limited number of officers, and administrative tasks that must be completed simultaneously. This condition often causes work pressure and has the potential to disrupt interpersonal communication in the ministry. This study aims to determine the relationship between the workload of health workers and the quality of interpersonal communication in services at the West City Health Center. The type of quantitative research with the analytical survey method uses a cross-sectional design. The research population of all health workers at the West City Health Center amounted to 58 health workers with a total sampling technique. Data collection using workload questionnaires and interpersonal communication quality questionnaires. Data analysis using the Spearman's Rank test. Based on data analysis, it was shown that there was no relationship between workload variables and the quality of interpersonal communication in health workers at the West City Health Center with the Spearman's Rank test value p-value = 0.428 (P > 0.05), there was no relationship between workload and the quality of interpersonal communication. In conclusion, even though the workload is quite high, health workers at the West City Health Center continue to carry out interpersonal communication well between health workers in the service. Suggestions, health workers are expected to maintain effective, empathetic, and open communication with patients and colleagues.

Abstract

Type 2 Diabetes Mellitus is a non-communicable disease whose prevalence continues to increase globally, nationally, and locally. Globally, the International Diabetes Federation reports that the prevalence of diabetes reaches 10.5% in the age group of 20-79 years and is projected to increase to 783.2 million cases by 2045. In Indonesia, the number of people with diabetes reaches around 19.47 million people with a prevalence of 10.6%, placing Indonesia in fifth place in the world. Meanwhile, in Gorontalo Province in 2024, there will be 10,735 cases (4%) of Diabetes Mellitus. This condition shows an increase in the burden of disease, including in the early adulthood age group. This study aims to analyze the determinants of the incidence of type 2 Diabetes Mellitus in early adulthood at the South City Health Center. This study uses an analytical quantitative design with a cross sectional approach. The sample amounted to 87 respondents who were selected through accidental sampling techniques. Data were collected using questionnaires and direct measurements, then analyzed by Chi-Square test at a 95% confidence level (?=0.05). The results showed that 35.6% of respondents experienced type 2 Diabetes Mellitus. There was a significant association between hereditary history (p=0.000) and central obesity (p=0.033) with the incidence of type 2 diabetes mellitus. In contrast, physical activity showed no significant association (p=0.614). It is recommended to health centers to increase early screening of at-risk individuals and strengthen obesity control programs through education and lifestyle interventions.
