Abstract

Medical records are files that contain records and documents about patient identity, examination, treatment, actions and other services that have been provided to patients. Medical records must be kept either electronically or in writing that is clear and concise. Medical record documents that have been completed by the doctor within less then 24 hours after completion of the service or after the inpatient is decided to go home. This research is a qualitative research. The research instrument is the researcher himself who is subjective, so the data obtained is checked for validity using triangulation techniques, namely source triangulation, technical triangulation and time triangulation. Informants are the Doctor in Charge, Nurse, Midwife, Head of Medical Records Installation and hospital management, namely the Head of Nursing and Midwifery Services Section. Interviews were conducted according to Esterberg and literature studies. The results showed that the determinants of the completeness of Medical Record files were influenced by Man, namely the indiscipline in filling out Medical Record files, Machine, namely the absence of a policy from management if human resources was not disciplined in filling out Medical Record files, Method, namely the lack of motivation in filling in accordance with the SOP, Material, namely sometimes slow provision of parts Medical Record dan Money forms, that is there is no reward dan punishment if the Medical Record is complete.

Abstract

Performance is defined as success in completing duties, both in terms of quality and quantity. The performance of health workers reflects the quality of health services, which contributes to public satisfaction. Motivation is the main factor that influences the performance of health workers. A strong motivation influences individual behavior in various contexts, including in learning and working. The objective of this research is to determine the effect of motivation on ASN ( State Civil Apparatus ) performance in UPT( Integrated Service Unit ) Puskesmas ( Community Health Center ) Pulo Brayan in 2023. This is a quantitative survey research design with a cross sectional approach. The number of samples is 31 State Civil Apparatus using a total sampling technique. The data are collected by a questionnaire and they are analyzed with univariate, bivariate and multivariate analysis. The results of the research show that there is an effect of motivation on the performance of the ASN  in Puskesmas Pulo Brayan in terms of extrinsic motivation (p-value = 0.026) and intrinsic motivation (p-value = 0.000). Age has no effect on the performance of the ASN in  Puskesmas Pulo Brayan (p-value = 0.556), neither length of work on performance of the ASN at the Pulo Brayan Community Health Center (p-value = 0.954). The most dominant variable in this research  is intrinsic motivation with OR = 28.000. It is suggested that  Puskesmas continue to improve and sustain the motivation so that the ASN’s performance can affect the service and patient/visitor satisfaction. Community Health Centres need to encourage the employees to strengthen relationships and collaboration among members of ASN.

Abstract

Dental and oral hygiene is identified as an important factor in maintaining oral health. This importance is emphasized by government efforts, such as the provision of the dental clinics at Puskesmas (Community Health Centers). Dentist who work in dental clinic have a crucial role in providing care and its effects on patient satisfaction. Interpersonal communication between doctors and patients is a key factor in the success of treatment and in achieving patient satisfaction. The objective of this research is  to analyze the effect of interpersonal communication on patient satisfaction at the dental clinic of Puskesmas Pulo Brayan  in 2023. This is  quantitative survey research design with a cross sectional approach. The number of samples is 93 people. The Data are collected using questionnaires and analyzed univariate, bivariate and multivariate analysis. The results of the research show  that there is an effect of interpersonal communication dimension on patient satisfaction at Dentalt Clinic of Puskesmas Pulo Brayan in 2023 with the respect dimension  (p-value = 0.005) OR = 4.557; empathy dimension (p-value = 0.001) OR = 5.614; audible dimension (p-value = 0.027) OR = 3.009; clarity dimension (p-value = 0.000) OR = 12.571; humble dimension (p-value = 0.009) OR = 3.600, and the most dominant dimension  interpersonal communication dimension is the clarity dimension. It is recommended that the Puskesmas continue to improving its services in terms of interpersonal communication through training such as service excellence.

Abstract

Community health centers must be accredited to ensure good quality of service. Community health center accreditation is recognition given to community health centers after meeting accreditation standards. Medan City has 41 community health centers, 39 of which have been accredited, predominantly basic and intermediate status (87%). The preliminary study describes the problems in implementing community health center accreditation, including communication, resources, attitudes of leaders and employees, organizational structure and support from health services. The aim of the research is to analyze the implementation of community health center accreditation policies in Medan City. This research uses a descriptive analysis method with a qualitative approach by means of observation, in-depth interviews and collecting documentation from research informants. The selection of research informants used purposive sampling. The main informants are people who are directly involved in implementing accreditation policies at community health centers. Research variables include communication, resources, disposition/attitude, organizational structure, and health service support. The research results show that communication at the community health center is running well, but the instruments and standards for community health center accreditation are still poorly understood. Human resources are not distributed evenly and some have excessive workloads, budget support and facilities are not optimal. The attitude of officers in general is good towards accreditation, but the distribution of tasks is not fair and equitable. The organizational structure has been determined, work procedures are in place. However, the commitment of the leadership and officers to carry out accreditation has not been developed. The health department supports the implementation of accreditation. However, coaching and supervision has not been scheduled and is not continuous. The conclusion was that communication, resources, disposition/attitude, organizational structure, and support from the health service were not fully functioning well, and obstacles were found in understanding and fulfilling accreditation standard documents. Suggestions for more intense, sustainable and quality guidance for community health centers in implementing accreditation policies.

Abstract

Pulmonary tuberculosis is an infectious disease of the lower respiratory tract. This disease occurs due to the presence of mycobacterium tuberculosis which can be transmitted through droplets that can occur from one individual to another and form colonization in the bronchioles or alveoli. This disease if left untreated or incomplete treatment can cause dangerous complications up to death. The estimated prevalence of TB in all cases is 660,000 and the estimated incidence is 430,000 new cases per year. The number of deaths from TB is estimated at 61,000 deaths per year according to the National Strategy for Pulmonary Tuberculosis Control. What often happens that can cause failure in treatment is because the patient does not carry out instructions or in determining the right and proper treatment given by health workers to patients, it does not necessarily go well if the patient himself does not take medication properly or according to the procedure for taking the drug for pulmonary TB patients. The formulation of the problem in this study was: "Analysis of adherence to drug use in patients with pulmonary tuberculosis on the success of therapy at the Sigli City Health Center, Pidie Regency in 2023. In this study the type of research used was a descriptive analytic approach with an analytic study design using a retrospective-prospective study observations on the independent and dependent variables at the same time. The sample used in this study was a total sample or total sampling of 72 people. The results in this study obtained data namely; There is a relationship between compliance with pulmonary TB patients and the success of therapy with a ρ-value of 0.000. There is no relationship between the age of patients with pulmonary TB and the success of therapy with a ρ-value of 0.403. There is no relationship between the gender of pulmonary TB sufferers and the success of therapy with a ρ-value. 0.246, There is a relationship between the knowledge of pulmonary TB sufferers and the success of therapy with a ρvalue of 0.016. There is no relationship between the educational level of pulmonary TB sufferers and the success of therapy with a ρ-value of 0.523. There is a relationship between the work of pulmonary TB sufferers and the success of therapy with a ρ-value. 0.002 and There is a relationship between family support for pulmonary TB patients and the success of therapy with a ρ-value of 0.013. the work variable is the most significant variable related to the success of TB patient therapy at the Sigli City Health Center, Pidie Regency in 2023 (p-value 0.005 <α = 0.05) with an Ods Ratio (Exp. B) of 36.075, which means that work has success with treatment 3.6 times more compared to those who do not work.

Abstract

The objective of this study is to determine and analyze the causing factors of non-compliance in paying National Health Insurance contributions (JKN) by independent participants and to find out the efforts made by the Social Security Agency on Health (BPJS) to increase compliance in paying JKN contributions and the efforts made by the Medan Government through Health Department to taking care the independent participants in arrears. This is a qualitative study with a phenomenological approach. Data collection is carried out by in-depth interviews and documentation. The informants in this research are Independent JKN Participants, BPJS Stakeholders and Health Department Stakeholders. The data processing uses the Miles Hubberman method. The results of the study show that the causing factors of non-compliance with paying JKN contributions are due to large amount of arrears, They are still able to seek health treatment using their ID card at the Puskesmas (Community Health Center), Economic Factors, Number of dependents (family members) and an Increasing in dues. Efforts that have been made by Health BPJS to increase compliance in paying JKN contributions are by providing convenience in payment methods, there are efforts to establish communication with independent JKN participants (through tele-collecting efforts, conduct WhatsApp blasts for delinquent participants, visit delinquent participants through JKN cadres, and conduct outreach in sub-districts and crowded places), as well as the existence of a Phased Payment Plan (REHAB) program where the arrears can be paid in installments. Efforts made by the Health Department and the Medan City Government include switching independent participants to JKN Contribution Assistance Recipient (PBI) participants and the Universal Health Coverage (UHC) Program through the Medan Berkah Health Insurance (JKMB) of Medan City.

Abstract

Tuberculosis is a direct infectious disease caused by Mycobacterium tuberculosis, most of the tuberculosis germs attack the lungs, but also affect other organs of the body. Tuberculosis is a health problem with a very high incidence and is a global health problem today. The healthy and sick status of family members and families influence one another. A disease that runs in a family affects the whole family and in turn affects the course of the disease and the status of family members. Therefore, the influence of the health/sickness status on the family influences or is highly dependent on one another. Family behavior in prevention plays an important role in reducing the risk of pulmonary TB transmission. The increasing number of pulmonary TB patients in Indonesia is caused by unhealthy behavior. The formulation of the problem in this study is the determinants of the incidence of pulmonary TB transmission in families who live in the same house at Putri Hijau Tkt Hospital. II Medan in 2023. The type of research is observational analytic research using the cross-sectional method, namely the independent variables and the dependent variable are observed simultaneously. The sample used in this study was a total sample or total sampling of 52 people. The results in this study indicate that there is no relationship between age and a P-value of 0.084, there is a relationship between gender and a P-value of 0.000, there is a relationship between work and a P-value of 0.001, there is a relationship between knowledge and a P-value of 0.001, There is a relationship between smoking habits with a P-value of 0.001, There is a relationship between nutritional status and a P-value of 0.001, There is a relationship between contact history and a P-value of 0.001. The most dominant factor related is the history of contact with Exp (B ) 108,168 in the prevention of pulmonary TB transmission in families at Putri Hijau Tkt Hospital. II Medan in 2023.

Abstract

This study aimed to assess the relationship between the knowledge of adolescent girls about anemia and its occurrence in Sigarang-Garang Village, Naman Teran Subdistrict, Karo District, in 2019. The pre-experimental research used a sample of 30 adolescent girls, revealing that a significant 70% had limited knowledge about anemia. Statistical analysis found a significant correlation between knowledge about anemia and its occurrence. The data further suggested that improving knowledge could potentially reduce the incidence of anemia. The study underscores the importance of educational interventions and outreach by healthcare professionals to bridge the knowledge gap and enhance the overall health of adolescent girls in the region.

Abstract

This study focused on comprehensive obstetric care provided to Mrs. I, a primigravida (G1P0A0) with a gestational age of 28 weeks 4 days, ranging from childbirth, postpartum, newborn care, to family planning counseling. The research location is Pratama Kita Berastagi Clinic in 2022. Through the case study method and qualitative approach, this research seeks to understand Mrs. I's experience and special needs in each phase she went through. The results showed that comprehensive midwifery care played an important role in improving the well-being of Mrs. I and her baby. The interventions provided during labor guarantee a smooth delivery process and minimize the risk of complications. During the postpartum, emotional support and education about self-care and newborns are crucial. Family planning counseling after childbirth also helped Mrs. I and her husband make the best decisions for their family's future. This study recommends the importance of a comprehensive approach in midwifery care to ensure the best quality of care for pregnant women, newborns, and family planning. These findings are expected to be a reference for clinics and other health practitioners in providing holistic midwifery care.

Abstract

One of the traditional rituals performed by the Toraja community, the Mangrara Banua ceremonial, involves the use of plants. The Tongkonan House, the Toraja tribe's traditional house, holds an annual celebration of thankfulness known as the Banua Mangrara. The goal of this ethnobotany study is to identify the kinds of plants that the Toraja people employ in their Banua Mangrara rite. The Mangrara Banua Tongkonan Randan Aya' Bebo's, the Northern Sangalla District, and the Banua Tungkonan Tok Serrek, Maruang Batu Rongko, Sopai District were the locations of the research from November 2022 to April 2023. Observations, interviews, and documentation are used to gather data. Purposive sampling and snowball sampling are used to choose respondents. Utilizing qualitative descriptive analysis, the data was examined. According to research, the Mangrara Banua Tongkonan ceremony of the Toraja tribe uses 23 species of plants from 15 families that are significant. Different roles for these plant species are played during the course of traditional ceremonies. The organs of plants that are most commonly used in this traditional ceremony are the leaves and stems, while certain species also use the fruit and umbilia. The plants used in these traditional ceremonies cannot be changed because they are an ancestor's tradition. Therefore, in an effort to protect the plants, the Toraja community is pursuing planting operations in the fields surrounding Tongkonan houses and gardens.
