Abstract

The implementation of strategic purchasing in Indonesia’s National Health Insurance (JKN) aims to enhance budget efficiency and service quality while preventing stagnant medical spending. This study seeks to evaluate the effectiveness of strategic purchasing, identify key implementation challenges, and formulate policy strategies to strengthen the scheme. Literature was sourced from Google Scholar and PubMed using relevant keywords and screened based on topic relevance and publication period (2015–2025). Six eligible articles were analyzed qualitatively. Findings suggest that strategic purchasing can improve access and quality of care, yet faces persistent issues such as unabsorbed capitation funds, overlapping regulations, and financial inefficiencies within BPJS Kesehatan. Strengthening governance, adopting digital health systems, and refining payment models are essential strategies to ensure a more sustainable and equitable JKN framework.

Abstract

The digitalization of health insurance administration offers considerable potential to enhance efficiency, transparency, and service accessibility. However, its implementation in Indonesia continues to face structural and operational challenges. This article aims to identify the opportunities and key barriers to adopting digital technology in the health insurance sector. A literature review method was applied, analyzing seven articles published between 2020–2025 obtained from Google Scholar and PubMed. The findings indicate that digital tools such as mobile applications, blockchain, and telemedicine have improved administrative processes and user experience. Nevertheless, issues such as digital literacy gaps, infrastructure limitations, and weak system integration continue to hinder the full realization of digitalization benefits. This study underscores the importance of strengthening human resource capacity, regularly updating digital platforms, and reinforcing regulatory frameworks to support an inclusive and sustainable digital transformation in health insurance services.

Abstract

Hypertension remains a global health concern demanding effective, affordable, and safe treatment alternatives. Orthosiphon stamineus Benth. (Java tea) has long been used traditionally to manage elevated blood pressure, yet no systematic review has comprehensively evaluated its efficacy and safety. This study aims to synthesize experimental and clinical evidence on the antihypertensive effects, mechanisms of action, and safety profile of O. stamineus. A systematic literature search was conducted using PubMed, Scopus, and ScienceDirect with the keywords “Orthosiphon stamineus,” “hypertension,” “blood pressure,” and related study designs. Article selection followed PRISMA guidelines, and risk of bias was assessed using SYRCLE for animal studies and RoB 2 for clinical trials. Twenty three relevant studies met the inclusion criteria, revealing consistent blood pressure-lowering effects through diuretic activity, inhibition of angiotensin-converting enzyme (ACE), and vasorelaxation via endothelial nitric oxide pathways. However, substantial heterogeneity in extraction methods, doses, and study designs limits conclusive generalization. The findings suggest that O. stamineus holds promise as a complementary therapy for hypertension, although further robust clinical trials are necessary to confirm its clinical utility. OR "Java tea") AND ("hypertension" OR "blood pressure") AND ("clinical trial" OR "animal study" OR "in vitro").

Abstract

Indonesia’s health system transformation is structured around six strategic pillars: primary care, referral services, system resilience, financing, human resources, and health technology. This study aims to assess the implementation progress and challenges associated with these pillars. A literature review was conducted using national and international databases (Google Scholar and ScienceDirect), focusing on peer-reviewed articles published between 2016 and 2025. Four studies were selected based on inclusion criteria that emphasized relevance to Indonesia’s health system transformation agenda. The review identified key challenges, including limited digital infrastructure, uneven distribution of the health workforce, rigid financing mechanisms, particularly within Public Service Agency (BLU) institutions and weak intersectoral coordination. Many facilities remain unprepared for digital integration, particularly the implementation of Electronic Medical Records (EMR). Although some improvements have been observed, structural barriers continue to hinder the overall progress. Strengthening digital readiness, enhancing workforce capacity, adopting more flexible budgeting mechanisms, and improving coordination, especially through platforms such as SATUSEHAT are recommended as strategic responses. However, this study has several limitations, such as the limited availability of articles that cover all six pillars comprehensively and most of the studies used emphasize legal and administrative aspects rather than specific analysis in the health sector. These limitations limit the generalization of the findings. These findings are expected to inform more adaptive and equitable health policy reforms in Indonesia.
