Abstract

Oral and dental hygiene remains a significant public health concern among adolescents, largely due to inadequate knowledge and limited exposure to effective health education. Preliminary findings at SMAN 1 Porong indicated that 50% of students exhibited poor plaque indices, reflecting insufficient awareness of proper oral hygiene practices. This study aimed to evaluate the effectiveness of TikTok-based educational media in improving students’ knowledge of oral and dental hygiene. A quasi-experimental design with a one-group pretest–posttest approach was employed. A total of 52 tenth-grade students were selected using simple random sampling. Data were collected through a structured questionnaire distributed via Google Forms before and after the intervention. The educational intervention consisted of delivering oral health content through TikTok media. Data analysis was conducted using the Wilcoxon Signed-Rank Test to assess differences in knowledge scores before and after the intervention. The results demonstrated a substantial improvement in students’ knowledge following the intervention. Prior to education, 96% of respondents were categorized as having low knowledge, whereas after the intervention, 88% achieved a good knowledge level. Statistical analysis revealed a significant difference between pretest and posttest scores (p = 0.000; p < 0.05), indicating the effectiveness of TikTok as an educational medium. In conclusion, TikTok-based health education significantly enhances adolescents’ knowledge of oral and dental hygiene. The findings suggest that integrating social media platforms into health promotion strategies can serve as an innovative and engaging approach to improve health literacy among students. Future research is recommended to explore the long-term impact of such interventions and to compare the effectiveness of different digital media platforms.

Abstract

Tooth impaction represents a disturbance in the normal eruption process, predominantly affecting permanent dentition, while its occurrence in primary teeth remains exceedingly rare. Such anomalies may lead to complications including delayed eruption of permanent successors, malocclusion, and potential damage to adjacent anatomical structures. However, limited evidence exists regarding the optimal management of impacted primary molars, particularly in atypical cases where the primary tooth is located inferior to its permanent successor. This study aims to report and evaluate the clinical management of a rare case involving an impacted mandibular first primary molar positioned beneath its succedaneous premolar, with emphasis on the outcomes of a conservative treatment approach. A 7-year-old male patient presenting with dental malposition underwent comprehensive clinical and radiographic examination, including panoramic radiography and computed tomography. Due to the close proximity of the impacted tooth to the mandibular canal and mental foramen, a non-invasive strategy consisting of periodic monitoring, space maintenance, and interceptive orthodontic treatment was implemented. Longitudinal follow-up over a five-year period demonstrated delayed but progressive development of the permanent premolar. Clinical and radiographic findings revealed initiation of eruption in 2024, followed by complete eruption in 2025 without surgical intervention or associated complications. These findings indicate that, despite initial developmental delay, spontaneous eruption remained achievable under careful supervision. In conclusion, conservative management with regular follow-up may represent a safe and effective alternative to surgical intervention in selected cases of impacted primary molars, particularly when anatomical risks are significant. This approach supports preservation of surrounding structures while allowing natural eruption processes, highlighting the importance of individualized, risk-based treatment planning in pediatric dentistry.

Abstract

Poor glycemic control and the high prevalence of complications among individuals with type 2 diabetes mellitus (T2DM) remain major public health challenges and are strongly associated with inadequate self-care behaviors. Despite the growing implementation of self-care–based interventions, evidence regarding their effectiveness on glycemic outcomes and self-care activities remains inconsistent. This systematic review aimed to evaluate the effectiveness of self-care enhancement–based interventions in improving glycemic control and self-care behaviors among adults with T2DM. A systematic search was conducted across four electronic databases PubMed, ScienceDirect, ProQuest, and Google Scholar using relevant keywords related to self-care, intervention, and T2DM. Studies published between 2019 and 2024, written in English, available in full text, and employing randomized controlled trial or quasi-experimental designs were included. Article selection followed the PRISMA framework, and methodological quality was assessed using the Joanna Briggs Institute (JBI) critical appraisal tools. Eight eligible studies were included in the final synthesis. The findings demonstrated heterogeneous outcomes across interventions. Most studies (n = 5) reported significant reductions in HbA1c levels accompanied by improvements in self-care activities, including diet management, blood glucose monitoring, and self-efficacy. One study showed a significant reduction in HbA1c without corresponding improvements in self-care behavior, while two studies reported improvements in self-care activities without significant changes in HbA1c. Interventions that incorporated structured, personalized education and ongoing monitoring, supported by family or community involvement, consistently yielded more favorable outcomes. In conclusion, self-care enhancement–based interventions are generally effective in improving glycemic control and self-care behaviors among individuals with T2DM, particularly when delivered through a structured, person-centered approach integrated with social support systems. These findings highlight the importance of comprehensive and context-sensitive intervention designs in diabetes self-management programs.

Abstract

Indonesia continues to face a high maternal mortality ratio, with delays in emergency obstetric care at referral hospitals remaining a critical contributor, despite the implementation of Comprehensive Emergency Obstetric and Neonatal Care (PONEK) services. These delays are frequently associated with inefficiencies in workflow and response time, particularly during post-triage management. This study aimed to explore healthcare workers’ lived experiences and to identify organizational, procedural, and systemic factors influencing workflow efficiency and response time in PONEK services at Y Hospital, Jakarta, a teaching hospital, referral center, and certified Syariah-based institution. A qualitative descriptive phenomenological design was employed, involving 15 purposively selected participants consisting of obstetricians, emergency physicians, midwives, nurses, administrative staff, and hospital managers directly engaged in PONEK services. Data were collected through in-depth interviews, non-participant observations, focus group discussions, and document review, and were analyzed using thematic analysis supported by content and SWOT analyses to ensure analytical rigor and triangulation. The findings revealed that while triage response time consistently met international standards of less than five minutes, significant delays occurred in subsequent stages of care due to fragmented inter-unit coordination, reliance on informal communication, inconsistent implementation of Standard Operating Procedures, and suboptimal utilization of the Hospital Management Information System (SIMRS). Additional barriers included incomplete referral documentation and the presence of non-emergency cases within the PONEK pathway, which disrupted prioritization and prolonged patient length of stay. In conclusion, workflow inefficiencies in PONEK services at Y Hospital are primarily driven by coordination gaps, human resource limitations, and insufficient digital integration rather than infrastructural shortcomings. Strengthening cross-unit collaboration, improving referral accuracy, enhancing staff capacity through continuous training, and optimizing digital systems are essential strategies to improve response time, service quality, and maternal emergency outcomes, thereby supporting hospital performance and contributing to the achievement of Sustainable Development Goal 3.1 on maternal mortality reduction.

Abstract

The rapid expansion of digital technologies has transformed healthcare marketing; however, the adoption of Syariah-compliant maternity services remains relatively low despite high social media usage among executive patients in Indonesia. Limited evidence exists on how culturally and religiously aligned digital marketing strategies influence patient decision-making, particularly through Instagram. This study aims to analyze the effectiveness of Instagram-based digital marketing strategies in influencing executive patients’ intention to utilize Syariah maternity services at YARSI Hospital, Jakarta. A sequential explanatory mixed-methods design was employed. Quantitative data were collected from 19 executive patients and analyzed using Spearman correlation to examine the relationship between five marketing dimensions informativeness, usefulness, authenticity, entertainment, and content variety and patient visit intention. Subsequently, qualitative data from 19 key informants, including patients, medical staff, and marketing personnel, were analyzed thematically to provide deeper insights into the quantitative findings. The results indicate that informativeness (r = 0.533; p = 0.019), usefulness (r = 0.492; p = 0.032), and authenticity (r = 0.482; p = 0.037) have significant positive effects on patient visit intention. In contrast, entertainment (r = –0.596; p = 0.007) and excessive content variety show negative associations. Qualitative findings reinforce that clear, practical, and culturally aligned content enhances trust and engagement, whereas low posting frequency and limited interactivity hinder marketing effectiveness. In conclusion, digital marketing strategies that prioritize informative, useful, and authentic content are more effective in promoting Syariah-compliant maternity services. Integrating cultural and religious values into digital communication is essential to strengthen patient trust and engagement. These findings provide practical implications for healthcare providers to optimize social media strategies in culturally sensitive contexts.

Abstract

Therapeutic communication represents a fundamental component of professional nursing practice, directly influencing patient satisfaction, treatment adherence, and overall healthcare outcomes. Despite its recognized importance, nurses often encounter challenges in implementing therapeutic communication effectively due to various interpersonal, organizational, and technological complexities. Therefore, this study aims to systematically synthesize existing evidence on the dimensions, barriers, and innovative strategies of therapeutic communication in nursing practice to enhance patient outcomes. This study employed a systematic review design, analyzing articles retrieved from databases including PubMed, Scopus, and ProQuest. The search was limited to publications between 2018 and 2022. A total of 103 articles were initially identified, and after removing duplicates and conducting eligibility screening based on predefined inclusion criteria, 8 studies were selected following the PRISMA guidelines. Data were extracted and analyzed to identify key themes and patterns related to therapeutic communication. The findings revealed several critical dimensions of therapeutic communication, including empathy, emotional support, verbal and non-verbal communication skills, and the establishment of collaborative nurse–patient relationships. Additionally, common barriers such as workload, time constraints, and communication gaps were identified. The integration of digital technologies and artificial intelligence also emerged as an innovative approach to enhancing communication effectiveness in healthcare settings. In conclusion, therapeutic communication remains a vital determinant of patient-centered care and positive health outcomes. Strengthening nurses’ communication competencies, addressing systemic barriers, and leveraging technological innovations are essential strategies to optimize nurse–patient interactions. Future research should focus on developing standardized communication frameworks and evaluating the effectiveness of technology-assisted communication interventions in diverse clinical contexts.

Abstract

Smoking remains a major public health concern and is widely recognized as an important behavioral risk factor for poor dental and oral health. Tobacco exposure contributes to plaque accumulation, calculus formation, gingival inflammation, and impaired oral tissue healing. However, evidence regarding the relationship between smoking habits and oral hygiene status in Indonesian primary health care settings is still limited. This study aimed to analyze the association between smoking habits and dental and oral hygiene status among patients attending the Dental Clinic of the Kesamben Community Health Center, Jombang Regency. A quantitative cross-sectional analytical design was employed in March 2025. The study involved 30 male patients aged ≥25 years who were selected using purposive sampling. Smoking habits were assessed using a structured 20-item questionnaire and categorized into low, moderate, and high levels based on cumulative scores. Dental and oral hygiene status was evaluated using the Simplified Oral Hygiene Index (OHI-S), which classifies hygiene conditions into good, moderate, and poor categories. Data were analyzed using univariate and bivariate statistical methods, including the Chi-square test, Spearman correlation analysis, and Phi effect size, with a significance level set at p < 0.05. The results showed that the majority of respondents had high smoking intensity (93.4%) and poor oral hygiene status (96.6%). Statistical analysis revealed a significant association between smoking habits and oral hygiene status (χ² = 7.226; p = 0.007). Spearman correlation analysis indicated a moderate positive relationship between smoking intensity and worsening OHI-S scores (r = 0.52; 95% CI: 0.18–0.74), with a moderate effect size (φ = 0.49). In conclusion, higher smoking intensity is significantly associated with poorer dental and oral hygiene among adult patients at the Kesamben Community Health Center. These findings highlight the importance of integrating smoking cessation counseling with preventive oral health education in primary health care services to improve overall oral health outcomes.

Abstract

Pediatric obstructive sleep apnea (OSA) represents a significant yet often underdiagnosed respiratory disorder associated with adverse neurocognitive, cardiovascular, and developmental outcomes. Despite its clinical importance, early detection remains challenging, particularly in cases where symptoms overlap with common pediatric conditions. This study aims to highlight the critical role of pediatric dental assessment in the early identification and multidisciplinary management of severe OSA through a detailed case report. An 8-year-old male patient was referred for dental malpositions and molar-incisor hypomineralization. Comprehensive clinical, radiographic, and functional evaluations revealed craniofacial abnormalities indicative of airway obstruction. Screening using a validated pediatric questionnaire suggested OSA, which was subsequently confirmed by ventilatory polygraphy, demonstrating a severe apnea–hypopnea index (AHI) of 34.5 events/hour. A multidisciplinary treatment approach was implemented, including rapid maxillary expansion, mandibular advancement orthosis, nasal dilation, and myofunctional therapy. Due to limited access to polysomnography, follow-up relied on clinical observations and caregiver-reported outcomes. After six months, the patient exhibited notable improvements in respiratory function, sleep quality, and daytime alertness, along with enhanced facial aesthetics and oral function. These findings are consistent with existing evidence supporting orthodontic and functional interventions in managing pediatric OSA, particularly in cases associated with craniofacial abnormalities. In conclusion, this case underscores the pivotal role of pediatric dentists as frontline healthcare providers in recognizing early signs of OSA and initiating timely referrals. Integrating dental assessments into routine screening protocols can facilitate early diagnosis and coordinated multidisciplinary care, ultimately improving patient outcomes and preventing long-term complications.

Abstract

Maternal mortality remains a major public health challenge in Indonesia, and limited knowledge of high-risk pregnancy is one of the factors that may delay appropriate decision-making and reduce women’s confidence in managing pregnancy-related complications. This study aimed to examine the effect of flipchart-based health education on the knowledge and self-efficacy of pregnant women regarding high-risk pregnancy. A pre-experimental study with a one-group pretest-posttest design was conducted at the Kapas Health Center, Bojonegoro Regency. The population consisted of 302 pregnant women, and a sample of 51 participants was selected using the Lemeshow formula. The independent variable was health education delivered through flipchart media, while the dependent variables were knowledge and self-efficacy regarding high-risk pregnancy. Data were collected using structured questionnaires and analyzed using the Wilcoxon signed-rank test with a significance level of 0.05. The results showed a statistically significant improvement in knowledge after the intervention (p = 0.008). Before the intervention, 27 participants (53%) had poor knowledge, 20 (39.2%) had adequate knowledge, and 4 (7.8%) had good knowledge. After the intervention, the proportion with poor knowledge decreased to 16 participants (31.4%), while 24 (47%) had adequate knowledge and 11 (21.6%) had good knowledge, indicating that 68.6% achieved adequate-to-good knowledge after the intervention. A significant improvement was also observed in self-efficacy (p = 0.000). Prior to the intervention, 12 participants (23.5%) had low self-efficacy and 39 (76.5%) had high self-efficacy; after the intervention, all participants (100%) demonstrated high self-efficacy. In conclusion, flipchart-based health education was effective in improving both knowledge and self-efficacy among pregnant women regarding high-risk pregnancy. This approach may serve as a practical, low-cost, and accessible educational strategy for use in community health centers to support better maternal awareness, confidence, and decision-making.

Abstract

Stunting remains a major public health challenge, particularly in developing countries, because it negatively affects children’s physical growth, cognitive development, and future productivity. The first 1,000 days of life constitute a critical period for growth and development, during which nutritional deficiencies and inadequate maternal understanding of child health may increase the risk of stunting. Limited knowledge and unfavorable attitudes among mothers toward stunting prevention may hinder early preventive efforts. Therefore, effective educational interventions are needed to improve maternal awareness and promote positive health-related attitudes during this crucial period. This study aimed to determine the effect of animated video-based health education on mothers’ knowledge and attitudes regarding stunting prevention during the first 1,000 days of life. A quasi-experimental study with a pre-test–post-test control group design was conducted in the working area of the Sidotopo Wetan Health Center, Surabaya, Indonesia. A total of 44 respondents were selected using cluster sampling and divided into intervention and control groups. The independent variable was health education delivered through animated videos, while the dependent variables were mothers’ knowledge and attitudes toward stunting prevention. Data were collected using structured knowledge and attitude questionnaires. Statistical analysis was performed using the Wilcoxon Signed Rank Test to assess within-group differences and the Mann–Whitney U Test to compare differences between groups.  The findings revealed a statistically significant improvement in mothers’ knowledge and attitudes after receiving animated video-based health education on stunting prevention during the first 1,000 days of life (p < 0.05). Mothers in the intervention group demonstrated better post-test outcomes than those in the control group. In conclusion, animated video-based health education is effective in improving mothers’ knowledge and attitudes toward stunting prevention. This approach may serve as an engaging and practical educational strategy to strengthen maternal awareness and support early stunting prevention programs in community health settings.
