Abstract

Hypertension remains a major global health problem with a high prevalence in Indonesia and requires long-term management. Although pharmacological therapy is effective, patient adherence is often suboptimal due to concerns about long-term side effects, highlighting the need for safe, simple, and self-managed non-pharmacological alternatives. This study aimed to evaluate the effectiveness of Mudra Sujok therapy as a tool-free non-pharmacological intervention in hypertension management, focusing on blood pressure reduction, sleep quality improvement, and patient adherence. An observational case study design was employed involving two hypertensive patients who performed Mudra Sujok therapy independently for 14 days at Pandanwangi Primary Health Center, Malang City. Data were collected through interviews, blood pressure measurements, and sleep quality assessments. The results demonstrated reductions in both systolic and diastolic blood pressure, improved sleep quality, and high levels of adherence to the therapy among both subjects. The novelty of this study lies in the application of Mudra Sujok as a single, tool-free Sujok method that is simple, safe, and easily replicated, thereby supporting patient self-management and family involvement in hypertension care. Mudra Sujok therapy can be performed independently with family support, such as reminding therapy schedules, creating a relaxing home environment, and encouraging adherence to non-pharmacological hypertension management. These findings contribute to the development of a more holistic hypertension management model in primary healthcare settings. Further studies with larger sample sizes and experimental designs are recommended to confirm the long-term effects of Mudra Sujok therapy.

Abstract

Diabetes Mellitus (DM) is a chronic disease that requires strong adherence in its management. However, dietary compliance is often influenced by demographic, psychosocial, resource, and cultural factors. The purpose of this study was to analyze the influence of demographic factors, cognition and emotion, personal preferences, resources, and culture on dietary adherence among DM patients at Bhayangkara H.S. Samsoeri Mertojoso Hospital, Surabaya. This study employed a cross-sectional design with a sample of 105 DM patients. Data were collected using a structured questionnaire and analyzed through logistic regression. The findings revealed that the majority of respondents were non-adherent to the DM diet. Demographic factors (age, sex, education, occupation, marital status) contributed 41.3% to dietary adherence. Knowledge, attitudes, and personal preferences explained 63.7% of the variation, while limited resources and negative cultural practices increased the risk of non-adherence by 27.8–41.7 times. Respondents who adhered to the dietary recommendations tended to have better-controlled blood glucose levels. Logistic regression analysis indicated that demographic factors collectively influenced dietary adherence (p = 0.000). In addition, cognition and emotion (knowledge and attitudes) along with personal preferences significantly affected DM dietary compliance (p = 0.000). Furthermore, resource constraints and cultural influences related to lifestyle and health history were also significantly associated with dietary adherence (p = 0.000). In conclusion, dietary adherence in DM is not solely a matter of individual discipline but rather the result of a complex interaction between knowledge, attitudes, social support, access to resources, and cultural values. Culturally sensitive educational approaches, strengthening family support, and human-centered interventions are essential strategies to ensure successful DM management

Abstract

Acid-fast bacilli (AFB) smear testing in TB patients plays an important role in diagnosis and is crucial to the success of TB control. Many factors influence compliance with AFB smear testing. The aim of this study was to analyze knowledge of AFB testing as a predictor of TB patient compliance with AFB testing at Bhayangkara HS Samsoeri Mertojoso Hospital in Surabaya. This study used a cross-sectional design from March to May 2025 with a sample of 85 TB patients selected by simple random sampling according to inclusion criteria. Data were collected using a questionnaire and then analyzed using logistic regression. The results of the study explain that the majority of respondents were of productive age, worked as private employees, lived <10 km from health facilities, had a high school education, and had good knowledge about BTA. Knowledge had a significant effect on compliance (P value = 0.000; R2 = 0.389 and PR = 9.249). Patients with good knowledge were 9.249 times more likely to comply with BTA examinations. Respondent compliance was high but not optimal in terms of the timeliness of examinations and retests after treatment. Patients' technical understanding of the function of sputum smear microscopy is still lacking, affecting their perception of compliance. These findings are consistent with the Health Belief Model and Social Support Theory, emphasizing that education, access, and family support integrated into patient support strategies can improve compliance. TB control efforts require contextual education, family involvement, and continuous monitoring to ensure that sputum smear microscopy is performed on time and completely.

Abstract

Open defecation remains a serious challenge in public health development, especially in rural areas of Indonesia. This study was conducted to analyze the influence of cultural factors and the availability of sanitation facilities on open defecation behavior in the working area of the Krejengan Probolinggo Community Health Center. This study used a cross-sectional design. The study population consisted of all married patients who visited the Krejengan Community Health Center in Probolinggo Regency, totaling 13,871 patients, with a sample of 100 people who met the inclusion criteria. Data were collected using questionnaires and analyzed using logistic regression. The results showed that, partially, cultural factors were also proven to have an effect, with a contribution of 8.9%, where negative culture increased the likelihood of open defecation by 2.972 times. The availability of sanitation facilities had the strongest influence, with a contribution of 9.2% and a prevalence ratio of 3.009. Multivariate analysis showed that only the availability of sanitation facilities variable was significantly influential when all variables were tested together. This confirms that sanitation infrastructure is a key enabling factor in changing open defecation behavior. These findings reinforce the importance of a structural approach to public health interventions, in which the provision of basic facilities must be a top priority before attempting to change behavior through education. Community- and culture-based interventions remain necessary to support the sustainability of the Stop Open Defecation program.

Abstract

Breech presentation pregnancies often cause physical discomfort and anxiety, which can impact maternal well-being. The knee-chest intervention is a non-pharmacological intervention that, physiologically, has the potential to improve maternal comfort, but scientific evidence remains very limited. This study aimed to analyze the effect of the knee-chest position on the comfort of pregnant women with breech presentation, providing evidence that can inform clinical practice and improve patient care. This study used a quasi-experimental design with a control group, involving nineteen third-trimester pregnant women with breech presentation divided into an intervention group (n=10) and a control group (n=9). The intervention group received routine knee-chest exercises, while the control group received standard antenatal care. Comfort levels were measured using the General Comfort Questionnaire (GCQ) before and after the intervention. Data analysis was both descriptive and inferential, comparing changes in comfort scores across groups to ensure reliable results for healthcare professionals and researchers. The intervention group showed a significant increase in the total GCQ comfort score after the knee-chest intervention, encompassing physical, psychospiritual, and environmental aspects, compared with the control group. Clarifying how these improvements translate to maternal well-being can help readers assess practical benefits

Abstract

Labor pain primarily arises from uterine contractions that activate the sympathetic nervous system, leading to changes in blood pressure, heart rate, and breathing patterns. When not managed properly, this pain may intensify anxiety, tension, fear, and psychological stress. This study aimed to examine the effect of counterpressure massage in reducing labor pain. This quantitative research used an experimental approach with a pre-experimental one-group pretest–posttest design. The study was conducted at Petrokimia Gresik Hospital. The population consisted of all women who delivered at the hospital during January–February 2025. Using accidental sampling, 40 respondents were recruited. The independent variable was the administration of counterpressure massage during the first stage of labor, while the dependent variable was the reduction of pain in the first stage of labor. Data were analyzed using SPSS, and the Wilcoxon test was applied for statistical testing. The study took place from January to February 2025. Results indicated that before receiving counterpressure massage, most respondents reported severe pain (27 participants; 67.5%). After the intervention, the majority experienced moderate pain (22 participants; 55%). Statistical analysis showed that counterpressure massage significantly reduced labor pain at Petrokimia Gresik Hospital (p = 0.000, p < 0.05). These findings suggest that counterpressure massage meaningfully lowers pain intensity during labor, helping the delivery process proceed more smoothly and with fewer complications. Therefore, incorporating counterpressure massage into an official standard operating procedure (SOP) in the delivery room is recommended as a formal pain-reduction measure, so that laboring mothers experience less anxiety and tension and a decreased perception of pain

Abstract

Schizophrenia is a chronic mental disorder that requires long-term treatment and high medication adherence to prevent relapse. However, non-adherence to medication among patients with schizophrenia remains a major problem, particularly after patients return to the family environment. As the closest support system, families play an important role in supervising, motivating, and assisting patients to adhere to medication regimens. This study aimed to analyze the relationship between family role and medication adherence among patients with schizophrenia at Sofifi Mental Hospital, North Maluku. This study employed a quantitative correlational design with a cross-sectional approach. The study population consisted of families of patients with schizophrenia at Sofifi Mental Hospital, with a sample of 60 respondents selected using accidental sampling. Data were collected using a family role questionnaire as a medication supervisor and the Morisky Medication Adherence Scale (MMAS-8). Data analysis was conducted using descriptive statistics and inferential analysis with the Spearman Rank correlation test. The results showed that most families had a moderate to good level of family role, with a mean score of 26.4 ± 5.8. Medication adherence among patients was categorized as moderate, with a mean score of 6.1 ± 1.7. The Spearman Rank test indicated a significant and positive relationship between family role and medication adherence among patients with schizophrenia (r = 0.56; p-value = 0.001). These findings indicate that better family involvement in providing support, supervision, and emotional control is associated with higher medication adherence among patients with schizophrenia. Therefore, family-based mental health nursing interventions should be optimized to enhance medication adherence and reduce relapse rates among patients with schizophrenia
