Abstract

Background & Objective: Retaining young health workers is a major challenge in the healthcare system, especially in the post-pandemic era. Transformational leadership is seen as one of the strategic approaches that has the potential to increase employee loyalty and well-being, but the mechanisms of its influence on retention intentions are not yet fully understood, particularly through psychological roles such as job satisfaction and well-being. This study aims to analyze the influence of transformational leadership on the retention of young healthcare workers and to test the mediating role of job satisfaction and well-being in this relationship. Method: The study employs a quantitative approach with an explanatory design. Data were collected from 132 young healthcare workers in Central Java via a closed-ended questionnaire. The instruments were adapted from the MLQ scale, Spector, WHO-5, and Tremblay. Path analysis was conducted to test direct and indirect relationships between variables, and the Sobel test was used to verify mediation. Result: The results indicate that transformational leadership significantly influences job satisfaction (β=0.451; p<0.001) and well-being (β=0.417; p<0.001), both of which also significantly influence retention. Leadership also has a direct effect on retention (β=0.178; p=0.041), indicating partial mediation. The Sobel test shows that job satisfaction and well-being are significant mediators. Conclusion: Transformational leadership plays a crucial role in shaping young healthcare workers' decisions to stay, through the creation of positive job satisfaction and psychological well-being. Managerial strategies that integrate humanistic leadership approaches and psychosocial interventions are deemed essential for building a sustainable work system.

Abstract

Background & Objective: Bayan Community Health Center, with a total of 156,868 referrals during the period of January to December and a total of 1,056,436 visits. This number has increased, whereas in 2022, the number of BPJS referral patients was only 53,065 with a total of 392,868 visits. To determine patients' perceptions of the implementation of the BPJS Health Patient Referral System at Bayan Community Health Center in Purworejo, an observational descriptive study was conducted. Method: A cross-sectional approach was used, and the data were analyzed quantitatively. Result: Patient perceptions regarding the BPJS referral system at the Bayan Health Center were categorized as high on the following indicators: ease of the referral process (average index of 68.1), satisfaction with referral services (average index of 67.8), effectiveness and appropriateness of referrals (average index of 67.3), and challenges in referral implementation in the moderate category with an average index of 61.8. Conclusion: Patients' perceptions of the BPJS referral system at the Bayan Community Health Center are high in terms of ease, satisfaction, and effectiveness of referrals. However, there are still obstacles in its implementation, which are in the moderate category.

Abstract

Background & Objective: Patient safety is a critical component in improving the quality of hospital services. However, various patient safety incidents are still occurring due to suboptimal implementation. The Quality Committee plays a strategic role in managing patient safety, including program development, incident investigation, reporting, and evaluation. This study aims to analyze patient safety management by the Quality Committee, focusing on inpatient nursing services at Rumah Sakit Islam Surakarta. Method: This research employed a qualitative approach with a phenomenological method. Data were collected through in-depth interviews with six inpatient nurses and direct field observations. Thematic analysis was conducted through data reduction, data presentation, and conclusion drawing. Result: The findings indicate that the implementation of patient safety management by the Quality Committee has not been fully optimized. Key obstacles include underreporting of incidents, limited staff understanding of standard operating procedures (SOPs), lack of regular education and training, and weak documentation and coordination systems. The committee’s role has not yet fully integrated the functions of planning, organizing, implementing, and controlling in the context of patient safety. Conclusion: Patient safety management at Rumah Sakit Islam Surakarta requires strengthening in terms of system structure, safety culture, and active nurse involvement. Stronger leadership support, enhanced human resource capacity, and optimal performance of the Quality Committee are essential to establish a structured and sustainable patient safety system.

Abstract

Introduction & Objective: Neonatal anemia is a common health problem, particularly in developing countries, and contributes to increased morbidity and mortality in infants. One effective intervention that can be implemented immediately after birth is delayed cord clamping, which involves delaying the clamping of the umbilical cord for 1–3 minutes to allow for blood transfusion from the placenta to the infant. This practice is associated with increased hemoglobin levels in newborns. Method: This study is a literature review analyzing seven selected research articles published between 2019 and 2024, from both national and international journals. Inclusion criteria included experimental or quasi-experimental studies evaluating the effects of delayed cord clamping on hemoglobin levels and newborns. Data were analyzed narratively and comparatively. Result: All analyzed studies showed that delayed umbilical cord clamping has a positive impact on increasing hemoglobin levels and the hematological status of newborns. Newborns who underwent delayed umbilical cord clamping had higher hemoglobin, hematocrit, and iron stores compared to those who had their umbilical cord clamped earlier. Some studies also noted improved blood pressure stability and organ perfusion, particularly in preterm infants. Conclusion: Delayed cord clamping is a simple, safe, and effective intervention that can increase hemoglobin levels and improve the hematological status of newborn infants. The routine implementation of delayed cord clamping should be considered in delivery protocols as a preventive measure against neonatal anemia.

Abstract

Background & Objective: Family planning is an effort to regulate childbirth, including the spacing and ideal age of childbirth, as well as pregnancy, through promotion, protection, and assistance in accordance with reproductive rights, in order to realize a quality family. Therefore, family planning is a government program designed to balance population needs. Contraceptive use aims to fulfill reproductive rights, help people plan the number and timing of their children, and prevent unwanted pregnancies. Proper contraceptive use can reduce the risk of maternal and infant mortality. Therefore, ensuring access to quality family planning programs should be a priority in health services. The purpose of this literature review is to determine the factors that influence the selection of contraceptives in women of childbearing age. Method: The research method used is a literature review study that tries to explore information about the factors that influence the selection of contraceptives in women of childbearing age.The type of data used is secondary data. Sources for conducting this literature review include a systematic search study of computerized databases on Google Scholar in the form of research journals totaling 5 articles. The research journals used are from 2020 to 2024. Result: The results of the research analyzed by the researchers, from 5 journals, showed that the main factors influencing the selection of contraceptives were knowledge and husband support. While other factors that influence the selection of contraceptives are age, education, occupation and attitude. Conclusion: Increase education to husbands and always involve husbands in family planning counseling. So that husbands can provide appropriate support to their wives and can strengthen couple communication in making contraceptive decisions.

Abstract

Introduction & Objective: Preeclampsia and eclampsia remain significant health issues contributing to high maternal and perinatal mortality rates, particularly in developing countries. Preeclampsia, characterized by hypertension, edema, and proteinuria during pregnancy, affects 2% to 8% of pregnancies globally and is a major cause of maternal and fetal death. This study aims to review the current management strategies for preeclampsia to prevent its progression into eclampsia and improve maternal and neonatal outcomes. Method: A literature review was conducted using secondary data sources. Articles were obtained through systematic searches in databases such as PubMed and Google Scholar, using keywords like "Management" and "Preeclampsia" from publications between 2020 and 2024 in English and Indonesian. Results: Management of preeclampsia involves several key components: blood pressure control using antihypertensives such as nifedipine and methyldopa; seizure prevention through magnesium sulfate administration; decision-making regarding timing and mode of delivery; careful fluid management to avoid pulmonary edema; nutritional management focusing on sodium restriction and balanced diet; physical activity to lower gestational hypertension risk; and long-term follow-up due to increased cardiovascular risk post-preeclampsia. The literature indicates that a combination of interventions is more effective than single therapy approaches. Conclusion: Comprehensive management of preeclampsia is essential to prevent disease progression and ensure favorable outcomes for both mother and child. Integrated short- and long-term strategies are necessary to improve maternal safety and long-term health quality.

Abstract

Background & Objective: Chronic kidney disease (CKD) leads to progressive loss of renal function requiring hemodialysis as a substitute therapy. Compliance with fluid restriction is crucial to prevent fluid overload (hypervolemia) which can cause complications such as pulmonary edema and hypertension. This study aims to analyze the relationship between fluid restriction compliance and hypervolemia occurrence in CKD patients undergoing hemodialysis at RSD Gunung Jati Cirebon. Method:. A descriptive correlational study with a cross-sectional approach was conducted on 98 CKD patients selected by purposive sampling. Data on fluid restriction compliance were collected using a validated questionnaire, while hypervolemia status was assessed by observation and body weight measurement. Data analysis used Spearman Rank correlation. Result: Among respondents, 63.3% were compliant with fluid restriction and 36.7% were non-compliant. Hypervolemia levels showed 40.8% mild, 28.6% moderate, and 30.6% severe. Statistical analysis found a significant negative correlation between fluid restriction compliance and hypervolemia occurrence (p=0.000; r=-0.756). Conclusion: There is a significant relationship between compliance with fluid restriction and hypervolemia among CKD patients undergoing hemodialysis. Increasing patient compliance is important to prevent complications. More specific and targeted treatment.

Abstract

Background & Objective: Tuberculosis (TB) is an infectious disease with high prevalence in Indonesia, often accompanied by psychological problems such as anxiety that may affect sleep quality. This study aimed to analyze the relationship between anxiety and sleep quality among TB patients in the working area of Sitopeng Puskesmas, Cirebon City. Method: This study used a descriptive correlational design with a cross-sectional approach. The total population of 40 TB patients was sampled using total sampling, resulting in 32 respondents. Anxiety was measured using the Hamilton Rating Scale for Anxiety (HRS-A), and sleep quality was assessed through a validated questionnaire. Data were analyzed using Chi-Square test with significance level of α=0.05. Results: Of the respondents, 75.0% experienced no anxiety, 6.3% had mild anxiety, 6.3% moderate anxiety, and 6.3% severe anxiety. Regarding sleep quality, 68.8% of patients had good sleep quality, while 31.2% had poor sleep quality. The analysis showed a significant relationship between anxiety and sleep quality (p = 0.009). Conclusion: There is a significant relationship between anxiety and sleep quality among tuberculosis patients. It is suggested that nurses provide psychological support and sleep hygiene education for TB patients to improve their well-being.

Abstract

Background & Objective: Back pain in pregnant women is one of the most common discomforts experienced by pregnant women. This back pain is typically felt by pregnant women during the third trimester of pregnancy, around 28–40 weeks of gestation. One approach to reducing back pain involves the use of kinesiotaping. Kinesiotaping is a new method of applying tape used to prevent and rehabilitate injuries. The objective of this study is to review literature, articles, and research documents on the management of back pain in pregnant women using kinesiotaping. Method: The method used is a literature review, involving the collection of data from the Google Scholar database from 2022 to 2025. The selected literature data were full-text journals in Indonesian. Result: Kinesio taping is a non-pharmacological method used to alleviate back pain in pregnant women. This therapy works by stabilizing joints, improving muscle function and position, and reducing pain receptor stimulation, thereby helping to reduce pain and strengthen abdominal muscles. Conclusion: Based on the literature review, kinesiotaping has been proven to be an effective non-pharmacological method for alleviating back pain in pregnant women, particularly in the third trimester. This therapy improves joint stability, enhances muscle function, and reduces pain receptor stimulation, thereby providing comfort and improving the quality of life for pregnant women.
