Abstract

The use of sodium cyclamate, an artificial sweetener, as an additive in food and beverage production is commonly found among vendors around schools and campuses. Artificial sweeteners such as sodium cyclamate are often used as substitutes for sugar to reduce production costs. Although its dosage has been regulated by government and health authorities, these regulations are frequently ignored by vendors who prioritize profit without considering product quality and nutritional value. Excessive use of artificial sweeteners, particularly sodium cyclamate, can have harmful health effects. Several studies indicate that excessive consumption may damage nerve cells. Therefore, this study aims to determine the effect of excessive sodium cyclamate consumption on the decline of olfactory nerve function (CN I) in mice (Mus musculus). The dosage used was adjusted according to BPOM regulations for humans, which is 11 mg/kg body weight, then converted for mice using the Laurence conversion table. Twelve male Swiss Webster mice were divided into four groups: a control group and three treatment groups (P1: 0.55 mg/kg BW, P2: 1.1 mg/kg BW, P3: 1.65 mg/kg BW), each consisting of three mice. Sodium cyclamate was administered for 25 days to ensure systemic exposure. An ammonia scent test was then conducted to assess olfactory nerve function decline. The results showed that higher doses were associated with greater reduction in ammonia detection ability, indicating decreased olfactory nerve function (CN I).

Abstract

Anemia remains a common health problem among adolescent girls, partly due to low knowledge, motivation, and adherence to Iron Supplement Tablet consumption. This study aimed to determine the effect of health education delivered through Instagram on knowledge, motivation, and adherence to consumption among female students at SMP Negeri 18 Samarinda. A quantitative one-group pretest-posttest design was employed involving 40 participants selected through proportionate stratified random sampling. Health education was provided via Instagram for seven days. Data were collected using questionnaires and analyzed with the Wilcoxon test. The findings showed significant improvements in knowledge, motivation, and adherence after the intervention. The proportion of respondents with good knowledge increased from 27.5% to 92.5%, strong motivation from 25.5% to 92.5%, and adherence from 27.5% to 82.5%. Statistical analysis indicated significant differences between pretest and posttest scores for all variables (p < 0.05). These results suggest that Instagram-based health education is effective in improving knowledge, motivation, and adherence to consumption among adolescent girls.

Abstract

The implementation of an Environmental Management System (EMS) based on ISO 14001:2015 is an approach to improving environmental performance, regulatory compliance, and organizational sustainability. This study aims to evaluate the level of ISO 14001:2015 compliance, assess organizational readiness, and identify environmental risks at the Water Supply Management Unit (UPT PAM) of the Public Works and Spatial Planning Agency (Dinas PUPR) of Belitung Regency. A quantitative descriptive method was employed using ISO 14001:2015 gap analysis, organizational readiness questionnaires, and risk analysis based on a risk matrix. The results indicate a compliance level of 51.72% (moderate category), suggesting that the organization still requires improvements before the full implementation of the Environmental Management System. The organizational readiness assessment yielded an average score of 3.08, indicating a moderately ready condition. Leadership and commitment achieved the highest scores, while human resource competence and operational procedures require further improvement. Risk analysis identified 14 environmental risks, consisting of 1 very high-risk, 3 high-risk, and 10 moderate-risk categories. The primary risk was raw water contamination caused by illegal tin mining activities, with a risk score of 20. Improvement priorities include strengthening EMS documentation, enhancing human resource competence, conducting internal audits and management reviews, and controlling environmental risks to support the implementation of ISO 14001:2015.

Abstract

Hypertension is a major contributing factor to premature death globally, with an incidence rate of 30.8% in Indonesia. In Banda Aceh, the Baiturrahman Community Health Center recorded the highest number of cases, with 895 cases in 2024. This study aims to analyze various factors influencing patient adherence to antihypertensive medication. The research method used a quantitative approach with a cross-sectional design with 98 respondents. Data were collected through structured interviews from June 16 to 26, 2025. Data processing was carried out sequentially, starting with univariate analysis, followed by bivariate analysis using the chi-square test, and then multivariate analysis through multiple logistic regression with a stepwise approach using Stata/MP 17 software. This study has received ethical approval from the Health Research Ethics Committee of the Aceh Ministry of Health Polytechnic (No: DP.04.03/12.7/266/2025). The results showed that 45.92% of respondents were compliant with taking medication, while 54.08% were non-compliant. Multivariate analysis showed significant predictors of adherence, namely gender (p = 0.040; OR = 3.53), the role of health workers (p = 0.037; OR = 3.13), access to health services (p = 0.036; OR = 3.36), health literacy (p = 0.014; OR = 10.14), and family support (p = 0.002; OR = 7.97). The model has a Pseudo R² value of 0.36 indicating that the model is able to explain approximately 36% of the existing differences. The conclusion of the study shows that adherence to antihypertensive medication is significantly influenced by gender, the role of health workers, access to services, health literacy, and family support.

Abstract

As a chronic pathology, diabetes mellitus (DM) exhibits a surging prevalence that severely deteriorates the well-being and quality of life of affected individuals. This investigation sought to analyze the specific determinants influencing the quality of life among productive-age individuals (ranging from 15 to 64 years) diagnosed with type 2 DM within the administrative territory of the Meuraxa Public Health Center, Banda Aceh, in 2025. Professionally, this research integrated a descriptive analytic framework utilizing a cross-sectional design. From a total population of 391 type 2 DM patients, a cohort of 80 participants was selected via an accidental sampling strategy. Empirical data were gathered through structured questionnaire-guided interviews and subsequently processed using univariate and bivariate statistical evaluations within the SPSS software environment. The descriptive univariate assessments indicated that a vast majority of the subjects experienced a moderate quality of life (80.0%). Regarding independent variables, 45.0% demonstrated negative attitudes, 26.3% possessed deficient knowledge, 50.0% had a long-standing disease duration, 48.8% reported a history of medication-induced adverse effects, and 51.3% presented with a history of diabetic ulcerations or wounds. Furthermore, the inferential bivariate modeling demonstrated a statistically significant correlation between quality of life and three major indicators: patient attitude (p = 0.003), health literacy or knowledge (p = 0.015), and wound history (p = 0.048). Conversely, no significant associations were observed for illness duration (p = 0.090) or the experience of pharmaceutical side effects (p = 0.317). To conclude, personal attitude, cognitive knowledge, and clinical wound history stand as pivotal elements dictating the quality of life in working-age type 2 DM patients. Consequently, systematic health counseling and targeted complication mitigation strategies must be intensified to optimize patient well-being.

Abstract

The inadequate dietary fiber intake among the Indonesian population contributes to an increased risk of degenerative diseases, including diabetes mellitus. The utilization of Moringa oleifera L. as a functional food ingredient has attracted considerable attention due to its relatively high fiber content and its potential to enhance the nutritional quality of ready-to-eat food products. This study aimed to investigate changes in dietary fiber content and the textural characteristics of snack bars formulated with different levels of moringa leaf flour incorporation. An experimental approach employing a Completely Randomized Design (CRD) was applied, consisting of four treatment formulations with moringa leaf flour substitution levels of 0%, 5%, 10%, and 15%, each conducted in duplicate. Dietary fiber content was determined using the AOAC enzymatic-gravimetric method, while textural properties were evaluated using a Texture Analyzer. Statistical analysis was performed using One-Way ANOVA followed by Duncan’s Multiple Range Test (DMRT) to identify differences among treatments. The findings demonstrated that increasing the proportion of moringa leaf flour significantly affected both dietary fiber content and the texture of the snack bars (p < 0,05), with fiber levels increasing from 2,51% to 6,87%. The 15% formulation met the criteria for a high-fiber food product, whereas the 5% and 10% formulations were classified as sources of dietary fiber. These results highlight the potential of moringa leaf flour as a valuable ingredient for the development of highly functional snack bar products.

Abstract

Ischemic stroke is catastrophic diseases and recognized as placing a considerable financial strain on patients and health systems. The difference between actual hospital expenditures and INA-CBGs reimbursement rates potentially threaten the financial stability of healthcare facilities. Therefore, this study aimed to analyze the comparison between actual hospital costs and INA-CBGs tariffs for hospitalized ischemic stroke patients covered by the National Health Insurance, using a retrospective approach based on medical record and JKN claim data obtained from ischemic stroke patients at Soerojo Hospital in 2025. A total of 217 patients met the inclusion criteria, and the analysis was conducted based on ward class and severity level to calculate the difference between actual costs and INA-CBGs tariffs. The results indicated at severity level 1, all ward classes yielded a positive difference (surplus), namely IDR 3,342,519 (class 1), IDR 3,068,501 (class 2), and IDR 1,486,293 (class 3). In contrast, at severity level 2, all classes recorded a negative difference (deficit) of IDR 1,062,231, IDR 2,377,175, and IDR 2,191,417, respectively. The deficit widened considerably at severity level 3, with negative differences of IDR 22,515,300 (class 1), IDR 7,634,467 (class 2), and IDR 18,819,214 (class 3). Therefore, an evaluation and adjustment of INA-CBGs tariffs are warranted to ensure that the reimbursement rates better reflect the actual cost of hospital care.

Abstract

Cantrang fishing operations involve a high level of occupational safety and health (OSH) risk due to the use of heavy fishing gear, mechanical equipment, and dynamic marine working conditions. This study aimed to identify potential hazards, assess risk levels, and develop occupational safety and health risk control measures for cantrang vessel crews using the Hazard Identification and Risk Assessment (HIRA) method. The study was conducted at Tegalsari Coastal Fishing Port, Tegal City, Indonesia, through field observations, interviews, hazard identification, and risk assessment based on likelihood and severity parameters. The results showed that hauling posed the highest risk (Risk Level = 20), followed by setting and towing (Risk Level = 16), while net arrangement and loading–unloading activities (Risk Level = 12) were classified as high-risk activities. Engine inspection (Risk Level = 9), vessel preparation, and catch sorting (Risk Level = 6) were categorized as moderate-risk activities. Risk control should prioritize the implementation of standard operating procedures (SOPs), routine equipment inspections, the use of personal protective equipment (PPE), and regular safety training for vessel crews. These findings provide a basis for implementing occupational safety and health risk management in cantrang fishing operations.

Abstract

The School Health Program (Usaha Kesehatan Sekolah/UKS) plays a strategic role in promoting healthy behaviors among students, including adequate water consumption. However, its implementation has not fully succeeded in establishing consistent drinking habits. This study aimed to evaluate the implementation of the School Health Program in relation to the adequacy of water consumption among fifth-grade students at SD Muhammadiyah 2 Surakarta. A descriptive study with a mixed-methods approach was employed. Participants included students, teachers, and school stakeholders. Data were collected through observation, interviews, and measurement of water intake based on individual fluid requirements adjusted for body weight. Data were analyzed using descriptive quantitative and qualitative approaches. The findings revealed that 85% of students achieved adequate water intake, while 15% were categorized as inadequate. Despite this, the program implementation was not optimal, as indicated by the absence of structured habituation, lack of continuous monitoring, and limited teacher involvement in shaping drinking behaviors In conclusion, although most students met their fluid requirements, the School Health Program has not been effective in fostering consistent water consumption behavior. Strengthening behavioral-based interventions is essential to improve program effectiveness.

Abstract

Dengue Hemorphic Fever (DHF) remains a significant public health challenge in Banda Aceh City, characterized by fluctuating annual case trends. Population density is heavily suspected of influencing transmission risk due to intense human interaction and vector presence. This ecological study aims to analyze the relationship between population density and DHF incidence, while identifying spatial distribution patterns and case clusters in Banda Aceh City from 2021 to 2025. Employing a quantitative descriptive-analytic design based on Geographic Information System (GIS), aggregate secondary data were analyzed using QGIS 3.34.11, SaTScan, GeoDa, and Google Earth software. The results revealed a total accumulation of 1,330 DHF cases, with the highest prevalence observed in males and the child-to-adolescent age groups. Statistical testing demonstrated no significant relationship between population density and DHF incidence (p-value > 0.05). However, spatial analysis confirmed the existence of dynamic annual case clustering, with several sub-districts identified as high-risk areas. In conclusion, although population density is not directly correlated, the distribution of DHF cases in Banda Aceh exhibits a distinct spatial clustering pattern. Consequently, DHF control efforts must be specifically targeted at high-risk cluster areas through a location-based intervention approach.
