Abstract

Early childhood is one groups with the highest risk of nutritional problems because in this phase the first rapid growth process takes place. The impact of malnutrition includes impaired growth and development, decreased productivity and resistance to disease, impaired brain structure and function, and behavioral changes in the next life. Nutritional problems are directly caused by food intake and history of infection, while the availability and utilization of health services, family parenting, food availability and household sanitation will indirectly increase the risk. The research was conducted using a mixed method approach. In the early stages of the study, nutritional status measurements of all toddlers will be carried out followed by extracting information on intake history and other related information needed. Furthermore, information will be extracted from informants to provide answers related to the social situation of the community and individual experiences. Toddlers with normal nutritional status are nurtured with a Positive deviance pattern in the form of feeding, parenting, hygiene, and good health service utilization. Expanding the knowledge and ability of mothers in implementing of effective parenting for toddlers is important to improve the attitude and behavior of mothers to obtain good nutritional status and prevent early childhood from experiencing growth failure conditions such as stunting.

Abstract

Pulmonary tuberculosis is one of the top 10 diseases that need to be addressed and is one of the most common diseases that cause death. Tarutung Subdistrict with Hutabaginda Health Center is the subdistrict with the highest cases of pulmonary tuberculosis in North Tapanuli Regency. The number of cases of pulmonary tuberculosis patients in 2022 was obtained as many as 59 cases, of which 40 cases with complete and traceable addresses. This research is a descriptive analysis research using quantitative research method with Cross Sectional research design. This study analyzed the distribution mapping and risk factors for the incidence of pulmonary tuberculosis in the Working Area of Puskesmas Hutabaginda, Tarutung Subdistrict, North Tapanuli Regency with a total sampling technique with a sample size of 40 people. Data were collected directly by the researcher through observation and interview with a research questionnaire. Data analysis showed that the majority of research respondents were male (70%), productive age (77.5%), with low education level (80%), still working (52.5%), not consuming alcohol (52.5%), not smoking (55%), no history of diabetes (62.5%), unqualified residential density (80%), unqualified house temperature (70%), unqualified house ventilation (77.5%), unqualified house humidity (97.5%), qualified wall type (60%), qualified floor type (57.5%), and qualified house lighting (62.5%). It is recommended that patients with pulmonary tuberculosis be able to maintain the health of the housing environment, and the Puskesmas is expected to be able to increase the promotion of clean and healthy living behaviors (PHBS) through socialization and counseling.

Abstract

The waist-hip ratio (WHR) is the comparison between an individual's waist circumference and hip circumference. This ratio is frequently used in health research to assess body fat distribution and can indicate health risks associated with obesity. This study aims to evaluate the relationship between the consumption of sugar-sweetened beverages and physical activity with the waist-hip ratio among students of MAS Plus Al-Ulum Medan. This research employs a quantitative method with a cross-sectional approach. The results indicate that most students consume excessive amounts of sugar (≥50 grams/day) and have low levels of physical activity. Bivariate analysis using Spearman's rank correlation showed a significant relationship between the consumption of sugar-sweetened beverages and WHR, with a strong and positive correlation. Additionally, there is a significant relationship between the level of physical activity and WHR, with a strong and inverse correlation. Students of MAS Plus Al-Ulum Medan are encouraged to reduce their consumption of sugar-sweetened beverages and increase their physical activity. Keywords: SSB’s, physical activity, WHR, adolescents

Abstract

Nutritional problems in children in Indonesia stem from what they eat, including the consumption of school snacks. Education on the nutritional content of school snacks must be provided to overcome the risk of snacks in school-age children to obtain balanced nutrition. The purpose of this study was to provide education on nutritional content and sanitation at SD Islam Ulun Nuha Medan. The design of this study used a pre-experiment with a one group pre-post-test design with a sample of 129 students from grade 5 to grade 6. Education was provided through lectures containing the concept of nutritional content and sanitation in children's snacks. Pre-test and post-test were conducted before and after the education using 10 question in questionnaire. The results of this activity show the pre-test and post-test that have been distributed before and after education, which shows that there is an increase in respondents' knowledge before and after being given education, which is 6.66 with a p-value of 0.0001. It’s shows that there is an influence of nutrition education on students' knowledge about nutritional and sanitation in snacks. The increase in knowledge can be caused by using lecture and demonstration methods with slide presentation media and examples of healthy and unhealthy snack foods. The use of the right methods and media greatly affects the results of education where the more senses are exposed, the easier it is to understand a message.

Abstract

Scabies is an infectious disease caused by the parasite Sarcoptes scabiei var. hominis which is transmitted through interactions between humans with clinical symptoms of lesions characterized by itching, especially at night. In 2023 World Health Organization (WHO) estimates there are more than 200 million people worldwide at any one time. In Southeast Asia, scabies has a DALYs burden of 135.47 cases. The incidence of scabies in 2021 in North Sumatra Province was 811 people. At the class I State Detention Center in Medan, scabies is the highest infectious disease that is mostly suffered by 250 inmates. This study aims to determine the factors related to the incidence of scabies among inmates at the Medan Class I State Detention Center. This research method uses analytical methods with a cross-sectional design. The number of samples used in the research was 95 male inmates using a purposive sampling technique. Collecting research data through interviews using questionnaires and observation. The results of bivariate analysis showed that there was a significant relationship between knowledge (p<0.035), frequency of bathing (p< 0.025), cleanliness of hands and nails (p<0.001), clean clothing (p<0.001), bedding (p<0.001), and density (p<0.001) with the incidence of scabies in inmates in Medan class I state detention center. Inmates should maintain individual cleanliness, namely not borrowing and borrowing personal equipment, not washing clothes together with cellmates, drying bedding and other sleeping equipment at least once a week and continuing to keep the cell environment clean.

Abstract

Unclean hands can transfer Escherichia coli from the body, feces, or other sources to food which when consumed can cause diarrhea. One solution to maintain hand hygiene is the use of natural antiseptic. This study aims to determine the effectiveness of cinnamon and cloves as antiseptics against the growth of Escherichia coli. This study is a quantitative study with a true experimental method with a post-test only control design. Testing the inhibition of Escherichia coli in antiseptic extracts using the Kirby-Bauer test with 3 repetitions. The extracts used were obtained from the maceration process and made into several concentrations, namely 20%, 40%, 60%, 80%, and 100%. The positive controls used were Chloramphenicol Palmitate antibiotics and 70% alcohol antiseptic. The negative control in this study was aquadest. The measurement results after 24 hours showed that concentrations of 20%, 40%, 60%, 80%, and 100% respectively effectively inhibited the growth of Escherichia coli because there were clear zones of 10,067 mm, 10,867 mm, 11,267 mm, 12,833 mm, and 13,4 mm. Data analysis using the Kruskal-wallis test showed p- value was 0,026. This means there is a significant difference in the average diameter of the inhibition zone. Through the Post Hoc Multiple Comparison test, it was found that 20% until 80% concentrations had no significant difference with 70% alcohol antiseptic, p-value > 0,05, but 100% concentration had a significant difference with 70% alcohol antiseptic because it had a p-value= 0,036. Starting from 20% concentration, can replace 70% alcohol antiseptic.

Abstract

Drinking water is vital for daily consumption and having physical qualities that meet health standards is critical. A study (Mason et al.) in 2018 at the University of New York showed that 93% of bottled drinking water contains microplastics. The study used 259 drinking water bottle samples, including those sold in Indonesia from 11 different brands and the size of microplastics found were between 6.5-100 micrometers. Furthermore, microplastics are also found in refillable gallon drinking water that could create microplastics fragments. This study was done to analyze the physical qualities and presence of microplastics in bottled and refillable gallon drinking water in Medan Selayang in 2023. This was a descriptive study with observational and interview methods. There were 12 water samples obtained from simple random sampling. Our study showed that the drinking water physical qualities has met the health standards (100%) as per the regulation of Ministry of Health (Permenkes No.2, 2023). However, the equipment condition in all drinking water depots did not meet the health standards (100%). The processing and cleaning in the drinking water depots have all met the health standards (100%). All samples from drinking water depots contained microplastics in the form of fibers and fragments per liter. It is recommended for water depots in Medan Selayang to pay more attention to the quality of drinking water through physical, chemical, and biological parameters, and to do periodical examination of the water through the public health office.

Abstract

Atopic dermatitis is often considered a common skin problem, that has a high prevalence of cases. Panusupan Village, one of the working areas of the Cilongok II Public Health Center, has the highest number of cases of Atopic Dermatitis, which is 89 cases in 2024. The purpose of this research was to know the factors that affect the incidence of atopic dermatitis in Panusupan Village, Cilongok District. Quantitative research used a control case design. The research sample were 123 people (41 cases and 82 controls). Case data was obtained from the Cilongok II Public Health Center from January to September 2024. The variables studied included family history of allergies, temperature, smoking behavior, food allergies, and stress. The Data were analysed by univariate, bivariate, and multivariate analysis. The results of the research showed that related factors (p-value < 0.05), namely family allergy history, temperature, and food allergies, unrelated variables (p-value > 0.05), namely smoking behavior and stress. Factors that affect the incidence of atopic dermatitis are family allergy history and temperature. Recommendations that can be made are to conduct counseling related to atopic dermatitis and routinely apply moisturizer for people with atopic dermatitis.

Abstract

Waste remains a national issue. The waste generated in Medan City amounted to 628.7 thousand tons per year in 2022, sourced primarily from household waste. The phenomenon of online shopping among students contributes to the generation of waste in the form of plastic and cardboard. This study aims to analyze the relationship between online shopping patterns and waste quantity among students of the Faculty of Public Health. This research is an analytical survey with a cross-sectional study design. The study was conducted at the University of North Sumatra. The population comprised 361 students from the Faculty of Public Health, Class of 2022, with a sample size of 78 students determined using the Slovin formula. Data was collected through observation, direct interviews using questionnaires, and measurement of waste quantity using a digital scale. The data analysis method was conducted in two stages: univariate analysis using frequency and bivariate analysis using the chi-square test with a significance level of α = 5% (0.05). The results showed a significant relationship between online shopping patterns based on online shopping expenses (p = 0.001) and shopping frequency (p = 0.001). However, there was no significant relationship between online shopping patterns based on types of shopping (p = 0.091), students' knowledge (p = 0.450), and students' attitudes (p = 1.000) toward waste quantity. The study suggests that parents and students should pay more attention to online shopping patterns and waste management as an effort to reduce waste.

Abstract

Minimum Service Standards (MSS) for health emphasize the importance of the type and quality of basic services to achieve optimal and equitable health services, especially in the health sector of non-communicable diseases, which continue to increase in prevalence. Non-communicable diseases (NCDs), especially hypertension, are health problems which cases continue to increase. The prevalence of hypertension in 2018 was 34.11%, decreasing to 30.8% in 2023, but this figure has not met the national target. East Java ranks fourth with the highest prevalence of hypertension at 34.30%. The third lowest hypertension service achievement is Malang Regency. Puskesmas X had a low hypertension treatment rate of 12.71% in 2023. The purpose of this study was to analyze the implementation of MSS in hypertension health services at Puskesmas X and identify inhibiting factors based on input, process, and output variables. This study used a qualitative method with in-depth interviews involving 11 informants with an interview guide instrument and researchers conducted observations with an observation sheet instrument to record field data. This study used source triangulation and method triangulation. The results showed that hypertension MSS implementation at Puskesmas X faced several challenges, particularly limited human resources, suboptimal training, lack of educational media, weak cross-sector coordination, and ineffective data recording and reporting. Although MSS achievement increased from 3.7% (2021) to 31.3% (2024), it is still far from the national target of 100%. Overall, improvements in MSS implementation in hypertension health services at Puskesmas X have not fully met the set targets.
