Abstract

ABSTRACTWilms tumor (nephroblastoma) is rare in adults. We present a case of 28-year old female with Wilms tumor diagnosed with pre-operative abdominal magnetic resonance imaging, histopathological analysis and immunohistochemistry. She had relapse tumor two years after primary open radical nephrectomy. She was managed with chemotherapy with the ICE regimen (ifosfamide, carboplatin, and etoposide) for six cycles with partial response and excellent functional status. Keywords: ABSTRAKTumor Wilms (nefroblastoma) adalah kasus yang jarang ditemukan pada orang dewasa. Kami melaporkan satu kasus, pasien perempuan, 28 tahun dengan tumor Wilms yang didiagnosis dengan pemeriksaan magnetic resonance imaging pre-operasi, histopatologi, dan imunohistokimia. Pasien tersebut mengalami relaps dua tahun pasca-radikal nefrektomi. Pasien kemudian menjalani kemoterapi dengan regimen ICE (ifosfamid, karboplatin, dan etoposid) selama enam siklus dengan repons parsial dan status fungsional baik.

Abstract

Pada setiap individu, terdapat keterkaitan antara sistem biologis, sistem psikologis dan sistem sosial. Penyakit yang dialami individu akan memberikan pengaruh besar dalam emosi, penampilan dan perilaku sosial individu. Dr. Elisabeth Kiibler-Ross mengidentifikasi lima tahap yang mungkin dilewati oleh pasien penyakit terminal, yang divonis tidak akan hidup lama lagi. Melalui tahapan tersebut maka dapat terlihat suatu gambaran mengenai proses perubahan psikologis pada pasien terminal dalam menghadapi kematian atau rasa kehilangannya sehingga pasien dapat mendapatkan bantuan untuk melewati tahapan-tahapan tersebut. Dengan adanya perawatan paliatif memungkinkan pasien dan keluarga pasien kanker stadium lanjut tidak hanya mendapatkan perawatan fisik namun juga perawatan secara psikologis dan sosial dalam menghadapi penyakit dan masalah psikologis dan sosial yang dihadapi pasien dan keluarga pasien.Disamping pengobatan primer, melalui obat-obatan, Moos mengemukakan bahwa orang yang sakit perlu melakukan dua tipe tugas penyesuaian dalam proses coping, yaitu: tugas yang berkaitan dengan penyakit atau pengobatan, dan tugas yang berkaitan dengan fungsi psikososial umum. Kondisi psikologis dan sosial yang stabil, secara langsung ataupun tidak langsung, akan dapat meningkatkan kualitas hidup pasien menjadi lebih baik.Kata kunci: perawatan paliatif, perawatan menyeluruh, gangguan sosial dan psikologis.

Abstract

ABSTRACTIntroduction. Sacral tumors are rare, and experience of these tumors is usually limited to a small number of patients. Inthis study, we evaluated profile, survival rates, and functional outcome in a series of sacral tumor treated in our institution.Method. We retrospectively reviewed the records of 22 sacral tumor patients from January 1995 to February 2014 inCipto Mangunkusumo National Central Hospital, Jakarta, Indonesia. Kaplan-Meier method was used to describedsurvival and functional outcome. Their correlation with clinical profile, histological type, level of sacral involvement,treatment, and complication were analyzed by Log rank test.Results. From 22 patients, 5 of them were excluded from this study. Thus, there were 17 cases of sacral tumor, 16 ofthem were malignant and one case was benign. In Kaplan-Meier Analysis, there were no significant difference insurvival found between sex, age group, biopsy type, level of sacral involvement, treatment, and complication. There wassignificant difference in survival found between histopathology result (p=0.012), and giant cell tumor GCT showed thehighest survival, followed by chordoma, metastatic lession, and Ewing Sarcoma. Sacral tumor at the level of S2 andbelow had better functional outcome compared to the one above S2 (p=0.001). There were no difference in functionaloutcome found between histopathology type and treatment (p=0.137 and p=0.210).Conclusion. The majority of primary tumors of sacrum are chordoma which present with nonspecific early signs andsymptoms. Survival rate and functional outcome of the sacral tumor patients were determined by histopathology resultand level of sacral involvement. Lower level of sacral involvement lead to better survival and functional outcome.Keyword: sacral tumors, chordoma, giant-cell tumor, metastasis, survival, functional outcomeABSTRAKPendahuluan. Tumor sakrum sangat jarang ditemukan; dan pengalaman mengenai tumor sakrum biasanya terbataspada sejumlah kecil pasien. Dalam studi ini, kami mengevaluasi profil, tingkat kesintasan, dan luaran fungsional dariserangkaian tumor sakrum yang dirawat di institusi kami.Metode. Kami telah mengevaluasi 22 pasien tumor sakrum dari Januari 1995 sampai Februari 2014 secara retrospektif diRumah Sakit Umum Pusat Nasional Cipto Mangunkusumo Jakarta Indonesia. Metode Kaplan-Meier digunakan untukmenganalisis angka kesintasan dan luaran fungsional. Hubungan antara kesintasan dan luaran fungsional dengan profilklinis, jenis histopatologi, level sakrum yang terlibat, penatalaksanaan, dan komplikasi dianalisis dengan Log rank test.Hasil. Dari 22 pasien, hanya 17 pasien tumor sakrum yang dimasukkan ke dalam studi ini (5 kasus diekslusi), 16 kasusdengan tumor ganas dan 1 kasus dengan tumor jinak. Hasil analisis Kaplan-Meier menunjukkan tidak ada perbedaanyang bermakna pada angka kesintasan terhadap jenis kelamin, kelompok umur, jenis biopsi, level sakrum yang terlibat,penatalaksanaan, dan komplikasi. Hasil uji statististik menunjukkan perbedaan yang bermakna antara kesintasandengan hasil histopatologi (p = 0,012), giant cell tumor menunjukkan kesintasan hidup tertinggi, diikuti oleh Chordoma,lesi metastasis, dan Ewing Sarcoma. Tumor sakrum setinggi S2 ke bawah memiliki luaran fungsional yang lebih baikdibandingkan dengan level di atas S2 (p = 0,001). Hasil uji statistik menunjukkan tidak ada perbedaan yang bermakna antara luaran fungsional dengan jenis histopatologi dan penatalaksanaan(p = 0,137 dan p = 0,210). Kesimpulan. Mayoritas tumor primer pada tulang sakrum adalah chordoma yang muncul dengan gejala dan tanda-tanda awal yang tidak spesifik. Angka kesintasan dan luaran funsional pasien tumor sakrum dipengaruhi oleh jenis histopatologi dan level sacrum yang terlibat. Semakin rendah level sacrum yang terlibat, semakin baik kesintasan dan luaran fungsional.Kata Kunci: Tumor sakrum, Chordoma, giant cell tumor, metastasis,kesintasan, luaran fungsional

Abstract

Background: Thyroid cancer is the most common endocrine cancer, and its incidence has continuously increased in the last three decades all over the world. The national prevalence of thyroid cancer has not been recorded. Sex, obesity, and the size of thyroid nodules are the risk factors for thyroid cancer. The aim of this study is to acknowledge the characteristics of thyroid cancer patients in Haji Adam Malik General Hospital. Methods: This is a cross-sectional study and carried out among 97 patients with thyroid cancer at Haji Adam Malik General Hospital from 2013 until 2015. Secondary data were obtained by consecutive sampling techniques using medical records. The inclusion criteria are all patients diagnosed with thyroid cancer based on histopathological analysis. Data were analyzed descriptively using SPSS in percentage.Results: From 97 patients with thyroid cancer, it could be concluded that thyroid cancer is the most frequently diagnosed in the age group of 55–64 years (32.0%). Females were predominant that attributed to 71.1% than males (28.9%). The majority of thyroid cancer was diagnosed at stage IV (39.2%) with the most common histopathological feature of papillary thyroid carcinoma (46,4%). Thyroid cancer patients are usually found with normal BMI (37.1%) and nodule thyroid found with the diameter of ≥4cm (47.4%).Conclusions: In the 55–64-year age female group, thyroid cancer is most diagnosed at stage IV with papillary thyroid carcinoma. Thyroid cancer patients are usually found with normal BMI and the nodule size of ≥4 cm.

Abstract

Background: Fluoroquinolone has been routinely given as a prophylactic antibiotic to patients undergoing transrectal ultrasound (TRUS)-guided prostate biopsy. Currently, there is no data on the profile of fluoroquinolone-resistant bacteria, its association with post-biopsy sepsis, and alternative prophylactic antibiotics.Methods: A cross-sectional study was conducted in patients undergoing TRUS-guided prostate biopsy at Urology Clinic, Cipto Mangunkusumo National Central General Hospital between August and December 2015. Specimens were taken from rectal swabs prior to biopsy. Gramnegative bacilli and Gram-positive cocci were characterized on the Vitek®2 using GN and GP cards (BioMérieux, USA). Antimicrobial susceptibility testing was done by Kirby Bauer disc diffusion method. Post-biopsy infection was monitored within one week after biopsy by telephone call and diagnosed by clinical examination.Results: A total of 52 patients aged 52-80 years were enrolled. Sixty-six isolates grew from 52 swabs; 14 swabs among them grew 2 isolates. The commonest pathogen was Escherichia coli (78.8%), followed by Klebsiella pneumoniae (13.6%), Enterococcus faecium (3.0%), Acinetobacter haemolyticus, Morganella morganii subsp. morganii, and Enterococcus faecalis (1.5%), respectively. Ciprofloxacin-resistant bacteria were found in 33 (50.0%) isolates which were predominated by E. coli. Meanwhile, 90.4% and 96.2% of isolates were sensitive to amoxicillinclavulanate and ampicillin-sulbactam, respectively. Post-biopsy infection was established in 7 patients requiring no hospitalization.Conclusions: The proportion of ciprofloxacin-resistant bacteria in the rectum among patients undergoing prostate biopsy was 50.0%. Escherichia coli is the commonest resistant pathogen to fluoroquinolone. The recommended alternatives for prophylactic antibiotics are amoxicillinclavulanate and ampicillin-sulbactam. Routine pre-biopsy rectal swab cultures should also be encouraged.

Abstract

Indonesia mempunyai masalah yang sama dengan negara berkembang lainnya yaitu pasien kanker payudara yang datang ke rumah sakit dalam stadium lanjut. Oleh karena itu diperlukan upaya peningkatan kepedulian khususnya di lingkup tenaga medis terhadap kanker payudara melalui penyuluhan dan ketrampilan pemeriksaan payudara. Tujuan penelitian ini adalah mengumpulkan data awal untuk menentukan metode yang tepat untuk pelatihan pemeriksaan payudara bagi tenaga medis khususnya perawat, bidan dan dokter umum.Dilakukan penyuluhan bagi staf medis yang berminat dan pelatihan pemeriksaan payudara pada tenaga medis (perawat dan bidan) di RS.Kanker Dharmais periode April - Desember 2005. Untuk meningkatkan kepedulian dan kemampuan ketrampilan pemeriksaan payudara para peserta kemudian diminta untuk memberikan penyuluhan di lingkungannya dan mengajak wanita yang bersedia dilakukan pemeriksaan payudara dan USG payudara di RS. Kanker Dharmais.Sebanyak 25 orang perawat dan bidan serta 1 orang dokter umum yang berpartisipasi aktif, 856 wanita bersedia dilakukan pemeriksaan payudara dan USG payudara. Pemeriksaan payudara oleh tenaga medis yang telah dikonfirmasi dengan USG payudara ditemukan hasil normal 783 ( 91.5 %) dan 73 (8.5%) abnormal. Adapun dari hasil abnormal tersebut adalah 39 (53.4%) kista, 17 (23.3%) lesi solid benigna dan 8 (10.9%) dicurigai maligna. Sedangkan kelainan payudara lain yang ditemukan adalah mastitis, galactocele, duktal ektasis serta mamaria aberans.Pelatihan pemeriksaan payudara membutuhkan tidak hanya pengetahuan dan ketrampilan pemeriksaan payudara tetapi juga ketrampilan berkomunikasi dan menjalin kerjasama dengan para wanita di lingkungannya sehingga partisipasi masyarakat dapat ditingkatkan.Kata kunci: kanker payudara, deteksi dini, pemeriksaan payudara, USG payudara.

Abstract

Penentuan imunofenotipe merupakan salah satu pemeriksaan yang disyaratkan dalam diagnosis dan klasifikasi leukemia. Dalam tulisan ini, kami melaporkan data peranan penentuan imunofenotipe dalam diagnosis leukemia akut dikaitkan dengan pemeriksaan sitomorfologi dan pemeriksaan sitokimia Sudan Black B (SBB) yang dilakukan di Laboratorium Patologi Klinik Rumah Sakit Kanker Dharmais (RSKD) sejak Januari 2005 sampai Desember 2007.Menurut imunofenotipe, kami menemukan 13 kasus leukemia limfoblastik akut (LLA) galur T, 82 kasus LLA galur B, dan 110 kasus leukemia mieloblastik akut (LMA). Ko-ekspresi antigen dari galur lain ditemukan pada 38% kasus LLA-T, 28% kasus LLA-B, dan 37% LMA. Antigen limfoid yang paling sering ditemukan ekspresinya pada LMA adalah CD19 diikuti CD7.CD33 ditemukan pada 94,6% kasus LMA dan CD13 pada 87,8% kasus LMA. CD19 ditemukan pada 96% LLA-B, CD10 pada 68% kasus, diikuti CD20 (50%) dan CD22 (33%). CD7 ditemukan pada 92% LLA-T, dan CD3 positif pada 85% kasus LLA-T.Disimpulkan bahwa penentuan imunofenotipe sangat bermanfaat dalam penentuan diagnosis galur, membedakan antara LLA dan LMA, khususnya pada kasus dengan SBB negatif seperti pada kasus LMA-M0 dan M5a, membedakan LLA-B dan LLA-T, serta mendiagnosis kasus leukemia bifenotipe/leukemia galur campuran.Kata kunci: Leukemia akut, imunofenotipe, galur myeloid, LLA-B, LLA-T.

Abstract

Background: Immunotherapy using immune checkpoint inhibitors has currently emerged as an effective treatment for a subset of colorectal cancer (CRC) patients. The roles of PD-1 and PD-L1 expression levels in peripheral blood to predict patient’s response to immune checkpoint inhibitors are not well established. Therefore, we analyzed PD-1 and PD-L1 mRNA expression levels of peripheral blood in Indonesian CRC patients and explored the association with the clinicopathological features.Methods: Peripheral blood of 25 CRC patients and 10 healthy individuals were collected in Dharmais Hospital-National Cancer Center from 2017 to 2018. PD-1 and PD-L1 mRNA expression levels were analyzed using real time PCR. The associations with clinicopathological variables were analyzed with fisher-exact test or chi square test.Results: PD-1 mRNA expression levels were significantly lower in CRC patients compared to healthy individuals (HI) (mean: 0.0015 ± 0.0013 and 0.017 ± 0.010 respectively, p < 0.001). Although PD-L1 mRNA expression levels were lower in CRC patients, the difference was not statistically significant (mean in CRC and HI: 0.021 ± 0.013 and 0.034 ± 0.028 respectively, p = 0.125). The expression of PD-L1 was higher in CRC females compared to males (p = 0.03). The expression levels of PD-L1 were not associated with different ages (p = 0.673), stages (p = 0.298), histological type of colorectal cancer (p=0.852), patient status (p = 1.000), and body mass index (p = 0.514).Conclusions: The mRNA expression levels of PD-1 and PD-L1 were lower in CRC patients compared to healthy controls. Expression of PD-L1 were correlated with sex, but not correlated with ages, stages, histological type of CRC, patient status, and body mass index.

Abstract

Kanker kolorektal merupakan salah satu jenis kanker yang banyak diderita oleh masyarakat dan dapat disembuhkan jika terdeteksi dini. Salah satu penyebab kanker adalah terjadinya mutasi pada gen-gen tertentu. Mutasi pada gen K-RAS sering ditemui pada tahap awal kanker kolorektal. Cetuximab dan Panitumumab merupakan contoh antibodi monoklonal yang telah digunakan dalam penanganan kanker kolorektal. Dari penelitian-penelitian yang telah dilakukan, penanganan pasien kanker kolorektal menggunakan Cetuximab dan Panitumumab menunjukkan respon yang positif terhadap pasien yang tidak memiliki mutasi pada gen K-RAS. Pemberian Cetuximab atau Panitumumab pada pasien yang memiliki mutasi pada gen K-RAS memiliki kemungkinan kecil untuk mendapatkan manfaat klinis sehingga hal ini dapat menjadi pertimbangan dalam pemberian obat kepada pasien.Kata kunci: KRAS, kanker kolorektal, antibodi anti EGFR

Abstract

Karsinoma tiroid merupakan keganasan sistem endokrin yang paling sering ditemukan dan dalam tiga dekade terakhir insidensinya meningkat tajam. Hampir 90% kasus tergolong kedalam Karsinoma Tiroid diferensiasi baik yang tumbuh lambat dengan prognosis yang baik. Diagnosis ditegakkan berdasarkan pemeriksaan sitologi dari biopsi jarum halus atau melalui pemeriksaan histopatologi dari lobektomi tiroid.Sampai saat ini tindakan bedah dalam penatalaksanaan Karsinoma Tiroid diferensiasi baik masih kontroversial terutama untuk kasus dengan risiko rendah. Satu pihak menganjurkan untuk tiroidektomi total, pihak yang lain cukup lobektomi tiroid saja. Kontroversi juga terjadi dalam hal perlu tidaknya diseksi limfatik level sentral bersama-sama tindakan tiroidektomi total pada KTP.American Thyroid Association merekomendasikan tindakan tiroidektomi total dan diseksi limfatik level sentral untuk Karsinoma Tiroid Papilifer khususnya untuk tumor primer yang invasif dan untuk Karsinoma Tiroid Folikuler cukup tiroidektomi total saja.Kata kunci: Karsinoma Tiroid diferensiasi baik, tindakan bedah.
