Abstract

Cost Recovery Rateat the Royal Prima Medan Hospital, there was a surplus every year below 5% and even decreased in 2018 to less than 100%. The CRR surplus is still below 5% and the CRR decline is related to costs, tariffs and utilization. This study aims to analyze tariffs, costs and utilization in increasing the Cost Recovery Rate (CRR) at RSU Royal Prima Medan. This type of research is quantitative and qualitative research is descriptive analytic. The population and sample were the Director, Director of Medical Services, Head of Finance Subdivision, Head of Accounting and Marketing Subdivision at RSU Royal Prima Medan, totaling 5 informants. Data collection with primary and secondary data and data analyzed by data reduction, data display and data verification. The results of the study show that there is a change in the Cost Recovery Rate (CRR) every year, how to determine service product rates at RSU Royal Prima Medan and compare them to rates at competing hospitals in Medan, the number of sick people totaling 4222 people was only about 53 people who came out dead for various reasons, not because of the disease they suffered when they entered and the strategy carried out by RSU Royal Prima Medan in increasing CRR is to carry out promotions for several superior service products. It is recommended that because this research is limited to using only quantitative descriptive methods with a qualitative approach, the authors suggest that the leadership can re-examine the success of this research by opening other researchers using different methodologies and employees need to increase motivation and quality of service from year to year so as to make RSU Royal Prima Medan is indeed a hope for the wider community.

Abstract

Hypertension is still a health problem in the elderly group. As a result of rapid development and a prosperous standard of living and health services can increase life expectancy, so that the number of elderly people increases every year. This increase in age is often followed by an increase in degenerative diseases and other health problems in this group. Psychological interventions are believed to be able to change a person's behavior towards a better direction in disease control and prevention. This study measures the impact of cognitive behavioral therapy (CBT) interventions on self-care management of hypertensive patients.This type of research is a quasi-experimental design with a pretest-posttest and control group design. The research subjects were divided into two groups, namely the intervention group (35 people) and the intervention group (35 people). control group (35 people). The research was conducted at the Royal Prima Marelan Hospital in 2022. Data were analyzed using the t-dependent test (α = 0.05).The results showed that in the intervention group there were differences in self-care management of hypertensive patients before and after being given CBT which included diet (p <0.001; 95% CI -6.663-(-4.796)), physical activity (p<0.001; 95 %CI -5.344-(-4.370)), and attitude (p=<0.001; 95%CI -13.086-(-10.115)). In the control group, there was no difference in self-care management of hypertensive patients, including diet n (p= 0.083; 95% CI -0.183-0.012), physical activity (p= 0.058; 95% CI- 0.291-0.005), and attitude T2DM patients (p = 0.094; 95% CI -0.225-0.003). This means that CBT is effective in improving self-care management of Hypertension patients.It is suggested to the Management of Royal Prima Marelan Hospital to implement routine physical activities such as hypertension gymnastics in order to prevent hypertension among young people.

Abstract

A good work program is needed by using the WISN (Workload Indicators of Staff Needing) method, which helps see the workload given to and received by medical workers in health facilities. This study aims to determine the appropriate number of employees using the WISN (Workload Indicators of Staff Needing) method to reduce the workload in the hospital's medical rehabilitation polyclinic. This research is a descriptive study with quantitative data analysis conducted at the H. Abdul Manan Simatupang Hospital in 2021. The data taken is medical record data on the total number of patient visits at the medical rehabilitation polyclinic and staffing data for the rehabilitation polyclinic. H. Abdul Manan Simatupang Kisaran Hospital. We found a relatively high number of visits at the Medical Rehabilitation Polyclinic; the workforce data obtained totaled four employees who handled patients directly. The study's results found that the available work time was not directly proportional to the existing standard workload, where the available work time is 135,360 minutes per year while the Standard Workload is 347,424 minutes per year. The workload in the Medical Rehabilitation Polyclinic is too high, and we can obtain the cause from the number of operational staff on duty.

Abstract

Tuberculosis as a disease caused by Mycobacterium tuberculosis. Tuberculosis can attack the lungs and can attack all parts of the body. Just as Pneumonia is a respiratory infection that most often causes death in infants and toddlers. The purpose of this study was to evaluate the cost of Pulmonary TB and Pneumonia therapy for the period July 2020 to July 2021 Based on INA-CBGs at RSU Royal Prima Medan. This type of research is descriptive with a qualitative approach. The sample in this study were informants consisting of officers in the INA-CBGs tariff coding section, pulmonary specialists, administrative management staff/staff and patients who were treating pulmonary tuberculosis and pneumonia. The determination of informants was carried out by purposive sampling. Analysis of research data using source triangulation, method triangulation and time triangulation. The results of the study show that the direct costs of patients for pulmonary TB therapy are based on the INA-CBGs rates, these costs are distributed into several costs such as registration fees, consultation fees, laboratory fees, drug costs and X-ray fees. Indirect costs incurred for pulmonary TB therapy patients and indirect costs incurred for pneumonia therapy patients who undergo therapy at RSU Royal Prima Medan. This fee is the sum of transportation costs, food costs, losses due to leave, patient care costs, and drug costs that are not available at the hospital. The duration of therapy for pulmonary TB patients at RSU Royal Prima Medan was in the majority > 5 days in 23 (65.7%) patients, and 1-5 days in 12 (34.3%) patients. The majority of Pneumonia patients at RSU Royal Prima Medan spent 1-5 days in 24 (68.6%) patients, and > 5 days in 11 (31.4%) patients.

Abstract

Tinuknuk is a special sauce from Simalungun Regency, North Sumatra Province, tinuktuk warms the body and is believed to be able to cure various internal diseases, especially to help post-natal recovery. This study aims to determine the optimal nutritional value of the main components, namely red ginger with black pepper in the ratio composition: 70:30%, 60:40%, 50:50%, 40:60%, and 30:70% using the method oven at a temperature of 105℃. Then the ash content, protein content, and salt content were tested. The results showed that the ash content was 6.98%, respectively; 6.91%; 6.90%; 6.84%, and 6.73%. The salt content is 0.57%, respectively; 0.57%; 0.57%; 0.57%, and 0.57%. The protein content was 10.43%, respectively; 10.93%; 12.43%; 11.81%, and 10.62%.

Abstract

Community diagnosis is an activity to determine the existence of a problem by collecting data on the community. Community diagnosis is an activity to explore the main issues faced by the community based on existing facts and take strategies and follow-up plans to resolve these problems. The study aimed to determine the description of health problems through community diagnosis in Dukuh Demangan, Sewon District, Bantul Regency. The method used in this research is quantitative andqualitative. The study design used in implementing the Field Learning Experience is adescriptive study. After determining the priority of the problem using the Hanlon method. The result shows that hypertension is the priority of health problems in Dukuh Demangan.

Abstract

Anemia in pregnancy is a condition of the mother with a hemoglobin level of less than 11.0 gram/dl. Anemia can be prevented by consuming a balanced nutritious diet with sufficient iron intake to meet the body's needs. Heme iron is found in green vegetables, red meat, egg yolks, raisins, prunes, liver, oysters and some fortified cereals. Knowing the effectiveness of giving beetroot juice on Hb levels in third trimester pregnant women at Budh Asih Hospital. The research design is Quasi Experimental, with the Pretest and Posttest Control Group design. The population in this study were third trimester pregnant women with a gestational age of 28-36 weeks in the working area of Budhi Asih Hospital. The sampling technique used purposive sampling with the number of respondents being 30 third trimester pregnant women. The statistical tests used are the Mann-Whitney and Wilcoxon tests. The mean level of anemia before being given beetroot juice therapy in the intervention group was 6.33 and 6.93 in the control group. The mean level of anemia after being given beetroot juice therapy in the intervention group was 11.20 and in the control group was 4.80. beetroot juice therapy is effective for reducing the level of anemia in third trimester pregnant women with p value = 0.000 ≤ 0.05. Giving beetroot juice is effective for increasing hemoglobin levels in third trimester pregnant women at Budhi Asih Hospital. It is hoped that midwives can provide regular education to mothers who experience anemia about the benefits of beetroot juice to increase hemoglobin levels in third trimester pregnant women. Abstract

Labor pain is the main problem experienced by maternity mothers (Rahayu, 2018). In Brazil, labor pain reaches more than 50% of the birth rate in a hospital which is the highest percentage worldwide (Alam, 2020). A preliminary study conducted by the authors, in May 2022, of 10 maternity mothers, 7 (70%) mothers felt severe pain, 2 (20%) mothers felt moderate pain, and 1 (10%) mothers felt mild pain. Therefore, the author is interested in conducting research on pain management with non-pharmacological methods in the form of relaxation and massage, on this occasion the author will conduct research on the difference between abdominal breathing techniques and deep back massage on reducing labor pain during the active phase I in physiological childbirth at Cengkareng Hospital in 2022. Knowing the difference in the influence between abdominal breathing techniques and deep back massage on reducing labor pain during the active phase at cengkareng hospital in 2022. This type of research is quantitative research, research design using quasi-experiments with one group pretest and posttest design. The sampling technique is a consecutive sampling technique, with a total sample of 44 respondents. Data processing using the T-test. Based on the results of independent analysis-test, a p value of 0.000 (p value < α 0.05) was obtained with a deep back massage standard deviation of 0.512 < 0.528 so that the results were obtained that H0 was rejected and Ha was accepted. From these results, it is concluded that there is a difference between abdominal breathing techniques and deep back massage in overcoming labor pain during the I active phase. Thus from both techniques more effective against pain adaptation response is deep back massage. The conclusion of this study is that deep back massage is more effective than abdominal breathing techniques with a standard deviation of 0.512 < 0.528 and a p value of 0.000 (p value < 0.05).

Abstract

Pregnant women most commonly experience anemia during the first trimester of pregnancy as a result of poor diet, which causes nausea and a lack of appetite. Chronic energy deficiency (CED) in pregnant women is caused by a long-lasting lack of energy intake in the mother, causing health problems. Pregnant women's nutritional intake has a significant impact on fetal growth during pregnancy, the incidence of low birth weight during delivery, and the baby's growth and development. Avocado (Persea americana) is a fruit that is very well known and popular with the public. Apart from having a delicious taste and soft texture, avocados also have several health benefits. The main components of avocados are carotenoids, fatty acids, minerals, phenolics, phytosterols, proteins, and many vitamins. Avocado is a fruit that is rich in vitamin A. Vitamin A has a role in erythropoiesis related to its function of synthesizing protein so that it will affect the growth of bone cells. Bone marrow is the site of the formation of erythrocytes. Vitamin A is needed in several essential processes in the body, such as metabolism, hematopoiesis, and erythropoiesis; it also regulates cell differentiation and plays a role in the immune system.The approach used in this research is experimental research. Experimental research is research that is carried out by manipulating, and it aims to determine the effect of manipulation on the observed individual's behavior. Manipulation carried out can be in the form of certain situations or actions given to individuals or groups, and after that, the effect can be seen. This experiment was conducted to determine the effects of a treatment given intentionally by the researcher. Giving this treatment is a peculiarity of experimental research compared to other studies. The results showed that there was a significant relationship between giving avocado fruit and improving the nutrition of first-trimester pregnant women, as indicated by the p value of 0.000. This is supported by research data showing that giving avocado fruit increases nutrition more, namely by as much as 70%, while not regularly consuming a lot of avocado fruit does not increase nutrition by 30%.

Abstract

In 2020 UNICEF reported that the average coverage of exclusive breastfeeding in the world for infants aged 0-6 months only reached 38%. In Indonesia in 2018, the coverage rate for exclusive breastfeeding was 52%, in West Java Province in 2020 the coverage for exclusive breastfeeding for infants was 72.6%. The low coverage of exclusive breastfeeding can be caused by various factors, one of which is the production of breast milk which is not optimal, so that many babies have less nutritional needs because mothers cannot provide maximum breast milk according to the nutritional needs of babies. Breast milk production can be increased by doing complementary therapies, one of which is consuming young papaya stew. To determine the effectiveness of consuming young papaya stew to increase the smooth production of breast milk in nursing mothers. Quasy experiment with one group pretest-posttest design. The sample in this study were all postpartum mothers at PMB Midwife Masniar in November 2022 as many as 30, the sampling technique was total sampling. The smooth distribution of milk production before consuming young papaya stew was mostly substandard 60.0% and after consuming unripe papaya stew most of it was smooth 63.3%. There is an effectiveness of consuming young papaya stew to increase the smooth production of breast milk in nursing mothers (p value 0.000). There is the effectiveness of consuming young papaya stew in increasing the smooth production of breast milk in nursing mothers. It is hoped that midwives can provide midwifery care for postpartum mothers who experience breast milk difficulties by consuming young papaya stew.
