Abstract

Indonesia has various traditional plants that are potential to be used as medicinal agents. The telang plant (Clitoria ternatea L.) has been studied regarding its’ potency as a traditional medicine. The parts of the telang plant that are often used are leaves and flowers. Telang leaves and flowers contain several compounds with antioxidant activity including phenolics, flavonoids, anthocyanins, and tannins. Antioxidant activity can be tested using several methods, e.g. DPPH, ORAC, and FRAP. Based on the results of the literature review, it is known that telang leaves and flowers have antioxidant activity and do not show any potential toxicity, therefore they can be considered as safe to be used.

Abstract

Ocular tuberculosis (OTB) is a chronic disease caused by infection of M. tuberculosis in the eye with various manifestations and can lead to blindness. Indonesia has the second highest tuberculosis burden worldwide, with 11% of the case is extrapulmonary TB, including OTB. The most common clinical presentation of OTB is posterior uveitis, but it could also involve all surrounding ocular tissue. OTB has a good prognosis if patients receive full course of antituberculosis therapy, but there are so many challenges in the diagnosis and treatment initiation. There is a new international concensus published by the Collaborative Ocular Tuberculosis Study (COTS) working group which could aid clinicians to manage the disease.

Abstract

Introduction: Men who have sex with men (MSM) have higher risk of contracting sexually transmitted infections (STIs) such as syphilis, one of chronic and systemic STI. HIV and HPV infection can be risk factors for syphilis infection, while coinfection between syphilis and HIV facilitates the course of the two diseases. Case: A 22-year-old homosexual man complained of red scaly patches on both palms and feet, no itching, hair loss and lumps in the genital area. Examination revealed nonscarring alopecia on occipital region, bilateral patches with multiple erythematous plaques partly confluent hyperpigmentation and an overlying scale on palmar and plantar region, also multiple nodule papules with pale red smooth surface on perineal region. The acetowhite examination at the verrucous lesion was positive. The results of the CD4 examination was 112 cells/µl, VDRL/RPR was reactive 1:256; and TPHA was also reactive >1:5,120. Histopathological examination of the skin showed the features of skin syphilis and condyloma acuminata. Immunohistochemistry test revealed brownish Spirochaeta. The patient was diagnosed with secondary syphilis coinfection, condyloma acuminata, and HIV. Conclusion: We found a case of MSM patient that was coinfected with secondary syphilis, condyloma acuminata, and HIV. HPV and HIV infection are often coexisting because they both increase the risk of sexually transmitted infections.

Abstract

Psoriasis is a chronic recurrent inflammatory disease influenced by several risk factors, including genetics, immune system, environment, and hormones. Skin lesions associated with psoriasis are well-defined erythematous plaques covered in thick, layered, white scales. Psoriasis treatment varies according to the severity of the condition and may include topical or systemic medications, light therapy, or natural elements such as minerals and balneotherapy. Balneotherapy may be used in conjunction with other treatments for psoriasis due to its efficacy in healing psoriasis lesions, particularly plaque forms. Balneotherapy is a traditional medicinal treatment that involves bathing, drinking, or inhaling mineral-containing water. Mineral sources include mineral water, mineral mud (peloids), and natural gas from natural springs. Balneotherapy can thin the stratum corneum and lucidum, decrease the number of lymphocytes, histiocytes, granulocytes, and eosinophils, increase skin permeability, decrease inflammation, improve microcirculation and the skin immune system, and decrease pro-inflammatory cytokines in psoriasis. This article aims to explore the efficacy of balneotherapy as adjuvant therapy for psoriasis to accelerate clinical improvement and reduce disease recurrence.

Abstract

Background: Molluscum contagiosum is an infectious disease of the skin caused by poxvirus. The virus may be transmitted through direct skin and mucous membrane contact, including sexual contact, and through contaminated fomites. The prevalence of sexually transmitted molluscum contagiosum has increased in recent years. This disease causes anxiety and discomfort for the patients. Diagnosis of molluscum contagiosum is made based on history taking, physical examination, dermoscopic examination, and histopathological examination. Proper diagnosis and management can help to reduce risk of recurrence. Case: A 26-year-old woman with main complaint of developing shiny white rashes on her thighs and buttocks since 3 months ago. The nodules felt itchy yet painless. At first, only 4 papules appeared on the buttocks. In the last month, the nodules multiplied and spread to the thighs. The patient had tried to squeeze the nodule and rice-like white mass came out. The patient had history of unprotected sexual intercourse. Dermatological examination on the right and left femoral regions and the gluteus, found that there were discrete, multiple shiny white pearl-like papules of 0.1-0.5 cm in size, smooth surface, well-defined, dome-shaped, umbilicated with caseous plug. Dermoscopic examination showed a white to yellow polylobular appearance with crown vessel. Histopathological examination showed an appearance of intracytoplasmic eosinophil inclusion bodies (molluscum bodies). Discussion: Clinical findings such as discrete multiple, pearl-colored, umbilicated dome-shaped papules, are characteristic features of molluscum contagiosum. Dermoscopic examination results and histopathological images also support the diagnosis of molluscum contagiosum. Treatment was carried out using curettage on the lesions and prescription of oral cimetidine 2x800 mg for 2 months. No recurrence was found after 5 months of follow-up

Abstract

Background: Candidiasis is caused by Candida sp. infection, mainly Candida albicans. The skin, mucous membranes, nails, and digestive tract are organs that can be affected by this infection. The risk of infection is higher in immunocompromised individuals. Objective: To determine the profile of candidiasis patients in Dr. Moewardi Hospital Surakarta between January 2016-December 2019. Method: This is a retrospective, descriptive study that evaluate the distribution of sex, age, primary diagnosis, comorbidities, and candidiasis type among candidiasis patients in RSUD Dr. Moewardi. Results: During study period, there were 177 candidiasis patients (147 outpatients and 30 inpatients). Pseudohyphae was seen in all inpatients’ KOH test. Majority of the patients are women (77,97%), age distribution varies between outpatients and inpatients. Cutaneous candidiasis was found in 53,74% outpatients and 66,67% inpatients. Type 2 diabetes mellitus was the most frequent (20%) comorbidity among those patients. Topical miconazole 2% and systemic fluconazole 150 mg were given to 42,37% and 33,90% patients, respectively.

Abstract

Colorectal cancer was infrequently diagnosed few decades ago, but the number of cases increased dramatically until it becomes the third most prevalent malignancy after breast and lung cancer. Population aging and on top of unhealthy lifestyle were major contributor to the significant rise of the disease. Management of colorectal cancer has markedly evolved, allowing improvement in terms of survival and advance in quality of life. Nevertheless, the rate of survival could be boosted by raising public awareness over colorectal cancer, education program that may lead to better health behaviour, as well as health system policy that support disesase screening, considering the characteristic of the disease that is often asymptomatic, particularly in the early development.

Abstract

Background: Diabetes mellitus (DM) is a chronic condition that requires continuous medical care with multifactorial management strategy. Soybean is known as source of isoflavones that possess beneficial effect to help controlling blood glucose level. This study aims to evaluate the effect of soybean product diet to reduce fasting blood glucose and improve insulin resistance. Methods: This is an experimental study using 30 streptozotocin-induced diabetic Wistar rats that divided into 5 groups, i.e. normal rats, negative control, and three treatment groups that receive tempeh, tofu, and soy flour diet for 2 weeks. Fasting blood glucose was tested before treatment, after diabetes induction, and at the end of the study. HOMA-IR was calculated using fasting blood glucose and fasting insulin level after diabetes induction and at the end of the study. MANOVA test was used to analyzed the data statistically (p<0,05). Results: There was a significant effect from soybean product diet (tempeh, tofu, and soy four) in reducing fasting blood glucose and HOMA-IR, with slightly better result shown in tempeh group. Conclusion: Soybean product such as tempeh, tofu, and soy flour have beneficial effect in reducing fasting blood glucose and improving insulin resistance in streptozotocin-induced diabetic rats.

Abstract

Background: General anesthesia is the most preferred anesthetic management for lower back surgery, considering the location and practicality. However, multiple studies showed that regional anesthesia, particularly thoracal segmental, spinal anesthesia, had more advantages compared to general anesthesia. Case presentation: We report a 19-year-old woman admitted to the OR for infected lipoma in lower back region, located in thoracal 12th, who underwent successful thoracal segmental spinal anesthesia. Conclusion: This is an original case report which showed significance of thoracal segmental spinal anesthesia and the advantages compared to general anesthesia.

Abstract

Today is the era of nanotechnology, where almost all fields use nanotechnology. Likewise, the field of pharmaceutical technology is currently intensively developing pharmaceutical preparations using nanosystem. One of these nanosystems is polymeric micelles, which generally have particle size below 100 nm. Research showed that polymeric micelles have been proven to increase the solubility and bioavailability of hydrophobic drugs. There are several methods that can be used in polymeric micelles preparation, each of which has advantages and disadvantages. In order to characterize polymer micelles, parameter evaluation includes determination of critical micelle concentration (CMC), particle size, surface characteristics, stability, and release tests. Although most of the research on polymer micelles was applied in the field of oncology by the intravenous route of administration, researchers are currently developing drug delivery system using polymer micelles for oral route of administration. It can be further expected to initiate clinical research like such that has been done on intravenous administration.
