Abstract

​​Background: Depression is the most common mental health problem experienced by nurses, especially in stressful environments. It can affect performance, reduce effectiveness, and decrease work productivity. One effective method to reduce depression is deep breathing techniques, which help enhance comfort and increase the body’s resilience. Additionally, positive affirmations short, simple statements spoken aloud and repeated slowly also help build confidence and combat depression. Objective: To determine the effectiveness of combining deep breathing techniques and positive affirmations in reducing depression among nurses at Bantul Public Health Center, Yogyakarta. Method: This study used a quantitative approach with a pre-experimental design employing a one-group pretest-posttest design. The sampling technique was non-probability sampling, specifically purposive sampling. The sample consisted of 17 respondents. Data collection was conducted using pre-test and post-test questionnaires. The DASS-42 instrument was used to measure depression levels. Data analysis was performed using the Wilcoxon signed-rank test to compare depression scores before and after the intervention within the same respondents. Results: Among the 17 respondents, 15 showed a decrease in depression scores, 1 showed an increase, and 1 showed no change. The average depression score significantly decreased from 3.29 to 0.41 following the deep breathing and positive affirmation intervention, with a Wilcoxon test significance value of 0.002 (p < 0.05). Conclusion: This study demonstrated a significant reduction in depression scores following the intervention combining deep breathing and positive affirmations, indicating that the intervention is effective in reducing depression among nurses at Bantul Public Health Center. Therefore, it is recommended to implement this intervention routinely as an effort to improve the mental health of healthcare workers.

Abstract

Preventive Maintenance (PM) is crucial for ensuring the reliability, safety and longevity of medical equipment in healthcare settings. This review evaluates key PM strategies emphasizing their applicability to resource-constrained hospitals. A systematic review was conducted, analyzing 19 articles selected from 37 screened, focusing on standardization, predictive maintenance, criticality-based prioritization, lifecycle management and data-driven approaches. Advantages, limitations and contextual relevance were concluded. Five main approaches were identified, standardization improves operational consistency and efficiency, while predictive maintenance enhances diagnostic accuracy and reduces downtime. Criticality-based prioritization optimizes resource allocation for high-risk devices and lifecycle management ensures sustainable use of high-value equipment. Data-driven strategies provide decision-making support but require significant technological investment. Combining standardization, criticality-based prioritization and basic predictive techniques is ideal for small and medium-sized hospitals. Larger hospitals can benefit from integrating lifecycle management and advanced data systems. A phased, flexible implementation ensures the reliability of medical equipment and the safety of patients.

Abstract

The aging process experienced by the elderly causes problems in physical, mental, and behavioral development. The physical abilities of the elderly decline with age, causing problems with cognitive decline in the elderly. One intervention that can be done in the elderly to stop cognitive decline is Crossword Puzzle Therapy (CPT) which can improve cognitive abilities, enhance memory, and improve understanding. To determine the effect of Crossword Puzzle Therapy (CPT) on the cognitive function of the elderly in the Tanjung Unggat Community Health Center working area in Tanjungpinang City. The research method used was a quasi experimental design with a One Groups Pretest-Posttest Design approach. The sample in this study consisted of 33 elderly patients treated at the Tanjung Unggat Community Health Center in Tanjungpinang City, and the sampling technique used was purposive sampling. This study used a normality test with a sig. value > 0.005 and continued with a non-parametric test, namely the Wilcoxon test. The results of the study showed a P value of 0.000, which means that the significance value is < 0.05, so that Crossword Puzzle Therapy (CPT) has a significant effect on cognitive function, namely H0 is rejected and Ha is accepted. There is an effect of Crossword Puzzle Therapy (CPT) on cognitive function in the working area of the Tanjungunggat Community Health Center in Tanjungpinang City with a P value of 0.000. It is hoped that community health centers can implement Crossword Puzzle Therapy (CPT) to improve the health quality of the elderly, especially in handling their cognitive function.

Abstract

This study aims to address the challenges in managing the pharmaceutical warehouse at Metro Islamic Hospital (RSIM), which has not fully complied with the First In, First Out (FIFO) principle. This non-compliance has the potential to cause a buildup of expired stock, financial losses, and decreased operational efficiency, as well as endangering patient safety. This report is the result of Field Learning Practice (PBL) conducted at RSIM, identifying the causes of the problem and evaluating its impact.. Method: The approach used in this study was direct observation of the storage and arrangement of pharmaceutical supplies in the RSIM warehouse. Data was collected through field observations, interviews with relevant staff, and internal document analysis. Priority issues were identified using the USG (Urgency, Seriousness, Growth) method to determine the main issues that needed immediate attention.. Conclusion: The results of this study indicate that the issue of drug arrangement that does not comply with FIFO is a top priority. Implementing a strategy to improve the layout of the pharmaceutical warehouse based on FIFO at RSIM is crucial to minimize the risk of expired drugs, reduce financial losses, improve patient safety through timely and high- quality drug administration, and enhance compliance with good pharmaceutical practice (GPP) regulations.Keyword: Pharmaceutical Warehouse, FIFO, Metro Islamic Hospital, Operational Efficiency, Patient Safety.

Abstract

Employee retention has become a major challenge in human resource management within hospitals, including for non-medical staff. A high turnover rate can disrupt the operational stability of a hospital. Therefore, human resource management strategies such as recruitment, selection, training, and career development are considered essential to improve employee retentio. This study aims to analyze the influence of recruitment, selection, training, and career development on the retention of non-medical employees at RS X. This research applied a quantitative approach with a cross-sectional design. The sample consisted of 60 non-medical employees selected through total sampling. Data were collected using a Likert-scale questionnaire and analyzed using Spearman rank correlation and logistic regression. Partially, the variables of recruitment, selection, training, and career development have a significant influence on the retention of non-medical employees at RS X. However, simultaneously, the selection variable does not show a significant effect on employee retention. Recruitment has a significant influence on the retention of non-medical employees at RS X, both partially and simultaneously. Selection has a partial influence on employee retention but does not have a significant simultaneous effect. Training significantly influences employee retention both partially and simultaneously. Career development also has a significant influence on employee retention, both partially and simultaneously

Abstract

The increasing use of electronic cigarettes (vapes) among adolescents has become a significant public health concern, driven by the perception that vaping is safer than traditional smoking and by strong social and media influences. This study aimed to analyze the factors influencing electronic cigarette use among students of SMA Muhammadiyah 1 Pekanbaru, including perceived health risks, peer pressure, media exposure, and family role. A quantitative cross-sectional design was employed with a total sample of 113 students selected through simple random sampling. Data were collected using questionnaires and analyzed using the Chi-Square test and Cramer's V. The results indicated significant associations between vaping behavior and perceived health risks (p = 0.012; Cramer’s V = 0.25), peer pressure (p = 0.001; Cramer’s V = 0.36), media exposure (p = 0.004; Cramer’s V = 0.29), and family role (p = 0.018; Cramer’s V = 0.24). Peer pressure was identified as the strongest influencing factor. These findings suggest that adolescent vaping behavior results from interactions among individual perceptions, social dynamics, and media influence. Comprehensive prevention strategies should include health education, strengthened parental supervision, peer-based interventions, and digital literacy programs to reduce vape use among students. Abstract

Effective communication in hospitals is crucial for improving healthcare service quality. This study aims to identify effective digital health communication media used in hospitals and explore the challenges and potential of digital platforms in enhancing interactions between patients and medical professionals. The research method employed is a systematic literature review (SLR), analyzing various global research articles on hospital health communication media from 2021 to 2025. he article selection method used PRISMA (Preferred Reporting Items for Systemic Reviews and Meta-Analyses) and was reviewed with the principles of the PICO (Participants, Interventions, Comparisons and Outcomes) framework. The findings indicate that social media, mobile applications, hospital websites, telemedicine, and interactive digital media effectively enhance patient engagement, provide fast and accurate information, and facilitate medical decision-making. Additionally, digital media simplifies communication between patients and healthcare providers, particularly for those in remote locations This study concludes that integrating various digital communication media in hospitals can create a more efficient and effective communication system by optimizing the use of social media, mobile applications, and telemedicine. The research highlights the importance of developing communication models that combine multiple media types to enhance patient accessibility and engagement in their care. Future studies should include long-term analyses of the impact of communication media on patient health outcomes, as well as the need for more holistic communication models.

Abstract

Metformin is the first-line therapy for type 2 diabetes mellitus; however, its use in patients with impaired renal function remains controversial. This study aimed to compare the effectiveness of combination antidiabetic therapy with and without metformin in patients with type 2 diabetes mellitus and renal impairment. A cross-sectional retrospective study was conducted using medical records of patients at Hospital X in Surakarta from January to December 2024. Patients with an estimated glomerular filtration rate below 60 mL/min/1.73 m² who received combination antidiabetic therapy for at least three months were selected through purposive sampling. Treatment effectiveness was evaluated based on the achievement of HbA1c levels below 7%. A total of 36 patients met the study criteria. The results showed no significant association between age, sex, comorbidities, or stage of chronic kidney disease and glycemic target achievement. None of the patients receiving metformin achieved the HbA1c target, while some patients in the non-metformin group did, although the difference between the two regimens was not statistically significant (p = 0.076). In conclusion, combination therapy with metformin did not demonstrate superior effectiveness compared to non-metformin combination therapy in this patient population.

Abstract

Repeat visit intention is an essential indicator of service continuity in primary healthcare facilities, reflecting patient perceptions of service quality, satisfaction, accessibility, and overall experience. The outpatient visit rate at Talang Banjar Health Center has shown fluctuations, indicating the need to identify factors influencing repeat visit interest. This quantitative study used a cross-sectional design with 106 respondents selected through accidental sampling. Data were collected using structured questionnaires and analyzed using the Chi-square test to examine relationships between service quality, patient satisfaction, accessibility, waiting time, and repeat visit intention. The findings showed that 63.2% of respondents had an interest in returning, while 36,8% did not. Significant relationships were found between repeat visit intention and service quality (p = 0.006; PR = 3.034), patient satisfaction (p = 0.001; PR = 4.348),  accessibility (p = 0.005; PR = 3.100). and waiting time (p = 0.001 ; PR = 3.912). These results indicate that improving service quality, enhancing patient satisfaction,  accessibility, and waiting time are crucial to strengthening patient interest in returning to the health center. Strengthening these aspects may support better service continuity in primary healthcare settings.

Abstract

The dynamics of health services demand strong doctor–nurse collaboration, which often involves the delegation of medical authority. Recent regulatory changes through Law Number 17 of 2023 and Government Regulation Number 28 of 2024 have created significant implications for the boundaries of legal liability, particularly within the grey areas between mandates and delegation. This study analyzes the shifting paradigm of legal responsibility in the delegation of medical authority and the validity of electronic medical instructions as evidence in medical disputes. This research employs a normative juridical method using a statute approach and case approach, relying on secondary data analyzed qualitatively. The findings, based on Government Regulation Number 28 of 2024, confirm that delegated authority transfers full legal responsibility to the receiving nurse, while under a mandate, liability remains with the physician. However, medical instructions delivered via messaging applications (such as WhatsApp) without integration into Electronic Medical Records (EMR) carry a high risk of being deemed legally invalid, placing nurses in a vulnerable position regarding malpractice claims. In remote and underserved areas (DTPK), legal protection becomes conditional, depending on urgency and the unavailability of medical personnel. Therefore, the formalization of rigid standard operating procedures (SOPs) regarding the validation of digital instructions within EMR systems is urgently required to ensure legal certainty for healthcare workers.
