Abstract

The main problem in conducting lupus treatment is that most drugs for the therapy provided have not been authorized so that they are still included in the class of off-label drugs for the treatment of lupus. The use of off-label drugs can carry the potential for side effects in Lupus patients which have an impact on increasing health costs. This study aims to determine the impact of side effects of using off-label drugs on health costs in lupus patients, as an integrated evaluation in seeking to improve the quality of life and ensure proper care in Lupus patients. This study was conducted using a cross-sectional method by means of a survey using the Naranjo Algorithm instrument (PESO Book) to determine the incidence rate of side effects that exist in LSE therapy using off-label drugs and calculate the health costs of lupus patients. The results showed that the most respondents were in adulthood (19 respondents or 47.5%), then the most respondents were female (37 respondents or 92.5%). Data on the incidence of side effects obtained the most respondents in the Probable category (most likely to occur ROM) of 14 respondents or 35%. In addition, data on health costs incurred by more respondents experienced an increase, namely 23 respondents or 57.5%. The research data shows the calculated r value is 0.3044 and the Sig (2-tailed) value is 0.000 with a confidence level (CI) of 0.05. The results of the analysis clearly show that there is a significant relationship between the side effects of using off-label drugs on the health costs of lupus patients, in this correlation there is a unidirectional relationship, where if the value of side effects experienced by respondents is high, the value of respondents' health costs is also high.

Abstract

This study investigated the hepatoprotective effects of Phak Khom (Andrographis paniculata) using in vitro and in vivo models. Phak Khom extract showed dose-dependent cytotoxicity on hepatocyte cells, significantly reducing cell viability at concentrations above 100 µg/mL. In addition, the extract showed a reduction in oxidative stress markers, with a 40% reduction in reactive oxygen species (ROS) and inhibition of pro-inflammatory cytokines (TNF-α and IL-6). In vivo, Phak Khom treatment in rats resulted in a significant reduction in liver enzyme levels (ALT and AST), indicating improved liver function. Histopathological analysis revealed decreased liver necrosis and fibrosis, while oxidative stress markers such as malondialdehyde (MDA) were decreased, and superoxide dismutase (SOD) activity was increased. Statistical analysis confirmed the efficacy of the extract at both low and high doses. These findings validate the traditional use of Phak Khom in liver-related disorders and support its potential as an adjunct therapy for hepatitis. However, further clinical studies, especially randomized controlled trials, are essential to confirm its safety and efficacy in humans, and to explore its synergy with conventional treatments.

Abstract

This study investigated the potential of Ocimum basilicum var. thyrsiflora (Thai basil) extract as a natural adjunct therapy for the management of type 2 diabetes. The increasing prevalence of diabetes globally and the limitations of conventional therapies require alternative approaches, especially in regions where Thai basil is culturally accepted. The objective was to evaluate the effects of Thai basil on blood glucose levels and glycemic control in patients with type 2 diabetes. A randomized controlled trial was conducted in 2024 with 200 participants, aged 30–65 years, diagnosed with type 2 diabetes. Participants were assigned to either the Thai basil extract group (500 mg daily) or the placebo group, with clinical data collected at baseline, mid-intervention, and post-intervention. Findings showed that the intervention group experienced significant reductions in fasting blood glucose and HbA1c levels, indicating improved long-term glycemic control. In addition, lipid profiles, including cholesterol and triglyceride levels, were positively affected. However, no significant weight loss was observed. This review highlights the therapeutic potential of Thai basil bioactive compounds, including flavonoids and essential oils, which have antioxidant, anti-inflammatory, and antidiabetic properties. Limitations such as sample size and study duration are noted, indicating the need for further studies with larger, more diverse populations and longer duration. Future studies should explore the optimal dosage, formulation, and long-term effects of Thai basil. This study supports the inclusion of Thai basil in diabetes care, especially in regions where basil is widely available and culturally accepted.

Abstract

Cissus quadrangularis is a plant known for its traditional use in bone healing, has shown potential in modern pharmacological applications due to its bioactive compounds. This study investigates the therapeutic effects of Cissus quadrangularis extract on bone healing, antioxidant activity, and gastric protection. The primary objective was to evaluate its efficacy in accelerating bone regeneration, mitigating oxidative stress, and protecting the gastric mucosa from NSAID-induced damage. The methodology involved a series of in vivo experiments using rats, with treatment groups receiving the extract and control groups given a placebo. Bone healing was assessed radiographically, antioxidant activity was measured using the DPPH assay and SOD enzyme activity, and gastric protection was evaluated by counting gastric lesions. The findings revealed a 40% improvement in bone healing, a 25% increase in SOD activity, and a 60% reduction in gastric lesions in the treatment group compared to controls. These results suggest that Cissus quadrangularis has significant potential as a natural remedy for bone fractures, oxidative stress, and gastric ulcers. Future research should focus on human clinical trials, molecular mechanisms, and long-term safety to support its broader medical application.
