Abstract

Diarrhea is still the second cause of morbidity and mortality in children under five in Indonesia. Data from Basic Health Research (Riskesdas) in 2013 showed that the incidence of diarrhea in toddlers in Indonesia was 6.7%, a similar figure was also found in West Nusa Tenggara Province at 6.6%. Several factors are related to the incidence of diarrhea, namely exclusive breastfeeding and clean water facilities. This study aims to determine the relationship between exclusive breastfeeding and clean water facilities with the incidence of diarrhea in toddlers in Selagalas Village, the working area of the Cakranegara Community Health Center. The research design was cross sectional on 104 toddlers conducted in 3 selected environments in Selagalas Village. Data was collected by structured interviews using questionnaires and observation sheets. Data were analyzed bivariately with Stata SE 12.1. The results of this research were that 67 (64.42%) toddlers had experienced diarrhea. The variable related to the incidence of diarrhea is clean water facilities. There is no significant relationship between exclusive breastfeeding and the incidence of diarrhea. The importance of providing good and adequate clean water facilities, especially for mothers in caring for children.

Abstract

A caesarean section is an artificial birth that is carried out by making a small incision in the abdominal wall and uterine wall, but the uterus must be intact and the fetus weighs >500 grams. The high cost of a cesarean section has prompted the Indonesian government to launch the National Health Insurance (JKN) program as an implementation of the National Health Insurance (JKN) which has been regulated regarding to the payment patterns to advanced health facilities by applying Indonesian Case Based Groups (INA-CBG's). This study aims to determine the difference between real hospital cost rates and INA-CBGs rates for payment of claims by JKN participants for caesarean section inpatients at Dr. Soedomo Regional Hospital Trenggalek. The research method is analytical observation with a cross sectional approach which refers to the hospital's perspective by comparing the direct medical costs of BPJS’s (Social Security Agency on Health) participant patients and the INA-CBG's rates. The populations of this study were BPJS recipients of class I, II, III inpatients who received mild category of caesarean section at Dr. Soedomo Regional Hospital Trenggalek in 2020. The sample used in this study was medical record documents that met the inclusion and exclusion criteria. Purposive sampling method was used. In treatment of I, II, III classes, a significant difference was found between the average total direct medical costs of mild category of caesarean section patients (O-6-10-I) compared to the INA-CBGs rates. The treatment of class I had a difference of IDR 886,582. (p<0.05). The treatment of class II had a difference of IDR 754,881. (p<0.05) and the treatment of Class III, the difference obtained was IDR 1,109,368. (p<0.05).

Abstract

One of the main risk factors for chronic kidney disease (CKD) is excessive alcohol consumption. Alcohol consumption can damage the kidneys and disrupt their function because the kidneys filter toxins and waste products from the blood. This study aims to estimate the magnitude of the relationship between alcohol consumption and the incidence of chronic kidney failure. This research is a systematic review and meta analysis with PICO as follows, population: Adults. Intervention: alcohol consumption, Comparison: no alcohol consumption. Outcome: chronic failure events. The articles used in this research were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles are "Alcohol" AND "Chronic Kidney Disease" OR CKD AND Multivariate. Articles used from 2018 – 2023. Article selection was carried out using the PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 application. A total of 6 cohort studies, namely Asia and Europe, were selected for systematic review and meta analysis. Cohort studies show that alcohol consumption influences the incidence of CKD. Patients who consume alcohol have a risk of experiencing CKD that is 1.05 times compared to those who do not consume alcohol, but the resulting increase in risk is not statistically significant (aHR= 1.04 95% CI= 0.85 to 1.27; p= 0.710). Forest Plot also shows heterogeneity of effect estimates between primary studies I2= 84%; p= 0.040, which means that the estimated effect between primary studies in this meta-analysis varies. Thus, the calculation of the average effect estimate was carried out using a random effect model approach.

Abstract

Exclusive breastfeeding had many benefits for mothers and babies. Maternal employment was one of the factors that was closely related to the decline in exclusive breastfeeding coverage. Therefore, there was a need for education for mothers of toddlers to continue providing breast milk even though they are working. Objective of this research was to analyze the effect of simulation-based teaching on about the management of expressed breast milk on the mother's knowledge of the baby in Toddler Posyandu in Pelem Village, Bendo Community Health Center. The method was research quantitative. The reasearch was carried out at the Posyandu Balita in Pelem Village, Bendo Community Health Center on Saturday, January, 20th 2024. This activity was attended by 40 mothers of toddlers. There was an influence of health education using the simulation-based teaching method on the mother's knowledge about the management of expressed breast milk with a wilcoxon test result of 0.002.

Abstract

Phlebitis is defined as inflammation of the vein wall where 2 complications that may develop as a result of phlebitis are: infection resulting from an accumulation of neutrophils; and thrombus, which may lead to complete occlusion of the vein. As healthcare professionals cannot look inside veins during IV therapy, they must rely on what they can see, feel and ask, i.e. detectable changes in skin color, texture, temperature and sensitivity, to identify the development of phlebitis. The absence of a good blood flow and the presence of a poor infusion rate have also been cited as clinical indicators of phlebitis Observation of the patient for clinical signs and symptoms of phlebitis is therefore the key to preventing serious venous damage. Health professional hands are a common of spreading microorganisms, thus very important for health workers followed one of precaution is to attention wash your hands of properly. Phlebitis is caused by chemical factors (type of net liquids), mechanical factors (insertion measures in place), and the most common factors (hand washing techniques including the factors of the patient's age), have been caused by phlebitis. The same results in some research conducted by some researchers show that the causes of phlebitis, among them, the types of liquidity and knowledge of nurses, type of therapy, and development of children, the location and the application of these measures were not yet put in place. The design of research is a quantitative study, with a cross-sectional approach. The sampling technique in this study was accicental sampling, with a sample of 56 respondents. The study was conducted on November 2022 in National Hospital Guido Valaradres Dili, Timor Lest. The result of study showed that the cannula factor influence the phlebitis of the correlations is 0.605 and significant P value is 0.000, time/duration using cannula factor influence the phlebitis of the results correlations is 0.633 and significant P value is 0.000 and the age factor influence the phlebitis of the correlations = 0.736 and significant P value is 0.000

Abstract

Childhood is the very vulnerable period since children have to depend on their basic need and decision making. Nevertheless, there are many children who cannot get care from their parents or family due to various reasons therefore they are sent to orphanages. Thus, health education for children is something that needs to be given to them. This study aims to analyze the effect of snakes and ladders game on the level of knowledge and attitudes of children about reproductive health. This research is an experimental study conducted on 30 orphanage children using purposive sampling. The intervention is snakes and ladders game. Before and after the intervention, children's level of knowledge and attitudes about reproductive health was measured. Data collection was carried out using pre-test and post-test questionnaires. The statistical analysis showed that there were significant differences in the level of knowledge (p-value: 0.000) and attitude (p-value: 0.001) before and after the intervention. The snake and ladder game are able to increase children's knowledge and attitudes about reproductive health.

Abstract

Babies who are exclusively breastfed will get all the advantages of breast milk and maximum nutrition. On the other hand, the incidence of malnutrition in the future is largely determined by breastfeeding patterns. The aim of this research is to determine the relationship between breastfeeding patterns and the nutritional status of babies aged 6–24 months at the Islamic Boarding School I Community Health Center, Kediri City. This study used a cross-sectional design. The research population was mothers who had babies aged 6–24 months at the Pesantren I Health Center in Kediri City in August–October 2023, for a total of 150 babies. The research sample consisted of several mothers who had babies aged 6–24 months at the Pesantren I Health Center in Kediri City who met the inclusion and exclusion criteria. There were 110 respondents. The research instrument for assessing breastfeeding patterns uses a questionnaire, while nutritional status is assessed using height and weight measurements. Data analysis used the Spearman rho statistical test. Half of the mothers had a pattern of predominant breastfeeding, namely 50%. Based on nutritional status, undernutrition 28.2%,. The Lambda test results show p = 0.016, meaning that there was a relationship between breastfeeding patterns and the nutritional status of children aged 6-24 months. The magnitude of the correlation was 0.152 which indicates the correlation very weak category. Breast milk given exclusively during the first 6 months of life can provide sufficient nutrition for the baby's growth and development.

Abstract

Sleep is a factor that affects the development of infants. Babies need optimal sleep time to produce developmental hormones. Poor sleep quality in infants has a negative impact such as babies becoming easily emotional, concentration and body immunity decreases. One of the therapies to improve sleep quality is to provide external stimulus in the form of baby massage therapy using lavender oil and baby sleep music (lullaby). The aim of this study was to determine the effect of a combination of baby massage using lavender oil and baby sleep music (Lullaby) on the sleep quality of infants aged 6-12 months in the working area of PMB Supianah, Malang. This study uses a quantitative research design and the research approach used is an experimental approach. The sample in this study was a portion of infants aged 6-10 months at PMB Supianah, S.Tr.Keb., Bd, Malang, with a total of 30 respondents. The sampling technique used is purposive sampling. The analysis used is paired samples t test. The research results obtained from 30 respondents most of the respondents (infants) experienced mild sleep problems, namely 25 infants (83.3%). Whereas at the time of the posttest, it was found that most of the babies did not experience problems in their sleep, namely as many as 25 babies (83.3%). Based on statistical analysis of paired samples t test , it was obtained that the value of p = 0.000 <0.05, then H0 was rejected and H1 was accepted, which means that it can be concluded that there is a combined effect of baby massage using lavender oil and baby sleep music (lullaby) on the sleep quality of infants aged 6-12 months in the working area. PMB Supianah City of Malang. With a correlation coefficient value of 0.80, which means the level of the relationship is in the strong relationship category, where the range of correlation values between 0.60-0.799 is included in the strong relationship category. The combination of baby massage using Lavender Oil and Baby Sleep Music (Lullaby) is effective in improving the sleep quality of babies aged 6-12 months.     Keywords: Baby Massage, Lavender Oil, Baby Sleep Music, Sleep Quality

Abstract

Introduction. Lack of awareness of type 2 DM patients and the duration of suffering are factors that inhibit type 2 DM sufferers from carrying out self-management properly. The purpose of this study was to determine the effect of family support and long suffering with self-management in patients with Diabetes mellitus (DM) type 2. Methods. The study design used cross sectional. The population is patients with type 2 diabetes mellitus. The total sample was 167 respondents, taken according to inclusion and exclusion criteria. The sampling technique used is purposive sampling. The independent variables are family support and long suffering. The dependent variable is self-management. Data was collected using questionnaires that have been tested for validity and reliability using product moments. The data were analyzed using spearman rho analysis. Results. The results showed that there was an association of family support for self-management (p = 0.000) and no association of long suffering with self-management diet (p = 0.879). Discussion. High family support can make people with type 2 DM have good self-management. long suffering from DM does not necessarily make DM sufferers have good self-management. As long as people with type 2 DM have the awareness to carry out self-management well, complications can be prevented. For health workers, it is expected to further optimize health education and counseling in increasing spiritual intelligence in carrying out self-management of type 2 DM.

Abstract

The incidence of phlebitis will increase along with poor infusion insertion actions or not in accordance with hospital SPO. This study aims to analyze the relationship between nurse compliance in the implementation of infusion SPO with the incidence of phlebitis in the inpatient room. This study used a cross sectional approach. The sampling technique used in this study used total sampling with a sample size of 22 nurses and 22 inpatients at RSI Mabarrot MWC NU Bungah in February 2024. The instruments used in this study were observation sheets for compliance with SPO for infusion and VIP (Visual Infusion Phlebitis) screening sheets to measure the occurrence of phlebitis. The results showed that out of 22 nurses, 20 nurses were compliant with SPO for infusion with 18 patients (72.7%) not experiencing phlebitis, and 4 patients (18%) patients experiencing phlebitis. The results of data analysis showed a significance value of 0.015 <0.05, which means that there is a relationship between nurse compliance in implementing SPO infusion with the incidence of phlebitis in patients. The compliance of nurses who carry out the SPO for infusion installation will obediently reduce the incidence of phlebitis in patients. It is recommended for nurses to always improve compliance with infusion installation according to the SPO set by the hospital to minimize the occurrence of phlebitis in patients during hospitalization.
