Abstract

Background: Basic Health Research (RISKESDAS) data in 2013 showed that the prevalence of overweight among adolescents aged 13-15 years was 10.8%. Karanganyar district is ranked fourth by the number of overweight adolescents cases in Central Java with prevalence of 8.2%. This study aimed to determine the asssociation between socio-cultural, lifestyle factors, and school environment on the risk of overweight in adolescents, Karanganyar District, Central Java, using social cognitive theory (SCT), PRECEDE-PROCEED model, and multilevel analysis.Subjects and Method: This was an analytic observational study with cross-sectional design. The study was conducted at 25 junior high schools in Karanganyar district, Central Java, in December 2017. A sample of 200 junior high school students were selected for this study by stratified sampling and fixed disease sampling. The dependent variable was overweight. The independent variables were nutritional intake, snacking habits, physical activity, self efficacy, paternal BMI, maternal BMI, peer group influence, and school environment. Body weight data was measured by digital scale. Contextual effect of school environment was measured by the number of snack sellers at school. Other data were collected by questionnaire and analyzed by multilevel analysis.Results: High nutritional intake (b= 1.56, SE = 0.73, p= 0.032), high snacking habits (b= 1.27, SE = 0.70; p= 0.069), paternal BMI ≥23 (b= 1.60; SE= 0.79; p= 0.042 ), maternal BMI ≥23 (b= 2.29; SE = 0.76; p = 0.002), parent income ≥minimum  regional  wage (b= 2.17; SE= 0.76; p= 0.004), and strong peer group influence (b= 2.22; SE = 0.65; p= 0.001) increased the risk of overweight in adolescents. High self-efficacy (b= -3.61, SE= 0.75, p <0.001) and high physical activity (b= -2.92; SE= 0.80; p <0.001) decreased the risk of overweight in adolescents. Intraclass correlation= 9.92% indicating considerable contextual effect of school environment.Conclusion: Nutritional intake, snack habits, paternal BMI, maternal BMI, family income, and peer influence increase the risk of overweight in adolescent. Self-efficacy and high physical activity decrease the risk of overweight in adolescent. Abstract

Background: Cyber-bullying involves the use of electronic media with the aim of causing damage, humiliation, suffering, fear, and despair for individuals who are the targets of aggression. The impact of cyber-bullying is not only affect their daily lives but may continue to adulthood. KiVa is research-based anti-bullying program that has been developed in the University of Turku, Finland, with funding from the Ministry of Education and Culture. The effectiveness of KiVa has been shown in a large randomized controlled trial. This study aimed to describe the health problems and evidence-based intervention programs that support trauma healing through KiVa program.Subjects and Method: A systematic review was conducted by searching the following databases: PubMed, SAGE Journals and Virtual Health Library from 2010 until 2018. The keywords for this review included cyber-bullying AND children, cyber-bullying AND mental health AND Children, cyber-bullying in children AND intervention. The inclusion criteria were systematic review, clinical review, cohort study, cross-sectional study, clinical study and guidelines. After the review process, 19 articles and 1 RCT were included.Results: Internationally, the impact of cyber-bullying on children has been linked to poor mental health including overall psychological health, low self-esteem, depression and anxiety and violent behavior. Another study showed that bully victims had the highest risk of self-injury, suicidal ideation, suicide plan and effort. KiVa is a school-based anti-bullying program from Finland involving teachers, students and parents. The KiVa principle is the prevention of bullying by cultivating character in students by forming group norms. The forms vary, can be videos, online games, surveys, posters, and guidebooks for parents. Children are taught a sense of responsibility and empathy. Previous RCT studies which involved 2,042 students in grades 4 and 6 showed that KiVa reduced intimidation, pro-intimidation, and victimization. And increasing attitude and empathy for victims. Overall, the findings provide evidence of the effectiveness of the KiVa Program.Conclusion: Cyber-bullying is associated with emotional stress and even suicide attempts. Parents, health personnels, and educators must be aware of the long-term risks so that prevention and treatment programs such as KiVa should be considered to be applied in Indonesia.Keywords: Cyber-bullying, children, mental healthCorrespondence: Aris Widiyanto. Abstract

Background: Every five years, the American Heart Association (AHA) releases new guidelines on CPR. Previous guidelines have focused on Airway-Breathing-Circulation (ABC) advocacy. However, in 2010 guidelines for CPR recommended changes in the sequence of BLS steps to CAB (chest compression, airway, and breathing) for adults, children and infants (excluding newborns). The purpose of this review is to explain the reasons and provide scientific evidence about the results of CAB actions compared to ABC.Subjects and Method: Systematic reviews were done by searching the database through PubMed, Google Scholar, and Science Direct. Key words for this review include: AHA Guideline AND CPR 2010, CAB in CPR, CAB, and RCT (Randomized Controlled Trial) guidelines and why does ABC turn into CAB? The inclusion criteria are systematic reviews, clinical reviews and guidelines.Results: The results are in the form of 3 review articles and 1 RCT study. The basic reasons for changing ABC to CAB are (1) The most common case of cardiac arrest in adults, and the initial element of critical BLS (basic life support) is chest compression and early defibrillation by changing sequentially to CAB, chest compression will start faster; (2) Most victims of cardiac arrest outside the hospital do not get CPR observers, this is an obstacle to opening the airway and exhaling; and (3) Chest compression provides vital blood flow to the heart and brain. One RCT study showed that the time to do the first resuscitation with the CAB technique (mean = 25; SD = 10) was faster than the ABC technique (mean = 32; SD = 12) and statistically significant (p = 0.002).Conclusion: Chest compression is the most important aspect of heart attack management because airway maneuver takes a long time. Abstract

Background: Palliative care is an essential right for infants and children with a life-threaten­ing ill­ness. However, palliative care is still un­ava­il­able in some developing countries. This stu­dy aim­ed to explore the issues, challenges, and po­ten­tial of palliative care in pediatric nursing care.Subjects and Method: A systematic review was conducted by searching published articles in PubMed, CINAHL, and EMBASE databases, from year 2000 to 2018. The keywords used for this stu­dy included “palliative” AND “children”, “pal­lia­tive” AND “nursing” AND “care”, “palliative ca­re” AND “pediatric care”. The data were analyzed by PRISMA flow diagram.Results: Palliative care is evolving continuously, meeting new challenges, and expand­ing beyond the care of terminally ill pediatric patients. A ma­jor challenge in palliative care is effective com­mu­­nication. However, only a few communication trainings exist for nurses. Obstacles of palliative care in pediatrics nursing included clinical, ope­ra­tional, and financial factors. However, pallia­tive care has a substantial potential to reduce morbidity in infants and children. In addition, a cohort study conducted in 265 pedia­tric cancer patients reported that palliative care reduced pain and improved their life expectancy.Conclusion: Palliative care is a comprehensive app­roach of care and its delivery to pediatric pa­tients is challenging. Abstract

Background: Maternal mental disorders during pregnancy are associated with a range of adverse health outcomes for infants. Re­cent studies indi­cate the possible mechanism of maternal mental health disorders asso­ciated with fetal develop­ment through prog­ram­­ming effect. This study aimed to sys­tema­t­ically review the effect of ante­natal mental health disorders on fetal growth.Subjects and Method: This was a sys­tem­atic review. The articles were selected from Psyc­INFO, Medline, Web of Science, Em­­­base, and PubMed journal databases pu­blis­hed from July to Septem­ber 2018. The keywords for this review included maternal mental health AND fetal growth, post­partum de­p­ress­­ion AND fetal growth, maternal mental health AND fetal growth AND review. As many as 11 articles were selected for this stu­dy from 575 articles.Results: Maternal mental health during preg­nancy was associated with fetal head cir­cum­­ference growth through 2 mechanism: (1) In­creased maternal stress response modifies neu­ro­en­docrine function, which included changes in cortisol regulation, adreno­corti­co­tropic, adrena­line and nor­adrenaline hor­mo­nes levels; and (2) Decreased 11β-hydroxy­steroid dehydrogenase (11β-HSD2) enzyme regulation in placenta. It decreased11β-HSD2 level, increa­s­ed fetal gluco­corticoids circu­la­tion, and affect­ed on fetal head growth restriction.Conclusion: Maternal mental health during pregnancy is associated with fetal head circ­um­­fe­rence growth restriction.Keywords: maternal mental health, fetal head circumference, growth, systematic re­viewCorrespondence: Rina Tri Handayani. Abstract

Abstract : In Indonesia ranges from 12-20% of early marriages performed by a new partner. Typically, early marriages performed at a young age couples average age between 16-20 years old. Nationally, early marriages by age of bride and groom under the age of 16 as much as 26.95%. This reserch aims to determine the relationship of age of marriage to parenting on children in the village of democratic Gedongan, Sragen Regency. Research methods a type of observational analytic study with cross sectional design, with a population of 220 pairs of sampling done by purposive sampling techniques that meet the criteria for inclusion of 70 pair. Results is There was a relationship of marriage age of democratic parenting style in children, found 39 people (55.7%) with 33 young married people (47.1%) who had a good upbringing of children and 6 men (8.6%) had a pattern foster child is not good, while 31 people (44.3%) were married sufficient age to have good parenting. The results of chi-square test with a confidence level of 95% or ? = 0.05 obtained greater value X2 table (5.216&gt; 3.841) and p-value = 0.022 (p &lt;0.05), which means Ha Ho accepted and rejected, so no relationship to the pattern of the democratic age of marriage on child care. Conclusion there is a marriage age of democratic parenting style on children in the village of Gedongan, Sragen Regency.                                           Keyword : Age of Marriage, Parenting Democratic  Abstrak : Di Indonesia pernikahan diusia dini 12-20% yang dilakukan oleh pasangan baru. Pernikahan dini dilakukan pada pasangan usia muda usia rata-rata umurnya antara 16-20 tahun. Secara nasional pernikahan dini dengan usia pengantin di bawah usia 16 tahun sebanyak 26,95%. Penelitian ini bertujuan untuk mengkaji hubungan usia perkawinan terhadap pola asuh demokratis pada anak di Desa Gedongan, Kabupaten Sragen. Jenis penelitian ini adalah observasional analitik dengan rancangan cross sectional, dengan populasi 220 pasangan dilakukan pengambilan sampel dengan teknik purposive sampling yang memenuhi kriteria inklusi sebanyak 70 pasangan. Hasil penelitian ini adalah Ada hubungan usia perkawinan  terhadap pola asuh demokratis pada anak, didapatkan 39 orang (55,7%) menikah muda dengan 33 orang (47,1%)  yang mempunyai pola asuh anak baik  dan 6 orang (8,6%) mempunyai pola asuh anak tidak baik, sedangkan 31 orang (44,3%) menikah cukup usia yang mempunyai pola asuh baik. Hasil uji chi-square dengan tingkat kepercayaan sebesar 95% atau ? = 0,05 didapatkan nilai X2 lebih besar  X2 tabel (5,216&gt;3,841) dan nilai p=0,022 (p&lt;0,05) yang artinya Ha diterima dan Ho ditolak, sehingga ada hubungan usia perkawinan terhadap pola demokratis pada asuh anak. Kesimpulannya adalah Ada hubungan usia perkawinan terhadap pola asuh demokratis pada anak di Desa Gedongan, Kabupaten Sragen.    Kata Kunci : Usia perkawinan, pola asuh demokratis

Abstract

ALS and BLS are the treatment options that are always taken when finding a cardiac arrest patient outside the hospital. The purpose of this review is to determine the effectiveness of the two emergency treatments that are correct according to the conditions and also their success in reducing mortality. This review was conducted by the author in June - July 2019, by searching databases including PubMed, Google scholar, and open access science direct to find out the potential of eligible studies. In this study it was explained that BLS was more likely to improve the ability of life of patients after being discharged from the hospital and improve the neurological function of patients after the rescue action from cardiac arrest.

Abstract

Background: Telemedicine has been practiced since 1960. The objective of telemedicine was to establish feasible interactive telecommunication for medical diagnosis and treatment of patients at remote sites. Nowadays, with healthcare costs on the rise, telemedicine is increasing­ly being seen as a strategy for healthcare organizations to make cost-saving. This study aimed to explore the opportunity of telemedicine utilization in cost savings to patients and the health care system.Subjects and Method: A systematic review was conducted by searching the published articles from Pubmed, MEDLINE, EMBASE, CINAHL, and Science Direct databases. Keywords for this study were ?telemedicine? AND ?patient saticfaction?, ?telemedicine? AND ?cost-utility and cost-effective­ness?, ?tele­medicine? AND ?systematic review?, and ?telemedicine? AND ?meta analysis?. The data were analyzed by PRISMA flow diagram.Results: 8 articles were selected for this study. These studies reported that telemedicine utiliza­tion in dermatology, radiology, pediatrics, and intensive care unit (ICU) rooms reduced health cost by 56% and patients travel cost to health care by 94%. Telemedicine advantages for patients were reduced transportation time or cost, eliminated time off of work, on-demand option, and reduced time in the waiting room, so that it can increase patient?s satisfaction. A study reported that telemedicine utilization at the pediatrics department increased hospital?s revenue by USD 101,744 per year.Conclusion: Telemedicine is an alternative health care to generate cost savings for patient and hospital and it can increase patient?s satisfaction.Keywords: Telemedicine, cost effectiveness, patient satisfactionCorrespondence: Joko Tri Atmojo. Abstract

The World Health Organization has stated that COVID-19 is a "global pandemic". However, community attitudes and compliance in maintaining protocols during the pandemic were minimal and raised the issue of "herd immunity". What exactly is herd immunity, how exactly does the body's immune system react to COVID-19 and what is a COVID-19 pandemic? The purpose of writing articles is to provide information about the definition of a pandemic, the body's immune system in dealing with COVID-19, and herd immunity. A systematic review was carried out in May 2020 by searching databases: PubMed, google scholar, NIH, CDC, and Science Direct. Keywords: "Pandemic and COVID 19" and "COVID 19 rapid test and accuracy", "PCR accuracy" and "PCR COVID 19 vs Rapid test" and "Herd immunity". The COVID-19 pandemic occurred because there was a discovery and a new mutation of the SARS-CoV virus becoming very infectious and of high virulence. Herd Immunity cannot be said to be the relevant treatment for COVID-19. It requires a large minimum threshold of immunity to achieve Herd Immunity, and it is very unethical to allow large numbers of individuals to be infected.

Abstract

Australasian triage scale (ATS) adalah algoritma triage gawat darurat yang terdiri dari lima tingkat yang terus dikembangkan di Australia, yang memiliki koefisien keandalan gabungan adalah  0,428 (95% CI 0,340-0,509). Review ini menjelaskan pembagian sistem triase ATS, cara kerja, dan tingkat keandalannya pada unit gawat darurat. Penelitian ini dilakukan dengan mencari dan menseleksi data dari beberapa database diantaranya : PubMed, Science Direct, Web of Science, Springer Link dan Cochrane Database. Dengan menggunakan kata kunci: national triage scale ATAU australasian triage scale ATAU Skala Triase Australia ATAU Reliability and australasian triage scale ATAU Validity and australasian triage scale ATAU Guideline and australasian triage scale. Penelitian dilakkan pada bulan Oktober 2019. Sebanyak 5 artikel telah memenuhi kriteria untuk dibahas lebih lanjut. Kategori ATS menggunakan warna antara lain:  Merah (Kategori 1), Oranye (Kategori 2), Hijau (Kategori 3), Biru (Kategori 4) dan Putih (Kategori 5) dengan waktu penentuan kategori dan penanganan segera hingga batas waktu maksimal 120 menit sejak kedatangan pasien pada unit gawat darurat.
