Abstract

Chronic kidney failure (CKF) occurs when the kidneys experience a decline in their function and duties. This condition affects various body systems, leading to fluid imbalances, edema, increased blood pressure, and heart rhythm disturbances. One of the replacement therapies is hemodialysis. However, hemodialysis can also lead to clinical complications, one of which is uremic pruritus, a chronic itching sensation often experienced by patients that impacts their comfort and quality of life. One non-pharmacological intervention that can be applied to address this complaint is the use of olive oil as a natural moisturizer, which functions to reduce pruritus and improve skin condition. This study aims to describe the administration of olive oil to chronic kidney failure patients at risk of skin integrity impairment. The intervention was implemented using a case study method with an Evidence-Based Nursing (EBN) approach on 5 patients. The intervention was performed twice, once a day for 5 minutes. Evaluation was conducted using the Pruritus Severity Scale (PSS) questionnaire. The results showed a significant decrease in pruritus scale and improvement in skin condition after the administration of olive oil. The application of this therapy has been proven effective in reducing the risk of skin integrity disorders in CKF patients undergoing hemodialysis.

Readiness for improved health management is one of the family issues with DM that can be addressed through education. The research aims to describe the management of health management improvement readiness through education. The type of research is a descriptive case study with a nursing care approach. The research sample was 1 family with a risk of diabetes. Management was carried out for 1 week, with the results of the assessment indicating that the client's family had a history of diabetes, the client stated they always took medication regularly according to the doctor's instructions, and had been regularly participating in the Prolanis program for 3 years. The intervention consisted of health education using a booklet. Implementation included explaining the definition, causes, signs and symptoms, prevention, and both medical and non-medical care, as well as explaining the client's healthy lifestyle with diabetes. Evaluation showed an increase in knowledge for both the client and their family regarding diabetes, and the booklet was effective in improving knowledge. Recommendations for nurses to be more thorough in managing diabetic patients and to make the most of the booklet medium.

Abstract

Typhoid fever presents with hyperthermia, a condition that can affect preschool children. Preschool children have an underdeveloped immune system and poor personal hygiene habits, it makes susceptible to infections such as typhoid fever, which is characterized with hyperthermia. The goal of this study is describing the application of water tepid sponges (WTS) in reducing the body temperature of preschool children with typhoid fever. This study uses a descriptive method with a case study approach of nursing process, on one preschool child with typhoid fever and hyperthermia. The intervention provided was hyperthermia management through water tepid sponge compresses for 3 days management, monitoring of changes in body temperature before and after the intervention. Data collection techniques included assessment, nursing diagnosis, nursing intervention, nursing implementation, and nursing evaluation. The results of the management are that hyperthermia can be resolved, with a decrease in body temperature on the first day, the patient's body temperature before being given a water tepid sponge was recorded at 38.9°C, then decreased to 37.5°C after the procedure within 15 minutes. On the second day, the initial temperature of 38.5°C decreased to 36.9°C after being given WTS. On the third day, the patient's body temperature was 36.5°C, indicating that the body temperature had returned to normal. Suggestions for families are expected to implement the health education that has been provided by nurses, especially in carrying out water tepid sponge (WTS) compress actions at home as an independent effort in treating children when they have a fever.

Abstract

Diabetic ulcers, a complication of diabetes mellitus, often require surgery, which triggers significant anxiety in patients. Preoperative anxiety can negatively impact the patient's physiological and psychological stability. Benson relaxation is a non-pharmacological intervention proven to reduce anxiety. The goal of this study to determine the effect of Benson relaxation on preoperative anxiety levels in patients with diabetic ulcers in the IBS ward at Fastabiq Sehat Hospital, PKU Muhammadiyah Pati. This was a quantitative study with a quasi-experimental design using a one-group pretest-posttest approach. A sample of 33 respondents was selected using a purposive sampling technique. Anxiety data were measured using the HARS and analyzed using the Wilcoxon test. Before the intervention, the majority of patients experienced moderate (51.5%) and severe (48.5%) anxiety. After Benson relaxation, mild anxiety increased to 54.5%. The Wilcoxon test showed a p-value of 0.000 (p < 0.05). Benson Relaxation is effective in reducing preoperative anxiety in patients with diabetic ulcers and can be implemented in preoperative nursing care

Abstract

Hemodialysis is renal replacement therapy which is important for the patient of chronic renal failure. Hemodialysis procedures are currently experiencing rapid development, it’s often stated that many patients experience medical problems during hemodialysis. Common complications during hemodialysis include hemodynamic disturbances. The  goal of this research is to determine if there is a correlation between the lenght of hemodialysis sessions and blood pressure levels in chronic renal failure patients in the hemodialysis ward of an Pati Islamic Hospital. This research was a cross-sectional approach which is monitoring and measuring the patient’s blood pressure before and after the hemodialysis session.  The statiscal test used spearman rank In this research, it was found that the matority of hemodialysis sessions were short in duration whic is 37 (77,09%) respondents. While blood pressure is hypertension, which is 36(75%) respondents. As indicated by Spearman’s rank correlation, the obtained value is p value 0,406 with a = 0,05. If p value >a, it can be concluded that there is no correlation between hemodialysis duration and the blood pressure of patients with chronic renal failure. The research showed that there was no relationship between the duration of hemodialysis and the blood pressure of patients with chronic renal failure.

Abstract

Pulmonary tuberculosis remains a health problem in Indonesia. The main obstacle to treating pulmonary tuberculosis is low patient adherence to taking medication regularly, which can lead to drug resistance and treatment failure. Patient adherence is influenced, among other factors, by education and attitude. The research aims to determine the relationship between education level and attitude with medication adherence in pulmonary TB patients who receive outpatient care at Pati Islamic Hospital. The research used an analytical quantitative design with a cross-sectional approach. The number of respondents is 40 patients, selected using purposive sampling. The research instruments were an education level questionnaire, a Guttman scale attitude questionnaire, and the MMAS-8 medication adherence scale. The research analysis was performed using the Spearman Rank test. The results showed that the majority of respondents had a secondary education level (35%) and moderate attitudes (60%). The most common level of adherence was moderate (52.5%). There was a significant relationship between education level and adherence (p = 0.000; ρ = 0.65) and between attitude and adherence (p = 0.000; ρ = 0.72). Conclusion of the study: The higher the level of education and positive attitude of patients, the higher the adherence to taking tuberculosis medication.

The research type is cross-sectional. The population consists of all chronic kidney failure patients undergoing hemodialysis, with a sample size of 48 individuals selected thru purposive sampling. The research instruments were the Perceived Social Support - Family (PSS-Fa) survey and the Kidney Disease Quality of Life (KDQOL) questionnaire. The analysis used the Spearman rank test with a significance level of 5%. The results of the study showed that the majority of respondents had a low quality of life (47.9%) and strong family support (52.1%). Statistical analysis showed a p-value of 0.369 (p > 0.05) and a correlation coefficient of r = 0.132. Conclusion: There is no significant relationship between family support and the quality of life of chronic kidney failure patients undergoing hemodialysis at Pati Islamic Hospital.
