Abstract

Teenage pregnancy can cause several risks to both the mother and the fetus, for the fetus there can be complications of premature birth and LBW. In the mother there can be bleeding, perineal tears and prolonged labor. In 2022, as many as 12.8 million mothers who gave birth were aged 15-19 years. In this case, the mother with a young pregnancy aged 18 years accompanied by condyloma acuminata disease has the potential for respiratory problems, vocal cord damage, and blindness in the eyes of babies infected with HPV. The methods used in compiling the Report are case reports, midwifery management approaches and SOAP documentation (Subjective, Objective, Analysis, Management). Data collection techniques use interview techniques, physical examinations, observations, documentation studies, and literature studies. Analysis in this case Mrs. D,18 years old pregnant 31 weeks with condyloma acuminata, care provided in collaboration with a SPog doctor by administering 80-90% trichloroacetic acid ointment and 0.1% gentamicin sulfate ointment therapy, education on STDs and sexuality, after two months of care condyloma acuminata resolved. The conclusion of the results of care is in accordance with the standards of pregnancy care, but there are obstacles, namely the husband who is not cooperative. The suggestion in this case is to invite the husband to undergo treatment so that the sexually transmitted disease in this couple is resolved.

Abstract

Vaginal discharge is a fluid released from the genital area, excluding blood, and often occurs due to poor hygiene in the genital area. Pathological vaginal discharge is characterized by milky white, thick fluid accompanied by itching and is commonly caused by bacterial, viral, fungal, or parasitic infections. Over the past three months, 28 cases of pathological vaginal discharge were reported out of 53 reproductive health complaints at Parung Community Health Center. This report aims to apply midwifery care for Miss R, an 18-year-old patient, with pathological vaginal discharge at Parung Community Health Center.The method used a case report with a SOAP approach (Subjective, Objective, Analysis, Management). Data were collected through interviews, observations, physical examinations, documentation studies, and literature reviews. The analysis showed that Miss R experienced pathological vaginal discharge caused by fungal infection (Candida albicans), triggered by unhealthy habits such as wearing non-absorbent underwear and inadequate genital hygiene.Management included pharmacological therapy with a single dose of oral oral metronidazole 500mg and antifungal cream to relieve itching, as well as non-pharmacological education on genital hygiene, using cotton underwear, and avoiding scented hygiene products. Care was provided over 10 days, resulting in significant improvement in symptoms.In conclusion, the management of pathological vaginal discharge tailored to the patient’s needs produced optimal results. Education for the patient and her family is essential to detect signs of pathological vaginal discharge and prevent recurrence in the future.

Abstract

Globally, in 2022, the prevalence of contraceptive use was recorded at 65%. The purpose of this study is to determine the relationship between knowledge and maternal parity with the use of 1-month injectable birth control at PMB Hj. Zuniawaty Palembang in 2021. The research design used is a quantitative research that is an analytical survey with a cross-sectional design. The independent variable is knowledge and parity, the dependent variable is the use of 1-month Injectable Birth Control. The population is all mothers who came to PMB Hj. Zuniawati Palembang from November – December 2021 which amounted to 32 people. The sampling technique uses the accidental sampling technique, which is a sampling method by selecting respondents who happen to be there/found. The instrument useD a questionnaire. The data analysis carried out was univariate and bivariate. The chi-square test showed a significant relationship between knowledge (p=0.001) and parity (p=0.004) with the use of 1-month injectable birth control. Suggestions for PBMs can improve services, especially services for mothers who use 1-month injectable birth control and provide complete information about good knowledge of good 1-month injectable contraceptives and are willing to use injectable contraceptives as an effective contraceptive.

Abstract

Pregnancy triggers complex metabolic changes, particularly in glucose homeostasis. This vital adaptation, marked by physiological insulin resistance in the second and third trimesters, ensures an adequate glucose supply for the fetus. However, a failure of pancreatic beta-cell compensation can lead to Gestational Diabetes Mellitus (GDM), a significant obstetric complication. GDM shares pathophysiological similarities with type 2 diabetes (T2D) and poses substantial risks, including adverse perinatal outcomes (e.g., macrosomia and preeclampsia) and long-term consequences such as increased maternal T2D risk and offspring susceptibility to obesity via fetal programming. Despite these known impacts, GDM diagnosis remains challenging due to a lack of uniform diagnostic standards and often-delayed screening. This review, therefore, aims to comprehensively examine the physiological adaptations, factors influencing glucose dysregulation, and clinical implications of GDM. Furthermore, it evaluates current management strategies and identifies future research directions, emphasizing the crucial role of midwives in developing innovative, prevention-based approaches from preconception through the breastfeeding period to improve overall maternal and child health. Abstract

According to the World Health Organization (2013), approximately 15% of pregnant women experience complications that can endanger their lives, often due to a lack of understanding about bodily changes during pregnancy. This study aimed to examine factors associated with ANC K1 visits among pregnant women at the Lima Ilir Health Center in 2023. An accidental sampling method was applied, involving all pregnant women who underwent ANC K1 examinations at the Lima Ilir Health Center from November to December 2023, with a total sample of 30 respondents. Data were analyzed using univariate and bivariate methods. The findings revealed that 21 participants (70%) attended ANC K1 visits, while 9 participants (30%) did not. Chi-Square test results at a significance level of α = 0.05 indicated no significant association between mothers’ knowledge (p = 0.815) and parity (p = 0.09) with ANC K1 visits, but a significant relationship was found between educational level and ANC K1 attendance (p = 0.016). It is recommended that the Lima Ilir Health Center enhance health education programs to emphasize the importance of ANC K1 visits in monitoring both maternal and fetal health.

Abstract

The Intrauterine Device (IUD) was the effective, safe, and reversible long-term contraceptive method. Despite its numerous advantages, IUD adoption still faces challenges such as a lack of information about IUDs, negative public perceptions, misconceptions, and side effects. If IUD side effects are not properly managed, they can cause anxiety and unpreparedness, potentially leading to complications, leading to premature IUD removal. The purpose of this Final Project Report is to provide family planning midwifery care to Mrs. S, 39 years old, P3A0, a new IUD recipient.This report uses a midwifery care approach with documentation using the SOAP (Subjective, Objective, Analysis, and Management) approach. Data were collected through history taking, physical examination, observation, documentation, and literature review.The subjective data assessment revealed that Mrs. S complained of vaginal discharge, no itching, no odor, and abdominal pain three days after IUD insertion. The pain scale used a Numeric Rating Scale (NRS) instrument with a scale of 5. Objective data from a genital examination revealed a white, watery vaginal discharge. Mrs. S, 39 years old, P3A0, a new IUD acceptor, was analyzed. The management provided included providing information about IUDs, conducting genital and IUD examinations, explaining the causes of vaginal discharge and abdominal pain, advising on maintaining personal hygiene, teaching deep breathing relaxation techniques, and recommending warm compresses on the painful abdominal area. The conclusion from the care provided to Mrs. S was that the mother understood the IUD, no longer experienced vaginal discharge or abdominal pain, and was not worried about her condition.

Abstract

ABSTRACT Platelet count examination is a crucial parameter in evaluating hemostatic function, especially in diagnosing diseases such as dengue fever. This study compares the manual method using Ammonium Oxalate reagent with the digital imaging method utilizing ImageJ software on the Improved Neubauer chamber. The research aims to evaluate the differences between these methods and compare them with the automated method (Hematology Analyzer) as the gold standard. The study employed an observational analytic design with a cross-sectional approach. Samples consisted of blood from patients undergoing complete hematology examinations at RSUD Bedas Kertasari, Bandung Regency. Results from the three methods were compared to determine accuracy, efficiency, and objectivity. The results of the study showed the quantitative distribution of platelet counts using automatic (Hematology Analyzer), manual (Ammonium oxalate) and digital imaging (ImageJ) tools obtained the average values ​​respectively: 302,400 cells/mm3, 297,500 cells/mm3, 305,000 cells/mm3. There was no statistically significant difference in the test of platelet count examination data pairs between the automatic method (Hematology Analyzer) and the manual method (Ammonium oxalate) and between the automatic method data pairs (Hematology Analyzer) and the digital imaging method (ImageJ). While in the test of platelet count examination data pairs in the Improved Neubauer counting chamber between the manual method (Ammonium oxalate) and the digital imaging method (ImageJ) there was a statistically significant difference, but clinically the difference was still within the permissible threshold.

Abstract

Background: According to SKI 2023, the number of people with Diabetes Mellitus in West Java has increased, including in Bandung Regency, which recorded 62,344 clients. Cinunuk Public Health Center is one of the subdistricts with the highest number of cases, totaling 2,307 clients. The large number of clients indicates the need for rehabilitative efforts to prevent decreased productivity and more severe complications. Educational media is needed as a tool to improve knowledge and control health-related behaviors, thereby preventing higher morbidity rates. Objective: To develop a self-guided rehabilitative educational media using the Research and Development (R&D) method with the 4D approach model. Method: The development stages included define, design, develop (involving expert validation in content and media, small-scale trial), and a quantitative evaluation using a quasi-experimental one group pre-test post-test design on 40 respondents, followed by dissemination. Results: Clients require a printed educational media in the form of a self-guided booklet containing rehabilitative materials and a behavioral monitoring table. The developed media showed a positive impact on clients’ knowledge. The media was also disseminated via QR code and link through WhatsApp and print distribution. Conclusion: The rehabilitative self-guided educational media is feasible for use as a strategy to support rehabilitative behavioral control in clients with type 2 Diabetes Mellitus.

Abstract

Introduction: Diabetes mellitus remains a major global health challenge, with microvascular complications—retinopathy, nephropathy, and neuropathy—contributing significantly to morbidity and reduced quality of life. Glycemic control, primarily measured by HbA1c, is a cornerstone of diabetes management, but the strength and consistency of its association with microvascular outcomes across diabetes types and patient subgroups require comprehensive synthesis. Methods: We conducted a systematic review of 80 studies investigating the association between HbA1c and microvascular complications in Type 1 and Type 2 diabetes. Studies were screened based on predefined criteria including study design, population, measurement of HbA1c, assessment of microvascular outcomes, and availability of quantitative data. Data were extracted on HbA1c definitions, complication types, study characteristics, effect measures, and confounding control. Results: Poor glycemic control (HbA1c >7%) was consistently associated with increased risk and progression of microvascular complications. In Type 1 diabetes, intensive control reduced retinopathy by 76%, nephropathy by 39–56%, and completely prevented clinical neuropathy over 24 years. In Type 2 diabetes, intensive control reduced retinopathy progression by 23–33% and nephropathy by 21–26%. However, benefits diminished in older patients, those with advanced complications, or long disease duration. Glycemic variability and metabolic memory effects were also significant. Intensive control increased severe hypoglycemia risk approximately twofold. Discussion: The association between HbA1c and microvascular complications is strong but modulated by diabetes type, disease stage, age, and glycemic variability. Early intensive control yields lasting benefits via metabolic memory, especially in Type 1 diabetes. In Type 2 diabetes, individualized targets are essential to balance microvascular benefits against hypoglycemia and mortality risks. Conclusion: Glycemic control is fundamentally important in preventing and delaying microvascular complications, but treatment must be personalized. Future research should focus on variability metrics, early intervention windows, and integrative management strategies.

Abstract

Introduction: Critical Congenital Heart Disease (CCHD) is a leading cause of infant mortality, often presenting with subtle or absent clinical signs in the newborn period. Early detection is crucial for timely intervention and improved outcomes. Pulse oximetry has emerged as a potential screening tool, but its diagnostic performance, optimal implementation protocol, and cost-effectiveness, especially compared to existing methods like physical examination and prenatal ultrasound, require comprehensive evaluation (Ewer, 2013; Thangaratinam et al., 2012). Methods: This systematic review adhered to the PRISMA 2020 guidelines. We identified 68 studies (2002-2025) including systematic reviews, meta-analyses, and primary studies (prospective/retrospective cohorts, RCTs). Eligibility criteria focused on newborns (0-28 days), pulse oximetry as a screening tool for CCHD, and reporting of diagnostic accuracy measures. Data extraction covered CCHD definitions, screening protocols, diagnostic performance, population characteristics, and comparison with other screening methods. Results: The pooled sensitivity of pulse oximetry for CCHD detection varied widely (47%-92%), while specificity was consistently high (93%-99.9%). The sensitivity variation was attributed to differences in CCHD definitions, screening timing (higher sensitivity but more false positives if done before 24 hours), and protocol details. Physical examination alone showed lower sensitivity (32%-69%). Combining pulse oximetry with physical examination significantly improved sensitivity to 92%-93%. Pulse oximetry was found to be cost-effective, with an incremental cost per timely diagnosis substantially lower than universal screening echocardiography. Implementation success depended on standardized protocols, staff education, and decision support tools (van Vliet et al., 2023; Aranguren Bello et al., 2019; Knowles et al., 2005; Londoño et al., 2017; Hom et al., 2019). Discussion: Pulse oximetry is a valuable, specific, and moderately sensitive tool for CCHD screening. Its greatest utility is realized when used in combination with physical examination, applied after 24 hours of birth to balance sensitivity and specificity, and adapted for specific populations (e.g., adjusting thresholds for altitude). It is particularly beneficial in settings with low prenatal detection rates and limited resources. However, it cannot detect all CCHD cases, especially non-cyanotic lesions like isolated coarctation of the aorta. Conclusion: The evidence strongly supports the integration of pulse oximetry into universal newborn screening programs as a cost-effective adjunct to physical examination. Future efforts should focus on standardizing CCHD definitions, optimizing context-specific protocols, and improving implementation fidelity in diverse healthcare settings, particularly in low- and middle-income countries.
