Abstract

The selection of unit cost calculation method in hospital services is a challenge for hospitals. Errors in the selection of the calculation method will make a big difference in the final results of the total costs required after the service. Yogyakarta City Hospital still uses traditional methods of determining service rates. Design of this study is a qualitative study with a case study approach to the unit costs required for uncomplicated section Caesarea which is calculated using the activity-based costing method compared to the unit cost of section Caesarea calculated using traditional methods. The purposes are to know the differences between the two methods. The results of this study indicate the unit cost of uncomplicated section Caesarea using the ABC method is IDR 2,703,919. the action rate by the hospital is IDR. 3,051,000, greater than the ABC method. The difference obtained is Rp 347,081. The Conclusions is a positive difference between the unit cost of the ABC method and the hospital rate so it is necessary to evaluate the service rates in Yogyakarta City Hospital.

Abstract

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis signified by the formation of granuloma by the infected tissue. Old TB is a case of TB that once suffered from tuberculosis infection and declared as being cured. This research aims to know the presence of description on spirometry disorder in the old TB patients who were suffering from dyspnea in BBKPM Surakarta. Type of this research is descriptive research using Cross-Sectional approach. Population which becomes the subject of this research is the outpatient in the Non-TB Clinic of BBKPM Surakarta according to the criteria. Sampling was conducted using the method of Purposive Sampling and it was obtained 50 samples of old TB patients who were suffering from Dyspnea. Data were then processed by editing, coding, and tabulation. From the results of the data processing through editing, coding, and tabulation, there were 27 patients who were suffering from Obstruction disorder, 5 patients were suffering from Restriction disorder, 17 patients were suffering from mixed disorder (Obstruction and Restriction), and 2 patients were Normal. This research concluded who was suffering from dyspnea, in BBKPM Surakarta.

Abstract

Buying behavior in choosing health care is a long process. Segmentation is one of the things that underlies the decision of the patient. This study aims to determine the buying behavior of patients who do the first child labor and its relationship with buying behavior in RSIA Puri Bunda (PB) Malang. This research uses the cross-sectional method. The sampling technique used the total sampling method, ie all inpatients who gave birth to the first child at the time of this study were executed. The number of samples was 55 respondents in April-May 2017. Data were analyzed using multinomial logistic regression. The results showed that customer segmentation of RSIA PB was from Blimbing sub-district, 21-25 years old, last high school education, income <Rp.1.500.000,00, housewife, Islam, class of 3rd class and payment method using BPJS . Buying behavior of respondents is included in complex type and dissonance reducing buying behavior. Geographic, demographic, psychographic and behavioral segmentation is not related to buying behavior. Segments that have been detected are expected to get attention from the hospital to determine the marketing efforts that will be done next. In its marketing efforts, RSIA PB should pay more attention to customers who have a high-involvement type in order to provide all the information expected by the customer.

Abstract

The patient safety incidents is closely related to treatment services. Medication errors in pediatric patients are generally three times higher than adult patients. 40.8% medication errors occur in prescribing drugs such as antibiotics. This study was to analyze the management of antibiotic prescribing in hospitalized pediatric patients at X Hospital for patient safety. This study used mixed methods namely quantitative by observational cross sectional and qualitative case study. It used a purposive sampling method with inclusion and exclusion criteria obtained from 494 medical record data from 2367 total patients in 2017. Analysis of the antibiotic rationality of the Gyssens method and deep interviews of pediatricians, pharmacists and patient safety management. The most antibiotic use is cefotaxime 51,83%. It used rational antibiotics (40.69%). The irrational antibiotics were in category V (22.8%), IVD category (25.10%), IIIA category (8.10%), IIC category (2.63%). Giving antibiotics rationally is not maximized so that it needs better regulation and systems. Patient safety incidents occur in patients' identity errors and LASA drugs (look-alike and sound-alike) do not cause serious incidents. Prescribing rational antibiotics in hospitalized pediatric patients has not been maximized and patient safety incidents are still found. Need support from management in optimizing rational antibiotic use programs as well as patient safety culture.

Abstract

Infectious diseases are still the leading cause of morbidity and mortality in the world. One of the infections was a nosocomial infection. Nurses have appreciable influence regarding nosocomial infection incident because nurses have a great deal of interaction with the patient during 24 hours. The prevention of nosocomial infection can be conducted by a nurse is with hand hygiene practice. The purpose of this research is to know factors influencing nurses’ compliance level in the application of hand hygiene at in inpatient ward of Muntilan General Hospital. This was a quantitative research with research design cross-sectional. The sampling technique used was the Proportionate Stratified Random Sampling with the number of respondents was 54 nurses. The instrument of the research applied a questionnaire and checklist. Analyses of the data used were the Chi-Square test. This research indicated there was a correlation between motivation with a compliance level of nurses in application of hand hygiene (p-value 0.000<0,05).There was a correlation between attitudes with a compliance level of nurses in application of hand hygiene (p-value 0.000<0,05).There was no correlation between work period with a compliance level of nurses in application of hand hygiene in of Muntilan General Hospital (p-value 0.459>0,05).

Abstract

Access to a hospital and continuity of services are very important for the continuity of the hospital so that people who need health services are interested in visiting the hospital and utilizing the health services. The aims of the research are to find out the implementation of access to the hospital and continuity of service at X Hospital. The design of the research was descriptive qualitative research with observation and interview to collect the data. In observation, the subjects are 180 patients registered. In the interview, step is human resources who concerned on hospital access and continuity of service. Data analysis used interviews coding and calculated the observation result. Results and discussion are Observations on the implementation of a standard operating procedure for hospital access and continuity of services have been carried out in excess of 80%, but there are obstacles in the implementation, one of which is not all clinical staffs know the step in standard operational procedure. The conclusion is Clinical staff has implemented the standard operational procedure of Hospital access and service continuity exceeds 80%.

Abstract

The implementation of best Patient-Centered Collaborative Care training concept becomes one of the important aspects of providing health services in a hospital. The improvement of health services conducted bt the hospital is aimed to support healing and recovery with the full attention of patient comfort. This study used Mixed Method Research or a combination of quantitative and qualitative approaches os Sequential Explanatory, and qualitative data analysis in the second stage, to support the quantitative research finding which is done in the first stage. This study used One Group Pre – Test – Post – Test Design without Control, is a way of performing one pretest- before treatment (treatment/training) and afterward measuring again (post-test) without giving a control treatment. There are 96 respondents with one year of minimum work period in X Hospital. The respondent's observations are included gender, educational background, work period, status, children, residence and the workplace. The result of this study shows that the highest level of nurse’s knowledge is influenced by the age, work experience, educational background, and motivation of self-development. Statistically, there are no signification differences, but from training appreciation, a nurse is unrealized that there is the influence of PCC implementation at the Surgical and Intern Ward Inpatient Unit X Hospital and supported by nurse’s performances. Good knowledge of PCC concept proved to have an effect on PCC implementation inpatient Surgery and Interna Ward X Hospital supported by the improvement of nurse performance in inpatient room.

Abstract

Indonesia government through Indonesia Ministry of Health has developing Health Information System, including Primary Health Care Information System Management (SIMPUS), as a tool to acquire an accurate data to improve society health quality. SIMPUS has been implemented in several districts in Indonesia, including Bantul District, Yogyakarta. But, founded several problems during the implementation. This research aimed to evaluate SIMPUS eHealth implementation compared to IHIS. Qualitative with case study approach was used to capture the data in Piyungan Public Health Centre. PIECES was employed as an evaluation method. Our respondents were SIMPUS manager, SIMPUS officer in mother and child health, SIMPUS officer in patient registration, and SIMPUS officer in the medical record. Our research revealed that eHealth was powerfull than IHIS. Nevertheless, there were several limitations on eHealth such as less of control-security and service. For future research suggested to include more number of Puskesmas as a sample to figure out the comprehensive problem.

Abstract

This research used quasi-experimental design with pre and post test design. Then, for the method used in this research was pre and post intervention which analyze the primer data and secondary data. Furthermore, the research also used descriptive approach to compare the result of pre and post the standard operating procedure was socialized. This research used 52 people as the sample. The research about waiting time of elective surgery and find the main problem bring in the chosen solution by the researcher which is this solution showed the result of the comparison in pre and post socialization the standard operating procedure that increases about 25%. This comparison showed that there is an enhancement of the number of staffs in the operating room who could performance the surgery on time and also showed the reduction number of waiting time in elective surgery.

Abstract

This Research Purpose to identify critical waste, root cause of critical waste, up to giving recommended improvement to minimize critical waste in the outpatient pharmacy. This research uses qualitative approach with case study design. The health service process is visualized with value stream mapping, the amount of critical waste is obtained by distributing questionnaires, while the root cause of critical waste is acquired by 5 why method of interview. The recommended improvementis accomplished by discussion between team and expert panel. Based on the value stream mapping, it has obtained 16,67% of ratio value added to waste for non concoction prescription and 14.52% of ratio value added to waste for concoction prescription. Based on waste questionnaire distribution, motion got the highest rank as the most often exist waste with 19% percentage. The root causes of this motion waste is nonexistence of a routine schedule on the work space organization, which affecting the effectiveness of pharmacy staff to complete their task. The recommended improvement to minimize this waste motion is to conduct 5S method.
