Abstract

In 2015, the maternal mortality rate (MMR) in Tanggamus Regency amounted to 16 events consisting of 3 maternal deaths, 4 maternal deaths, 9 postpartum maternal deaths. Efforts to accelerate the reduction of MMR can be done by ensuring that every mother is able to access quality maternal health services, one of which is health care for pregnant women (antenatal care/ANC). ANC must meet the minimum frequency of 4 visits. However, the uneven distribution of health workers and the low knowledge of health cadres about the importance of ANC is still low, so the number of ANC visits is still low. This study used the quassy experimental method with the design of one group pretest-posttest design. The sampling technique is total sampling. The research was conducted from April to July 2018, located in Gunungtiga Village, Ulubelu District, Tanggamus Regency, Lampung. The samples that were obtained were 18 Posyandu cadres. The median value of the knowledge pre-test is 30, while the median value of the post-test knowledge is 80. The results of the bivariate analysis p = 0,000. There was a significant difference between the training regarding the importance of antenatal care for the knowledge of Posyandu cadres in Gunungtiga Village.

Abstract

The elderly group is included in the vulnerable population because of the process of decreasing the ability to avoid disease and exposure to risk factors for chronic diseases such as Diabetes Mellitus (DM). DM can increase the risk of tuberculosis (TB) infection three times greater than the normal population, and can also increase the risk of reactivation of TB and latent TB. The study conducted was a case report. This case report aims to implement evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and management with a patient-centered approach and a family approach. Primary data were obtained through anamnesis and physical examination by making home visits, filling in family folders, and filling in patient files.

Abstract

Henoch Schonlein Purpura is a clinical syndrome caused by vasculitis which is most common in children. Annual HSP incidence reaches 10-20 per 100,000. Henoch Schonlein Purpura can affect all ages, but 50% of cases occur at less than 5 years of age and 75% of cases occur at less than 10 years of age. The diagnosis of HSP is based on clinical symptoms. If 2 of the 4 criteria are found, namely ≤20 years of age on pain, palpable nontrombocytopenia purpura, diffuse acute abdominal pain that gains weight after eating, and biopsy showing granulocytes in the walls of the arterioles or venules. Gastrointestinal manifestations of Henoch Schonlein Purpura occur in 60-65% of patients. The core explanation regarding the pathophysiology of gastrointestinal disorders in HSP is the deposition of immune complexes in the blood vessels of the gastrointestinal organs. In this case, the patient is suspected of having intussusception, this is in accordance with the literature which states that intussusception in 2-6% of cases can be a complication of gastrointestinal tract vasculitis in HSP. Most HSP can recover without treatment. In mild circumstances, supportive management is given in the form of hydration, nutrition, and symptomatic drug administration. The controversy of corticosteroid therapy is mainly related to the exact time of administration. At present, corticosteroids have been widely used to treat HSP with gastrointestinal and kidney manifestations.

Abstract

Labor has its risks for both mother and fetus, such as illness until death risk. There is two types of labor: vaginal labor and abdominal labor. The optimal productive age for a healthy reproduction is between 20 – 35 years old. The risk will increased at the age below 20 years old and above 35 years old. This research is an quantitative research with an observational analytic design. The population of this research is the women in labor at RSUD Dr. H. Abdul Moeloek Lampung Province between July 2017 – September 2017. The sample of this research is all the women in labor between July – September 2017, in total of 191 sample. Sample is taken with purposive sampling. Research data collection was obtained from medical record. Bivariate analysis using Chi Square. The result of this research showed that pregnant women below 20 years old and above 35 years old who gave birth by vaginal labor is 52,5%. While abdominal labor is 47,5%. For pregnant woman between 20 – 35 years old who gave birth by vaginal labor is 69,7% and abdominal labor is 30,3%. The result of Chi Square analysis showed that there are effects of age factor of pregnant women and the types of labor at RSUD Dr. H. Abdul Moeloek Lampung Province with p = 0,034. OR score obtained at OR = 2,007 thus pregnant woman in risk age (below 20 and above 35 years old) has twice abdominal labor risk compared with woman with reproductive age (20-35 yearls old). Abstract

Staphylococcus aureus is one of the bacteria that is pathogenic to humans and is most often transmitted from hand to hand. Moringa leaf gel as an alternative as an antibacterial. The purpose of this research is to determine the antibacterial activity of Moringa oleifera leaf extract in gel formulation against Staphylococcus aureus. This type of research is an experimental laboratory with the method of sump on Mueller Hinton Agar media. Moringa oleifera leaf extract was obtained from the Organic Chemistry Laboratory of the University of Lampung with maceration techniques using 96% ethanol. Gel formulations were made at Cendikia Farma Husada Pharmacy Vocational School Bandar Lampung. Moringa oleifera leaf extract is divided into several concentrations namely 5%, 10%, 20%, 40% and 80%. As a negative control, distilled water gel and positive control were erythromycin (Erymed®) gel. The data obtained is based on the results of the measurement of the inhibition zone formed around the sump and measured by the calipers. Data were tested by One Way ANOVA. The results of this study indicate that the diameter of the inhibitory zone formed in the Moringa oleifera leaf extract in gel formulation in concentration of 5%, 10%, 20%, 40% and 80%. Sequentially, namely 5.85 mm, 10.00 mm, 11.00 mm, 15.40 mm and 21.05 mm. In the negative control group it was 0 mm and positive control was 32.20 mm (p value = 0,000). There is an antibacterial activity of Moringa oleifera extract in gel formulation against Staphylococcus aureus.

Abstract

Diarrheal diseases is still health problems of the world with high number of mortality and morbidity. Traditional medicine which has a bitter taste and antibacterial effect, one of which has the effect of antibacterial honey, bitter taste and antioxidants. This study to find out the potential effects of honey supplementation on reducing the frequency of acute diarrhea in children in the Dr. H. Abdul Moeloek. This study used quasi-experimental design with nonequivalent control group design. Subjects of study were children with acute diarrhea who were divided into two intervention groups and control group, each group amounted to 15 respondents. Data obtained directly from research subjects through primery data. From the data obtained then the analysis is performed using unpaired samples t-test. This study indicates the frequency of first-day acute diarrhea of intervention group was more than control group. The frequency of acute diarrhea on the second, third and fourth days in intervention group was less than children who were controls group. The statistical test results show the potential effects of honey supplementation on decreasing frequency of acute diarrhea. Provision of honey supplementation was proven to reduce the frequency of acute diarrhea in the Dr. H Abdul Moeloek Hospital Bandar Lampung.

Abstract

Metformin is the most widely used oral hypoglycemia drug in the world. This drug become the first choice for the treatment of type 2 diabetes melitus (DM). However, metformin is known to have the effect of creating a vitamin B12 deficiency in type 2 DM patients who consume it. The existing hypothesis states that metformin interferes with the absorption of vitamin B12 through the mechanism of intestinal bacterial overgrowth, the disruption of the vitamin B12 complex with calcium and cubulin receptors. The effect of metformin is known to increase with increasing doses and the length of time uses. The incidence of worsening peripheral neuropathy and megaloblastic anemia have been reported to be associated with the effects of vitamin B12 deficiency in type 2 DM patients taking metformin. To date, there are no clear guidelines that discuss how to diagnose, manage and prevent vitamin B12 deficiency due to metformin, but some research results can be taken into consideration. Vitamin B12 deficiency should be suspected in type 2 DM patients who have taken metformin ≥3 years, at a dose of >1500 mg, experiencing hematological abnormalities or worsening peripheral neuropathy. Prevention that can be done is to give vitamin B12 with dose of 1000 μg intramuscularly per year. As for treatment, an intramuscular injection of vitamin B12 can be given a dose of 1000 μg per day for seven days, followed by an injection once a week for four weeks.

Abstract

Indonesia is one of the country that has a wealth of natural resources that are diverse and scattered throughout the region. These natural resources are widely used as traditional medicine by people in Indonesia. One of these natural resources is the eucalyptus plant. Eucalyptus leaves (Melaleuca leucadendron L.) are known to have various benefit for health. One of the active compounds contained in eucalyptus leaf extract is 1,8-cineol. This compound is known to have antimicrobial, antioxidant, and anti-inflammatory activity. Besides, eucalyptus leaves also contain α-terpineol, α-pinen, and ß-pinen. These compounds also have antibacterial activity by damaging cell membranes, inhibiting enzymes, and destroying genetic material in bacteria so that they are thought to inhibit bacterial growth.

Abstract

The prevalence of chronic energy deficiency (CED) in Lampung Province in 2013 was 21,3% for pregnant women and 17,5% in non-pregnant women, in Bandar Lampung was 24,5%. The problem of chronic energy deficiency (CED) in women is influenced by various factors, namely food intake, incorrect understanding of nutrition, the entry of undernourished food products from outside and chronic infectious diseases.This study was conducted to determine the relation of food intake with CED in pregnant women in working area of Sukaraja health center in Bandar Lampung.This type of researchwas quantitative using cross sectional approach with consecutive sampling technique. The inclusion criteria were pregnant women in the first and second trimesters, aged 20-35 years, and willing to be respondents. The exclusion criteria were women with chronic infection, anemia, and hyperemesis gravidarum. The research respondents were 71 pregnant women. Food intake data were obtained from the Semi quantitative food frequency questionnaire and CED data by measuring the upper arm circumference (LILA). Data were analyzed by fisher exact test.The results showed that there were 8.5% of respondents suffering from CED. Respondents who have less energy intake (36.6%), less carbohydrate intake (69.0%), less protein intake (11.3%), and less fat intake (39.4%). Based on the results of the analysis, it was found that energy and protein intake was associated with CED (p = 0.022) and (p = 0.017), while carbohydrate and fat intake was not related to CED (p = 0.167) and (p = 0.204). The conclusion of this study is that there is a significant relationship between energy and protein intake with the incidence of CED in pregnant women in the work area of Sukaraja Health Center, Bandar Lampung City.
