Abstract

Wounds are damage or loss of body tissue due to a factor that interferes with the body's protection system. These factors include trauma, temperature changes, chemicals, explosions, electric shock, or animal bites. mangroves can be used for medicine because there are several compounds such as alkanoids, flavonoids, steroids, difenoid and sapronin.This study used 30 rats divided into five groups, namely K-groups were only given normal saline, K + was given silver sulfadiazine, P1, P2 and P3 were given mangrove extract with concentrations of 20%, 40% and 80% respectively. This research was conducted for 14 days. Before the mice were given cuts on the back and then treated. The wound healing process macroscopically between the administration of silver sulfadiazine with extracts of mangrove bark with various concentrations of 20%, 40% and 80% in Wistar strain male rats (Rattus norvegicus), concluded that the administration of magrove bark extract (Buguiera gymnorrizha) has an effect on the healing process of wounds in male white mice (Rattus norvegicus) wistar strain because the value of p = 0,000 was obtained pvalue

Abstract

Gout is a progressive disease due to deposition of monosodium urate (MSU) crystals in joints, kidneys, and other connective tissues as a result of chronically elevated serum uric acid levels above normal. The prevalence of hyperuricemia and gout in Indonesia is higher than other Asian countries. Dietary factors with excessive puring consumption are one of the risks factor of gout. Therefore, in an effort to treat disease, the role of the family is needed to achieve good therapeutic goals. The purpose of this study is to identify internal and external risk factors in patients, apply a holistic and comprehensive family doctor approach, and perform management based on Evidence Based Medicine that is familyapproach and patient-centered. A 59-year-old female patient came with complaints of pain in both knees. Initially the pain was only felt in the patient's right knee, a few weeks later the patient began to feel pain in the patient's left knee. Complaints are accompanied by swelling, redness and heat. The patient has been diagnosed with gout since 2 years ago. Patients do not routinely control their illness and only seek treatment if complaints interfere with their activities. Internal risk factors are the patient does not know the target of normal uric acid levels, what types of food can cause uric acid levels, the pattern of curative treatment, and the patient does not like to exercise. The external risk to the patient is that the family does not know how to prepare food according to the patient's diet and appropriate physical activity. Intervention was carried out regarding the type of food and appropriate physical activity and the importance of routinely checking uric acid in 3 home visits. After the evaluation, the knowledge of the patient and family about the disease were increased.

Abstract

Smoking is a lifestyle that is currently one of the causes of health problems. Every year the number of smokers has increased. Airway irritation by cigarette smoke and other toxic substances will cause an airway inflammatory reaction. Various ways to avoid cigarette smoke bad effect have been tried, one of them is by placing the tongue-in-law (Sansevieria sp) in the room that can absorb air pollution, especially cigarette smoke. The purpose of this study was to determine the effect of tongue-in-law (Sansevieria sp) as a biofilter on tracheal cell damage in Sprague Dawley rats. The method used in this study is a randomized controlled design with a Post Test Only Control Group Desig. This research was conducted by using randomly selected samples and divided into 3 treatment groups with each group consisting of 6 white rats. In group 1 was negative control, rats were given food and drink as usual and were not exposed to cigarette smoke. In group 2 was a positive control, rats were exposed to cigarette smoke as much as 2 cigarettes per day. In group 3 is a treatment group with rats exposed to cigarette smoke and placed the tongue-in-law plant in the cage as a biofilter for 14 days. In this study was found that there were significant difference in the positive control group (who were exposed to cigarette smoke every day as much as 2 cigarettes for 14 days without the biofilter of the tongue-in-law plant) and the treatment group (with Sanseviera sp). So that it can be concluded that there was an effect of the tongue-in-law (Sansevieria sp) as a biofilter on tracheal cell damage in Sprague Dawley rats.

Abstract

Burst abdomen is a postoperative complication that involves tissue damage at the surgical incision site where there is separation due to suture removal that occurs in the surgical wound before the wound healing process is complete. The distended abdomen is part of the complete type of abdominal dehiscence wound. The risk factors for burst abdomen can be classified into three groups, namely pre-operative risk factors, intra-operative risk factors, and post-operative risk factors. This study is a case report at RSUD Abdul Moeloek Bandar Lampung. It is reported that a 48-year-old woman came with complaints of abdominal pain due to open stitches since 1 week ago, after undergoing laparotomy for ovarian cysts 2 months ago at an external hospital. Abdominal pain occurs around the open surgical wound about 8 cm x 1cm x 4cm with pus and blood. The risk factors obtained in the patient were pre-operative risk factors in the form of a history of diabetes mellitus and anemia. The patient was treated in the form of a re-closure operation with the Smead Jones Suture suture technique.

Abstract

Pelvic organ prolapse (POP) is a condition that affects a woman's quality of life. Pelvic organ prolapse can include uterine prolapse, cystocele and rectocele. POP occurs due to weakness of the muscles, ligaments and fascia that support the pelvic organs. This condition can be influenced by factors such as age, parity, menopause, body mass index and occupation. This study aims to determine the relationship of these factors to the incidence of POP. The study is an observational analytic with cross sectional approach, using secondary data from the medical records of gynecological patients at H. Abdul Moeloek Hospital in the period 1 January 2014 to 31 December 2018 with a total sampling technique. Then univariate and bivariate analyzes were performed using the chi-square test. The incidence of POP in the period of 1 January 2014 to 31 December 2018 in the H. Abdul Moeloek Hospital was 33 cases. Factors that had a relationship with the highest risk sequence are parity (p = 0.000; OR 7,959), age (p = 0.002; OR 7,250), menopause (p = 0.014; OR 5.019), body mass index (p = 0.036; OR 3.297) and type of work (p = 0.036; OR 3.297).All factors studied is parity, age, menopause, body mass index and type of work were associated with the incidence of pelvic organ prolapse in patients at H. Abdul Moeloek Hospital, Lampung Province January 2014 to 31 December 2018

Abstract

Schizoaffective disorder is a chronic mental disorder characterized by psychotic and mood symptoms. Psychotic disorders became the top 10 most common diseases at the Gedong Air Community Health Center in the course of January - June 2021 with a prevalence of 4.05%. A proper and holistic treatment using family medicine approach is required to improve the patient's quality of life. This study is a descriptive study of case reports. Primary data were obtained through autoanamnesis and alloanamnesis, mental status examination, and home visits. Secondary data were garnered from the patient's medical record. Patient Mrs. SH, 44 years old, complained of sleeping difficulty during night since 2 months ago followed by frequent soliloquies, asociality, fatigue, auditory hallucination, visual hallucination, and sense of foreshortened future. Clinically, the patient was diagnosed with schizoaffective depressive type (ICD-10 F25.1; ICPC-2:P98). The patient received medical treatment in the form of antidepressants, second-generation antipsychotics, anticholinergics, and phenothiazine antipsychotics. Patients also received interventions in the form of invitations to socialize, write diaries, and paint, as well as education using poster media. The authors conducted an evaluation 16 days post-intervention. The patient became more aware of her disease and had better behavior changes that affected the success of therapy. Patients with schizoaffective depressive type require holistic management with family medicine approach to support the success of therapy and improve the patient's quality of life.

Abstract

Malaria is one of the endemic infectious disease cases in several areas in Indonesia. Indonesia had the highest malaria incidence in Southeast Asia. High levels of morbidity and mortality and antimalarial resistance problems encourage the need to find new antimalarial agents, such as red fruit. Red fruit contains many antioxidants and has been reported to have effect of reducing parasitemia. This study aims to determine the effects of red fruit oil on reducing parasitemia in mice infected with Plasmodium berghei. This experimental study with a post-test only control group design used 32 mice as samples and divided it into 8 groups (control group (aquadest); chloroquine group 0.5 mg / grBB, red fruit oil group doses of 0.1 cc, 0.2 cc and 0.4 cc; combination group of red fruit oil doses of 0.1 cc, 0.2 cc and 0.4 cc, with chloroquine 0.5 mg/grBB). Abstract

Scabies is a skin disease caused by the mite Sarcoptei scabei hominis variety. This disease is a public health problem, especially in tropical and subtropical climates. According to the World Health Organization (WHO), globally it is estimated that as many as 300 million people are currently infected with scabies with a prevalence ranging from 0,2% to 71%. Scabies attacks in groups. Because easy transmission occurs through skin contact either directly or indirectly. Appropriate holistic management using a family medicine approach is needed to achieve therapeutic success and prevent transmission to the community. Patient R, a male student aged 10 years, complained of itching between the fingers, especially at night, since 3 weeks ago. Internal risks in the form of lack of personal hygiene, lack of knowledge, curative treatment patterns, and early adolescence. External risks include similar complaints in the surrounding environment, poor family hygiene, humid houses and poor ventilation, low socioeconomic status, lack of family knowledge. A holistic and comprehensive management effort uses a family medicine approach in the form of education about the causes, transmission, treatment and prevention of disease. The results obtained in the form of a better understanding of the disease and changes in behavior that have an impact on the success of therapy. Holistic management with a family medicine approach is needed for patients with scabies to identify problems in various aspects in order to support the success of therapy and prevent transmission.

Abstract

Diabetes mellitus needs to be managed comprehensively, including pharmacological and nonpharmacological treatment. The treatment goals, complication prevention, and increased quality of life can be achieved by understanding the disease to build self-awareness to modify lifestyles, both diet, as well physical activity. To identify risk factors, clinical problems, conduct patient management holistically by applying family doctors according to evidence based medicine through a patient centered and family approach. Design study is a case report. The primary data obtained through anamnesis (autoanamnesis), physical examination, and home visits. Secondary data were obtained from the patient's medical record. Mrs. S, 56 years old, with type two diabetes mellitus since 5 years ago, concerns about her worsening disease. Knowledge limitations of the patient's health condition, unhealthy diet, and physical inactivity are the factors causing problems in patient’s family. Holistic and comprehensive management of the problems of Mrs. S has been carried out by providing counseling using posters to increase the knowledge of the patient and the patient's family regarding the patient's diet and the importance of physical exercise, as well as routine control to the nearest health facility. After the evaluation, it was found that the patient’s blood sugar decreased while in patient.

Abstract

Scabies is a parasitic infection of the skin caused by Sarcoptes scabei var hominis. Synonyms or other names for scabies are scabies, the itch, gudig, budukan, and agogo itching. Many factors support the development of this disease, including low socioeconomic conditions, poor personal hygiene, level of knowledge, age and contact with sufferers both directly and indirectly. This situation facilitates the transmission and infestation of Sarcoptes scabei. This study is a case report. Primary data were obtained through history taking, physical examination and home visits. Assessment is based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. In the results obtained a 7-year-old boy with complaints arise pimples accompanied by itching, especially at night since 11 months ago. Patients have aspects of internal risk in the form of poor personal hygiene, habits of children who sleep in the same room with their parents, bed linen, pillowcases, and curtains are rarely washed, repeated use of clothes before washing and use towels together. The external risk aspects are low income family with a fairly dense home environment, lack of cleanliness of the home environment, and low patient and family knowledge about scabies. Home visits were carried out 3 times to intervene in patients and families. The diagnosis and management of these patients is done holistically, patient centered, family approached and based on several theories and the latest research. In the process of behavior change, An. M has reached the trial stage. The process of behavior change in An. M and family in personal hygiene have been seen after intervention.
